    A cystic dilatation of the terminal portion of the minor pancreatic duct (duct of Santorini) is referred to as a santorinicele. It is usually associated with pancreas divisum and has been suggested to be a cause of relative stenosis of the minor papilla, often leading to recurrent pancreatitis. While this anomaly has been reported in the paediatric population, it is more commonly found in the elderly.     Menstrual health management: Knowledge and practices among adolescent girls

    Objectives: To assess the knowledge and attitude of adolescent girls regarding menstruation and menstrual hygiene.Material and Methods: After approval by the ethical committee , the study was conducted on 340 adolescent girls aged 12-19 years, belonging to an urban slum area of Delhi. This was a questionnaire based cross-sectional study conducted over a period of 9 months in a tertiary hospital. Confidentiality of the subjects was ensured.Results: Out of 340 girls 69 % were between 16-19 years. Source of information about menstruation in majority of the cases was mother (60%). Only 48% girls knew menstruation before menarche and 24% girls knew uterus as the organ for menstruation. 71 % girls used sterile sanitary napkins as absorbent. Satisfactory changing of pads (>2pads/day) was done by 68 % and 77% girls cleaned the genitalia satisfactorily (>2 times/day). 41% girls were aware of the fact that unhygienic use of pad could be a source of genital infection and 8% girls had associated vaginal discharge. Social restrictions during menses in the form of religious activities, drop out from school, avoiding certain foods etc were practiced in many families.Conclusion: Although the menstrual practices appear to be satisfactory in major percentage of girls but knowledge regarding menstrual hygiene is worrisome, as maximum girls are unaware of menarche and physiology of menstruation. Inclusion of such information in the school curriculum and wider coverage in mass media will help to bridge this gap.Keywords: Adolescence; hygiene; menstruation healt

    On the optimality of gluing over scales

    We show that for every α>0\alpha > 0, there exist nn-point metric spaces (X,d) where every "scale" admits a Euclidean embedding with distortion at most α\alpha, but the whole space requires distortion at least Ω(αlogn)\Omega(\sqrt{\alpha \log n}). This shows that the scale-gluing lemma [Lee, SODA 2005] is tight, and disproves a conjecture stated there. This matching upper bound was known to be tight at both endpoints, i.e. when α=Θ(1)\alpha = \Theta(1) and α=Θ(logn)\alpha = \Theta(\log n), but nowhere in between. More specifically, we exhibit nn-point spaces with doubling constant λ\lambda requiring Euclidean distortion Ω(logλlogn)\Omega(\sqrt{\log \lambda \log n}), which also shows that the technique of "measured descent" [Krauthgamer, et. al., Geometric and Functional Analysis] is optimal. We extend this to obtain a similar tight result for LpL_p spaces with p>1p > 1.Comment: minor revision

    Effect of plant growth regulators on flowering behavior of cashew cv. Vengurla-4 grown in the hilly tracts of South Gujarat

    A trial was conducted at Subhir and Chikhalda locations in Dang district of South Gujarat, India to assess the effect of Ethrel, NAA and GA3 on the flowering behavior of cashew cultivar Vengurla-4 during 2013-14. Three concentrations each of GA3 (50, 75, 100 ppm), Ethrel (10, 30, 50 ppm) and NAA (50, 75, 100ppm) were applied as foliar sprays 20 days before blossoming and 20 days after full bloom in twenty year old trees of cashew cultivar Vengurla-4. Trees sprayed with 50 ppm Ethrel had significantly the highest number of flowering panicles per squaremeter (13.09), number of perfect flowers per panicle (87.11) and sex ratio (0.24) across locations and in pooled data. However, this was at par with 10 ppm Ethrel which emerged as the second best treatment of the trial. This study demonstrated the potential of Ethrel in improving various flowering parameters of cashew which are important determinations in increasing nut production

    Dynamic changes in biochemical markers of renal function with thyroid status- A study in Indian population

    Thyroid dysfunction is known to cause significant changes in glomerular filtration rate. The present cross-sectional study was performed to evaluate the changes in biochemical markers of renal function in hypothyroid subjects before and after treatment. Thyroid function tests (T3, T4 and TSH levels) were assayed in 385 subjects. Based on TSH levels, subjects were classified as euthyroid (n=198), sub-clinical hypothyroid (n=98; TSH 6.1 to 19.9 μIU/ml) and overt hypothyroid (n=89; TSH ≥ 20 μIU/ml, abnormally low T4 levels). Forty-eight hypothyroid patients were re-evaluated after 3 months of thyroxine replacement therapy. Renal function tests were carried out in all subjects and statistically analyzed. Serum creatinine was significantly increased in subclinical and overt hypothyroid groups as compared to euthyroid subjects. Serum creatinine showed a significant negative correlation with T3 &T4 levels in overt group(r = -0.372 and r = - 0.371), whereas a positive correlation was observed with TSH (r=0.283). Uric acid levels were significantly increased in the overt group as compared to euthyroid subjects. Uric acid levels showed a significant negative correlation with T3 levels in the overt group (r= -0.298). After 3 months of thyroxine replacement therapy, creatinine and uric acids levels decreased significantly and were comparable to euthyroid levels.     Multi-bit quantum random number generator from path-entangled single photons

    Measurement outcomes on quantum systems exhibit inherent randomness and are fundamentally nondeterministic. This has enabled quantum physics to set new standards for the generation of true randomness with significant applications in the fields of cryptography, statistical simulations, and modeling of the nondeterministic behavior in various other fields. In this work, we present a scheme for the generation of multi-bit random numbers using path-entangled single photons. Without losing their intrinsic randomness, the protocol allows us to engineer the distribution from which we sample random numbers. For the experimental demonstration, we use single photons generated using spontaneous parametric down-conversion (SPDC), and assign a multi-bit commitment along the path. One-bit and two-bit random numbers are then generated from measuring entangled states in the path basis. In addition to passing the NIST tests for randomness, we also demonstrate the certification of quantumness and self-certification of quantum random number generator (QRNG) using Clauser, Horne, Shimony and Holt (CHSH) inequality violation. The path-entangled states can generate higher bitrates compared to heralded single photon or entangled photon schemes which are limited by the coincidence counts. The scheme involves distribution of photons along multiple paths resulting in multiple bits from one photon and avoids the limitation imposed by the detection dead time of one detector. We demonstrate this by generating a high rate of about 80 Mbps when the single photon detector saturates at around 28 Mcps.Comment: 11 Pages, 8 figures, functionally identical to the published versio

    Assessment of selenium levels and risk factors for stroke and other cardiovascular disease: a cross sectional study in a seleniferous area of Punjab, India

    Background and aims: Rural areas of Punjab in India have been found to have soil rich in selenium (Se); about 2160 hectare area is seleniferous and is populated by about 10,000 inhabitants. Selenium concentrations in these villages were reported to be as high as 65 times over non-seleniferous areas. The aim of this cross-sectional study was to evaluate selenium levels in blood, hair and nails in a group of subjects living in this area, and to evaluate the correlation between selenium exposure levels and a relevant cardiovascular risk factor and blood pressure. Methods: In a random sample of rural residents in three districts of a seleniferous area of Punjab, we determined selenium concentration in hair, nail clippings and serum samples. Analyses were carried out using atomic absorption spectrophotometry at National Dairy Research Institute (NDRI), Karnal, India. Data analysis was performed using the STATA 15.0 software (STATA Corp. TX). Results: A total of 680 human subjects were recruited in this study, with a male/female ratio of 0.65 and a median age of 43 (IQR 32-52). Medium selenium levels in blood, hair and nail were 86.7 µg/l (IQR 55.9-200.3), 20.7 µg/g (IQR 12.6-40.3) and 56.9 µg/g (IQR 42.8-83.9), respectively, with lower levels in women in all three kind of samples. Concerning systolic blood pressure, Pearson’s correlation coefficients were 0.102 (95 % CI -0.025 to 0.226, p=0.116); 0.076 (95% CI -0.010 to 0.160, p=0.085); 0.072 (95% CI -0.015 to 0.157, p=0.104) with blood, hair and nail, respectively. For diastolic blood pressure, Pearson’s correlation coefficients are 0.106 (95% CI -0.022 to 0.230, p=0.104), 0.036 (95% CI -0.050 to 0.122, p=0.409), 0.049 (95% CI -0.038 to 0.135, p=0.272), respectively. Conclusions: Our findings indicate a positive correlation between selenium content in blood, hair and nails and increasing systolic and diastolic pressure levels, in line with previous epidemiologic findings, indicating a possible health concern for this highly exposed population. The possible relation between selenium over-exposure and onset of hypertension and other cardiovascular diseases deserves further investigation

    Mortality implications and factors associated with nonengagement in a public epilepsy care initiative in a transient population

    BACKGROUND: Community-based, public care programs are a requisite to close the epilepsy treatment gap in disadvantaged communities in low- and middle-income countries (LMICs). Potential beneficiaries may, however, choose not to engage in these programs. AIMS: The aim of the study was to describe factors associated with and mortality consequences of nonacceptance of a public epilepsy care initiative. METHODS: In this cross-sectional study, we contacted 207 (36%) people out of 575 who screened positive for epilepsy during a population-based survey of 59,509 people. They were invited for neurological evaluation and care provision (including antiseizure medications (ASMs)) but chose not to engage. Structured questionnaires and qualitative interviews were conducted to determine reason for their nonengagement. Factors associated with nonengagement were evaluated by univariate and multivariate analysis. We conducted verbal autopsies for those who had died. RESULTS: Ten (5%) of the 207 individuals died since the initial screening; six with epilepsy-related causes. Of those who could be contacted (n = 48), 40 (19%) were confirmed to have epilepsy. Nonengaging individuals were likely to be older (odds ratio (OR): 1.02; 95% confidence interval (CI), 1.01, 1.11), locals (OR: 4.32; 95% CI, 1.55, 12.03), and earn less than US$ 78/month (OR: 3.6; 95% CI, 1.62, 8.06). Reasons for not engaging included a belief that epilepsy is inconsequential, loss of daily wages owing to healthcare facility visit and physical infirmity. CONCLUSIONS: Nonacceptance of a community-based public epilepsy care initiative is associated with high premature mortality, mostly attributed to epilepsy-related causes. Older age, ethnic status, and economic deprivation are factors associated with nonacceptance, though the underlying reasons may be varied

    Effect of target material on fast-electron transport and resistive collimation.

    The effect of target material on fast-electron transport is investigated using a high-intensity (0.7 ps, 1020  W/cm2{10}^{20}\text{ }\text{ }\mathrm{W}/{\mathrm{cm}}^{2}) laser pulse irradiated on multilayered solid Al targets with embedded transport (Au, Mo, Al) and tracer (Cu) layers, backed with millimeter-thick carbon foils to minimize refluxing. We consistently observed a more collimated electron beam (36% average reduction in fast-electron induced Cu K\ensuremath{\alpha} spot size) using a high- or mid-ZZ (Au or Mo) layer compared to Al. All targets showed a similar electron flux level in the central spot of the beam. Two-dimensional collisional particle-in-cell simulations showed formation of strong self-generated resistive magnetic fields in targets with a high-ZZ transport layer that suppressed the fast-electron beam divergence; the consequent magnetic channels guided the fast electrons to a smaller spot, in good agreement with experiments. These findings indicate that fast-electron transport can be controlled by self-generated resistive magnetic fields and may have important implications to fast ignition
