    Mosquito-borne disease and community engagement in high-income countries: scoping review

    Background & Aim: High-income countries in sub-tropical and tropical regions are at increasing risk of Aedes mosquito borne disease outbreaks. As the Aedes mosquito predominately lives and breeds in and around people's homes, engaging communities in mosquito borne disease management is an important part of preventing disease outbreaks. Historically government led strategies have dominated community engagement efforts rather than empowering or co-production strategies designed and led by the community. A scoping review was conducted in 2018 to: 1.To describe the community engagement approaches used in aedes mosquito-borne disease management within high income countries, and identify any reported outcomes of these methods 2.To characterize the extent to which empowering approaches are included as part of these community engagement approaches Method: A systematic search was conducted of peer-reviewed literature using electronic databases MEDLINE (OVID), Informit, CINAHL and Scopus published between 2004 and 2018. Grey literature was also searched primarily through key Mosquito Association websites. Results & Conclusions: Twenty studies matched the review criteria. Community engagement strategies within each study were critiqued against eight community empowerment domains. Results showed few community engagement strategies incorporated more than one domain of empowerment. Review suggests over reliance on government led approaches. The rationale for using predominantly government led approaches was not clear; further research is required to understand the barriers to employing empowering approaches in mosquito management in high-income countries

    Instructional Practices of Career and Technical Teachers toward English Language Learners

    In Midwestern high schools, English language learners (ELLs) who are enrolled in Career and Technical Education (CTE) classes transition from school to the workforce at unacceptably low levels. This loss of opportunity has been linked to CTE instructional practices. The purpose of this study was to explore the instructional practices used by CTE teachers to support ELL instruction and how the teachers perceive those practices to improve ELL transition from school to the workforce. The conceptual framework included Freire\u27s critical consciousness theory, which holds that it is important to include learners in the learning process. The framework grounded the study by linking student-centered teaching research to improved ELL outcomes. This basic qualitative study was conducted in secondary school settings in the urban Midwest. Data were collected from 8 CTE education teachers through semistructured interviews and data analyzed by using open and a priori codes. The codes were placed into categories from which themes emerged. Primary themes indicated that CTE teachers apply certain instructional practices with ELLs and perceive that they help ELLs transition to the workforce. These practices include classroom management techniques, adapting curriculum, instructional consistency, questioning techniques, developing community partnerships, and teaching culturally relevant subject matter. A project, in the form of a policy recommendation paper, was created and may translate to an increased number of high school ELL graduates who are prepared to enter the workforce with the skills necessary to be successful. The findings contribute to positive social change through increased understanding of CTE instructional practices toward ELLs

    Presence of a pre-hospital enhanced care team reduces on scene time and improves triage compliance for stab trauma

This study investigated the relationship between presence of pre-hospital enhanced care teams (ECT) (Critical Care Paramedics (CCPS) or Helicopter Emergency Medical Service (HEMS)), on the scene time and triage compliance, of penetrating trauma patients in a UK ambulance service. The primary outcome was whether scene time reduces when an ECT is present. A secondary outcome was whether the presence of an ECT improved compliance with the trust's Major Trauma Decision Tree (MTDT). Methods: All suspected penetrating trauma incidents involving a patient's torso were identified from the Trust's computer-aided dispatch (CAD) system between 31st March 2017 and 1st April 2018. Only patients who sustained central penetrating trauma were included. Any incidents involving firearms were excluded due to the prolonged times that can be involved when waiting for specialist police units. Data relevant to scene time for each eligible incident were retrieved, along with the presence or absence of an ECT. The results were analysed to identify trends in the scene times and compliance with the MTDT. Results: One hundred seventy-one patients met the inclusion criteria, with 165 having complete data. The presence of an ECT improved the median on-scene time in central stabbing by 38% (29m50s vs. 19m0s, p = 0.03). The compliance with the trust's MTDT increased dramatically when an ECT is present (81% vs. 37%, odds ratio 7.59, 95% CI, 3.70-15.37, p < 0.0001). Conclusions: The presence of an ECT at a central stabbing incident significantly improved the scene time and triage compliance with a MTDT. Ambulance services should consider routine activation of ECTs to such incidents, with subsequent service evaluation to monitor patient outcomes. Ambulance services should continue to strive to reduce scene times in the context of central penetrating trauma.Peer reviewe

    Access to a range of services, including health care, ranks among the key determinants of health and wellbeing. It varies with both health system supply factors and consumer demand characteristics. For rural populations, access to health services can be restricted for a variety of reasons, contributing to poorer health outcomes compared with metropolitan populations. Access to health care differs between communities, despite commonly being seen as homogenous in terms of lack of service and poor access. This article seeks to examine consumer perceptions of access to health service in four shires in rural Victoria and explore differences between rural areas. These insights may assist health services to reorient their modes of service provision to be more accessible to rural health consumers. A confidential self-administered questionnaire was mailed to randomly selected households in the four shires. A total of 1,271 questionnaires were returned (35 percent response rate) with 75 percent of respondents reporting good access to health care overall. Many factors contributed significantly to the perception of health access; however, these factors were unique to each rural community. The implication of this heterogeneity is that rural health care services must be tailored to promote equitable, quality health care outcomes with attention to local community needs at the core of efforts. Only locally-targeted actions will achieve optimal health service planning and delivery

    The facilitators and barriers to exercise in the Noongar Aboriginal population in Perth, Australia

    Indigenous Standpoint Theory forms the epistemological foundation for this study and methodological choices were made within this theoretical framework to ensure culturally responsive research processes that engaged the Indigenous agenda of self-determination and rights. The objectives of this research were to determine: (i) Indigenous perceptions of the facilitators and barriers to exercise; (ii) The potential feasibility and sustainability of an exercise intervention. In this context, Participatory Action Research methods were used to design the data-gathering instrument for the studyβ€”a questionnaire, co-designed with the Noongar Aboriginal community of Perth, Western Australia. This self-administered questionnaire, distributed to participants by email, post and manual delivery, sought to elicit the factors that impact uptake and retention of regular exercise activities. Questionnaire data included individual demographic detail and specific question responses on labelled 5 point Likert Scales. Specific question responses were tabulated by Likert Scale label category and the response distribution for each question was enumerated. Simple descriptive statistics (measures of central tendency and variance) were used to characterize the data set and the Chi squared test was used to evaluate frequency differences between males and females. A total of 133 participants (71 females) completed the questionnaire. The results indicated that people valued exercise. The most common barriers indicated by participants were exercising with an injury (63%), changing diet (58%), finding time to exercise every day (55%) and exercising the next day with pain from exercising the day before (54%). A larger proportion of males (34%) than females (24%) reported greater ease in finding time to exercise every day (p \u3c 0.05). Facilitators mainly related to the potential social and community benefits of exercising with other people, preferably in small groups, and the importance of a culturally secure venue. These findings shed light on what a culturally secure exercise programme might involve for the Noongar community. As this may have implications for other Aboriginal and Torres Strait Islander and international First Nations’ Peoples, more focused research is needed on the place of traditional physical activities and the nature of culturally secure exercise programmes and spaces to enable wider application

    Background: Complex, ongoing social factors have led to a context where metabolic syndrome (MetS) is disproportionately high in Aboriginal Australians. MetS is characterised by insulin resistance, abdominal obesity, hypertension, hypertriglyceridemia, high blood-sugar and low HDL-C. This descriptive study aimed to document physical activity levels, including domains and intensity and sedentary behaviour, and MetS risk factors in the Perth Aboriginal (predominately Noongar) community. Methods: The Global Physical Activity Questionnaire (GPAQ), together with a questionnaire on self-reported MetS risk factors, was circulated to community members for completion during 2014 (n = 129). Results: Data were analysed using chi-squared tests. The average (SD) age was 37.8 years (14) and BMI of 31.4 (8.2) kg/m2 . Occupational, transport-related and leisure-time physical activity (PA) and sedentary intensities were reported across age categories. The median (interquartile range) daily sedentary time was 200 (78, 435), 240 (120, 420) and 180 (60, 300) minutes for the 18–25, 26–44 and 45+ year-olds, respectively (p = 0.973). Conclusions: An in-depth understanding of the types, frequencies and intensities of PA reported for the Perth Aboriginal community is important to implementing targeted strategies to reduce the prevalence of chronic disease in this context. Future efforts collaborating with community should aim to reduce the risk factors associated with MetS and improve quality of life

    Mathematical model of a telomerase transcriptional regulatory network developed by cell-based screening: analysis of inhibitor effects and telomerase expression mechanisms

    Cancer cells depend on transcription of telomerase reverse transcriptase (TERT). Many transcription factors affect TERT, though regulation occurs in context of a broader network. Network effects on telomerase regulation have not been investigated, though deeper understanding of TERT transcription requires a systems view. However, control over individual interactions in complex networks is not easily achievable. Mathematical modelling provides an attractive approach for analysis of complex systems and some models may prove useful in systems pharmacology approaches to drug discovery. In this report, we used transfection screening to test interactions among 14 TERT regulatory transcription factors and their respective promoters in ovarian cancer cells. The results were used to generate a network model of TERT transcription and to implement a dynamic Boolean model whose steady states were analysed. Modelled effects of signal transduction inhibitors successfully predicted TERT repression by Src-family inhibitor SU6656 and lack of repression by ERK inhibitor FR180204, results confirmed by RT-QPCR analysis of endogenous TERT expression in treated cells. Modelled effects of GSK3 inhibitor 6-bromoindirubin-3β€²-oxime (BIO) predicted unstable TERT repression dependent on noise and expression of JUN, corresponding with observations from a previous study. MYC expression is critical in TERT activation in the model, consistent with its well known function in endogenous TERT regulation. Loss of MYC caused complete TERT suppression in our model, substantially rescued only by co-suppression of AR. Interestingly expression was easily rescued under modelled Ets-factor gain of function, as occurs in TERT promoter mutation. RNAi targeting AR, JUN, MXD1, SP3, or TP53, showed that AR suppression does rescue endogenous TERT expression following MYC knockdown in these cells and SP3 or TP53 siRNA also cause partial recovery. The model therefore successfully predicted several aspects of TERT regulation including previously unknown mechanisms. An extrapolation suggests that a dominant stimulatory system may programme TERT for transcriptional stability

    Revisiting the Hetero-Fertilization Phenomenon in Maize

    Development of a seed DNA-based genotyping system for marker-assisted selection (MAS) has provided a novel opportunity for understanding aberrant reproductive phenomena such as hetero-fertilization (HF) by observing the mismatch of endosperm and leaf genotypes in monocot species. In contrast to conventional approaches using specific morphological markers, this approach can be used for any population derived from diverse parental genotypes. A large-scale experiment was implemented using seven F2 populations and four three-way cross populations, each with 534 to 1024 individuals. The frequency of HF within these populations ranged from 0.14% to 3.12%, with an average of 1.46%. The highest frequency of HF in both types of population was contributed by the pollen gametes. Using three-way crosses allowed, for the first time, detection of the HF contributed by maternal gametes, albeit at very low frequency (0.14%–0.65%). Four HF events identified from each of two F2 populations were tested and confirmed using 1032 single nucleotide polymorphic markers. This analysis indicated that only 50% of polymorphic markers can detect a known HF event, and thus the real HF frequency can be inferred by doubling the estimate obtained from using only one polymorphic marker. As expected, 99% of the HF events can be detected by using seven independent markers in combination. Although seed DNA-based analysis may wrongly predict plant genotypes due to the mismatch of endosperm and leaf DNA caused by HF, the relatively low HF frequencies revealed with diverse germplasm in this study indicates that the effect on the accuracy of MAS is limited. In addition, comparative endosperm and leaf DNA analysis of specific genetic stocks could be useful for revealing the relationships among various aberrant fertilization phenomena including haploidy and apomixis

    Creating an online world flora by 2020 : a perspective from South Africa

    At the 10th Conference of the Parties of the Convention on Biodiversity (CBD), which was held in Nagoya, Japan, in October 2010, an updated Global Strategy for Plant Conservation (GSPC) was adopted as part of the plan of work of the CBD. Target 1 of the GSPC aims to produce an online Flora for all the plants of the world by 2020. Governments that have ratified the CBD will have to report over the next several years on progress towards achieving this challenging target. Floras are still widely regarded as a means of providing descriptive information and identification tools for the plants that occur in a specified region. Historically, Floras have included identification keys; scientific names with authorship for all taxa known to occur in the area; synonymy; descriptions; distributions within the region in question; specimen citations; habitat; literature references; and illustrations. Of these, nomenclature, descriptions, identification tools, illustrations and distributions are critical components. The approach being taken by South Africa, a biodiversity-rich country, in working towards achieving Target 1 of the GSPC by 2020 is presented and discussed, outlining a methodology that may be of practical use to other countries.
