    Extracorporeal Cardiopulmonary Resuscitation

    ECMO, or extracorporeal membrane oxygenation, is an advanced life support technique that provides cardiac and pulmonary support similar to cardiopulmonary bypass. ECPR (extracorporeal cardiopulmonary resuscitation) is the rapid deployment of VA-ECMO when conventional cardiopulmonary resuscitation fails to provide return of spontaneous circulation. Evidence in the literature is sparse, but with expanding reported applications, ECPR has shown promise to improve outcomes of cardiac arrest. ECPR is superior to conventional CPR for both survival and neurologic outcomes. ECPR has been successfully used to manage arrests secondary to cardiac and non-cardiac causes. Arrests secondary to primary cardiac causes have the best overall outcome. Other determinants of outcomes of ECPR include duration of low flow state and on-ECMO complications. A narrow list of ECPR contraindications exists, and includes severe neurologic injury and irreversible primary disease process. Various complications can occur with ECPR, and include mechanical, cardiovascular, pulmonary, hematologic, renal, and neurologic complications. Neurologic complications are the most serious, and significantly affect mortality or quality of life. ECPR is a nascent field, and substantial work remains to be done to optimize its application. Given the small number of patients at each institutional level, this is a field ripe for collaborative work and rewarding results

    Teaching institutions have realized that it is necessary to use new and propositive tools to train students to interact with this type of instruments to make them more competitive and facilitate their learning. The objective of the investigation was to determine the possibilities in the management of virtual worlds as a training environment in the Faculty of Computer Science, Electronics and Communications of the University of Panama according to gender. A survey was used to verify the use of virtual worlds as a training environment. The use of virtual worlds and the possibilities of using them as a training environment indicates that there is much willingness to use this environment in the University of Panama. The Faculty of Informatics, Electronics and Communications is a reliable sample that studies at this level should be given. All this implies that the study programs and modalities must be adapted in order to be able to work with a pilot plan.Las instituciones de enseñanza se han dado cuenta que es necesario el uso de herramientas nuevas y propositivas para ir formando estudiantes que interactúen con este tipo de instrumentos para hacerlos más competitivos y facilitar su aprendizaje. El objetivo de la investigación fue determinar las posibilidades en el manejo de los mundos virtuales como entorno de formación en la Facultad de Informática, Electrónica y Comunicaciones Universidad de Panamá, según género. Se utilizo una encuesta para constatar el uso de los mundos virtuales como entorno de formación. Las posibilidades de uso de los mundos virtuales como entorno de formación, nos indica que hay mucha disposición para que se pueda utilizar dicho entorno en la Universidad de Panamá. La facultad de Informática, Electrónica y comunicaciones es una muestra fehaciente de que los estudios a este nivel deben darse. Todo esto implica que se deben adecuar los programas de estudios y las modalidades, para así poder trabajar con un plan piloto

    The objective of this research is to evaluate different eCommerce in Panamanian companies, with KPI indicators, where the different evaluation items will be shown using online tools. Currently, due to the current events, the creation of ecommerce to offer different products and services has increased. For this research 56 companies that offer ecommerce in Panama were used, divided into different categories such as: Restaurants, Banks, Universities, Hotels among others.  A cross-sectional time horizon will be used, in addition, the research strategy to be implemented will be theoretical. The approach of this research will be deductive by case analysis. It is intended that this research is achieved, to know how Panamanian companies are in terms of ecommerce, using KPI indicators.El objetivo de esta investigación es evaluar distintos eCommerce en empresas  panameñas, con indicadores de KPI, en donde se mostrará los diferentes item de evaluación utilizandos herramientas que se encuentran en línea. Actualmente por los acontecimientos se ha poliferado la creación de ecommerce para ofrecer diferentes productos y servicios. Para esta investigación se utilizaron 56 empresas que ofrecen ecommerce en Panamá, divididas en diferentes rubros como: Restaurantes, Bancos, Universidadesm, Hoteles entre otras.  Se manejará un horizonte de tiempo de tipo transversal, además, la estrategia de investigación a implementar será la teórica. El enfoque de esta investigación será deductivo por análisis de casos.     Pediatric Cardiac Arrest

    This chapter will focus on four important topics in pediatric cardiac arrest. We will highlight recent developments in pediatric CPR quality, medications used in cardiac arrest, ECPR, and post-cardiac arrest care (PCAC) and discuss the existing literature behind AHA guidelines and gaps in knowledge. Optimization of CPR quality is critical during cardiac arrest. We will summarize literature regarding current guidelines which target provider-centered goals and discuss evidence behind patient-centered goals. We will also discuss the evidence behind drugs used in the PALS guidelines. In cases of refractory cardiac arrest, ECMO can be lifesaving; however, there are still many gaps in our knowledge of this field. We will summarize the literature regarding determination of candidacy, cannulation strategies, resuscitation practices during ECPR, and outcomes. After a cardiac arrest, PCAC is crucial to minimize further injury from post-cardiac arrest syndrome (PCAS). The main goals of PCAC are to prevent further brain injury, treat myocardial dysfunction, and systemic ischemia/reperfusion injury. We will discuss AHA guidelines on oxygenation and ventilation goals, targeted temperature management, hemodynamic monitoring, and neuromonitoring

    Common Molecular Pathways Mediate Long-Term Potentiation of Synaptic Excitation and Slow Synaptic Inhibition

    SummarySynaptic plasticity, the cellular correlate for learning and memory, involves signaling cascades in the dendritic spine. Extensive studies have shown that long-term potentiation (LTP) of the excitatory postsynaptic current (EPSC) through glutamate receptors is induced by activation of N-methyl-D-asparate receptor (NMDA-R)—the coincidence detector—and Ca2+/calmodulin-dependent protein kinase II (CaMKII). Here we report that the same signaling pathway in the postsynaptic CA1 pyramidal neuron also causes LTP of the slow inhibitory postsynaptic current (sIPSC) mediated by metabotropic GABAB receptors (GABAB-Rs) and G protein-activated inwardly rectifying K+ (GIRK) channels, both residing in dendritic spines as well as shafts. Indicative of intriguing differences in the regulatory mechanisms for excitatory and inhibitory synaptic plasticity, LTP of sIPSC but not EPSC was abolished in mice lacking Nova-2, a neuronal-specific RNA binding protein that is an autoimmune target in paraneoplastic opsoclonus myoclonus ataxia (POMA) patients with latent cancer, reduced inhibitory control of movements, and dementia

    Effect of angiotensin-converting enzyme inhibitor and angiotensin receptor blocker initiation on organ support-free days in patients hospitalized with COVID-19

    IMPORTANCE Overactivation of the renin-angiotensin system (RAS) may contribute to poor clinical outcomes in patients with COVID-19. Objective To determine whether angiotensin-converting enzyme (ACE) inhibitor or angiotensin receptor blocker (ARB) initiation improves outcomes in patients hospitalized for COVID-19. DESIGN, SETTING, AND PARTICIPANTS In an ongoing, adaptive platform randomized clinical trial, 721 critically ill and 58 non–critically ill hospitalized adults were randomized to receive an RAS inhibitor or control between March 16, 2021, and February 25, 2022, at 69 sites in 7 countries (final follow-up on June 1, 2022). INTERVENTIONS Patients were randomized to receive open-label initiation of an ACE inhibitor (n = 257), ARB (n = 248), ARB in combination with DMX-200 (a chemokine receptor-2 inhibitor; n = 10), or no RAS inhibitor (control; n = 264) for up to 10 days. MAIN OUTCOMES AND MEASURES The primary outcome was organ support–free days, a composite of hospital survival and days alive without cardiovascular or respiratory organ support through 21 days. The primary analysis was a bayesian cumulative logistic model. Odds ratios (ORs) greater than 1 represent improved outcomes. RESULTS On February 25, 2022, enrollment was discontinued due to safety concerns. Among 679 critically ill patients with available primary outcome data, the median age was 56 years and 239 participants (35.2%) were women. Median (IQR) organ support–free days among critically ill patients was 10 (–1 to 16) in the ACE inhibitor group (n = 231), 8 (–1 to 17) in the ARB group (n = 217), and 12 (0 to 17) in the control group (n = 231) (median adjusted odds ratios of 0.77 [95% bayesian credible interval, 0.58-1.06] for improvement for ACE inhibitor and 0.76 [95% credible interval, 0.56-1.05] for ARB compared with control). The posterior probabilities that ACE inhibitors and ARBs worsened organ support–free days compared with control were 94.9% and 95.4%, respectively. Hospital survival occurred in 166 of 231 critically ill participants (71.9%) in the ACE inhibitor group, 152 of 217 (70.0%) in the ARB group, and 182 of 231 (78.8%) in the control group (posterior probabilities that ACE inhibitor and ARB worsened hospital survival compared with control were 95.3% and 98.1%, respectively). CONCLUSIONS AND RELEVANCE In this trial, among critically ill adults with COVID-19, initiation of an ACE inhibitor or ARB did not improve, and likely worsened, clinical outcomes.
