    Impacto do etanol e consumo de café na qualidade de sono de acadêmicos de medicina

    Introduction: The comsuption of ethanol and coffee may be retracted as a sleep disruptor principally in medical students and this can compromise their welfare and daily activities. Objectives: The purpose of this study was to evaluate sleep quality and your possible correlation between comsuption of ethanol and caffeine in medical students of Faculdade de Medicina de Jundiaí. Methodology: This is a cross-sectional study with 143 medical students that answered specific questionnaires about sleep quality (Pittsburg Sleep Quality Index), alcohol comsuption (AUDIT) and caffeine comsuption. Chi-square tests and multivariate logistic regression models were used to identify statistically significant association between these variables. Results: The results showed that the prevalence of poor sleep quality (PSQI > 5 points) was found to be 65,7%; 71,3% had low risk comsuption of ethanol. There was a weak correlation (R = 0.251), however statistically significant (p = 0.020) only between sleep quality and ethanol comsuption. Conclusion: These results designate the necessity of projects and wariness to aim for promotion of medical student’s quality of life.Introdução: O consumo de etanol e café podem ser retratados como disruptores de sono principalmente em estudantes de medicina, o que pode prejudicar o bem estar e as atividades cotidianas desses graduandos. Objetivo: O objetivo deste estudo foi avaliar a qualidade de sono e sua possível correlação entre o consumo de etanol e café em estudantes da Faculdade de Medicina de Jundiaí. Modelo de estudo: Trata-se de um estudo transversal com 143 estudantes de medicina que responderam os questionários específicos referentes à qualidade de sono (PSQI–BR), consumo de etanol (AUDIT) e consumo de café. A associação entre essas variáveis foi avaliada através de testes de qui-quadrado e análise de regressão linear. Resultados: Os resultados demonstraram que 65,7% dos estudantes que responderam o questionário apresentaram índice de qualidade de sono ruim (PSQI–BR > 5 pontos), 71,3% apresentaram consumo de baixo risco de etanol. A qualidade de sono apresentou correlação fraca (R = 0.251), porém significativa (p = 0.020) somente com o consumo de etanol. Conclusão: A presença desse resultado sinaliza a necessidade de projetos e cuidados que visem a promoção da qualidade de vida dos estudantes de medicina

    Malignant Transformation Of An Odontogenic Cyst In A Period Of 10 Years.

    Primary intraosseous carcinoma of the jaws (PIOSCC) might arise from odontogenic epithelium, more commonly from a previous odontogenic cyst. The aim of this case is to illustrate that the clinician should consider that an apparent benign dentigerous cyst can suffer malignant transformation and that all material removed from a patient must be evaluated histologically. A 44-year-old man presented in a routine periapical X-ray an impacted lower left third molar with radiolucency over its crown. Ten years later, the patient complained of pain in the same region and the tooth was extracted. After one month, the patient still complained of pain and suffered a fracture of the mandible. A biopsy was performed and carcinoma was diagnosed. The patient was treated surgically with adjuvant radio- and chemotherapy and after 8 years, he is well without signs of recurrences. This report describes a central mandibular carcinoma probably developed from a previous dentigerous cyst.201476296

    Oral Metastasis of Metaplastic Breast Carcinoma in a Patient with Neurofibromatosis 1

    Neurofibromatosis type 1 (NF1) has been associated with an increased risk for development of malignancy, especially malignant peripheral nerve sheath tumors. In addition, recently, literature has demonstrated an increased risk of breast cancer in women with NF1. The present paper shows a 53-year-old woman with NF1 who presented with metaplastic breast carcinoma and developed multiple metastases, including mandible. Furthermore, we reviewed the English literature, found 63 cases showing the association between NF1 and breast cancer, and added one more case. The present study demonstrated an important association between NF1 and breast cancer. Until the present time, there has been only one case of metaplastic breast carcinoma associated with NF1. Curiously, in our case the oral metastasis corresponded to sarcomatous component of metaplastic breast carcinoma

    Biological markers and prognosis in recurrent oral cancer after salvage surgery

    Objective: To analyze the prognostic effect of epidermal growth factor receptor (EGFR), matrix metalloproteinases 2 and 9, and vascular endothelial growth factor expression in patients with locally recurrent oral carcinoma after salvage surgery. Design: Retrospective cohort study. Settings: Tertiary center cancer hospital. Patients: The charts of 111 patients with local recurrence of oral carcinomas were retrospectively analyzed. The previous treatment consisted of surgery in 33 patients (30.0%), radiotherapy with or without chemotherapy in 46 patients (41.0%), and surgery with adjuvant radiotherapy in 32 patients (29.0%). The expression of EGFR, matrix metalloproteinases 2 and 9, and vascular endothelial growth factor was analyzed with a tissue microarray immunohistochemical technique. Main Outcome Measures: Overall survival and cancer-specific survival (CSS). Results: The recurrences were diagnosed in less than 1 year in 69 patients (62.2%) and in more than 1 year in 42 patients (37.8%). The prognosis was worse in the group with the disease-free interval of less than 1 year (P=.01). Patients with more advanced disease (clinical stage of recurrence, III/IV) had worse rates of CSS (P=.04). Cases that were positive for EGFR had a 3-year CSS of 27.2%, while EGFR-negative cases had a 3-year CSS of 64.3% (P=.001). The expression of matrix metalloproteinases 2 (P=.83) and 9 (P=.15) and vascular endothelial growth factor (P=.86) was not significant in this group. In multivariate analysis, only the disease-free interval and the overexpression of EGFR were associated with a higher risk of cancer death. Conclusions: Local recurrence in oral carcinomas carries a poor prognosis. A disease-free interval of more than 1 year and a EGFR-negative expression are the main prognostic factors related to better CSS in patients treated with salvage surgery
