    Quantifying the If, the When, and the What of the Sublime: A Survey and Latent Class Analysis of Incidence, Emotions, and Distinct Varieties of Personal Sublime Experiences

    Sublime encounters provide a compelling example of the peaks of our shared emotional and cognitive experiences. For centuries, these have been a target for philosophy and, more recently, for psychology, with its renewed focus on profound or aesthetic events. The sublime has been theoretically connected to multiple contexts, from interactions with overpowering nature, to beauty, music, even interpersonal engagements, and to multiple emotions—danger, awe, pleasure, fear—often with diametrically opposing arguments for what constitutes these events. However, despite this prolonged discussion, there is still a scarcity of actual systematic research. It is not known whether sublime encounters are common, nor how they are described by individuals, or if reports match theoretical arguments: Are there one or more, or no, distinct sublime types? We address these questions by matching historical discussions to 402 participants’ (Western adults) reports of whether they have ever experienced the sublime and, if so, how these are described in terms of cognitive/emotional and contextual factors. Roughly half reported having had at least one sublime experience, with accounts involving a range of contexts that essentially cover the full spectrum of past theoretical arguments. At the same time, when we considered the cognitive/affective descriptions using network science and latent class analysis of reported feelings, 90.8% fit one model, with involved communities (or interrelated clusters) of positive emotions, discrepancy, self-awareness, transformation/insight, and, notably, not including negative emotions/fear. We conclude with a discussion of how this approach and findings might be used as a basis for considering sublime theory and shaping future research

    Network structure of the Wisconsin Schizotypy Scales–Short Forms: Examining psychometric network filtering approaches

    Schizotypy is a multidimensional construct that provides a useful framework for understanding the etiology, development, and risk for schizophrenia-spectrum disorders. Past research has applied traditional methods, such as factor analysis, to uncovering common dimensions of schizotypy. In the present study, we aimed to advance the construct of schizotypy, measured by the Wisconsin Schizotypy Scales-Short Forms (WSS-SF), beyond this general scope by applying two different psychometric network filtering approaches-the state-of-the-art approach (lasso), which has been employed in previous studies, and an alternative approach (information-filtering networks; IFNs). First, we applied both filtering approaches to two large, independent samples of WSS-SF data (ns = 5,831 and 2,171) and assessed each approach's representation of the WSS-SF's schizotypy construct. Both filtering approaches produced results similar to those from traditional methods, with the IFN approach producing results more consistent with previous theoretical interpretations of schizotypy. Then we evaluated how well both filtering approaches reproduced the global and local network characteristics of the two samples. We found that the IFN approach produced more consistent results for both global and local network characteristics. Finally, we sought to evaluate the predictability of the network centrality measures for each filtering approach, by determining the core, intermediate, and peripheral items on the WSS-SF and using them to predict interview reports of schizophrenia-spectrum symptoms. We found some similarities and differences in their effectiveness, with the IFN approach's network structure providing better overall predictive distinctions. A new method, based on monitoring the equivalent widths of strong lines, shows promise but is yet to be confirmed.     Consensus review of best practice of transanal irrigation in adults

    Study design: Review article. Objectives: To provide a consensus expert review of the treatment modality for transanal irrigation (TAI). Methods: A consensus group of specialists from a range of nations and disciplines who have experience in prescribing and monitoring patients using TAI worked together assimilating both the emerging literature and rapidly accruing clinical expertise. Consensus was reached by a round table discussion process, with individual members leading the article write-up in the sections where they had particular expertise. Results: Detailed trouble-shooting tips and an algorithm of care to assist professionals with patient selection, management and follow-up was developed. Conclusion: This expert review provides a practical adjunct to training for the emerging therapeutic area of TAI. Careful patient selection, directly supervised training and sustained follow-up are key to optimise outcomes with the technique. Adopting a tailored, stepped approach to care is important in the heterogeneous patient groups to whom TAI may be applied. Sponsorship: The review was financially supported by Coloplast A/S.     A falls prevention programme to improve quality of life, physical function and falls efficacy in older people receiving home help services: study protocol for a randomised controlled trial

    BACKGROUND: Falls and fall-related injuries in older adults are associated with great burdens, both for the individuals, the health care system and the society. Previous research has shown evidence for the efficiency of exercise as falls prevention. An understudied group are older adults receiving home help services, and the effect of a falls prevention programme on health-related quality of life is unclear. The primary aim of this randomised controlled trial is to examine the effect of a falls prevention programme on quality of life, physical function and falls efficacy in older adults receiving home help services. A secondary aim is to explore the mediating factors between falls prevention and health-related quality of life. METHODS: The study is a single-blinded randomised controlled trial. Participants are older adults, aged 67 or older, receiving home help services, who are able to walk with or without walking aids, who have experienced at least one fall during the last 12 months and who have a Mini Mental State Examination of 23 or above. The intervention group receives a programme, based on the Otago Exercise Programme, lasting 12 weeks including home visits and motivational telephone calls. The control group receives usual care. The primary outcome is health-related quality of life (SF-36). Secondary outcomes are leg strength, balance, walking speed, walking habits, activities of daily living, nutritional status and falls efficacy. All measurements are performed at baseline, following intervention at 3 months and at 6 months' follow-up. Sample size, based on the primary outcome, is set to 150 participants randomised into the two arms, including an estimated 15-20% drop out. Participants are recruited from six municipalities in Norway. DISCUSSION: This trial will generate new knowledge on the effects of an exercise falls prevention programme among older fallers receiving home help services. This knowledge will be useful for clinicians, for health managers in the primary health care service and for policy makers

    Escaping the Ashby limit for mechanical damping/stiffness trade-off using a constrained high internal friction interfacial layer.

    The development of new materials with reduced noise and vibration levels is an active area of research due to concerns in various aspects of environmental noise pollution and its effects on health. Excessive vibrations also reduce the service live of the structures and limit the fields of their utilization. In oscillations, the viscoelastic moduli of a material are complex and it is their loss part - the product of the stiffness part and loss tangent - that is commonly viewed as a figure of merit in noise and vibration damping applications. The stiffness modulus and loss tangent are usually mutually exclusive properties so it is a technological challenge to develop materials that simultaneously combine high stiffness and high loss. Here we achieve this rare balance of properties by filling a solid polymer matrix with rigid inorganic spheres coated by a sub-micron layer of a viscoelastic material with a high level of internal friction. We demonstrate that this combination can be experimentally realised and that the analytically predicted behaviour is closely reproduced, thereby escaping the often termed 'Ashby' limit for mechanical stiffness/damping trade-off and offering a new route for manufacturing advanced composite structures with markedly reduced noise and vibration levels

    Circuit dissection of the role of somatostatin in itch and pain

    Stimuli that elicit itch are detected by sensory neurons that innervate the skin. This information is processed by the spinal cord; however, the way in which this occurs is still poorly understood. Here we investigated the neuronal pathways for itch neurotransmission, particularly the contribution of the neuropeptide somatostatin. We find that in the periphery, somatostatin is exclusively expressed in Nppb+ neurons, and we demonstrate that Nppb+somatostatin+ cells function as pruriceptors. Employing chemogenetics, pharmacology and cell-specific ablation methods, we demonstrate that somatostatin potentiates itch by inhibiting inhibitory dynorphin neurons, which results in disinhibition of GRPR+ neurons. Furthermore, elimination of somatostatin from primary afferents and/or from spinal interneurons demonstrates differential involvement of the peptide released from these sources in itch and pain. Our results define the neural circuit underlying somatostatin-induced itch and characterize a contrasting antinociceptive role for the peptide
