Subclinical hyperthyroidism and the risk of coronary heart disease and mortality

L'hyperthyroïdie infra-clinique est une perturbation de la fonction thyroïdienne, définie par une thyrotropine (TSH) basse et des taux normaux de thyroxine libre (T4L) et triiodothyronine (T3). Cette dysfonction affecte de 1% à 5% des adultes de plus de 65 ans, surtout les femmes, et pourrait être associée avec les maladies cardiovasculaires, la fibrillation auriculaire et l'insuffisance cardiaque. Toutefois, les conclusions des différentes études de cohortes sont contradictoires, avec des limites méthodologiques empêchant leur comparaison de manière formelle. L'objet du travail de thèse était d'estimer le risque de mortalité de toute cause, le risque de mortalité de cause cardiovasculaire, le risque d'événements cardiovasculaires et le risque de fibrillation auriculaire associés à l'hyperthyroïdie infra-clinique dans toutes les grandes études de cohorte prospectives disponibles à ce jour.
Méthode et Résultats
Les données individuelles de 52'674 participants provenant de 10 études de cohorte prospectives des Etats-Unis, d'Europe, du Brésil et d'Australie ont été analysées pour évaluer les risques à long-terme de l'hyperthyroïdie infra-clinique. L'euthyroïdie était définie par une TSH entre 0.45 et 4.49 mUI/l et l'hyperthyroïdie infra-clinique par une TSH inférieure à 0.45 mUI/l avec un taux normal de T4L, après exclusion des participants prenant des médicaments pouvant perturber la thyroïde.
Sur les 52'674 participants, 2188 (4.2%) avaient une hyperthyroïdie infra-clinique. Pendant un suivi de plus de 8 ans, 8527 participants sont décédés (dont 1896 de cause cardiovasculaire), 3653 sur 22'437 ont eu un événement cardiovasculaire et 785 sur 8711 ont développé une fibrillation auriculaire. Dans des analyses ajustées pour l'âge et le sexe, l'hyperthyroïdie infra-clinique était associée à une hausse de la mortalité de toute cause (hazard ratio [HR] 1.24, intervalle de confiance à 95% [IC] 1.06-1.46), de la mortalité cardiovasculaire (HR 1.29, IC 1.02-1.62), des événements cardiovasculaires (HR 1.21, IC 0.99- 1.46) ainsi qu'une hausse de l'incidence de fibrillation auriculaire (HR 1.68, IC 1.16-2.43). Les risques ne différaient pas significativement dans les analyses stratifiées selon l'âge, le sexe ou la présence de maladies cardiovasculaires préexistantes, et étaient similaires après ajustement multiple pour les facteurs de risque cardiovasculaire. Le risque de mortalité cardiovasculaire et de fibrillation auriculaire était plus élevé avec une TSH très basse (< 0.10 Ul/I) comparé à une TSH modérément abaissée (0.10-0.44 mUI/l, valeurs ρ for trend < 0.03).
Conclusions et perspectives
L'hyperthyroïdie infra-clinique est associée à un risque augmenté de mortalité de toute cause, de cause cardiovasculaire et de fibrillation auriculaire, avec un risque plus élevé quand la TSH est inférieure à 0.10 mUI/l. Ces résultats sont cohérents avec les dernières recommandations internationales conseillant de considérer un traitement de l'hyperthyroïdie infra-clinique pour les adultes de plus de 65 ans ou les patients avec maladie cardiaque, en particulier si la TSH est inférieure à 0.10 mUI/l. Toutefois, des études cliniques randomisées sont encore nécessaires pour prouver l'efficacité du traitement et déterminer si l'on devrait dépister les problèmes de thyroïde dans la population générale
Phase Transition in a One-Dimensional Extended Peierls-Hubbard Model with a Pulse of Oscillating Electric Field: II. Linear Behavior in Neutral-to-Ionic Transition
Dynamics of charge density and lattice displacements after the neutral phase
is photoexcited is studied by solving the time-dependent Schr\"odinger equation
for a one-dimensional extended Peierls-Hubbard model with alternating
potentials. In contrast to the ionic-to-neutral transition studied previously,
the neutral-to-ionic transition proceeds in an uncooperative manner as far as
the one-dimensional system is concerned. The final ionicity is a linear
function of the increment of the total energy. After the electric field is
turned off, the electronic state does not significantly change, roughly keeping
the ionicity, even if the transition is not completed, because the ionic
domains never proliferate. As a consequence, an electric field with frequency
just at the linear absorption peak causes the neutral-to-ionic transition the
most efficiently. These findings are consistent with the recent experiments on
the mixed-stack organic charge-transfer complex, TTF-CA. We artificially modify
or remove the electron-lattice coupling to discuss the origin of such

Dynamical estimates of chaotic systems from Poincar\'e recurrences
We show that the probability distribution function that best fits the
distribution of return times between two consecutive visits of a chaotic
trajectory to finite size regions in phase space deviates from the exponential
statistics by a small power-law term, a term that represents the deterministic
manifestation of the dynamics, which can be easily experimentally detected and
theoretically estimated. We also provide simpler and faster ways to calculate
the positive Lyapunov exponents and the short-term correlation function by
either realizing observations of higher probable returns or by calculating the
eigenvalues of only one very especial unstable periodic orbit of low-period.
Finally, we discuss how our approaches can be used to treat data coming from

On the spectrum of Farey and Gauss maps
In this paper we introduce Hilbert spaces of holomorphic functions given by
generalized Borel and Laplace transforms which are left invariant by the
transfer operators of the Farey map and its induced version, the Gauss map,
respectively. By means of a suitable operator-valued power series we are able
to study simultaneously the spectrum of both these operators along with the
analytic properties of the associated dynamical zeta functions.Comment: 23 page
Comparison of averages of flows and maps
It is shown that in transient chaos there is no direct relation between
averages in a continuos time dynamical system (flow) and averages using the
analogous discrete system defined by the corresponding Poincare map. In
contrast to permanent chaos, results obtained from the Poincare map can even be
qualitatively incorrect. The reason is that the return time between
intersections on the Poincare surface becomes relevant. However, after
introducing a true-time Poincare map, quantities known from the usual Poincare
map, such as conditionally invariant measure and natural measure, can be
generalized to this case. Escape rates and averages, e.g. Liapunov exponents
and drifts can be determined correctly using these novel measures. Significant
differences become evident when we compare with results obtained from the usual

On Renormalization Group Flows and Polymer Algebras
In this talk methods for a rigorous control of the renormalization group (RG)
flow of field theories are discussed. The RG equations involve the flow of an
infinite number of local partition functions. By the method of exact
beta-function the RG equations are reduced to flow equations of a finite number
of coupling constants. Generating functions of Greens functions are expressed
by polymer activities. Polymer activities are useful for solving the large
volume and large field problem in field theory. The RG flow of the polymer
activities is studied by the introduction of polymer algebras. The definition
of products and recursive functions replaces cluster expansion techniques.
Norms of these products and recursive functions are basic tools and simplify a
RG analysis for field theories. The methods will be discussed at examples of
the -model, the -model and hierarchical scalar field

A solvable model of the genesis of amino-acid sequences via coupled dynamics of folding and slow genetic variation
We study the coupled dynamics of primary and secondary structure formation
(i.e. slow genetic sequence selection and fast folding) in the context of a
solvable microscopic model that includes both short-range steric forces and and
long-range polarity-driven forces. Our solution is based on the diagonalization
of replicated transfer matrices, and leads in the thermodynamic limit to
explicit predictions regarding phase transitions and phase diagrams at genetic
equilibrium. The predicted phenomenology allows for natural physical
interpretations, and finds satisfactory support in numerical simulations.Comment: 51 pages, 13 figures, submitted to J. Phys.
