Hospital treatment, mortality and healthcare costs in relation to socioeconomic status among people with bipolar affective disorder
BACKGROUND: Evidence regarding the relationships between the socioeconomic status and long-term outcomes of individuals with bipolar affective disorder (BPD) is lacking. AIMS: We aimed to estimate the effects of baseline socioeconomic status on longitudinal outcomes. METHOD: A national cohort of adult participants with newly diagnosed BPD was identified in 2008. The effects of personal and household socioeconomic status were explored on outcomes of hospital treatment, mortality and healthcare costs, over a 3-year follow-up period (2008ā2011). RESULTS: A total of 7987 participants were recruited. The relative risks of hospital treatment and mortality were found elevated for the ones from low-income households who also had higher healthcare costs. Low premium levels did not correlate with future healthcare costs. CONCLUSIONS: Socioeconomic deprivation is associated with poorer outcome and higher healthcare costs in BPD patients. Special care should be given to those with lower socioeconomic status to improve outcomes with potential benefits of cost savings in the following years. DECLARATION OF INTEREST: None. Distribution specificities of long-period comets' perihelia. Hypothesis of the large planetary body on the periphery of the Solar System
The present paper reviews selected aspects of the Guliyev's hypothesis about
the massive celestial body at a distance of 250-400 AU from the Sun as well as
the factor of comets transfer. It is shown, that the conjecture of the point
around which cometary perihelia might be concentrated, is not consistent. On
the issue of perihelia distribution, priority should be given to the assumption
that there is a plane or planes around which the concentration takes place. A
total of 24 comet groups were investigated. In almost all cases there are
detected two types of planes or zones: the first one is very close to the
ecliptic, another one is about perpendicular to it and has the parameters: ip =
86{\deg}, {\Omega}p = 271.7{\deg}. The existence of the first area appears to
be related to the influence of giant planets. The Guliyev's hypothesis says
that there is a massive perturber in the second zone, at a distance of 250-400
AU. It shows that number of aphelia and distant nodes of cometary orbits in
this interval significantly exceeds the expected background. Analysis of the
angular parameters of the comets, calculated relative to the second plane
(reference point is the ascending node of a large circle) displays clear
patterns: shortage of comets near i' = 180{\deg}, excess of them near B'=
0{\deg} (ecliptic latitude of perihelion) and shortage near B'=-90{\deg}. The
analysis also shows irregularity of distant nodes, overpopulation of perihelion
longitudes in the range 350{\deg}-20{\deg}. Plotted distributions of aphelia
N(Q) and distant cometary nodes clearly indicate a perturbation of the natural
course near 300 AU. On the basis of collected cometary data, we have estimated
orbital elements of the hypothetical planetary body: a = 337 AU; e = 0.14;
{\omega} = 57{\deg}; {\Omega} = 272.7{\deg}; i = 86{\deg}
Ocular post-transplant lymphoproliferative disorder
AbstractWe report a case of an iris tumor with mutton-fat keratic precipitates in a young patient after liver transplantation surgery. A 6-year-old girl underwent liver transplantation for congenital biliary atresia and was subsequently immunosuppressed with oral cyclosporine. We examined her 5 years after transplantation because of a āwhite nodule in her left eye,ā which had been detected by her father one day before visiting our clinic. Ophthalmological examinations revealed symmetric visual acuity and normal afferent papillary reflex. Slit-lamp examination revealed a depigmented iris nodule approximately3 Ć 2 mm with mutton-fat keratic precipitates in the anterior chamber. Fundus examination was unremarkable, and computed tomography (CT) of the head, neck, and abdomen showed normal findings. Based on the suspicion of post-transplant lymphoproliferative disorder (PTLD), therapy was initiated, which included tapering cyclosporine and topical mydriatics. After 2.5 months, the lesion resolved and no more mutton-fat keratic precipitates were identified in the anterior chamber. In this PTLD case, the patient presented with an iris nodule and mutton-fat keratic precipitates, and the ocular PTLD presentation resolved spontaneously after tapering cyclosporine
Lipid-related residual risk and renal function for occurrence and prognosis among patients with first-event acute coronary syndrome and normal LDL cholesterol
<p>Abstract</p> <p>Background</p> <p>We investigated relationship of low levels of high density lipoprotein cholesterol (HDL-C), high levels of triglycerides, and renal function for the odds, prognosis and survival following acute coronary events among patients with a first event and normal low density lipoprotein cholesterol levels.</p> <p>Results</p> <p>A case-control study based on 557 patients and 1086 matched control subjects was conducted. Case patients were followed up for survival with a median of 1.9 years. Participants in the higher quintiles of HDL-C had lower odds to develop acute coronary events (the adjusted odds ratios were 0.24 for the second, 0.24 for the third, 0.10 for the fourth and 0.05 for the fifth quintile). Patients with normal glomerular filtration rate were at a lower risk for all-cause death. However, a reverse association between triglycerides and death risk was found: patients with higher triglycerides were at a lower risk for all-cause death (adjusted relative risk, 0.38 for triglycerides ranging from 82 to 132.9 mg/dL, and 0.14 for triglycerides > = 133 mg/dL).</p> <p>Conclusions</p> <p>Low HDL-C was significantly associated with acute coronary events, and triglyceride levels as well as renal function were inversely related to all-cause deaths after the coronary event.</p
Dementia Assessment Using Mandarin Speech with an Attention-based Speech Recognition Encoder
Dementia diagnosis requires a series of different testing methods, which is
complex and time-consuming. Early detection of dementia is crucial as it can
prevent further deterioration of the condition. This paper utilizes a speech
recognition model to construct a dementia assessment system tailored for
Mandarin speakers during the picture description task. By training an
attention-based speech recognition model on voice data closely resembling
real-world scenarios, we have significantly enhanced the model's recognition
capabilities. Subsequently, we extracted the encoder from the speech
recognition model and added a linear layer for dementia assessment. We
collected Mandarin speech data from 99 subjects and acquired their clinical
assessments from a local hospital. We achieved an accuracy of 92.04% in
Alzheimer's disease detection and a mean absolute error of 9% in clinical
