    Health-related And Socio-demographic Factors Associated With Frailty In The Elderly: A Systematic Literature Review.

    Frailty is a syndrome that leads to practical harm in the lives of elders, since it is related to increased risk of dependency, falls, hospitalization, institutionalization, and death. The objective of this systematic review was to identify the socio-demographic, psycho-behavioral, health-related, nutritional, and lifestyle factors associated with frailty in the elderly. A total of 4,183 studies published from 2001 to 2013 were detected in the databases, and 182 complete articles were selected. After a comprehensive reading and application of selection criteria, 35 eligible articles remained for analysis. The main factors associated with frailty were: age, female gender, black race/color, schooling, income, cardiovascular diseases, number of comorbidities/diseases, functional incapacity, poor self-rated health, depressive symptoms, cognitive function, body mass index, smoking, and alcohol use. Knowledge of the complexity of determinants of frailty can assist the formulation of measures for prevention and early intervention, thereby contributing to better quality of life for the elderly.301143-6

    The role of earth observation in an integrated deprived area mapping “system” for low-to-middle income countries

    Urbanization in the global South has been accompanied by the proliferation of vast informal and marginalized urban areas that lack access to essential services and infrastructure. UN-Habitat estimates that close to a billion people currently live in these deprived and informal urban settlements, generally grouped under the term of urban slums. Two major knowledge gaps undermine the efforts to monitor progress towards the corresponding sustainable development goal (i.e., SDG 11—Sustainable Cities and Communities). First, the data available for cities worldwide is patchy and insufficient to differentiate between the diversity of urban areas with respect to their access to essential services and their specific infrastructure needs. Second, existing approaches used to map deprived areas (i.e., aggregated household data, Earth observation (EO), and community-driven data collection) are mostly siloed, and, individually, they often lack transferability and scalability and fail to include the opinions of different interest groups. In particular, EO-based-deprived area mapping approaches are mostly top-down, with very little attention given to ground information and interaction with urban communities and stakeholders. Existing top-down methods should be complemented with bottom-up approaches to produce routinely updated, accurate, and timely deprived area maps. In this review, we first assess the strengths and limitations of existing deprived area mapping methods. We then propose an Integrated Deprived Area Mapping System (IDeAMapS) framework that leverages the strengths of EO- and community-based approaches. The proposed framework offers a way forward to map deprived areas globally, routinely, and with maximum accuracy to support SDG 11 monitoring and the needs of different interest groups

    Decision aid for prostate cancer screening in Brazil

    OBJECTIVE To present the development and validation processes of a decision aid for prostate cancer screening in Brazil. METHODS Study with qualitative-participatory design for the elaboration of a decision aid for prostate cancer screening, with the participation of a group of men and physicians inserted in primary health care in 11 Brazilian states. Evidence synthesis, field testing, and use in clinical scenarios were performed to adapt the content, format, language, and applicability towards the needs of the target audience in the years 2018 and 2019. The versions were subsequently evaluated by the participants and modified based on the data obtained. RESULTS We elaborated an unprecedented tool in Brazil, with information about the tests used in the screening, comparison of their possible benefits and harms and a numerical infographic with the consequences of this practice. We verified the decision aid usability to assist in the communication between the doctor and the man in the context of primary health care, besides identifying the need for greater discussion about sharing decisions in clinical scenarios. CONCLUSION The tool was easy to use, objective, and has little interference in consultation time. It is a technical-scientific material, produced by research, with the participation of its main target audience and which is available free of charge for use in Brazilian clinical scenarios.OBJETIVO Apresentar o processo de desenvolvimento e validação de uma ferramenta de apoio à decisão para o rastreamento do câncer de próstata no Brasil. MÉTODOS Estudo com desenho qualitativo-participativo para elaboração de uma ferramenta de apoio à decisão para o rastreamento do câncer de próstata, com a participação de um grupo de homens e médicos inseridos na atenção primária à saúde de 11 estados brasileiros. Realizou-se síntese de evidências, teste de campo e utilização nos cenários clínicos, de modo a adaptar o conteúdo, formato, linguagem e a aplicabilidade às necessidades do público-alvo nos anos de 2018 e 2019. As versões foram avaliadas de forma subsequente pelos participantes, sendo modificada a partir dos dados obtidos. RESULTADOS Foi elaborada uma ferramenta inédita no Brasil, com informações sobre os exames utilizados no rastreamento, comparação dos seus possíveis benefícios e malefícios e um infográfico numérico com as consequências dessa prática. Verificou-se utilidade da ferramenta para auxiliar na comunicação entre o médico e o homem no contexto da atenção primária à saúde, além de identificar a necessidade de maior discussão sobre o compartilhamento das decisões nos cenários clínicos. CONCLUSÃO A ferramenta foi avaliada como de fácil utilização, objetiva e com pouca interferência no tempo de consulta. É um material técnico-científico, produzido por meio de pesquisa, com a participação do seu principal público-alvo e que se encontra disponível gratuitamente para utilização nos cenários clínicos do Brasil

    Effectiveness of daily and weekly iron supplementation in the prevention of anemia in infants

    OBJETIVO: Avaliar a efetividade da suplementação universal profilática com sulfato ferroso, em administração diária ou semanal, na prevenção da anemia em lactentes. MÉTODOS: Ensaio de campo randomizado com crianças de seis a 12 meses de idade, atendidas em unidades básicas de saúde do município do Rio de Janeiro, em 2004-2005. Foram formadas três coortes concorrentes com suplementação universal com sulfato ferroso com grupos: diário (n=150; 12,5mgFe/dia), semanal (n=147; 25mgFe/semana) e controle (n=94). A intervenção durou 24 semanas e foi acompanhada por ações educativas promotoras de adesão. A concentração de hemoglobina sérica foi analisada segundo sua distribuição, média e prevalência de anemia (Hb<110,0g/L) aos 12 meses de idade. A avaliação da efetividade foi realizada segundo intenção de tratar e adesão ao protocolo, utilizando-se análises de regressão múltipla (linear e de Poisson). RESULTADOS: Os grupos mostraram-se homogêneos quanto às variáveis de caracterização. A intervenção foi operacionalizada com sucesso, com elevada adesão ao protocolo em ambos os grupos expostos a ela, sem diferença estatística entre eles. Após ajuste, somente o esquema diário apresentou efeito protetor. Na análise por adesão, o esquema diário apresentou evidente efeito dose-resposta para média de hemoglobina sérica e prevalência de anemia, não sendo observado nenhum efeito protetor do esquema semanal. CONCLUSÕES: Apenas o esquema diário de suplementação universal com sulfato ferroso dos seis aos 12 meses de idade foi efetivo em aumentar a concentração de hemoglobina sérica e em reduzir o risco de anemiaOBJECTIVE: To evaluate the effectiveness of universal prophylactic targeting with iron sulfate on daily or weekly basis in the prevention of anemia in infants. METHODS: Randomized clinical field trial with children between ages six and 12 months seen at primary health care units in the municipality of Rio de Janeiro, Brazil, between 2004 and 2005. Three concurrent cohorts were compared: daily group (n=150; 12.5mg Fe/day); weekly group (n=147; 25mg Fe/week) and control group. The intervention consisted of universal supplementation with iron sulfate for 24 weeks, combined with educational adherence-promoting measures. Outcome: mean serum hemoglobin concentration, distribution and prevalence of anemia (Hb<110.0 g/l) at age 12 months. Effectiveness was evaluated considering both intent to treat and adherence to protocol, using multiple regression analysis (linear and Poisson). RESULTS: Groups were homogeneous in terms of descriptive variables. The intervention was implemented successfully, with high adhesion to protocol in both groups, and no statistical difference between them. After adjustment, only the daily regimen showed a protective effect. Adherence analysis demonstrated an evident dose-response effect on mean Hb and prevalence of anemia only for the daily regimen. No protective effect was detected for the weekly regimen. CONCLUSIONS: Universal supplementation with iron sulfate from six to 12 months of age was effective in increasing serum Hb and decreasing risk of anemia only when administered on a daily basi

    Development of high-gain gaseous photomultipliers for the visible spectral range

    We summarize the development of visible-sensitive gaseous photomultipliers, combining a semitransparent bi-alkali photocathode with a state-of-the-art cascaded electron multiplier. The latter has high photoelectron collection efficiency and a record ion blocking capability. We describe in details the system and methods of photocathode production and characterization, their coupling with the electron multiplier and the gaseous-photomultiplier operation and characterization in a continuous mode. We present results on the properties of laboratory-produced K2_2CsSb, Cs3_3Sb and Na2_2KSb photocathodes and report on their stability and QE in gas; K2_2CsSb photocathodes yielded QE values in Ar/CH4_4(95/5) above 30% at wavelengths of 360-400 nm. The novel gaseous photomultiplier yielded stable operation at gains of 105^5, in continuous operation mode, in 700 Torr of this gas; its sensitivity to single photons was demonstrated. Other properties are described. The successful detection of visible light with this gas-photomultiplier pave ways towards further development of large-area sealed imaging detectors, of flat geometry, insensitive to magnetic fields, which might have significant impact on light detection in numerous fields.Comment: 22 pages, 12 figures, for submission to JINS

    The outflow in Mrk 509: A method to calibrate XMM-Newton EPIC-pn and RGS

    We have analyzed three XMM-Newton observations of the Seyfert 1 galaxy Mrk 509, with the goal to detect small variations in the ionized outflow properties. Such measurements are limited by the quality of the cross-calibration between RGS, the best instrument to characterize the spectrum, and EPIC-pn, the best instrument to characterize the variability. For all three observations we are able to improve the relative calibration of RGS and pn consistently to 4 %. In all observations we detect three different outflow components and, thanks to our accurate cross-calibration we are able to detect small differences in the ionization parameter and column density in the highest ionized component of the outflow. This constrains the location of this component of the outflow to within 0.5 pc of the central source. Our method for modeling the relative effective area is not restricted to just this source and can in principle be extended to other types of sources as well.Comment: 11 pages, 9 figure

    Tendência temporal da amamentação na cidade do Rio de Janeiro: 1996-2006

    OBJETIVO: Analisar a tendência temporal da prática de aleitamento materno (AM) e de aleitamento materno exclusivo (AME). MÉTODOS: Foram analisados dados de sistema de monitoramento baseado em inquéritos realizados nos anos de 1996, 1998, 2000, 2003 e 2006 durante a Campanha Nacional de Imunização na cidade do Rio de Janeiro, RJ. A população de estudo foi constituída de 19.044 crianças menores de um ano de idade que compareceram aos postos de vacinação. Para cada ano foi estudada uma amostra probabilística por conglomerado (postos de vacinação), auto-ponderada representativa da população de crianças menores de 12 meses (<12). Foi aplicado questionário estruturado com questões fechadas sobre alimentação da criança no momento do estudo e características sociodemográficas da mãe. Foram adotados os indicadores de AM e de AME propostos pela Organização Mundial da Saúde. RESULTADOS: O AM<12 aumentou de 61,3% para 73,4% entre 1996 e 2006. Tendência similar foi observada em todas as faixas etárias analisadas. O AME em crianças <4 e <6 meses (AME<6) aumentou, respectivamente, de 18,8% para 42,4% e de 13,8% para 33,3%. Melhorias em AM>6 e em AME<6 foram observadas em todas as categorias de todas as variáveis sociodemográficas maternas. Para AME<6, a desvantagem observada em 1996 entre mulheres de menor escolaridade e em 1998 entre mulheres que trabalhavam não foi completamente superada até 2006. CONCLUSÕES: O AM e o AME aumentaram no período estudado independentemente da faixa etária da criança e das características sociodemográficas maternas. Não foram totalmente superadas diferenças observadas entre mulheres em diferentes situações sociodemográficas
