    The effect of periosteum on the union of the autoclaved bone graft-host bone: Experimental study in rabbits

    Amaç: Bu çalışmada tavşan radius kemiğinden alınan otolog segmenter kemik greftinin otoklavda bekletilerek intramedüller tespitle alındığı yere tekrar yerleştirilmesi ve periost dokusu ile bütünleşmesinin iyileşmeye etkisi incelendi. Gereç ve yöntemler: On altı erişkin tavşanda sol radiusun orta bölümünden 15 milimetre uzunluğunda kemik parça çıkarıldı. Çıkarılan kemikler 120 °C’de 15 dakika otoklavize edildi ve intramedüller Kirschner telleriyle yerine tekrar tespit edildi. Tavşanlar iki eşit gruba ayrıldı ve birinci gruptaki sekiz tavşanın greft-alıcı kemik birleşim bölgesi periost flebiyle örtülürken, ikinci gruptaki diğer sekiz tavşanın greft-alıcı kemik birleşim bölgesinde periost dokusu kullanılmadı. Cerrahi işlem sonrası 2, 4, 6. ve 8. haftalarda düz grafiler çekildi. Sekizinci haftanın sonunda elde edilen radyolojik bulgular Yang’ın modifiye radyolojik sayılama yöntemiyle değerlendirildi. Bulgular: Radyolojik bulgular 8. haftada birinci grupta (ort. 10.94) ikinci gruba (ort. 6.06) göre daha fazla kallus dokusuyla, daha erken ve daha fazla iyileşme elde edildiğini gösterdi. Fark istatistiksel olarak anlamlı bulundu (p=0.04, <0.05). Sonuç: Periost yaması kolay uygulanabilen ve greftalıcı bölgenin iyileşmesini hızlandıran biyolojik bir dokudur. Periost yama tekniği greft-alıcı kemik bölgesinin iyileşmesinde etkili bir yöntemdir ve bu teknik, yapısal allogreftlere uygulanmaya değer bulunmuştur

    Clinical Study Effects of Zoledronate on Mortality and Morbidity after Surgical Treatment of Hip Fractures

    We aimed to evaluate the effects of intertrochanteric femoral fractures on mortality, morbidity, and cost of zoledronate treatment in elderly patients treated by osteosynthesis. Based on Evans classification, 114 patients with unstable intertrochanteric femoral fractures were treated with osteosynthesis. After the surgical treatment of intertrochanteric fractures, the treatment group (M/F, 24/32; mean age, 76.7 ± SD years) received zoledronate infusion, and the control group (M/F, 20/38; mean age, 80.2 ± SD years) received placebo. Postoperative control visits were performed at 6-week, 3-month, 6-month, and 12-month time points. Functional level of patients was evaluated by the modified Harris hip score and Merle d&apos; Aubigné hip score. By 12 months, the mean HHS in treatment and control groups was 81.93 and 72.9, respectively. For time of death of the patients, mortality was found to be 57.1% (16/28) on the first 3 months and 92.9% (26/28) on the first six months. The mortality rate in the treatment and control groups was 14.3% (8/56) and 34.5% (20/58), respectively. The use of zoledronic acid after surgical treatment of intertrochanteric femoral fractures in osteoporotic elderly patients is a safe treatment modality which helps to reduce mortality, improves functional outcomes, and has less side effects with single dose use per year

    ÖZETTürkiye’de Elektronik Bankacılık ve Temassız Kredi Kartları’nın tez konusu olarak seçilmesinin nedeni, ödeme sistemlerinin geleceğinde kartlı ödeme sistemlerinin bulunması ve bu amaca uygun olarak düşük meblağlı ödemelerin de kredi kartı ile yapılmasına olanak sağlayan temassız kartların kullanıma sunulmasıdır.Bu tez çalışmasının birinci bölümünde gelişen teknoloji ile ticaretin farklılaşması ile birlikte ortaya çıkan e-ticaret kapsamı, amaçları ve türlerinden bahsedilmiştir. Elektronik ticaretin sürekli artan hacmi anlatılmış ve artan işlem hacmi ile birlikte, güvenlik konusunda kullanılmakta olan önlemler ve protokollere değinilmiştir. Ayrıca elektronik ticaretin bir şekli olan elektronik bankacılıktan ve elektronik bankacılık kanallarından bahsedilmiştir. Elektronik bankacılıkta bireysel dağıtım kanalları ve elektronik bankacılıkta bireysel ürünler anlatılmıştır.İkinci bölümde kredi kartı tanımı yapılmakla birlikte tarihsel gelişimleri ve günümüzde kullanılan kart türlerinden bahsedilmiştir. Türkiye’deki kartlı ödeme sistemleri tanıtılmıştır. Kredi kartı kullanımının sağladığı faydalara farklı taraflar açısından bakılmış, sonrasında ise kredi kartlarının makroekonomik etkilerine değinilmiştir.Üçüncü bölümde ise çipli kart teknolojisinin en yeni ürünü olan temassız kredi kartlarından anlatılmıştır. Temassız kredi kartlarının nasıl kullanıldığı ve kartlarda kullanılan işlem limitleri bilgilere yer verilmiştir. Temassız kartların kullanıldığı yerler, kullanım sıklıkları anlatılmakla beraber, temassız kartların sağladığı faydalar anlatılmıştır.Sonuç olarak ödeme sistemlerinde geleceğin biometrik ödeme olacağı yönünde düşüncelere yer verilmiş ve temassız kartların kullanımının yaygınlaştığı ve bu kullanım artışı ile ekonominin ve kişilerin kazançlarına değinilmiştir.     Determining students’ personality traits according to parental perspectives: A scale development study

    The educational community recognizes individual differences, but it is important to fully address them in educational settings. While there are existing studies on personality traits, none focus on the personality traits of students aged 6-18. This study aims to fill this gap, as personality traits significantly influence personal, academic, and professional life. Understanding student personalities allows for the development of more effective educational programs, facilitating their full potential. The study included parents of 2,229 students from a population of 16,505,271 aged 6-18 in Turkiye. Through exploratory factor analysis (EFA), a reliable 61-item 5-point Likert scale (α = .942) was derived. Confirmatory factor analysis (CFA) was performed on a second dataset of 916 parents, resulting in 794 due to outliers. The Scale for Determining Student Personalities According to Parental Perspectives, with 16 sub-dimensions and 46 items, demonstrated good reliability (α = .948). The study’s findings are expected to hold significant implications for education, parenting, and child development

    Can chondral healing be improved following microfracture? The effect of adipocyte tissue derived stem cell therapy

    WOS: 000378963600017PubMed ID: 27068292Background: We aimed to investigate the effect of adipose tissue-derived mesenchymal stem cells (ADSCs) on chondral healing using the microfracture (MF) technique. Methods: Thirty male rabbits were randomly divided into three groups. Standard cylindrical osteochondral defects (OCDs) were created in the weight-bearing areas of the medial condyles of all the right knees; the defects were four millimeters in diameter and two millimeters in depth. The control group (group A) was restricted to spontaneous healing. For group B, we performed MF with a 1.5-mm drill. For group C, we applied MF using the same method and then applied 3 x 10(6) ADSCs to the defect area. At eight weeks post-operation, the subjects were sacrificed, and the distal femoral joint surfaces were evaluated histopathologically for chondral healing. The samples were scored according to the International Cartilage Repair Society (ICRS) scale. Results: The results for group C were significantly better than those for group A in terms of the surface properties (p = 0.003). The matrix evaluation was better for group A than for group C (p = 0.01). The cell distribution, cell viability and subchondral bone parameters were similar between the groups (p = 0.198, p = 0.387 and p = 0.699). The cartilage mineralization parameter was better for group C than for group A (p = 0.001). The signs of healing were better for group C than for group B, but the differences were not significant (p = 0.185).     Practice of tourniquet use in Turkey: a pilot study

    Objective: The aim of the present pilot study was to evaluate patterns in the current practice of tourniquet use in Turkey. The results of this study can provide detailed information regarding tourniquet use and evaluate the need for guidelines on tourniquet use in Turkey. Methods: The questionnaire was sent to orthopedic residents and surgeons by either giving printed questionnaires directly or by establishing preliminary communication with surgeons and then sending questionnaires by e-mail. Participating staff consisted of 3 groups: Group 1: orthopedic surgeons; Group 2: orthopedic residents; and Group 3: orthopedic academic staff. Statistical differences in tourniquet use were analyzed among the groups. Results: Use of mechanical tourniquet was significantly higher in Group 1. Plain cuffs were used in orthopedic surgical practice more frequently. Assistant and orthopedic theatre personnel were commonly reported by participants as the tourniquet applicant. Periodic educational practice was not routine. The number of reported complications was higher in Group 3. Cuff padding was generally routine practice. Scientifically valid options at lowest inflation pressure were not observed among the results at the expected rates. Conclusion: The results of this pilot study indicate that there is wide variation in some aspects of tourniquet practice in Turkey. The differences are not acceptable because of the potential for significant complications with some practices. There is a need to provide and ensure adequate education to provide the best patient care. Furthermore, protocols should be developed for acceptable standards of tourniquet use
