    Abdominoplasty aims to reshape the abdominal wall by combining skin and subcutaneous tissue resection with musculo-aponeurotic reinforcement. The presence of a median or paramedian supraumbilical scar should always be considered in patients for whom an abdominoplasty is planned. In fact, advancement of the superior flap is very limited and leads to unfavorable aesthetic results, as the median scar resembles a cord under tension. A considerable number of these patients have supraumbilical scars consequent to bariatric surgery with massive weight loss. Simple transverse resection cannot guarantee adequate body contouring. © 2010 Springer-Verlag Berlin Heidelberg

    Considerações éticas sobre relatórios de casos e análises da qualidade de vida em pacientes com rinoplastia cosmética

    The paper discusses an ethical analysis of three common cases involving the most popular requests in rhinoplasty. As the nose plays a fundamental role in the aesthetics and physiognomy of the human face, the request for rhinoplasty may be an expression of both subjective discomfort and objective dysfunction. The paper aims, therefore, to fill the gap between qualitative-subjective impressions related to bodily self-perception and its quantitative-objective assessment. Ethical evaluation should start with consideration of the formal object and circumstances of the act, by posing the following question: Is this a real clinical case? Only after an undoubtedly positive answer to this question, should we consider specific case-related aspects; i.e., the best scientific evidence, available nosography and informed consent, before conducting an ethical evaluation of the case.DOI: 10.5294/pebi.2016.20.1.8  En el artículo se describe un análisis ético de tres casos comunes relacionados con las peticiones más habituales de la rinoplastia.     Cognitive Investigation Study of Patients Admitted for Cosmetic Surgery: Information, Expectations, and Consent for Treatment

    Background In all branches of medicine, it is the surgeon's responsibility to provide the patient with accurate information before surgery. This is especially important in cosmetic surgery because the surgeon must focus on the aesthetic results desired by the patient. Methods An experimental protocol was developed based on an original questionnaire given to 72 patients. The nature of the responses, the patients' motivation and expectations, the degree of patient awareness regarding the planned operation, and the patients' perceptions of the purpose of the required consent for cosmetic surgery were all analyzed using Fisher's exact test. Results Candidates for abdominal wall surgery had significantly more preoperative psychological problems than their counterparts did (P=0.035). A significantly different percentage of patients under 40 years of age compared to those over 40 years of age searched for additional sources of information prior to the operation (P=0.046). Only 30% of patients with a lower educational background stated that the preoperative information had been adequate, whereas 92% of subjects with secondary schooling or a postsecondary degree felt that the information was sufficient (P=0.001). A statistically significant difference was also present between patients according to their educational background regarding expected improvements in their quality of life postoperatively (P=0.008). Conclusions This study suggests that patients require more attention in presurgical consultations and that clear communication should be prioritized to ensure that the surgeon understands the patient's expectations

    Appearance distress and dysfunction in the elderly: International contrasts across italy and the UK using DAS59

    Background: As the global population grows, the percentage of those over 60 will escalate disproportionately. Their needs will become an ever more dominant feature of public policy and healthcare provision. Older adults' appearance is often removed from cultural ideals and stereotypes of beauty, often seen as synonymous with "youth." This has seen older adults' concerns about appearance and body image somewhat sidelined in practice and in research. This study investigates the extent to which self-consciousness of appearance is associated with distress and dysfunction in those over 60 years old. Furthermore, we contrast the extent of this phenomenon in two European nations, UK and Italy, and consider the direct impact and interaction of cultural context and participant gender. Method: To make an objective measurement of distress and dysfunction, we translated a widely used psychometric measure, the Derriford Appearance Scale 59 following an established and reliable translation protocol. Data were collected from community samples. Results: The Italian translation was sound, with acceptability in the Italian-speaking sample and acceptable internal consistency scores for full-scale and subscales. ANOVA analysis demonstrated that for overall adjustment, and all subscale scores, the Italians were more distressed about their appearance than UK comparators. Moreover, there were significant differences between Italian women and men, with Italian women more distressed than Italian men overall, and also general self-consciousness, sexual self-consciousness, and social self-consciousness subscales. Conclusion: These results are considered in the context of aging and cultural and gender issues in appearance, including the Italian concept of bella figura. © 2013 Springer Science+Business Media New York and International Society of Aesthetic Plastic Surgery
