Enabling self-directed computer use for individuals with cerebral palsy: a systematic review of available assistive devices and technologies
Aim  The purpose of this study was to systematically review published evidence on the development, use, and effectiveness of devices and technologies that enable or enhance self-directed computer access by individuals with cerebral palsy (CP).
Methods  Nine electronic databases were searched using keywords ‘computer’, ‘software’, ‘spastic’, ‘athetoid’, and ‘cerebral palsy’; the reference lists of articles thus identified were also searched. Thirty articles were selected for review, with 23 reports of development and usability testing of devices and seven evaluations of algorithms to increase computer recognition of input and cursor movements.
Results  Twenty-four studies had fewer than 10 participants with CP, with a wide age range of 5 to 77 years. Computer task performance was usually tested, but only three groups sought participant feedback on ease and comfort of use. International standards exist to evaluate effectiveness of non-keyboard devices, but only one group undertook this testing. None of the study designs were higher than American Academy for Cerebral Palsy and Developmental Medicine level IV.
Interpretation  Access solutions for individuals with CP are in the early stages of development. Future work should include assessment of end-user comfort, effort, and performance as well as design features. Engaging users and therapists when designing and evaluating technologies to enhance computer access may increase acceptance and improve performance
Effectiveness of robot-assisted therapy on ankle rehabilitation – a systematic review
Objective
The aim of this study was to provide a systematic review of studies that investigated the effectiveness of robot-assisted therapy on ankle motor and function recovery from musculoskeletal or neurologic ankle injuries.
Methods
Thirteen electronic databases of articles published from January, 1980 to June, 2012 were searched using keywords ‘ankle*’, ‘robot*’, ‘rehabilitat*’ or ‘treat*’ and a free search in Google Scholar based on effects of ankle rehabilitation robots was also conducted. References listed in relevant publications were further screened. Eventually, twenty-nine articles were selected for review and they focused on effects of robot-assisted ankle rehabilitation.
Results
Twenty-nine studies met the inclusion criteria and a total of 164 patients and 24 healthy subjects participated in these trials. Ankle performance and gait function were the main outcome measures used to assess the therapeutic effects of robot-assisted ankle rehabilitation. The protocols and therapy treatments were varied, which made comparison among different studies difficult or impossible. Few comparative trials were conducted among different devices or control strategies. Moreover, the majority of study designs met levels of evidence that were no higher than American Academy for Cerebral Palsy (CP) and Developmental Medicine (AACPDM) level IV. Only one study used a Randomized Control Trial (RCT) approach with the evidence level being II.
Conclusion
All the selected studies showed improvements in terms of ankle performance or gait function after a period of robot-assisted ankle rehabilitation training. The most effective robot-assisted intervention cannot be determined due to the lack of universal evaluation criteria for various devices and control strategies. Future research into the effects of robot-assisted ankle rehabilitation should be carried out based on universal evaluation criteria, which could determine the most effective method of intervention. It is also essential to conduct trials to analyse the differences among different devices or control strategies
Model based open-loop posture control of a parallel ankle assessment and rehabilitation robot
Ankle injuries are very common in daily life out of musculoskeletal or neurological reasons. Traditional ankle therapy usually requires cooperative and intensive efforts from therapists and patients. Robot-assisted ankle rehabilitation techniques have been actively researched in the past few decades. However, limitations exist such as inappropriate robot design, limited range of motion (ROM) and torque generation capability, or lack of measurement of the interaction between robots and patients. This paper proposes a novel ankle assessment and rehabilitation robot (AARR) that could perform a three-dimensional robotic training with real-time ankle assessment. This study focuses on the control of the robot for ankle rehabilitation rather than assessment. A preliminary test on a healthy subject was conducted using the AARR with a model based open-loop controller. Results show that the robotic training is continuous although the trajectory tracking accuracy is not high. It is concluded that the robotic design has potential for ankle rehabilitation. This model based open-loop controller provides continuous robotic training and is suitable for low precision tracking requirement. It can also serve advanced control schemes as the forward feedback

Designing theoretically-informed implementation interventions
Clinical and health services research is continually producing new findings that may contribute to effective and efficient patient care. However, the transfer of research findings into practice is unpredictable and can be a slow and haphazard process. Ideally, the choice of implementation strategies would be based upon evidence from randomised controlled trials or systematic reviews of a given implementation strategy. Unfortunately, reviews of implementation strategies consistently report effectiveness some, but not all of the time; possible causes of this variation are seldom reported or measured by the investigators in the original studies. Thus, any attempts to extrapolate from study settings to the real world are hampered by a lack of understanding of the effects of key elements of individuals, interventions, and the settings in which they were trialled. The explicit use of theory offers a way of addressing these issues and has a number of advantages, such as providing: a generalisable framework within which to represent the dimensions that implementation studies address, a process by which to inform the development and delivery of interventions, a guide when evaluating, and a way to allow for an exploration of potential causal mechanisms. However, the use of theory in designing implementation interventions is methodologically challenging for a number of reasons, including choosing between theories and faithfully translating theoretical constructs into interventions. The explicit use of theory offers potential advantages in terms of facilitating a better understanding of the generalisability and replicability of implementation interventions. Serotonin tranporter methylation and response to cognitive behaviour therapy in children with anxiety disorders
Anxiety disorders that are the most commonly occurring psychiatric disorders in childhood, are associated with a range of social and educational impairments and often continue into adulthood. Cognitive behaviour therapy (CBT) is an effective treatment option for the majority of cases, although up to 35-45% of children do not achieve remission. Recent research suggests that some genetic variants may be associated with a more beneficial response to psychological therapy. Epigenetic mechanisms such as DNA methylation work at the interface between genetic and environmental influences. Furthermore, epigenetic alterations at the serotonin transporter (SERT) promoter region have been associated with environmental influences such as stressful life experiences. In this study, we measured DNA methylation upstream of SERT in 116 children with an anxiety disorder, before and after receiving CBT. Change during treatment in percentage DNA methylation was significantly different in treatment responders vs nonresponders. This effect was driven by one CpG site in particular, at which responders increased in methylation, whereas nonresponders showed a decrease in DNA methylation. This is the first study to demonstrate differences in SERT methylation change in association with response to a purely psychological therapy. These findings confirm that biological changes occur alongside changes in symptomatology following a psychological therapy such as CBT
Continuity, Deconfinement, and (Super) Yang-Mills Theory
We study the phase diagram of SU(2) Yang-Mills theory with one adjoint Weyl
fermion on R^3xS^1 as a function of the fermion mass m and the compactification
scale L. This theory reduces to thermal pure gauge theory as m->infinity and to
circle-compactified (non-thermal) supersymmetric gluodynamics in the limit
m->0. In the m-L plane, there is a line of center symmetry changing phase
transitions. In the limit m->infinity, this transition takes place at
L_c=1/T_c, where T_c is the critical temperature of the deconfinement
transition in pure Yang-Mills theory. We show that near m=0, the critical
compactification scale L_c can be computed using semi-classical methods and
that the transition is of second order. This suggests that the deconfining
phase transition in pure Yang-Mills theory is continuously connected to a
transition that can be studied at weak coupling. The center symmetry changing
phase transition arises from the competition of perturbative contributions and
monopole-instantons that destabilize the center, and topological molecules
(neutral bions) that stabilize the center. The contribution of molecules can be
computed using supersymmetry in the limit m=0, and via the
Bogomolnyi--Zinn-Justin (BZJ) prescription in the non-supersymmetric gauge
theory. Finally, we also give a detailed discussion of an issue that has not
received proper attention in the context of N=1 theories---the non-cancellation
of nonzero-mode determinants around supersymmetric BPS and KK
monopole-instanton backgrounds on R^3xS^1. We explain why the non-cancellation
is required for consistency with holomorphy and supersymmetry and perform an
explicit calculation of the one-loop determinant ratio.Comment: A discussion of the non-cancellation of the nonzero mode determinants

Seeing Tree Structure from Vibration
Humans recognize object structure from both their appearance and motion;
often, motion helps to resolve ambiguities in object structure that arise when
we observe object appearance only. There are particular scenarios, however,
where neither appearance nor spatial-temporal motion signals are informative:
occluding twigs may look connected and have almost identical movements, though
they belong to different, possibly disconnected branches. We propose to tackle
this problem through spectrum analysis of motion signals, because vibrations of
disconnected branches, though visually similar, often have distinctive natural
frequencies. We propose a novel formulation of tree structure based on a
physics-based link model, and validate its effectiveness by theoretical
analysis, numerical simulation, and empirical experiments. With this
formulation, we use nonparametric Bayesian inference to reconstruct tree
structure from both spectral vibration signals and appearance cues. Our model
performs well in recognizing hierarchical tree structure from real-world videos

Supersymmetric QCD: Exact Results and Strong Coupling
We revisit two longstanding puzzles in supersymmetric gauge theories. The
first concerns the question of the holomorphy of the coupling, and related to
this the possible definition of an exact (NSVZ) beta function. The second
concerns instantons in pure gluodynamics, which appear to give sensible, exact
results for certain correlation functions, which nonetheless differ from those
obtained using systematic weak coupling expansions. For the first question, we
extend an earlier proposal of Arkani-Hamed and Murayama, showing that if their
regulated action is written suitably, the holomorphy of the couplings is
manifest, and it is easy to determine the renormalization scheme for which the
NSVZ formula holds. This scheme, however, is seen to be one of an infinite
class of schemes, each leading to an exact beta function; the NSVZ scheme,
while simple, is not selected by any compelling physical consideration. For the
second question, we explain why the instanton computation in the pure
supersymmetric gauge theory is not reliable, even at short distances. The
semiclassical expansion about the instanton is purely formal; if infrared
divergences appear, they spoil arguments based on holomorphy. We demonstrate
that infrared divergences do not occur in the perturbation expansion about the
instanton, but explain that there is no reason to think this captures all
contributions from the sector with unit topological charge. That one expects
additional contributions is illustrated by dilute gas corrections. These are
infrared divergent, and so difficult to define, but if non-zero give order one,
holomorphic, corrections to the leading result. Exploiting an earlier analysis
of Davies et al, we demonstrate that in the theory compactified on a circle of
radius beta, due to infrared effects, finite contributions indeed arise which
