    Evaluation of corn with different densities for broilers

    O trabalho foi realizado com o objetivo de determinar os valores nutricionais de milhos de quatro qualidades, obtidos por meio de estratificação em mesa densimétrica, para frangos de corte em diferentes idades. Os milhos foram designados como: MDA - milho de densidade alta; MDI - milho de densidade intermediária; MDB - milho de densidade baixa; e MDO milho de densidade original, composto de 25% de MDA, 50% de MDI e 25% de MDB. Três ensaios biológicos foram conduzidos utilizando-se o método tradicional de coleta total de excretas para determinação da energia metabolizável aparente corrigida (EMAn). O primeiro ensaio metabólico foi conduzido com pintos Cobb de 14 a 17 dias, o segundo, com pintos de 25 a 28 dias e o terceiro de 38 a 41 dias de idade. Foram realizadas análises químicas para determinação do perfil nutricional, classificação de grãos segundo a densidade e comparação por meio das estimativas de equações de predição do valor energético. Os valores da EMAn dos milhos de diferentes qualidades (MDA, MDI, MDB e MDO) determinados com frangos na fase inicial foram: 3562, 3382, 3198 e 3357 kcal/kg, para a fase de crescimento: 3576, 3555, 3229 e 3416 kcal/kg e, para a fase final 3610, 3554, 3354 e 3585 kcal/kg, respectivamente. Esses valores comprovam as significativas perdas energéticas dos milhos de mais baixa qualidade e a eficiência da mesa densimétrica em estratificar os grãos. A diferença da EMAn entre as idades das aves sugere melhor eficiência de utilização da energia com o avanço da idade. As equações de predição podem ser utilizadas para estimar o valor energético de milhos, tanto por sua classificação como pelas análises bromatológicas. Entretanto, o erro sistemático associado a cada equação de predição pode comprometer a exatidão de seu valor nutricional.The objective of this work was to determine the nutritional value of corn of four different qualities, obtained by stratification in densimetric table, for broilers at different ages. The corns were designed as: MDA - high density corn; MDI - medium density corn; MDB - low density corn and MDO - original density corn, composed of 25% MDA, 50% MDI and 25% MDB. Three biological assays were carried out by using the traditional total excreta collection method for determination of nitrogen corrected apparent metabolizable energy (AMEn). The first metabolism assay was carried out with Cobb chicks at 14 to 17 days of age, the second assay with chicks at 25 to 28 days of age and the third with chicks at 38 to 41 days of age. Chemical analyses were made for determination of the nutritional profile, classification of the grains according to density and comparison by estimates of energy value prediction equations. The values of AMEn of corns of different qualities (MDA, MDI, MDB and MDO) determined in broilers in initial phase were: 3562, 3382, 3198 and 3357 kcal/kg for growth phase and for final phase they were 3576, 3555, 3229 and 3416 kcal/kg and for the finish phase they were 3610, 3554, 3354 and 3585 kcal/kg, respectively. These results prove that significant energy losses of the lowest quality corns and the efficiency of the densimetric table to stratify the grains. The different AMEN values among ages of the birds suggest better energy use efficiency as age increases. The prediction equations can be used to estimate energy value of corn by its classification and by chemical analyses as well. However, the systematic error associated to each prediction equation can compromise the accuracy of the nutritional value

    Características dentárias e faciais de pacientes com artrite idiopática juvenil

    OBJECTIVE: It has been shown that the temporomandibular joint is frequently affected by juvenile idiopathic arthritis, and this degenerative disease, which may occur during facial growth, results in severe mandibular dysfunction. However, there are no studies that correlate oral health (tooth decay and gingival diseases) and temporomandibular joint dysfunction in patients with juvenile idiopathic arthritis. The aim of this study is to evaluate the oral and facial characteristics of the patients with juvenile idiopathic arthritis treated in a large teaching hospital. METHOD: Thirty-six patients with juvenile idiopathic arthritis (26 female and 10 male) underwent a systematic clinical evaluation of their dental, oral, and facial structures (DMFT index, plaque and gingival bleeding index, dental relationship, facial profile, and Helkimo's index). The control group was composed of 13 healthy children. RESULTS: The mean age of the patients with juvenile idiopathic arthritis was 10.8 years; convex facial profile was present in 12 juvenile idiopathic arthritis patients, and class II molar relation was present in 12 (P = .032). The indexes of plaque and gingival bleeding were significant in juvenile idiopathic arthritis patients with a higher number of superior limbs joints involved (P = .055). Anterior open bite (5) and temporomandibular joint noise (8) were present in the juvenile idiopathic arthritis group. Of the group in this sample, 94% (P = .017) had temporomandibular joint dysfunction, 80% had decreased mandibular opening (P = 0.0002), and mandibular mobility was severely impaired in 33% (P = .015). CONCLUSION: This study confirms that patients with juvenile idiopathic arthritis a) have a high incidence of mandibular dysfunction that can be attributed to the direct effect of the disease in the temporomandibular joint and b) have a higher incidence of gingival disease that can be considered a secondary effect of juvenile idiopathic arthritis on oral health.OBJETIVO: A articulação temporomandibular é freqüentemente afetada pela artrite idiopática juvenil, e esta doença degenerativa, durante o crescimento facial, resulta em disfunção mandibular grave. No entanto, não há estudos que avaliam conjuntamente alterações na saúde oral (cáries e doenças gengivais) e na articulação temporomandibular decorrentes da artrite idiopática juvenil. O objetivo deste estudo é avaliar a condição dentária e a função mandibular de pacientes com artrite idiopática juvenil tratados em um hospital escola. MÉTODO: Trinta e seis pacientes com artrite idiopática juvenil (26 meninas e 10 meninos) foram submetidos a uma avaliação clínica sistemática de suas estruturas dentárias, orais e faciais (índice CPO-D, índice de placa e sangramento gengival, relação dentária, perfil facial e índice de Helkimo para articulação temporomandibular). O grupo controle foi composto por 13 crianças saudáveis. RESULTADOS: A média de idade dos pacientes com diagnóstico de artrite idiopática juvenil foi 10,8 anos; o perfil facial convexo foi encontrado em 12 pacientes com artrite idiopática juvenil e relação molar classe II esteve presente em 12 deles (p=0,032). Os índices de placa e sangramento gengival foram significantes em pacientes com artrite idiopática juvenil que apresentavam maior número de articulações dos membros superiores comprometidas (p=0,055). Mordida aberta anterior (cinco) e ruídos articulares (oito) foram encontradas no grupo com artrite idiopática juvenil . Este grupo apresentou 94% dos pacientes com disfunção da articulação temporomandibular (p=0,017), além de amplitude mandibular diminuída (p=0,0002) e mobilidade mandibular gravemente comprometida em 33% (p=0,015). CONCLUSÃO: Este estudo confirma que pacientes com artrite idiopática juvenil: a) têm alto índice de disfunção mandibular, que pode ser atribuído ao efeito direto da doença sobre a articulação temporomandibular e b) maior índice de doença gengival, que pode ser considerado como efeito indireto da artrite idiopática juvenil na saúde oral

    Objetivou-se avaliar o fornecimento de misturas de extratos vegetais nas dietas de frangos de corte. Utilizaram-se 1.350 aves distribuídas em blocos casualizados, com seis repetições e cinco dietas, uma sem aditivos e outras quatro, cada uma contendo um dos aditivos: 10 ppm de avilamicina; 200 ppm de um produto contendo óleos essenciais de cravo, tomilho, canela e pimenta; 100 ppm de um produto comercial composto de óleos essenciais sintéticos de orégano e canela e óleo-resina de pimenta microencapsulados; 500 ppm de um produto comercial constituído de óleo de eucalipto, óleo essencial de canela-da-china, folhas de boldo-do-chile e sementes de feno-grego na fase inicial e 1.200 ppm nas fases de crescimento e final. Aos 21 e 42 dias, foram avaliadas as variáveis de desempenho e aos 44 dias de idade, as características de carcaça. Dos 27 aos 30 dias de idade, foi realizado o ensaio de metabolismo pelo método de coleta total de excretas e determinadas a energia metabolizável e a digestibilidade da proteína bruta das dietas. As dietas com misturas de extratos vegetais não tiveram efeito significativo sobre o desempenho se comparadas à dieta sem aditivo e à dieta com antibiótico. A ausência de desafio na criação e a utilização de dietas com ingredientes de alta digestibilidade podem ter contribuído para que as aves expressassem todo o seu potencial, tornando indetectável a melhora ocasionada pela inclusão de qualquer dos aditivos. Dietas contendo misturas de extratos vegetais promovem desempenho semelhante ao obtido com dietas contendo antibiótico.The trial was carried out to evaluate different blends of plants extracts in broilers diets. It was used 1,350 broilers distributed in a randomized block design with six replicates and five diets, one without additive and the four others, each containing one of the following additives: 10 ppm of avilamycin; 200 ppm of a product containing essential oils of carnation, thyme, cinnamon and microencapsulated capsicum; 100 ppm of a product containing essential oils of synthetic cinnamaldehyde and carvacrol and capsicum oleoresin microencapsulated; 500 ppm of a commercial product containing eucalypt oil, essential oil of Chinese cinnamon, leaves of Chilene boldo and seeds of fenu-Greek at the initial phase and 1200 ppm in the grower and finisher phases. Performance parameters were evaluated at 21 and 42 days of age and the carcass characteristics at 44 days of age. From 27 to 30 days of age, it was carried out the metabolism trial trough total excreta collection method and metabolizable energy and the crude protein digestibility of the diets were determined. The diets with plant extract blends had no significant effect on the performance when compared to diet without additives or diet with avilamycin. The lack of challenge in the experimental facilities and the use of diets with high digestibility ingredients might have contributed to broilers express all their potential making undetectable the improvement made by the inclusion of any of these additivies.     Genomic analysis and immune response in a murine mastitis model of vB_EcoM-UFV13, a potential biocontrol agent for use in dairy cows

    Bovine mastitis remains the main cause of economic losses for dairy farmers. Mammary pathogenic Escherichia coli (MPEC) is related to an acute mastitis and its treatment is still based on the use of antibiotics. In the era of antimicrobial resistance (AMR), bacterial viruses (bacteriophages) present as an efficient treatment or prophylactic option. However, this makes it essential that its genetic structure, stability and interaction with the host immune system be thoroughly characterized. The present study analyzed a novel, broad host-range anti-mastitis agent, the T4virus vB-EcoM-UFV13 in genomic terms, and its activity against a MPEC strain in an experimental E. coli-induced mastitis mouse model. 4,975 Single Nucleotide Polymorphisms (SNPs) were assigned between vB-EcoM-UFV13 and E. coli phage T4 genomes with high impact on coding sequences (CDS) (37.60%) for virion proteins. Phylogenetic trees and genome analysis supported a recent infection mix between vB-EcoM-UFV13 and Shigella phage Shfl2. After a viral stability evaluation (e.g pH and temperature), intramammary administration (MOI 10) resulted in a 10-fold reduction in bacterial load. Furthermore, pro-inflammatory cytokines, such as IL-6 and TNF-\u3b1, were observed after viral treatment.     Effectiveness of screening for tuberculosis in HIV: a pragmatic clinical trial

    OBJECTIVE: To verify the effectiveness of screening for tuberculosis (TB) on all-cause mortality and tuberculosis cases in newly diagnosed HIV-infected patients through a clinical algorithm based on recommendations of the World Health Organization. METHODS: From March 2014 to April 2016, a pragmatic randomized clinical trial was conducted with newly diagnosed and TB-free HIV-infected adults undergoing antiretroviral therapy for up to one month at a major tertiary hospital for HIV in the state of Pernambuco, Brazil. Participants were randomized into intervention and control groups using an automatically-generated random list, and followed-up for at least 6 months. The intervention group was screened for TB at hospital admission and at every follow-up visit through a series of questions addressing TB-related symptoms (cough, fever, night sweating, and weight loss). Patients presenting with any of these symptoms were referred to a pulmonologist and underwent sputum smear microscopy, sputum culture, and rapid molecular testing (GeneXpert). When at least one test result came back positive, TB treatment was initiated. In turn, if patients tested negative but presented with severe clinal symptoms, TB preventive treatment was initiated. Screening for TB was not performed systematically in the control group. The primary outcome assessed in this study was death from all causes, and secondary outcomes included sensitivity and specificity of this screening test, as well as its detection time. RESULTS: This study evaluated 581 patients, 377 in the intervention group (64.9%) and 204 in the control group (35.1%). In total, 36 patients died during the follow-up period. Of these, 26 (6.9%) were from the intervention group, reaching a cumulative mortality coefficient of 69 per 1,000 inhabitants, and 10 (4.9%) from the control group (p = 0.341), with a cumulative mortality coefficient of 49 per 1,000 inhabitants (p = 0.341)

    The use of telemedicine to support Brazilian primary care physicians in managing eye conditions : the TeleOftalmo Project

    Purpose: To determine whether teleophthalmology can help physicians in assessing and managing eye conditions and to ascertain which clinical conditions can be addressed by teleophthalmology in primary care setting. Methods: We evaluated the resolution capacity of TeleOftalmo, strategy implemented in the public health system of southern Brazil. Resolution capacity was defined as the ability to fully address patients’ eye complaints in primary care with remote assistance from ophthalmologists. Data from tele-eye reports were collected over 14 months. Resolution capacity was compared across different age groups and different ocular conditions. Results: Overall, 8,142 patients had a tele-eye report issued in the study period. Resolution capacity was achieved in 5,748 (70.6%) patients. When stratified into age groups, the lowest capacity was 43.1% among subjects aged �65 years, while the highest was 89.7% among subjects aged 13–17 years (p<0.001). Refractive error (70.3%) and presbyopia (56.3%) were the most prevalent conditions followed by cataract (12.4%) and suspected glaucoma (7.6%). Resolution capacity was higher in cases of refractive error, presbyopia, spasm of accommodation and lid disorders than in patients diagnosed with other condition (p<0.001). Conclusions: With telemedicine support, primary care physicians solved over two-thirds of patients’ eye or vision complaints. Refractive errors had high case resolution rates, thus having a great impact on reducing the number of referrals to specialty care. Teleophthalmology adoption in primary-care settings as part of the workup of patients with eye or vision complaints promotes a more effective use of specialty centers and will hopefully reduce waiting times for specialty referral

    Este estudo avaliou o custo de um serviço público de telediagnóstico em oftalmologia. O método de custeio baseado em atividades e tempo (TDABC) foi adotado para examinar os componentes de custos relacionados à teleoftalmologia. Com este método, também foi possível estabelecer o custo unitário padrão que o telediagnóstico deveria ter, dada a capacidade instalada e utilização de profissionais. Dados de um ano de telediagnósticos foram considerados, e avaliou-se a mudança do custo por telediagnóstico ao longo do período de adaptação da tecnologia no sistema. O TDABC merece destaque nesse sentido, pois permite obter informações mais precisas sobre custo da tecnologia, melhorando a capacidade de dimensionamento e gerenciamento da organização de saúde.This study evaluated the cost of public telediagnostic service in ophthalmology. The time-driven activity-based costing method (TDABC) was adopted to examine the cost components related to teleophthalmology. This method allowed us to establish the standard unit cost of telediagnosis, given the installed capacity and utilization of professionals. We considered data from one year of telediagnoses and evaluated the cost per telediagnosis change throughout technology adaptation in the system. The standard cost calculated by distance ophthalmic diagnosis was approximately R 119, considering the issuance of 1,080 monthly ophthalmic telediagnostic reports. We identified an imbalance between activities, which suggests the TDABC method’s ability to guide management actions and improve resource allocation. The actual unit cost fell from R783toR 783 to R 283 over one year – with room to approach the estimated standard cost. Partial economic evaluations contribute significantly to support the incorporation of new technologies. The TDABC method deserves prominence, as it enables us to retrieve more accurate information on the cost of technology, improving the scalability and management capacity of the healthcare system.Teleducaçã
