    Finiteness and Dual Variables for Lorentzian Spin Foam Models

    We describe here some new results concerning the Lorentzian Barrett-Crane model, a well-known spin foam formulation of quantum gravity. Generalizing an existing finiteness result, we provide a concise proof of finiteness of the partition function associated to all non-degenerate triangulations of 4-manifolds and for a class of degenerate triangulations not previously shown. This is accomplished by a suitable re-factoring and re-ordering of integration, through which a large set of variables can be eliminated. The resulting formulation can be interpreted as a ``dual variables'' model that uses hyperboloid variables associated to spin foam edges in place of representation variables associated to faces. We outline how this method may also be useful for numerical computations, which have so far proven to be very challenging for Lorentzian spin foam models.Comment: 15 pages, 1 figur

    Dual Computations of Non-abelian Yang-Mills on the Lattice

    In the past several decades there have been a number of proposals for computing with dual forms of non-abelian Yang-Mills theories on the lattice. Motivated by the gauge-invariant, geometric picture offered by dual models and successful applications of duality in the U(1) case, we revisit the question of whether it is practical to perform numerical computation using non-abelian dual models. Specifically, we consider three-dimensional SU(2) pure Yang-Mills as an accessible yet non-trivial case in which the gauge group is non-abelian. Using methods developed recently in the context of spin foam quantum gravity, we derive an algorithm for efficiently computing the dual amplitude and describe Metropolis moves for sampling the dual ensemble. We relate our algorithms to prior work in non-abelian dual computations of Hari Dass and his collaborators, addressing several problems that have been left open. We report results of spin expectation value computations over a range of lattice sizes and couplings that are in agreement with our conventional lattice computations.     Coupling of spacetime atoms and spin foam renormalisation from group field theory

    We study the issue of coupling among 4-simplices in the context of spin foam models obtained from a group field theory formalism. We construct a generalisation of the Barrett-Crane model in which an additional coupling between the normals to tetrahedra, as defined in different 4-simplices that share them, is present. This is realised through an extension of the usual field over the group manifold to a five argument one. We define a specific model in which this coupling is parametrised by an additional real parameter that allows to tune the degree of locality of the resulting model, interpolating between the usual Barrett-Crane model and a flat BF-type one. Moreover, we define a further extension of the group field theory formalism in which the coupling parameter enters as a new variable of the field, and the action presents derivative terms that lead to modified classical equations of motion. Finally, we discuss the issue of renormalisation of spin foam models, and how the new coupled model can be of help regarding this.Comment: RevTeX, 18 pages, no figure

    Predictors of outcome in sciatica patients following an epidural steroid injection:the POiSE prospective observational cohort study protocol

    INTRODUCTION: Sciatica can be very painful and, in most cases, is due to pressure on a spinal nerve root from a disc herniation with associated inflammation. For some patients, the pain persists, and one management option is a spinal epidural steroid injection (ESI). The aim of an ESI is to relieve leg pain, improve function and reduce the need for surgery. ESIs work well in some patients but not in others, but we cannot identify these patient subgroups currently. This study aims to identify factors, including patient characteristics, clinical examination and imaging findings, that help in predicting who does well and who does not after an ESI. The overall objective is to develop a prognostic model to support individualised patient and clinical decision-making regarding ESI. METHODS: POiSE is a prospective cohort study of 439 patients with sciatica referred by their clinician for an ESI. Participants will receive weekly text messages until 12 weeks following their ESIand then again at 24 weeks following their ESI to collect data on leg pain severity. Questionnaires will be sent to participants at baseline, 6, 12 and 24 weeks after their ESI to collect data on pain, disability, recovery and additional interventions. The prognosis for the cohort will be described. The primary outcome measure for the prognostic model is leg pain at 6 weeks. Prognostic models will also be developed for secondary outcomes of disability and recovery at 6 weeks and additional interventions at 24 weeks following ESI. Statistical analyses will include multivariable linear and logistic regression with mixed effects model. ETHICS AND DISSEMINATION: The POiSE study has received ethical approval (South Central Berkshire B Research Ethics Committee 21/SC/0257). Dissemination will be guided by our patient and public engagement group and will include scientific publications, conference presentations and social media.</p

    Patient Navigators Connecting Patients to Community Resources to Improve Diabetes Outcomes

    BACKGROUND: Despite the recognized importance of lifestyle modification in reducing risk of developing type 2 diabetes and in diabetes management, the use of available community resources by both patients and their primary care providers (PCPs) remains low. The patient navigator model, widely used in cancer care, may have the potential to link PCPs and community resources for reduction of risk and control of type 2 diabetes. In this study we tested the feasibility and acceptability of telephone-based nonprofessional patient navigation to promote linkages between the PCP office and community programs for patients with or at risk for diabetes. METHODS: This was a mixed-methods interventional prospective cohort study conducted between November 2012 and August 2013. We included adult patients with and at risk for type 2 diabetes from six primary care practices. Patient-level measures of glycemic control, diabetes care, and self-efficacy from medical records, and qualitative interview data on acceptability and feasibility, were used. RESULTS: A total of 179 patients participated in the study. Two patient navigators provided services over the phone, using motivational interviewing techniques. Patient navigators provided regular feedback to PCPs and followed up with the patients through phone calls. The patient navigators made 1028 calls, with an average of 6 calls per patient. At follow-up, reduction in HbA1c (7.8 ± 1.9% vs 7.2 ± 1.3%; P = .001) and improvement in patient self-efficacy (3.1 ± 0.8 vs 3.6 ± 0.7; P < .001) were observed. Qualitative analysis revealed uniformly positive feedback from providers and patients. CONCLUSIONS: The patient navigator model is a promising and acceptable strategy to link patient, PCP, and community resources for promoting lifestyle modification in people living with or at risk for type 2 diabetes
