    Paclitaxel, carboplatin and gemcitabine combination as induction chemotherapy for stage IIIA N2 bulky non-small cell lung cancer

    Background: Induct ion chemotherapy followed by surgical resection or definitive radiotherapy for patients affected by stage IIIA N2 bulky non-small cell lung cancer (NSCLC) has been investigated in several trials. Patients and Methods: In this present study, 52 patients with stage IIIA N2 bulky NSCLC with cytologically or histologically confirmed mediastinal lymph node involvement received paclitaxel 175 mg/mq on day 1, carboplatin AUC 5 on day 1 and gemcitabine 1,000 mg/mq on day 1 and 8 every 3 weeks for three cycles as induction chemotherapy. Results: Objective response (4 complete remission and 36 partial remission) was achieved in 40/52 patients. No early or toxic deaths were observed. Twenty-two patients were surgically explored. Fifteen were excluded for resection for biopsy-proven residual tumour in mediastinal nodes. Complete surgical resection was performed in 15 patients with confirmed pathological downstaging. Pathological complete response was achieved in 4 patients. No surgery-related mortality or significant morbidity was reported. Adjuvant radiotherapy was delivered in 15 patients, and 30 patients received definitive radiotherapy. Conclusion: In the present study, the combination of paclitaxel, carboplatin and gemcitabine has been a safe and active regimen in poor-prognosis stage IIIA N2 bulky NSCLC

    Production of intense highly charged ion beams with SERSE

    The source SERSE is operational at LNS since June 1998 and many improvements have been carried out in this period. The frequency has been increased from 14.5 GHz to 18 GHz and the use of two frequency heating has given positive results. Metallic ion production has been tested by means of a high temperature oven and the preliminary results are described. Tests of magnetic field scaling and frequency scaling have confirmed the results of previous tests with SC-ECRIS at lower frequency and seems to suggest that the upgrading of the source to higher frequency may be considered

    New food composition data on selected ethnic foods consumed in Europe

    Background: Reliable data on the composition of foods is needed to better understand individual diets, measure nutrient intakes and provide nutritional guidance for improving the health of the populations. Ethnic foods are becoming increasingly popular among all European consumers, and are the main source of nutrients in the diets of ethnic groups. However, there is limited information on the nutrient composition of ethnic foods in Europe. The objective of this study therefore was to generate new and reliable data on ethnic foods using harmonised methods for chemical analyses. Methods: New data on 128 ethnic foods were generated for inclusion in the national databases within the European Food Information Resource Network of Excellence through participants from France, Israel, Spain, Denmark, Italy, The Netherlands, Belgium and the United Kingdom. In each selected country, the list of prioritised foods and key nutrients, methods of analyses and quality assurance procedure were harmonised. Results: This paper presents the nutrient composition of 40 ethnic foods consumed in Europe. The nutrient composition of the foods varied widely because of the nature and variety of foods analysed, with energy content (kcal) ranging between 24 (biteku-teku, Blegium) and 495 (nachos, Italy) per 100 g of edible food. Polyunsaturated and monounsaturated fatty acids were generally higher in most ethnic foods consumed in Italy and Spain compared with ethnic foods of other countries. Conclusions: The new data were scrutinised and fully documented for inclusion in the national food composition databases. The data will aid effective diet and disease interventions, and enhance the provision of dietary advice, in all European consumers

    Application of Ferroelectric Cathodes to Enhance the Ion Yield in the Caesar Source at LNS

    With increasing RF power the electron concentration in the plasma of ECR ion sources is decreasing in comparison to the ion concentration, so that the plasma is charging up positively. Direct injection of electrons into the ECR plasma can increase the electron charge density and the ion current yield. We have used ferroelectric cathodes to inject electrons into the Argon plasma of the CAESAR ion source at INFN-LNS (Catania, Italy). The cathode was placed at about 10 cm from the hot plasma and a bipolar high voltage pulse of 1.6 kV was used to trigger the electron emission. No additional acceleration has been provided. The use of the ferroelectric cathode leads to an increase of about 30% of the Ar8+ intensity, which has been monitored during the test. In addition, magneto-hydrodynamic instabilities in the ECR source were damped during and after electron injection

    A phase II study of induction chemotherapy followed by concurrent chemoradiotherapy in elderly patients with locally advanced non-small-cell lung cancer.

    The optimal management of unresectable locally advanced non-small-cell lung cancer in older patients has not been defined to date. The present phase II study was planned to evaluate the activity and safety of platinum-based induction chemotherapy followed by concurrent chemoradiotherapy in elderly patients with locally advanced non-small-cell lung cancer. Patients received two cycles of paclitaxel (175 mg/m) and carboplatin (area under the curve: 5) day 1, every 3 weeks. Chemoradiotherapy (thoracic radiation therapy) was initiated on day 42 and consisted of 1.8 Gy daily, five times per week over 5 weeks (45.0 Gy target dose) followed by 10 2.0 Gy daily fractions. Concomitant chemotherapy was weekly paclitaxel 50 mg/mq followed by weekly carboplatin at an area under the curve of 2. The eligibility for patients: age 70 or older and histologically documented untreated non-small-cell lung cancer, locally advanced, unresectable, stage III A N2 bulky or III B. Thirty consecutive patients were enrolled onto the study. The median age was 73 (range 70-76). According to the intention-to-treat analysis, 1 month after the end of combined chemoradiotherapy, we observed complete and partial responses in one and 19 of the 30 patients, respectively, for an overall response rate of 66% (95% confidence interval, 45-76%). Median progression-free survival was 8.7 months (95% confidence interval, 3.4-37.8) and median survival was 15 months (95% confidence interval, 4.2-52.1). During the treatment, 12 patients (40.0%) experienced grade 3-4 neutropenia, two patients neutropenic fever, and three patients grade 3 anaemia and grade 3 thrombocytopenia, respectively. Grade 3 oesophagitis, during concomitant radiotherapy, was observed in six patients (20.0%). No treatment-related mortality was reported. The investigated sequential approach including induction chemotherapy followed by concurrent chemoradiotherapy appears safe and seems a reasonable chance for the treatment of locally advanced non-small-cell lung cancer in the elderly population

    Aspects of Dante’s theology of redemption: Eden, the Fall, and Christ in Dante with respect to Augustine

    My thesis offers an account of salvation theology in Augustine and Dante under three main aspects: prelapsarianism, the fall, and the redemptive work of God in Christ. Resting on an analysis of the precise doctrinal position in these authors, the thesis is historical in conception, but is arranged in such a way as to allow the patterns of thought advanced by Augustine and Dante to enter into a dialogue one with the other, its overall purpose, therefore, being a species of conversation transcending the historical pure and simple. In keeping with this, the thesis is in three chapters, the first chapter exploring the notion of man’s original righteousness in Augustine and Dante, the second their respective senses of the fall in its essential substance and meaning, and the third their understanding of the redemptive work of the Christ. More precisely, the first chapter compares and contrasts Augustine’s sense of how it is that man stands in need of grace for the purposes of good works even prior to the fall with Dante’s sense of his direct creation in the image of God and of the implications of this for his persisting in good works without God’s further assistance. The second chapter addresses the origins of sin, and, more particularly, compares Augustine’s sense of evil as a matter of privation with Dante’s account of it in terms of dysfunctionality on the plane of properly human loving. In Chapter Three I take up the question of the relationship between nature and grace, and, in consequence of the fall, the indispensability of the latter as that whereby man is brought home once again to God. But where in Augustine (and especially in the later Augustine) it is always a question of nature as moved by grace to its proper good, I argue that for Dante grace enters into nature for the purposes of empowering it from within itself to its proper righteousness and likeness to God. Basing my argument on a strict reading of the text, and taking care in the introduction to identify the main historical and contemporary approaches to the question of Dante and Augustine (and thus to preserve at every stage a properly scholarly perspective), I nonetheless aim in my thesis to recreate in a manner over and beyond the purely historical something of the dialogue which is taking place here, a dialogue at every point informed, for all its distribution and re-distribution of emphases, by a common existential intensity, a shared preoccupation with what it might mean for man to be both for self and for God

    BOVITA: a first overview on genome-wide genetic diversity of Italian autochthonous cattle breeds

    Analysis of genomic data is increasingly becoming part of the livestock industry and is an invaluable resource for effective management of breeding programs in small populations. The recent availability of genome-wide SNP panels allows providing background information concerning genome structure in domestic animals, opening new perspectives to livestock genetics. BOVITA was established to join local efforts and resources for the genomic characterization of Italian local cattle breeds. Despite the growing diffusion of some cosmopolite specialized breeds, several autochthonous breeds are still bred in Italy. The main aim of the BOVITA is to investigate the genomic structure of Italian local cattle breeds, to provide information on their genetic status that will be useful for the management of the genetic variability, as a contribution to biodiversity conservation and prioritization actions. A total of about 800 animals (20-32 per breed) belonging to thirty Italian cattle breeds (Agerolese, Bar\ue0-Pustertaler, Burlina, Cabannina, Calvana, Chianina, Cinisara, Garfagnina, Italian Brown, Italian Holstein, Italian Simmental, Marchigiana, Maremmana, Modenese, Modicana, Mucca Pisana, Pezzata Rossa d\u2019Oropa, Piedmontese, Pinzgau, Podolica, Pontremolese, Pustertaler, Reggiana, Rendena, Romagnola, Rossa Siciliana, Sarda, Sardo-Bruna, Sardo-Modicana and Ottonese-Varzese) and two cosmopolitan breeds (Charolaise and Limousine) genotyped with the Illumina BovineSNP50 v2 BeadChip array were collected for the analysis. The genotypes of several breeds were detected in the frame of the project, whereas for some breeds these data are derived by previous studies. The dataset will be analyzed to: study several aspects of population genetic diversity, multi-dimensional scaling plot, population structure, linkage disequilibrium, and runs of homozygosity. In addition, comparative analysis of conserved haplotypes will be conducted to identify genomic segments under selection pressure. Such information also provides important insights into the mechanisms of evolution and is useful for the annotation of significant functional genomics regions. Data analysis will also be useful to select SNPs suitable for parentage test and breed genetic traceability. The analysis of the data will pinpoint the genetic distinctiveness of Italian breeds. Moreover, the obtained results contribute to a better characterization of history and genetic structure of Italian cattle breeds

    The Girgentana Goat Breed: A Zootechnical Overview on Genetics, Nutrition and Dairy Production Aspects

    In recent years, there has been a great interest in recovering and preserving local livestock breeds. An interesting situation is represented by the Girgentana goat, an ancient local breed reared in Sicily. Over recent years, this breed has become almost extinct, in part as a consequence of the marked decrease in fresh goat milk consumption. On the basis of these considerations, several studies on its genetic structure and management aspects have been conducted in order to protect the Girgentana goat from the risk of extinction and recover its genetic and economic value. In this context, information on genetics, nutrition and dairy production aspects may have a crucial role in the improvement and management of the breed. Thus, this chapter describes some points of these applications through recent investigations on this goat breed

    Impact of the treatment of periodontitis on systemic health and quality of life: A systematic review

    AIM: To investigate the effect of treatment of periodontitis on systemic health outcomes, pregnancy complications, and associated quality of life. MATERIALS AND METHODS: Systematic electronic searches were conducted to identify randomized controlled trials with minimum 6-month follow-up and reporting on the outcomes of interest. Qualitative and quantitative analyses were performed as deemed suitable. RESULTS: Meta-analyses confirmed reductions of high-sensitivity C-reactive protein (hs-CRP) [0.56 mg/L, 95% confidence interval (CI) (−0.88, −0.25), p < .001]; interleukin (IL)-6 [0.48 pg/ml, 95% CI (−0.88, −0.08), p = .020], and plasma glucose [1.33 mmol/l, 95% CI (−2.41, −0.24), p = .016], and increase of flow-mediated dilation (FMD) [0.31%, 95% CI (0.07, 0.55), p = .012] and diastolic blood pressure [0.29 mmHg, 95% CI (0.10, 0.49), p = .003] 6 months after the treatment of periodontitis. A significant effect on preterm deliveries (<37 weeks) was observed [0.77 risk ratio, 95% CI (0.60, 0.98), p = .036]. Limited evidence was reported on quality-of-life (QoL) outcomes in the included studies. CONCLUSIONS: Treatment of periodontitis results in systemic health improvements including improvement in cardiometabolic risk, reduction in systemic inflammation and the occurrence of preterm deliveries. Further research is however warranted to confirm whether these changes are sustained over time. Further, appropriate QoL outcomes should be included in the study designs of future clinical trials
