    Cultural Genocide in International Law (Il Genocidio Culturale nel Diritto Internazionale)

    The aim of this study is to analyze the role assumed by the international law about the protection recognized to the bond among a people and his cultural heritage inside the discipline of a specific crime: the genocide. The existence of the cultural genocide as autonomous crime, in fact, is and has been over the time, as will be seen in the course of the discussion, a highly debated theme in doctrine. Despite in the last years more voices have been raised for its recognition, the prevailing position is that the crime of genocide doesn’t constitute if the behaviors that mine the culture of the protected groups are not accompanied by actions of physical or biological aggression. The paper therefore provides a definition of the concept also through the reconstruction of the genesis of its first theorization by the undisputed father of this specific crime Raphael Lemkin. SuntoScopo di questo studio è analizzare la posizione assunta dal diritto internazionale circa la tutela che si è ritenuto di dover accordare al legame tra un popolo ed il suo patrimonio culturale nell’ambito della disciplina di un crimine specifico: il genocidio. La sussistenza del genocidio culturale come fattispecie autonoma, infatti, è ed è stata nel tempo, come si vedrà nel corso della trattazione, un tema assai dibattuto in dottrina. Nonostante negli ultimi anni più voci si siano levate per il suo riconoscimento, la posizione che si è affermata quale prevalente è quella per cui non possa configurarsi il crimine di genocidio qualora siano perpetrate delle condotte che minano la cultura di uno dei gruppi protetti di cui si intende perseguire la distruzione, prescindendo però da forme di aggressione fisica o biologica nei confronti dei suoi membri.     Sex-driven vulnerability in stress and drug abuse

    A growing body of literature shows that a link exists between substance abuse and stress and that the crosstalk of sex hormones with the neuroendocrine system might differently prime vulnerability to drug addiction in male and female subjects. Thus, understanding the neurobiological mechanisms of addiction and the identification of sex-driven determinants in vulnerability to drug abuse may help to better devise and/or implement strategic (pharmacological, behavioural, social) interventions to prevent or face the issue of addiction. Differences between sexes can be found at all stages of life (in both the animal model and human studies) and may account for genetic, epigenetic and environmental/ hormonal factors that in turn affect the functionality of the whole organism leading also to a sex-driven differential vulnerability or resilience to non-communicable pathologies. These include the onset and precipitation of stress-related psychiatric disorders as well as “substance-related and addictive disorders” (as defined in the DSM-V). This paper reviews the scientific literature highlighting significant differences in male and female subjects in stress and neuroendocrine function and the implications for sex-dependent differential vulnerability to drug addiction

    Brivaracetam as Early Add-On Treatment in Patients with Focal Seizures: A Retrospective, Multicenter, Real-World Study

    Introduction In randomized controlled trials, add-on brivaracetam (BRV) reduced seizure frequency in patients with drug-resistant focal epilepsy. Most real-world research on BRV has focused on refractory epilepsy. The aim of this analysis was to assess the 12-month effectiveness and tolerability of adjunctive BRV when used as early or late adjunctive treatment in patients included in the BRIVAracetam add-on First Italian netwoRk Study (BRIVAFIRST). Methods BRIVAFIRST was a 12-month retrospective, multicenter study including adult patients prescribed adjunctive BRV. Effectiveness outcomes included the rates of sustained seizure response, sustained seizure freedom, and treatment discontinuation. Safety and tolerability outcomes included the rate of treatment discontinuation due to adverse events (AEs) and the incidence of AEs. Data were compared for patients treated with add-on BRV after 1-2 (early add-on) and >= 3 (late add-on) prior antiseizure medications. Results A total of 1029 patients with focal epilepsy were included in the study, of whom 176 (17.1%) received BRV as early add-on treatment. The median daily dose of BRV at 12 months was 125 (100-200) mg in the early add-on group and 200 (100-200) in the late add-on group (p < 0.001). Sustained seizure response was reached by 97/161 (60.3%) of patients in the early add-on group and 286/833 (34.3%) of patients in the late add-on group (p < 0.001). Sustained seizure freedom was achieved by 51/161 (31.7%) of patients in the early add-on group and 91/833 (10.9%) of patients in the late add-on group (p < 0.001). During the 1-year study period, 29 (16.5%) patients in the early add-on group and 241 (28.3%) in the late add-on group discontinued BRV (p = 0.001). Adverse events were reported by 38.7% and 28.5% (p = 0.017) of patients who received BRV as early and late add-on treatment, respectively. Conclusion Brivaracetam was effective and well tolerated both as first add-on and late adjunctive treatment in patients with focal epilepsy

    Sex-based electroclinical differences and prognostic factors in epilepsy with eyelid myoclonia

    Although a striking female preponderance has been consistently reported in epilepsy with eyelid myoclonia (EEM), no study has specifically explored the variability of clinical presentation according to sex in this syndrome. Here, we aimed to investigate sex-specific electroclinical differences and prognostic determinants in EEM. Data from 267 EEM patients were retrospectively analyzed by the EEM Study Group, and a dedicated multivariable logistic regression analysis was developed separately for each sex. We found that females with EEM showed a significantly higher rate of persistence of photosensitivity and eye closure sensitivity at the last visit, along with a higher prevalence of migraine with/without aura, whereas males with EEM presented a higher rate of borderline intellectual functioning/intellectual disability. In female patients, multivariable logistic regression analysis revealed age at epilepsy onset, eyelid myoclonia status epilepticus, psychiatric comorbidities, and catamenial seizures as significant predictors of drug resistance. In male patients, a history of febrile seizures was the only predictor of drug resistance. Hence, our study reveals sex-specific differences in terms of both electroclinical features and prognostic factors. Our findings support the importance of a sex-based personalized approach in epilepsy care and research, especially in genetic generalized epilepsies

    The spectrum of epilepsy with eyelid myoclonia: delineation of disease subtypes from a large multicenter study

    Objective Epilepsy with eyelid myoclonia (EEM) has been associated with marked clinical heterogeneity. Early epilepsy onset has been recently linked to lower chances of achieving sustained remission and to a less favorable neuropsychiatric outcome. However, much work is still needed to better delineate this epilepsy syndrome. Methods In this multicenter retrospective cohort study, we included 267 EEM patients from nine countries. Data on electroclinical and demographic features, intellectual functioning, migraine with or without aura, family history of epilepsy, and epilepsy syndromes in relatives were collected in each patient. The impact of age at epilepsy onset (AEO) on EEM clinical features was investigated, along with the distinctive clinical characteristics of patients showing sporadic myoclonia involving body regions other than eyelids (body-MYO). Results Kernel density estimation revealed a trimodal distribution of AEO, and Fisher-Jenks optimization disclosed three EEM subgroups: early onset (EO-EEM), intermediate onset (IO-EEM), and late onset (LO-EEM). EO-EEM was associated with the highest rate of intellectual disability, antiseizure medication refractoriness, and psychiatric comorbidities and with the lowest rate of family history of epilepsy. LO-EEM was associated with the highest proportion of body-MYO and generalized tonic-clonic seizures (GTCS), whereas IO-EEM had the lowest observed rate of additional findings. A family history of EEM was significantly more frequent in IO-EEM and LO-EEM compared with EO-EEM. In the subset of patients with body-MYO (58/267), we observed a significantly higher rate of migraine and GTCS but no relevant differences in other electroclinical features and seizure outcome. Significance Based on AEO, we identified consistent EEM subtypes characterized by distinct electroclinical and familial features. Our observations shed new light on the spectrum of clinical features of this generalized epilepsy syndrome and may help clinicians toward a more accurate classification and prognostic profiling of EEM patients

    Idealmente la presente tesi può suddividersi in due macroaree: i primi tre capitoli, sono stati dedicati all’analisi fenomenologica degli effetti in esame, al fine di delimitare la soglia scientifica del fenomeno investigato; a tal scopo è stata svolta un’ampia ricognizione della letteratura, nazionale e internazionale, che prende in esame, seppur indirettamente, l’argomento considerato, consultando lavori prodotti in diversi campi di studio. Il quarto e il quinto capitolo, di fatto il corpo centrale dello studio, si occupano di presentare come dare un valore agli effetti che la cooperazione potenzialmente potrebbe generare e che la scissione di certo dissolve. Al tal proposito, sono state esposte le metodologie più usate o ritenute più adatte al fenomeno sinergico, correlando la trattazione con casi applicativi, di modo da rendere concreta l’effettiva possibilità di ricorrere alle soluzioni proposte

    In this article, we present the command cub, which fits ordinal rating data using combination of uniform and binomial (CUB) models, a class of finite mixture distributions accounting for both feeling and uncertainty of the response process. CUB identifies the components that define the mixture in the baseline model specification. We apply maximum likelihood methods to estimate feeling and uncertainty parameters, which are possibly explained in terms of covariates. An extension to inflated CUB models is discussed. We also present a subcommand, scattercub, for visualization of results. We then illustrate the use of cub using a case study on students’ satisfaction for the orientation services provided by the University of Naples Federico II in Italy
