    Analyzing Sex Differences and the Dose-Response Relationship Between Aerobic Exercise and Cognitive Processing Speed in Young Active Adults

    Available research has identified a positive relationship between 10 minutes of aerobic exercise and improvements in cognitive processing speed (CPS) in young adults, although participant activity level was unclear Additionally, research indicates possible sex differences concerning exercise and CPS, defined as the rate in which human beings take in information and generate a response. PURPOSE: To investigate the potential effects of aerobic exercise bout length on cognitive processing speed in active adults. A secondary aim was to explore differences in CPS and aerobic exercise bout length between sexes. METHODS: Male (n=6) and female (n=6) participants who were classified as physically active based on ACSM guidelines participated in aerobic exercise sessions of different bout lengths (15, 20, and 25 minutes) in a balanced cross-over design. When participants arrived for the three testing trials, they first completed a computerized Symbol Search test. This matching test lasts two minutes and provides a score based on how many matches they answer correctly. Next, the exercise treatment consisted of a 5-minute warm-up, followed by a moderate intensity walk or jog on the treadmill (approximately 50-59% of Heart Rate reserve), and ending with a 5-minute cooldown. Participants then remained seated for ten minutes to allow for their heart rate to return to a resting state. Once in the resting state participants were administered the symbol search test again to determine if there were any changes in CPS following an exercise bout. All treatment sessions were performed at least 24 hours apart. Exercise bout length (T15, T20, T25) and time (pre-/post- exercise) were compared between sexes (M, F) using an ANOVA (1 between, 2 within) α=0.05. RESULTS: The main effects for bout length (p=0.849) and sex (p=0.232), bout length x sex interaction (p=0.563), bout length x time interaction (p=0.491), and bout length x time x sex interaction (p=0.956) were not significant. However, the main effect for time was significant (p=0.0001) where CPS was faster post-exercise (50+9) than pre-exercise (45+9) when pooled across bout length and sex. Also, there was a significant time x sex interaction (p=0.009) where, when pooled across bout length, there was greater CPS improvement from pre- to post-exercise in the males (Pre 46+8, Post 53+8) than in the females (Pre 43+11, Post 46+9). CONCLUSION: Active individuals experience improvements in CPS following an exercise bout. We did not find any significant distinction between bout lengths, indicating that active individuals do not require a specific exercise dose time to elicit improvements in CPS. However, male participants had a statistically significant increase in their processing speed assessment (pre-/post- exercise) compared to the females. This suggests that females may require additional exercise bouts or alternate exercise forms to experience similar improvements as the male participants

    Teamwork training in sport:A pilot intervention study

    The purpose of this study was to test the efficacy of a novel team building intervention that targets teamwork in sport. Using a 10-week pilot nonrandomized controlled intervention design, 12 interdependent sports teams comprising 187 athletes were assigned to one of two conditions: an experimental condition in which teams participated in two teamwork training sessions at Weeks 2 and 6 of the study (6 teams, 94 athletes) or a no-training control condition (6 teams, 93 athletes). Teamwork was measured at Weeks 1, 5, and 10 of the study. Overall, significant improvements in teamwork were shown for the experimental teams from baseline to Week 5; these effects were maintained through Week 10 of the study. In contrast, no significant changes in teamwork were observed for teams in the control condition over these 10 weeks. The results provide evidence that teamwork training can enhance the extent to which members of a sports team work effectively together

    Failure to Preserve β-Cell Function With Mycophenolate Mofetil and Daclizumab Combined Therapy in Patients With New- Onset Type 1 Diabetes

    OBJECTIVE This trial tested whether mycophenolate mofetil (MMF) alone or with daclizumab (DZB) could arrest the loss of insulin-producing β-cells in subjects with new-onset type 1 diabetes. RESEARCH DESIGN AND METHODS A multi-center, randomized, placebo-controlled, double-masked trial was initiated by Type 1 Diabetes TrialNet at 13 sites in North America and Europe. Subjects diagnosed with type 1 diabetes and with sufficient C-peptide within 3 months of diagnosis were randomized to either MMF alone, MMF plus DZB, or placebo, and then followed for 2 years. The primary outcome was the geometric mean area under the curve (AUC) C-peptide from the 2-h mixed meal tolerance test. RESULTS One hundred and twenty-six subjects were randomized and treated during the trial. The geometric mean C-peptide AUC at 2 years was unaffected by MMF alone or MMF plus DZB versus placebo. Adverse events were more frequent in the active therapy groups relative to the control group, but not significantly. CONCLUSIONS Neither MMF alone nor MMF in combination with DZB had an effect on the loss of C-peptide in subjects with new-onset type 1 diabetes. Higher doses or more targeted immunotherapies may be needed to affect the autoimmune process

    Gastric aspiration and its role in airway inflammation

    Gastro-Oesophageal Reflux (GOR) has been associated with chronic airway diseases while the passage of foreign matter into airways and lungs through aspiration has the potential to initiate a wide spectrum of pulmonary disorders. The clinical syndrome resulting from such aspiration will depend both on the quantity and nature of the aspirate as well as the individual host response. Aspiration of gastric fluids may cause damage to airway epithelium, not only because acidity is toxic to bronchial epithelial cells but also due to the effect of digestive enzymes such as pepsin and bile salts. Experimental models have shown that direct instillation of these factors to airways epithelia cause damage with a consequential inflammatory response. The pathophysiology of these responses is gradually being dissected, with better understanding of acute gastric aspiration injury, a major cause of acute lung injury, providing opportunities for therapeutic intervention and potentially, ultimately, improved understanding of the chronic airway response to aspiration. Ultimately, clarification of the inflammatory pathways which are related to micro-aspiration via pepsin and bile acid salts may eventually progress to pharmacological intervention and surgical studies to assess the clinical benefits of such therapies in driving symptom improvement or reducing disease progression

    Radiation Therapy for Esophageal Cancer

    Esophageal cancer develops in the mucosa of the esophagus and spreads toward the muscle layer. The nonsurgical treatment for localized, deeply invasive esophageal cancer has been external beam radiation therapy (EBRT) and concurrent chemotherapy. Recently, intraluminal brachytherapy showed a strong potential for the improvement of the therapeutic ratio. It was found that the fractionated high dose rate (HDR) brachytherapy offered beneficial palliation for a longer period of time with more durable symptom control. A similar was concluded for advanced unresectable esophageal cancer in previously irradiated patients. HDR brachytherapy may be a useful salvage treatment option for inoperable patients diagnosed with local esophageal cancer. Although better local control can be achieved with higher brachytherapy dose, this increases the risk of acute morbidity and late morbidity, especially in the setting of recurrence cancer. It was found that the moderate dose of EBRT and HDR brachytherapy could give a better local response than EBRT alone

    A corpus of images and text in online news

    In recent years, several datasets have been released that include images and text, giving impulse to new methods that combine natural language processing and computer vision. However, there is a need for datasets of images in their natural textual context. The ION corpus contains 300K news articles published between August 2014 - 2015 in five online newspapers from two countries. The 1-year coverage over multiple publishers ensures a broad scope in terms of topics, image quality and editorial viewpoints. The corpus consists of JSON-LD files with the following data about each article: the original URL of the article on the news publisher’s website, the date of publication, the headline of the article, the URL of the image displayed with the article (if any), and the caption of that image. Neither the article text nor the images themselves are included in the corpus. Instead, the images are distributed as high-dimensional feature vectors extracted from a Convolutional Neural Network, anticipating their use in computer vision tasks. The article text is represented as a list of automatically generated entity and topic annotations in the form of Wikipedia/DBpedia pages. This facilitates the selection of subsets of the corpus for separate analysis or evaluation

    Coagulation Profiles in Humans Exposed to Exertional Hypobaric Decompression Stress Determined by Calibrated Automated Thrombogram

Coagulation Profiles in Humans Abstract: The blood coagulation response to decompression stress in humans has yet to be fully investigated. Here we utilised calibrated automated thrombogram (CAT) on samples from healthy volunteers exposed to decompression stress to investigate real-time thrombin generation. To induce decompression stress, fifteen apparently healthy males (age 20-50 yr) were exposed to two consecutive ascents to 25,000 ft for 60 min (1st ascent) and then 90 min (2nd ascent) while breathing 100% oxygen. Citrated blood samples were taken prior to exposure (T0), following the 2nd ascent (T8) and at 24 h (T24). Thrombin generation curves were obtained using Thrombinoscope TM. Parameters determined were lag time (LAG), time to peak (TTP), peak thrombin (PEAK), endogenous thrombin potential (ETP) and velocity index (VEL). Of the 15 subjects, 12 had validated coagulation profiles. TTP and ETP showed no significant differences. However, there was a significant increase in VEL from T0 to T8 (p = 0.025) and from T8 to T24 (p = 0.043). A non-significant trend of an overall increase in PEAK was also observed from T0 to T8 (p = 0.069) and from T8 to T24 (p = 0.098). PEAK and VEL were found to be correlated. Taken together, these two parameters suggest an overall shift towards a more procoagulant profile following hypobaric stress

    An Interesting Case of Recurrent Small Bowel Obstruction

    Sclerosing mesenteritis is associated with a spectrum of diseases which include mesenteric lipodystrophy and mesenteric panniculitis. This inflammatory and fibrosing disorder can affect the small and large bowel wall and mesenteric vessels by exerting a mass effect. The following case highlights the difficulties with diagnosing and managing this unusual disease. A 64-year-old man presented with acute central abdominal pain, radiating to his back, and profuse vomiting. He was diagnosed clinically with small bowel obstruction. He had had an episode of small bowel obstruction 6 years earlier. At this time, he underwent an exploratory laparotomy, and a mass was identified in the small bowel mesentery. The features were thought to be in keeping with sclerosing mesenteritis. He had a dramatically favourable response to the initiation of prednisolone. He continued to be well and asymptomatic for a further 5 years on long-term maintenance low-dose steroids and 6-mercaptopurine. He re-presented in 2009 (six years after initial presentation) with very severe acute abdominal pain and vomiting. He had no recent change in weight or appetite, and had not had time off work. He underwent a second laparotomy and the tissue diagnosis was of metastatic carcinoid tumour involving the small bowel mesentery. This is the first case to our knowledge where sclerosing mesenteritis has been confirmed histologically on biopsy and then subsequently diagnosed with histologically proven carcinoid tumour. For this particular reason it must be always remembered that sclerosing mesenteritis is a ‘pathological’ and not a radiological diagnosis and that a large proportion of cases are associated with neoplasia
