    Comparison of domiciliary oxygen using liquid oxygen and concentrator in northern Taiwan

    Background/PurposeLong-term oxygen therapy has become standard treatment for patients with chronic respiratory insufficiency. However, patterns of long-term home oxygen therapy have not been well studied in Taiwan. Oxygen concentrator systems are commonly used in Taiwan, but liquid oxygen delivery systems are portable and may provide advantages over the concentrator system. This study compared oxygen usage between patients from a liquid oxygen group (LOG) and an oxygen concentrator group (OCG). The authors also assessed the physiologic responses of patients with chronic obstructive pulmonary disease (COPD) to ambulatory oxygen use at home.MethodsThe study used a retrospective, cross-sectional, observational survey design. The LOG comprised 42 patients, and the OCG comprised 102 patients. We recruited participants in northern Taiwan from July 2009 to April 2010. The questionnaire instruments that were used to collect data consisted of three parts: demographic characteristics, devices used in respiratory care, and activity status with portable oxygen. Two-minute walking tests were performed on COPD patients in their homes.ResultsCOPD was the most common diagnosis in our study, with more than 50% of patients who received oxygen long term in both groups having received this diagnosis. The LOG used oxygen for an average of 21.7 hours per day, whereas OCG averaged 15.2 hours per day (p<0.001). In the OCG, 92.2% of patients used a concentrator alone, whereas 23.8% of the LOG used liquid oxygen alone (p<0.001). The LOG patients were involved in significantly more outdoors activities (p=0.002) and reported traveling with oxygen more often (p<0.001) than the OCG patients. For patients with the same dyspnea level of COPD severity, those using liquid oxygen had a lower increase in pulse rate after the walking test, in comparison with the concentrator users.ConclusionPatients in the LOG used oxygen for longer hours, went on more outings, and were more likely to travel with oxygen than patients in the OCG. Being ambulatory with liquid oxygen might enable patients with COPD to walk more effectively

    Hospital treatment, mortality and healthcare costs in relation to socioeconomic status among people with bipolar affective disorder

    BACKGROUND: Evidence regarding the relationships between the socioeconomic status and long-term outcomes of individuals with bipolar affective disorder (BPD) is lacking. AIMS: We aimed to estimate the effects of baseline socioeconomic status on longitudinal outcomes. METHOD: A national cohort of adult participants with newly diagnosed BPD was identified in 2008. The effects of personal and household socioeconomic status were explored on outcomes of hospital treatment, mortality and healthcare costs, over a 3-year follow-up period (2008–2011). RESULTS: A total of 7987 participants were recruited. The relative risks of hospital treatment and mortality were found elevated for the ones from low-income households who also had higher healthcare costs. Low premium levels did not correlate with future healthcare costs. CONCLUSIONS: Socioeconomic deprivation is associated with poorer outcome and higher healthcare costs in BPD patients. Special care should be given to those with lower socioeconomic status to improve outcomes with potential benefits of cost savings in the following years. DECLARATION OF INTEREST: None.     Blockchain-enabled transparent traffic enforcement for sustainable road safety in cities

    With the progress of the times, cars have become an important means of transportation in our lives. However, with new cars of all kinds, road safety issues still cannot be effectively improved. While countries have responded by enforcing traffic laws and using electronic technology to enhance enforcement, challenges still need to be addressed, including blurred images and misjudgments by police agencies, and even exploiting loopholes in traffic enforcement frameworks to evade consequences through bribery or connections. exacerbating an accident and leaving victims and families in the lurch of the aftermath. To solve this problem, we proposed a data traceability law enforcement system based on blockchain and InterPlanetary File System (IPFS). The system ensures rapid traceability and protects law enforcement data from external attacks. The goal of leveraging blockchain’s decentralized nature and smart contracts is to instill accountability and fairness in road safety measures, mitigate the effects of corruption, and pave the way for a safer and more just transportation environment. Our experimental results show that under our stress test of 50 transactions per second, the throughput can be as high as 300 and the maximum delay can reach 2.01s, which is enough to experimentally prove that our system is feasible. Our approach is designed to solve the data corruption problem caused by the centralized server being paralyzed. It applies to all technology enforcement, with high flexibility and scalability for all participants to join or set privileges. we also provide an automated traffic law enforcement system in a smart city. To make the data storage more secure and fair, and not easily damaged by malicious people or tampered with by intentional attackers. This brings a breakthrough to the country’s program security

    HHP1 is involved in osmotic stress sensitivity in Arabidopsis

    HHP1 (heptahelical protein 1), a protein with a predicted seven transmembrane domain structure homologous to adiponectin receptors (AdipoRs) and membrane progestin receptors (mPRs), has been characterized. Expression of HHP1 was increased in response to abscisic acid (ABA) and salt/osmotic stress as shown by quantitative real-time PCR and HHP1 promoter-controlled GUS activity. The HHP1 T-DNA insertion mutant (hhp1-1) showed a higher sensitivity to ABA and osmotic stress than the wild-type (WT), as revealed by the germination rate and post-germination growth rate. The induced expression of stress-responsive genes (RD29A, RD29B, ADH1, KIN1, COR15A, and COR47) was more sensitive to exogenous ABA and osmotic stress in hhp1-1 than in the WT. The hypersensitivity in the hhp1-1 mutant was reversed in the complementation mutant of HHP1 expressing the HHP1 gene. The data suggest that the mutation of HHP1 renders plants hypersensitive to ABA and osmotic stress and HHP1 might be a negative regulator in ABA and osmotic signalling

    WBANs (Wireless Body Area Networks) are frequently depicted as a paradigm shift in healthcare from traditional to modern E-Healthcare. The vitals of the patient signs by the sensors are highly sensitive, secret, and vulnerable to numerous adversarial attacks. Since WBANs is a real-world application of the healthcare system, it’s vital to ensure that the data acquired by the WBANs sensors is secure and not accessible to unauthorized parties or security hazards. As a result, effective signcryption security solutions are required for the WBANs’ success and widespread use. Over the last two decades, researchers have proposed a slew of signcryption security solutions to achieve this goal. The lack of a clear and unified study in terms of signcryption solutions can offer a bird’s eye view of WBANs. Based on the most recent signcryption papers, we analyzed WBAN’s communication architecture, security requirements, and the primary problems in WBANs to meet the aforementioned objectives. This survey also includes the most up to date signcryption security techniques in WBANs environments. By identifying and comparing all available signcryption techniques in the WBANs sector, the study will aid the academic community in understanding security problems and causes. The goal of this survey is to provide a comparative review of the existing signcryption security solutions and to analyze the previously indicated solution given for WBANs. A multi-criteria decision-making approach is used for a comparative examination of the existing signcryption solutions.     Novel variant Pro143Ala in HTRA2 contributes to Parkinson’s disease by inducing hyperphosphorylation of HTRA2 protein in mitochondria

    Mutations in the gene encoding the mitochondrial protein high temperature requirement A2 (HTRA2) are inconsistently associated with a risk of Parkinson’s disease (PD). We assessed the presence of HTRA2 mutations among patients with PD and performed functional assay of identified mutations or variants. Among the total 1,373 subjects, the entire HTRA2 coding region was sequenced in 113 early-onset PD (EOPD), 20 familial PD patients and 150 control subjects. An additional 390 sporadic late-onset PD patients and 700 controls were subsequently screened to validate possible mutations found in the first set. We identified two novel heterozygous variants, c.427C > G (Pro143Ala) and c.906 +3 G > A, in 2 (1.5%) EOPD patients. The missense variant, Pro143Ala, was also observed in one late-onset PD patient but was absent in total 850 control subjects (relative risk 2.3, 95% CI 1.5–2.8, P = 0.04). Expressing Pro143Ala variant of HTRA2 in primary dopaminergic neurons causes neurite degeneration. Following exposure to rotenone, the ultra-structural mitochondrial abnormality, the percentage of mitochondrial dysfunction and apoptosis in cells carrying the HTRA2 Pro143Ala variant was significantly higher than wild-type cells. Mechanistically, protein level of phosphorylated HTRA2 was increased in cells carrying the Pro143Ala variant, suggesting Pro143Ala variant promotes HTRA2 phosphorylation with resultant mitochondrial dysfunction. Our results support a biologically relevant role of HTRA2 in PD susceptibility in Taiwanese. Further large-scale association studies are warranted to confirm the role of HTRA2 Pro143Ala variant in the risk of PD
