    Fine needle aspiration biopsy in the oral cavity and head and neck region

    The objective of the current study was to evaluate the sensitivity, specificity and accuracy of fine needle aspiration biopsy (FNAB) of submucous nodules from the oral cavity and head and neck region as an auxiliary diagnostic tool. Fifty patients with nodule lesions in the oral cavity and the head and neck region were selected. All of them were submitted to FNAB and to either incisional or excisional biopsy. The diagnoses from the FNABs were compared with the biopsy diagnosis as the gold standard. All the cases of FNAB were analyzed by a single oral pathologist prior to the biopsy diagnosis. The results showed that the sensitivity of FNAB was 75%, its specificity was 96% and its accuracy was 58.8%. The false positive and false negative rates were 6.7% and 13.3%, respectively. The positive predictive value was 86% and the negative predictive value was 93%. The inconclusive rate was 16/50. FNAB displayed a high success rate for identifying both malignant and benign lesions, but a low accuracy for making a final diagnosis

    Vimentin expression and the influence of Matrigel in cell lines of head and neck squamous cell carcinoma

    Vimentin is a cytoeskeletal intermediate filament protein commonly observed in mesenchymal cells; however, it can also be found in malignant epithelial cells. It is demonstrated in several carcinomas, such as those of the cervix, breast and bladder, in which it is widely used as a marker of the epithelial to mesenchymal transition that takes place during embryogenesis and metastasis. Vimentin is associated with tumors that show a high degree of invasiveness, being detected in invasion front cells. Its expression seems to be influenced by the tumor microenvironment. The aim of this study was to evaluate vimentin expression in head and neck squamous cell carcinoma (HNSCC) cell lines, and to investigate the contribution of the microenvironment to its expression. HNSCC cell lines (HN6, HN30 and HN31) and an immortalized nontumorigenic cell line (HaCaT) were submitted to a three-dimensional assay with Matrigel. Cytoplasmatic staining of the HN6 cell line cultured without Matrigel and of the HN30 and HN31 cell lines cultured with Matrigel was demonstrated through immunohistochemistry. Western Blotting revealed a significant decrease in vimentin expression for the HN6 cell line and a significant increase for the HN30 and HN31 cell lines cultured with Matrigel. The results suggest that vimentin can be expressed in HNSCC cells and its presence is influenced by the microenvironment of a tumor

    HOXB5 expression in oral squamous cell carcinoma

    Human HOX genes encode transcription factors that act as master regulators of embryonic development. They are important in several processes such as cellular morphogenesis and differentiation. The HOXB5 gene in particular has been reported in some types of neoplasm, but not in oral cancer. OBJECTIVE: The present study investigated the expression of HOXB5 in oral squamous cell carcinoma (SCC) and in non-tumoral adjacent tissues, focusing on verifying its possible role as a broad tumor-associated gene and its association with histopathological and clinical (TNM) characteristics. MATERIAL AND METHODS: RT-PCR was performed to amplify HOXB5 mRNA in 15 OSCCs and adjacent non-tumoral epithelium. A possible association with TNM and histopathologic data was verifed by the chi-square and post-hoc t-test. RESULTS: HOXB5 was amplifed in 60% non-tumoral epithelium and in 93.3% carcinomas. No statistically signifcant differences were found regarding the HOXB5 mRNA expression and TNM or histological grade. CONCLUSION: HOXB5 is expressed in OSCCs and its role in cancer progression should be further investigated.FAPESPCoordenacao de Aperfeicoamento de Pessoal de Nivel Superior (CAPES

    Adenocarcinoma polimorfo de baixo grau: relato de caso clínico

    O adenocarcinoma polimorfo de baixo grau (APBG) é uma neoplasia maligna incomum em região de cabeça e pescoço e que ocorre quase exclusivamente em glândulas salivares menores. Esta lesão apresenta baixo potencial biológico de malignidade e tem como principais diagnósticos diferenciais tanto clínico quanto histológico o adenoma pleomórfico e o carcinoma adenoide cístico. Descrevemos um caso de APBG em um paciente do sexo masculino com 59 anos de idade, leucoderma e que apresentava um nódulo assintomático de aproximadamente 10 dias de evolução em palato mole. Foi realizada uma biópsia incisional e o exame microscópico confirmou o diagnóstico de APBG. O tratamento dessas lesões envolve a completa excisão cirúrgica e raramente há recorrência ou desenvolvimento de doença metastática. Após três meses da excisão cirúrgica, o paciente não apresentou recorrências

    Background: Myoepitheliomas are rare tumours that may generally arise from the minor or major salivary glands. The differential diagnosis of this tumour should be performed along with several benign and malignant soft tissue neoplasms. The present case report describes an asymptomatic mass that arose in the soft palate of 42 year old black woman with duration of the six months.Methods: An incisional biopsy of soft palate lesion was carried out and submitted for histological evaluation under the clinical hypothesis of salivary gland tumour. To confirm the myoepithelial nature of neoplastic cells the immunohistochemical reactions for smooth-muscle actin, cytokeratins and S100 were performed.Results: The histological examination revealed the presence of tumour originating from a minor salivary gland and covered by a stratified squamous oral epithelium. The tumour cells were arranged in order to form a myxoid pattern and, individually, small and/or medium spindle-shaped cells with predominantly round or ovoid nuclei, as well as epithelioid and plasmocytoid cells were noted. The stroma was myxomatous and no ductal or syringomatous epithelial structures were observed. Following the histological and immunohistochemical diagnosis of myoepithelioma, the lesion was surgically removed. After the surgery, a follow-up of one year showed no signs and symptoms of reccurrence.Conclusions: The myoepithelioma should be carefully distinguished from the other soft tissue tumours, especially those arising from salivary glands, such as pleomorphic adenoma and adenoid-cystic carcinoma

    Objective: Functional in vitro studies are fundamental to understand the role of microRNAs, small non coding RNA molecules that function as post-transcriptional regulators, in cancer. The objective of this study was to determine the applicability of head and neck squamous cell carcinoma cell lines and human oral keratinocytes as models for functional studies on microRNAs previously identified as deregulated in head and neck squamous cell carcinomas. Methods: The expression level of four microRNAs was assessed in cell lines and in primary cultures of oral keratinocytes using specific real-time polymerase chain reactions. The identity of oral squamous cell carcinoma cell lines was confirmed by means of STR (short tandem repeats) profiling. The possible impact of feeder-layer gene expression in global microRNA expression results from keratinocyte primary culture was also evaluated. Results: Significant differences in microRNA gene expression were observed among squamous cell carcinoma cell lines, particularly among cells lines from distinct subsites, as well as between primary culture of human keratinocytes and immortalized keratinocyte cell lines. Conclusions: Primary cultures of human keratinocytes and diverse tumor cell lines are relatively easy to obtain. However, each cell model possesses a characteristic phenotype; whereas one may be useful for a specific study, it may be inappropriate for another. Therefore, it is imperative that suitable cell lines are cautiously selected for functional studies in cancer

    Nuclear metallothionein in oral squamous cell carcinoma: clinicopathological parameters and patient survival

    Abstract The aim of this study was to identify the immunoexpression of methallothionein in oral squamous cell carcinoma as well as to address the correlation with clinical features, histological grade and patient survival. Samples were collected from 93 patients with tongue squamous cell carcinoma who presented for follow-up. Immunohistochemical expression of methallothionein in all groups was performed. The scoring system has previously been published by Tsurutani in 2005, which is based on intensity and distribution of staining. We used Kappa index to evaluate the degree of observers’ agreement under metallothionein immunostaining and histological grade. Associations between methallothionein expression and clinical parameters (age, gender, smoking, tumor size, lymph node metastasis and disease stage) were examined for statistical significance using the chi-squared test. The overall survival rates were estimated by the Kaplan-Meier method and the relationship between protein expression and survival was compared using the log-rank test (p < 0.05). Our results showed no statistically significant association between methallothionein immunostaining and the selected clinicopathological variables. Immunohistochemistry results showed positive nuclear immunostaining for metallothionein in 62,37% (58/93) and negative for metallothionein 37,63% (35/93). The degree of examiners agreement by Kappa varied from substantial to perfect and both metallothionein immunostaining and histological grade were explored. The present study suggests that positive methallothionein expression found in tongue squamous cell carcinoma may not help to predict survival in the analyzed samples, as well as no relation between the protein and histological grade and clinical features was observed. In conclusion, the present study suggests that metallothionein is not associated with tongue squamous cell carcinoma clinicopathological characteristics and aggressiveness
