    Eight steps’ paradigm shift in men’s 110 metres hurdles: an 89 years retrospective study

    Men’s 110 metres hurdles is one of the oldest disciplines in the Olympic Games and yet scarce anecdotal reports are found to pinpoint technical elements indicators of success. The approach technique to the hurdles has been raised by coaches as a potential element of success however it seems that the approach strategies undertaken to the first hurdle have not been investigated to the current date. Therefore, this study aimed to examine the employment of the seven and eight steps strategy until the first hurdle in the 110 m discipline among the Olympic Games and World Championships in Athletics’ finalists. Public domain videos of the events in the period between 1924 and 2013 were assessed. An international level sprint and hurdles’ coach, using the Dartfish software, assessed the number of steps taken until the first hurdle. A left-tailed one sample sign test showed that although the seven steps strategy adoption seems to have begun in 1960 it was only in 2011 that it was considered a dominant tactic. Due to determination of factors that influence the choice of strategy or their respective influence on performance falling outside the scope of this study, future research is needed in this area. Notwithstanding, the success by the hurdlers that adopt the seven steps strategy, evidenced by the number of medals won, allowed to pinpoint the approach strategy to the first hurdle as a potential performance variable to be investigated

    Pursed-lip breathing in chronic obstructive pulmonary disease: a literature review

    Esta revisão teve por objetivo destacar os principais achados publicados nos últimos dez anos sobre os efeitos da respiração frenolabial (RFL) em pacientes com doença pulmonar obstrutiva crônica (DPOC). A busca dos artigos foi realizada nas bases de dados Lilacs, IBECS, MEDLINE e SciELO, por meio dos seguintes descritores da área da saúde (DeCS): doença pulmonar obstrutiva crônica, reabilitação, respiração, hiperinsuflação e dispneia, e suas respectivas versões na língua inglesa (MeSH), além do termo pursed-lip breathing. Após a eliminação dos títulos repetidos, foram selecionados somente os estudos que abordavam a RFL como tema principal, resultando em 12 artigos científicos, 10 ensaios clínicos e 2 revisões bibliográficas. Segundo os achados, a RFL proporciona: alterações sobre a gasometria arterial, caracterizada pelo aumento da saturação e pressão parcial de oxigênio; padrão ventilatório, com diminuição da frequência respiratória e aumento de tempo expiratório e do volume corrente; mecânica ventilatória, por meio do recrutamento de musculatura abdominal expiratória e dos músculos da caixa torácica e acessórios da inspiração; diminuição no consumo de oxigênio; alterações na modulação autonômica cardíaca induzida pelo aumento da atividade parassimpática e, por fim, melhora na qualidade de vida destes pacientes. A RFL é considerada uma manobra de grande importância, por repercutir de forma positiva em diversos sistemas e sobre a qualidade de vida de pacientes portadores da DPOC.This review aimed at standing out the main findings published in the last ten years on the effects of pursed-lip breathing (PLB) in patients with chronic obstructive pulmonary disease (COPD). The search for articles was performed in Lilacs, IBECS, MEDLINE, and SciELO database by using the descriptors of health (DeCS): doença pulmonar obstrutiva crônica, reabilitação, respiração, hiperinsuflação e dispneia, and their respective versions in the English language (MeSH), the term pursed-lip breathing was also in such list. After removing the repeated titles, only studies addressing PLB as the main theme were selected, resulting in 12 papers, 10 clinical trials, and 2 systematic reviews. According to the findings, the PLB provides: changes on arterial gases, which are characterized by increased oxygen saturation and partial pressure of oxygen; ventilatory pattern with decreased respiratory rate and increased expiratory time and tidal volume; respiratory mechanics, by recruiting the expiratory abdominal muscles and muscles of the rib cage and accessories inspired; decrease in oxygen consumption; alterations in cardiac autonomic modulation induced by increase in parasympathetic activity and, ultimately, improved quality of life of these patients. The PLB is considered a maneuver of great importance to have a positive effect on various systems and on the quality of life of patients with COPD

    Pain assessment in surgical patients with impaired cognition

    Pain is considered the 5th vital sign and its measurement/assessment and records are required and must be systematic. Ineffective pain management involves complications in clinical status of patients, longer hospitalization times and higher costs with health. In the surgical patient with impaired cognition, hetero measurements should be made, based on behavioural and physiological indicators. We used to determine the efficacy and efficiency of the Observer Scale, the Abbey Pain Scale and Pain Assessment in Advanced Dementia (PAINAD). Our study is an applied, non-experimental, quantitative, descriptive and analytical research. The data collection instrument consisted of patients’ sociodemographic and clinical data, the Observer Scale, the Abbey Pain Scale (Rodrigues, 2013) and PAINAD (Batalha et al., 2012). We assessed pain at an early phase and 45 minutes after an intervention for its relief. The sample is non-probabilistic for convenience, consisting of 76 surgical patients with impaired cognition, admitted to the surgery services of a central hospital, aged between 38 and 96 years. There was a positive correlation between the results of the three scales, most evident in the initial evaluation. Pain intensity in the same patient is higher when assessed with PAINAD (OM = 2.16) and lower when assessed with the Observer Scale (OM = 1.78). The most effective and efficient scale is PAINAD. Due to the small sample size, we suggest confirmatory studies so that the results can be generalized

    Endobronchial solitary fibrous tumor

    Solitary fibrous tumor (SFT) is a mesenchymal neoplasm that appears primarily in the pleura and rarely in intrapulmonary or endobronchial topography. The authors report the case of a 47-year-old woman who presented obstructive respiratory symptoms for 4 years. The chest computed tomography and bronchoscopy showed an obstructive polypoid lesion located between the trachea and the left main bronchus associated with distal atelectasis of the left lung. A resection of the lesion was performed and, macroscopically, the mass was oval, encapsulated, and firm, measuring 2.3 × 1.7 × 1.5 cm. Histology revealed low-grade mesenchymal spindle cell neoplasm, with alternating cellularity, myxoid areas, and mature adipose tissue outbreaks, as well as blood vessels with irregular walls. The immunohistochemical study was positive for CD34, CD99, and BCL2. The diagnosis was SFT in an unusual topography. The patient’s symptoms remitted after tumor excision, and no systemic problems were evident. SFTs primarily affect adults and often follow a benign course; however, their behavior is unpredictable. The presence of necrosis and mitotic activity may portend a poor prognosis.     Sports Injuries and Their Risk Factors in Adolescents who Practice Swimming

    Introdução: a natação é uma modalidade popular entre os adolescentes, no entanto, sua prática pode conduzir às lesões. Objetivos: caracterizar as lesões desportivas e associá-las com fatores de risco em adolescentes praticantes de natação de diferentes níveis de competitividade. Método: foram entrevistados 251 nadadores, com média de idade de 12,43±2,10 anos, ambos os sexos, recrutados ao acaso, classificadas em três níveis competitivos: categoria de iniciação, aperfeiçoamento e treinamento. Foi utilizado o inquérito de morbidade referida adaptado com as características da natação como instrumento de coleta que continha informações pessoais e de treinamento dos voluntários, como: idade, variáveis antropométricas e variáveis de treinamento e informações referentes às lesões ocorridas durante 12 meses. Todas as conclusões foram descritas no nível de 5% de significância. Resultados: Foram observadas 0,11 lesões por atleta e 1,03 lesões por atleta lesionado. Foi constatada associação entre presença e ausência de lesão com a variável estatura e tempo de treinamento. O mecanismo causal mais frequente foi o sem contato para as categorias de iniciação e treinamento. Os locais anatômicos mais acometidos foram os MMII e MMSS para todos os níveis de competitividade, assim como a gravidade leve, o treinamento e a ausência de recidiva predominaram. O retorno assintomático foi mais frequente para a iniciação, enquanto que o retorno sintomático prevaleceu na categoria de treinamento. Conclusão: a taxa de lesão nos adolescentes nadadores foi baixa. As variáveis estatura e tempo de treinamento foram associadas à ocorrência dos agravos. As características das lesões dependeram do nível de competitividade dos nadadores.Introduction: swimming is a popular sport among adolescents; however, your practice can lead to injuries. Objectives: to characterize the sport injuries and associate them with risk factors in teenage swimmers from different levels of competitiveness. Methods: 251 swimmers were interviewed, it an average age of 12.43±2.10, both sexes, recruited randomly and they were classified into three competitive levels: initiation, development, and training category. It wasused the morbidity survey adapted to the characteristics of swimming as collection instrument containing personal and training of volunteers data, as age, anthropometric variables and training variables and information relating to injuries sustained during a period of 12 months. All findings were described at the 5% significance level. Results: It was verified that there is an associationbetween the presence and absence of injury with the variable height and variable time. The causal mechanism most common was the non-contact mechanism for the categories of initiation and training. The most affected anatomical site was the lower limb and upper limb for all competitive levels. The mild severity, the training time and the absence of recurrence predominated. The asymptomatic return was more frequent for initiation, whereas the symptomatic return prevailed in the training category. Conclusion: the injury rate in teenage swimmers was low. The variables height and training time were associated with the occurrence of injuries. The characteristics of the injuries depended on the competitive level of the swimmers

    Microtomographic, histomorphometric, and molecular features show a normal alveolar bone healing process in iNOS-deficient mice along a compensatory upregulation of eNOS and nNOS isoforms

    Inducible nitric oxide synthase (iNOS) is one of the enzymes responsible for the synthesis of nitric oxide (NO), which is an important signaling molecule with effects on blood vessels, leukocytes, and bone cells. However, the role of iNOS in alveolar bone healing remains unclear. This study investigated the role of iNOS in alveolar bone healing after tooth extraction in mice. Methodology: C57Bl/6 wild type (WT) and iNOS genetically deficient (iNOS-KO) mice were subjected to upper incision tooth extraction, and alveolar bone healing was evaluated by micro-computed tomography (μCT) and histological/histomorphometric, birefringence, and molecular methods. Results: The expression of iNOS had very low control conditions, whereas a significant increase is observed in healing sites of WT mice, where iNOS mRNA levels peak at 7d time point, followed by a relative decrease at 14d and 21d. Regarding bone healing, both WT and iNOS-KO groups showed the usual phases characterized by the presence of clots, granulation tissue development along the inflammatory cell infiltration, angiogenesis, proliferation of fibroblasts and extracellular matrix synthesis, bone neoformation, and remodeling. The overall micro-computed tomography and histomorphometric and birefringence analyses showed similar bone healing readouts when WT and iNOS-KO strains are compared. Likewise, Real-Time PCR array analysis shows an overall similar gene expression pattern (including bone formation, bone resorption, and inflammatory and immunological markers) in healing sites of WT and iNOS-KO mice. Moreover, molecular analysis shows that nNOS and eNOS were significantly upregulated in the iNOS-KO group, suggesting that other NOS isoforms could compensate the absence of iNOS. Conclusion: The absence of iNOS does not result in a significant modulation of bone healing readouts in iNOS-KO mice. The upregulation of nNOS and eNOS may compensate iNOS absence, explaining the similar bone healing outcome in WT and iNOS-KO strains
