    Initial stage of plate lifting from a water surface

    This study deals with the flow induced by a rigid flat plate of finite length, initially touching a horizontal water surface, when it starts to move upwards with constant acceleration. In the present model, negative hydrodynamic pressures on the lower (wetted) surface of the plate are allowed, and thus, the water follows the plate due to the resulting suction force. The acceleration of the plate and the plate length are such that gravity, surface tension and viscous effects can be neglected during the early stages of the motion. Under these assumptions, the initial two-dimensional, potential flow caused by the plate lifting is obtained by using the small-time expansion of the velocity potential. This small-time solution is not valid close to the plate edges, as it predicts there singular flow velocities and unbounded displacements of the water-free surface. It is shown that close to the plate edges the flow is nonlinear and self-similar to leading order. This nonlinear flow is computed by the boundary-element method combined with a time-marching scheme.     Automated Classification of Breast Cancer Stroma Maturity from Histological Images

    OBJECTIVE: The tumour microenvironment plays a crucial role in regulating tumour progression by a number of different mechanisms, in particular the remodelling of collagen fibres in tumour-associated stroma, which has been reported to be related to patient survival. The underlying motivation of this work is that remodelling of collagen fibres gives rise to observable patterns in Hematoxylin and Eosin (H&E) stained slides from clinical cases of invasive breast carcinoma that the pathologist can label as mature or immature stroma. The aim of this paper is to categorise and automatically classify stromal regions according to their maturity and show that this classification agrees with that of skilled observers, hence providing a repeatable and quantitative measure for prognostic studies. METHODS: We use multi-scale Basic Image Features (BIF) and Local Binary Patterns (LBP), in combination with a random decision trees classifier for classification of breast cancer stroma regions-ofinterest (ROI). RESULTS: We present results from a cohort of 55 patients with analysis of 169 ROI. Our multi-scale approach achieved a classification accuracy of 84%. CONCLUSION: This work demonstrates the ability of texture-based image analysis to differentiate breast cancer stroma maturity in clinically acquired H&E stained slides at least as well as skilled observers

    Hard X-ray lags in active galactic nuclei: Testing the distant reverberation hypothesis with NGC 6814

    We present an X-ray spectral and temporal analysis of the variable active galaxy NGC 6814, observed with Suzaku during November 2011. Remarkably, the X-ray spectrum shows no evidence for the soft excess commonly observed amongst other active galaxies, despite its relatively low level of obscuration, and is dominated across the whole Suzaku bandpass by the intrinsic powerlaw-like continuum. Despite this, we clearly detect the presence of a low frequency hard lag of ~1600s between the 0.5-2.0 and 2.0-5.0 keV energy bands at greater than 6-sigma significance, similar to those reported in the literature for a variety of other AGN. At these energies, any additional emission from e.g. a very weak, undetected soft excess, or from distant reflection must contribute less than 3% of the observed countrates (at 90% confidence). Given the lack of any significant continuum emission component other than the powerlaw, we can rule out models that invoke distant reprocessing for the observed lag behavior, which must instead be associated with this continuum emission. These results are fully consistent with a propagating fluctuation origin for the low frequency hard lags, and with the interpretation of the high frequency soft lags - a common feature seen in the highest quality AGN data with strong soft excesses - as reverberation from the inner accretion disk

    Symmetric Biomechanically Guided Prone-to-Supine Breast Image Registration

    Prone-to-supine breast image registration has potential application in the fields of surgical and radiotherapy planning, image guided interventions, and multi-modal cancer diagnosis, staging, and therapy response prediction. However, breast image registration of three dimensional images acquired in different patient positions is a challenging problem, due to large deformations induced to the soft breast tissue caused by the change in gravity loading. We present a symmetric, biomechanical simulation based registration framework which aligns the images in a central, virtually unloaded configuration. The breast tissue is modelled as a neo-Hookean material and gravity is considered as the main source of deformation in the original images. In addition to gravity, our framework successively applies image derived forces directly into the unloading simulation in place of a subsequent image registration step. This results in a biomechanically constrained deformation. Using a finite difference scheme avoids an explicit meshing step and enables simulations to be performed directly in the image space. The explicit time integration scheme allows the motion at the interface between chest and breast to be constrained along the chest wall. The feasibility and accuracy of the approach presented here was assessed by measuring the target registration error (TRE) using a numerical phantom with known ground truth deformations, nine clinical prone MRI and supine CT image pairs, one clinical prone-supine CT image pair and four prone-supine MRI image pairs. The registration reduced the mean TRE for the numerical phantom experiment from initially 19.3 to 0.9 mm and the combined mean TRE for all fourteen clinical data sets from 69.7 to 5.6 mm

    Inattentive Consumers in Markets for Services

    In an experiment on markets for services, we find that consumers are likely to stick to default tariffs and achieve suboptimal outcomes. We find that inattention to the task of choosing a better tariff is likely to be a substantial problem in addition to any task and tariff complexity effect. The institutional setup on which we primarily model our experiment is the UK electricity and gas markets, and our conclusion is that the new measures by the UK regulator Ofgem to improve consumer outcomes are likely to be of limited impact

    Consensus Recommendations of the Multiple Sclerosis Study Group and the Portuguese Neuroradiological Society for the Use of Magnetic Resonance Imaging in Multiple Sclerosis in Clinical Practice: Part 2

    INTRODUCTION: Magnetic resonance imaging is recognized as the most important diagnostic test in the diagnosis of multiple sclerosis, differential diagnosis and evaluation of progression/therapeutic response. However, to make optimal use of magnetic resonance imaging in multiple sclerosis, the use of a standard, reproducible and comparable imaging protocol is of uttermost importance. In this context, the Portuguese Society of Neuroradiology and the Group of Studies of Multiple Sclerosis, after a joint discussion, appointed a committee of experts to create recommendations adapted to the national reality on the use of magnetic resonance imaging in multiple sclerosis. This document represents the second part of the first Portuguese consensus recommendations on the use of magnetic resonance imaging in multiple sclerosis in clinical practice. MATERIAL AND METHODS: The Portuguese Society of Neuroradiology and the Group of Studies of Multiple Sclerosis, after discussing the topic in national meetings and after a working group meeting held in Figueira da Foz, May 2017, appointed a committee of experts that have developed several standard protocols on the use of magnetic resonance imaging on multiple sclerosis by consensus. The document obtained was based on the best scientific evidence and expert opinion. Portuguese multiple sclerosis consultants and departments of neuroradiology scrutinized and reviewed the consensus paper; comments and suggestions were considered. Standardized strategies of magnetic resonance imaging referral in clinical practice for diagnosis and follow-up of multiple sclerosis were published in the first part of this paper. RESULTS: We provide magnetic resonance imaging acquisition protocols regarding multiple sclerosis diagnostic and monitoring and the information to be included in the report for application across Portuguese healthcare institutions. CONCLUSION: We hope that these first Portuguese magnetic resonance imaging guidelines will contribute to optimize multiple sclerosis management and improve patient care in Portugal.info:eu-repo/semantics/publishedVersio

    Heart rate changes during partial seizures: A study amongst Singaporean patients

    INTRODUCTION: Studies in Europe and America showed that tachycardia, less often bradycardia, frequently accompanied partial seizures in Caucasian patients. We determine frequency, magnitude and type of ictal heart rate changes during partial seizures in non-Caucasian patients in Singapore. METHODS: Partial seizures recorded during routine EEGs performed in a tertiary hospital between 1995 and 1999 were retrospectively reviewed. All routine EEGs had simultaneous ECG recording. Heart rate before and during seizures was determined and correlated with epileptogenic focus. Differences in heart rate before and during seizures were grouped into 4 types: (1) >10% decrease; (2) -10 to +20% change; (3) 20–50% increase; (3) >50% increase. RESULTS: Of the total of 37 partial seizures, 18 were left hemisphere (LH), 13 were right hemisphere (RH) and 6 were bilateral (BL) in onset. 51% of all seizures showed no significant change in heart rate (type 2), 22% had moderate sinus tachycardia (type 3), 11% showed severe sinus tachycardia (type 4), while 16% had sinus bradycardia (type 1). Asystole was recorded in one seizure. Apart from having more tachycardia in bilateral onset seizures, there was no correlation between side of ictal discharge and heart rate response. Compared to Caucasian patients, sinus tachycardia was considerably less frequent. Frequency of bradycardia was similar to those recorded in the literature. CONCLUSIONS: Significant heart rate changes during partial seizures were seen in half of Singaporean patients. Although sinus tachycardia was the most common heart rate change, the frequency was considerably lower compared to Caucasian patients. This might be due to methodological and ethnic differences.     An accurate test for homogeneity of odds ratios based on Cochran's Q-statistic

    Background: A frequently used statistic for testing homogeneity in a meta-analysis of K independent studies is Cochran's Q. For a standard test of homogeneity the Q statistic is referred to a chi-square distribution with K - 1 degrees of freedom. For the situation in which the effects of the studies are logarithms of odds ratios, the chi-square distribution is much too conservative for moderate size studies, although it may be asymptotically correct as the individual studies become large. Methods: Using a mixture of theoretical results and simulations, we provide formulas to estimate the shape and scale parameters of a gamma distribution to t the distribution of Q. Results: Simulation studies show that the gamma distribution is a good approximation to the distribution for Q. Conclusions: : Use of the gamma distribution instead of the chi-square distribution for Q should eliminate inaccurate inferences in assessing homogeneity in a meta-analysis. (A computer program for implementing this test is provided.) This hypothesis test is competitive with the Breslow-Day test both in accuracy of level and in power
