    Can we interrogate public databases to fill critical gaps in mental health epidemiology? Testing the association between cannabis and psychosis in the UK as an example

    The psychoactive properties of cannabis have been known forever. Since 1987, several prospective studies have suggested an increased risk of psychosis among cannabis users, with alternative explanations failing to account for such an effect. A cause-effect relationship has thus been implied. Further evidence has indicated that there is a dose-response relationship, and high-potency cannabis varieties confer the greatest risk of psychosis. As cannabis use has become more common over the last decades, one would expect a related increase in the number of schizophrenia cases. However, evidence in this regard remains equivocal for several reasons, including relying on databases that are not primarily designed to address such question and the issue that solid information regarding the incidence of schizophrenia is a relatively recent acquisition. Recent years have seen the development of online web publications, such as Google Trends and "Our World in Data", where data are explorable and interactable for tracking and comparing trends over specific periods and world regions. By using such databases, we believe that the question whether changes in cannabis use are associated with changes in schizophrenia rates can be answered, at least partly. Therefore, we tested these tools by evaluating trends in cannabis use and both cases and prevalence of schizophrenia in the United Kingdom, one of the countries where the incident rates for psychotic disorder have been suggested to be particularly increased by cannabis consumption. Crossing data from these tools revealed that interest in cannabis has been growing at the country level for over 10 years, with a parallel overlapping raise in psychosis cases and prevalence. Following up on this example, let us think of how many public health opportunities these public resources may offer. The question now is whether public health interventions for the benefit of the general population will follow suit

    Effects of environmental, living space and climate variability on the utilization of impregnated bed nets in west Cameroon: A community based survey for policy implementation

    Background: Despite the fact that Long Lasting Impregnated Mosquito Net (LLIN) represents one of the most effective tools in fighting malaria, its use remains limited. Our study aimed at determining how environmental, household characteristics and climate affect bed net use. Methodology: A cross sectional descriptive and analytic study was carried out from January to April 2014 in Mifi health district. Data collected were collected in households during a face to face interview with standard household questionnaires, entered and analyzed using Epi Info software version 3.5.3. Graphics and tables were obtained using MS Excel and Word. Results: Of the 317 participants interviewed, average age was 33.23 years (SD = 10.80) and female sex predominant (85.2%). Most participants had attended secondary education 53.6% (n= 170), married marital status was most represented (58.1%; n= 185).75.4% (n=239) of households had at least 1 LLIN and average district coverage estimated to 1 LLIN for 3.3 persons. 78% of occupants in households with at least one LLIN had slept under the night before the survey. The presence of a ceiling in a house reduced net usage by 2.5% (p = 0.67) compared to house lacking ceiling. Standing waters around the compound increased net utilization rate to 16.6% (p = 0.03), whereas the presence of a covered well decreased the rate by 1.4% (p = 0.86). The dry season was identified as the period during which 86.8% (n= 239) of respondents sleep less under a net. Heat (57.60% n = 138/239), increased choking (2.5%), reduction in vector breeding sites (39.90%; n = 95) were cited as main reasons. Conclusion: Although classified as zone of continuous transmission, our findings indicate that bed net usage by our study population depends on environmental, household characteristics and climate. There is therefore an urgent need to develop strategic communication and sensitization campaigns coupled to environmental management to help scale up and optimize malaria burden reductio

    Prevalence of HIV, HBV and Chlamydia infections in Cameroonian University context: case of the University of Dschang, in the Western Region

    Introduction: In sub-Saharan Africa HIV infection remains largely epidemic, whereas HBV infection is highly endemic (>8%). In Cameroon, HIV prevalence is 4.3%. Concerning HBV and chlamydia infections, their prevalence are both ≥10%. Young adults, including university students, are the population groups mostly affected. Epidemiological data on these infections, among university students could be helpful to implement specific prevention strategies. Methods: A descriptive study was performed in May 2013 among 624 students from the University of Dschang, Cameroon. Participants were screened for HIV, HBV and Chlamydia infections. Data was collected by a standard questionnaire and analyzed by Epi Info. Results: Average age of participants was 23.3 years (σ = 3.2) with female predominance (58.7%). Prevalence of HIV, HBV and Chlamydia infection was 1.1% (7/624), 2.8% (5/176) and 2.0% (2/100) respectively. 83.2% of participants were sexually active. Concerning sexual risk behaviors, participants reported having multi partners (14.8%), using condom occasionally (58.6%) or never (5.0%). 100%, 62.6% and 52.2% reported to be aware on HIV, HBV and Chlamydia infections respectively. In addition, only 5.5% and 21.3% of the participants were aware of their HBV and Chlamydia status respectively, versus 64.4% for HIV. The excessive cost of HBV and Chlamydia tests has been identified as the major barrier to testing (87.6%). Conclusion: Among college Cameroonian students the prevalence of HIV, HBV and Chlamydia infections seems to be relatively low if compared to general population. However, having multiple sexual partners in addition to non-systematic use of condoms during sexual intercourse represents risk behaviors among students. Awareness campaigns and screening facilitation on HBV and chlamydia infections need to be strengthened

    Pediatric emergency department mental health assessments in the 2 years following the COVID-19 outbreak reveal higher vulnerability for eating disorder and suicide risk

    Background: In the post-COVID era an increase in Emergency Department (ED) mental health assessments has been consistently reported among youth populations. Methods: Pediatric ED mental health assessments in the 2 years following the COVID-19 pandemic (March 2020-February 2022) were compared to those in the immediately preceding same period (March 2018-February 2020), in terms of rates and risk profiles. Results: During the pre-pandemic and post-pandemic periods, 158 and 268 ED accesses were counted respectively, and an overall 1.64 (95 % CI: 1.34–1.99) monthly IRR was estimated. During the post-pandemic period, youth accessing ED were less likely to have a personal history of psychiatric disorders (OR: 0.49; 95 % CI: 0.28–0.86) and to receive an extemporaneous administration of psychopharmacological therapy in ED (OR: 0.28; 95 % CI: 0.14–0.57), despite being more frequently discharged from ED with a background psychopharmacological therapy in place (OR: 2.02; 95 % CI: 1.02–4.01). Finally, during the post-pandemic period, an increase in ED accesses for eating disorder (OR: 2.77; 95 % CI: 1.49–5.13) and suicidal thoughts-self-harm (OR: 2.00; 95 % CI: 1.07–3.74) was observed, when compared to ED access for anxiety-agitation. Conclusions: This report suggests higher rates of post-COVID pediatric ED mental health assessments, especially for eating disorder and suicide risk, with a preponderance of youth whose ED access may be their first mental health specialist contact

    Psychological factors associated with vaccination hesitancy: an observational study of patients hospitalized for COVID-19 in a later phase of the pandemic in Italy

    Introduction: Vaccination against SARS-CoV-2 has been used to reduce the severity of COVID-19 disease and the incidence of new cases. However, a significant proportion of people have shown vaccination hesitancy. Methods: This study explored psychological factors related to vaccination hesitancy in a sample of Italian COVID-19 patients (N = 54), hospitalized during 2021, after vaccines had been made available and while the vaccination campaign was on-going. Consecutive patients, aged 18 or older, admitted to the hospital with a diagnosis of COVID-19 were assessed with a set of standardized measures. Results: In our sample, 48.1% was not vaccinated and 7.4% died within 6months after hospitalization, with a preponderance of deaths among non-vaccinated patients. Non-vaccinated participants had higher resilience scores at the CD-RISC-10 scale than vaccinated ones (33.6 ± 5.50 vs 28.6 ± 6.61; t40.2=+ 2.94, p = 0.005). No statistically significant differences were found between the two groups for any other measures. Discussion: Higher levels of resilience among non-vaccinated patients may reflect greater identity worth and self-esteem, in turn resulting in a decrease in vaccination likelihood. This finding may have important public health implications, as it indicates that specific psychological aspects, such as resilience, may result in vaccination hesitancy, with implications for hospitalization rates, and thus healthcare costs, as well as loss of lives

    pH-Dependent Capping Interactions Induce Large-Scale Structural Transitions in i-Motifs

    We study here a DNA oligonucleotide having the ability to form two different i-motif structures whose relative stability depends on pH and temperature. The major species at neutral pH is stabilized by two C:C+ base pairs capped by two minor groove G:C:G:C tetrads. The high pH and thermal stability of this structure are mainly due to the favorable effect of the minor groove tetrads on their adjacent positively charged C:C+ base pairs. At pH 5, we observe a more elongated i-motif structure consisting of four C:C+ base pairs capped by two G:T:G:T tetrads. Molecular dynamics calculations show that the conformational transition between the two structures is driven by the protonation state of key cytosines. In spite of large conformational differences, the transition between the acidic and neutral structures can occur without unfolding of the i-motif. These results represent the first case of a conformational switch between two different i-motif structures and illustrate the dramatic pH-dependent plasticity of this fascinating DNA motif
