    Localization of Energy and Momentum in an Asymptotically Reissner-Nordstr\"om Non-singular Black Hole Space-time Geometry

    The space-time geometry exterior to a new four-dimensional, spherically symmetric and charged black hole solution that, through a coupling of general relativity with a non-linear electrodynamics, is everywhere non-singular, for small rr it behaves as a de Sitter metric, and asymptotically it behaves as the Reissner-Nordstr\"{o}m metric, is considered in order to study the energy-momentum localization. For the calculation of the energy and momentum distributions, the Einstein, Landau-Lifshitz, Weinberg and M\o ller energy-momentum complexes have been applied. The results obtained show that in all prescriptions the energy depends on the mass MM of the black hole, the charge qq, two parameters % a\in \mathbb{Z}^+ and γR+\gamma\in \mathbb{R}^+, and on the radial coordinate rr. The calculations performed in each prescription show that all the momenta vanish. Additionally, some limiting and particular cases for rr and qq are studied, and a possible connection with strong gravitational lensing and micro lensing is attempted.Comment: To appear in Univers

    Antibiotic therapy in the treatment of acute otitis media

Worldwide acute otitis media affects about 8-11% of people a year. In acute otitis media, antibiotics may speed recovery but may result in side effects. Antibiotics are often recommended in those with severe disease or under two years old. Material and methods: Our study included a retrospective analysis of 117 patients with acute otitis media, whose medical records were examined. The information was analyzed statistically. Results: The results demonstrated an increased involvement of older people and women. The use of antibiotics to treat acute otitis media is determined by Cefazolin in 49-50% cases, Ceftriaxone - 49%, Cefotaxime – 48%, Cefoperazone – 14%, Amoxicillini Clavulanic acid in 16% of cases. Antibiogram was released for 53 patients, and according to it: Cefazolin – 23%, Amoxicilini Clavulanic acid – 15% and the rest were treated with cephalosporin third generation. Conclusions: Depending on severity, the treatment requires the use of antibiotics with broad spectrum. Antibacterial therapy according to antibiogram is contemporary and has the advantage of assessing the appropriate antibiotic

    Catedra Anesteziologie şi Reanimatologie nr. 2, USMF „Nicolae Testemiţanu”The purpose of this article is an informative analysis of the efficiency of prophylaxis of the pulmonary artery trombembolism. Out of 627 patients monitored, 473 had the benefit of a perioperatory tromboprophylaxis, while the other 154 patients got only a postoperatory tromboprophylaxis. The incidence of pulmonary artery trombembolism is 0.48%, this result refers only to patients operated on urgently. The incidence of lethal cases is 0.16%. Pulmonary artery trombembolism incidence varies depending on the combination of predisposing factors. We studied several combined methods of prophylaxis. Priority was the combination of mechanical methods (application of elastic bandages on the legs) with low molecular mass heparins. From observation of the study we determined an increased efficiency using the combination of mechanical methods with Enoxoparine (Klexan) in tromboprophylaxis of patients with moderate and high risk of pulmonary artery trombembolism. The priorities of Enoxoparine, to other low molecular weight heparins do not require the determination of bleeding and clotting time during administration. Also there was no change in platelet count in patients receiving Enoxoparine.Целью данной статьи является информативный анализ эффективности профилактики тромбоэмболии легочной артерии в пред-, интра- и постоперационном периоде. Из 627 обследованных пациентов, 473 больным проводилась, преимущественно, профилактика тромбоэмболии легочной артерии в пред-, интра- и постоперационном периоде, 154 пациентам - только в послеоперационном периоде. Полученный результат заболеваемости тромбоэмболией легочной артерии - 0,48%, относится только к больным, оперированным в срочном порядке. Смертность 0,16% варьирует в зависимости от сочетания предрасполагающих факторов. Было изучено несколько комбинированных методов профилактики. Приоритетным было сочетание механических методов (применение эластичного бинта на ногах) с гепаринами с низкой молекулярной массой. Исследование определялось повышением эффективности использования сочетания механических методов с еноксипарином (Klexan) в профилактике тромбоэмболии легочной артерии у пациентов с умеренным и высоким риском тромбоэмболии легочной артерии. Преимуществом еноксипарина и других низкомолекулярных гепаринов является то, что не требуется определение кровотечения и время свертывания крови во время приема. Также не определяются никакие изменения в количестве тромбоцитов у пациентов, получающих еноксипарин

    Catedra Anesteziologie şi Reanimatologie Nr.2 USMF “N.Testemițanu”, Congresul II Internaţional al Societăţii Anesteziologie Reanimatologie din Republica Moldova 27-30 august 2009Electrostimularea transcraniană cu aplicarea fronto-occipitală a electrozilor şi utilizarea pachetelor de curenţi impulsiformi cu frecvenţa 77,5 Hz şi valoarea sumară a curentului 4,5 ma, s-a dovedit a fi eficientă în tratamentul bolnavilor suferinzi de migrenă şi nevralgia nervului trigemin după 6-10 proceduri, contribuind şi la ameliorarea parametrilor hemodinamicii intracerebrale şi centrale. Studii aprofundate a savanţilor din ultimile decenii au demonstrat eficacitatea experimentală şi clinică a electrostimulării transcraniene cu curenţi impulsiformi rectangulari, având parametri cu valori critice. Metoda constă în influenţa transcutană, transcraniană a curentului electric impulsiform, care stimulează sistemul antinociceptiv cu actizarea structurilor endorfinice, serotoninergice ale creerului cu dezvoltarea efectelor analgezice, antistresorii, imunomodulatorii etc. Scopul acestui studiu a fost de a evalua eficacitatea metodei ESTC în tratamentul pacienţilor cu migrenă şi nevralgia nervului trigemin, care cu greu se supun tratamentului tradiţonal medicamentos. Cu ajutorul ESTC noi am tratat 25 pacienţi, inclusiv 20 femei şi 5 bărbaţi cu vârsta medie 43±6 ani, cu sindrom algic localizat central şi periferic, având durata medie 9±4 ani, care s-au tratat cu diverse remedii medicamentoase, dar fără rezultate importante. Bolnavi cu migrenă au fost 18, iar cu nevralgia nervului trigemin 7. Pentru ESTC am folosit aparatul DES produs de Î.S.CESID. Pacienţii au primit câte o cură de tratament care includea 7-10 şedinţe cu durata de 30-40 min. fiecare. În cadrul studiului s-a cercetat reoencefalografia, parametrii hemodinamicii şi evoluţia clinică. S-a constatat micşorarea esenţială sau chiar dispariţia durerii din prima şedinţă ESTC. Analgezia apare peste 15-17 min. de la iniţierea procedurii şi durează timp de 3-24 ore. Cu fiecare şedinţă durata efectului analgezic creşte şi ajunge la maximum după 4-5 şedinţe. Trebuie de menţionat că la bolnavii cu migrenă eficacitatea tratamentului se observă de la primele proceduri în proporţie de 85% iar remisia durează 3-7 luni după o cură de tratament. La bolnavii cu nevragia trigeminului efectul apare din prima procedură, iar după 4-6 proceduri durerile dispar definitiv. Hemodinamica intracerebrală şi centrală sa caracterizat prin ameliorare esenţială şi stabilizare. Astfel metoda ESTC s-a dovedit a fi eficientă în tratamentul migrenei şi nevralgiei nervului trigemin în cadrul monoterapiei. Este necesară continuarea aprofundată a studiului

    Expanding the MECP2 network using comparative genomics reveals potential therapeutic targets for Rett syndrome

    Inactivating mutations in the Methyl-CpG Binding Protein 2 (MECP2) gene are the main cause of Rett syndrome (RTT). Despite extensive research into MECP2 function, no treatments for RTT are currently available. Here, we used an evolutionary genomics approach to construct an unbiased MECP2 gene network, using 1028 eukaryotic genomes to prioritize proteins with strong co-evolutionary signatures with MECP2. Focusing on proteins targeted by FDA-approved drugs led to three promising targets, two of which were previously linked to MECP2 function (IRAK, KEAP1) and one that was not (EPOR). The drugs targeting these three proteins (Pacritinib, DMF, and EPO) were able to rescue different phenotypes of MECP2 inactivation in cultured human neural cell types, and appeared to converge on Nuclear Factor Kappa B (NF-κB) signaling in inflammation. This study highlights the potential of comparative genomics to accelerate drug discovery, and yields potential new avenues for the treatment of RTT

In purulent complications of acute abdominal inflammatory diseases, its frequency can be up to 60%. Difficulties in appropriate diagnoses and treatment and high lethality, which can be up to 95%, determined the actuality of this study. The main cause of multiple organ insufficiency syndromes are infectious complications, particularly in inadequate management, which cause immune, metabolic, and homeostatic disturbances. Синдром полиорганной недостаточности осложняет течение послеоперационного периода в 10-28% случаев. При гнойных осложнениях острых воспалительных заболеваний органов брюшной полости, частота его достигает 60%. Трудность своевременной диагностики и лечения, высокая летальность, достигающая 95%, обусловили особую актуальность и сложность исследования этой проблемы. Основной причиной развития синдрома полиорганной недостаточности являются инфекционные осложнения, особенно при их несвоевременной диагностике или неадекватном лечении, вызывающие нарушения иммунного статуса больных, метаболизма, а также системы гомеостаза

    The European Union’s Farm-to-Fork Strategy sets out its ambitions and approach to how European’s value food sustainability, and the accompanying opportunities for improving the environment as well as the lifestyles and health of EU citizens. It identifies the role of agroecological approaches in primary production as one of the means by which these ambitions can be achieved, through new knowledge and innovations in land management and supply chains. This article concludes the Special Issue on the EU Horizon 2020 projects LIFT and UNISECO, synthesising requirements for the realisation of transitions to agroecology in Europe. It reports the key needs identified of developing human capital, partnerships, cooperation and social capital, access to data and tools, and the links between these needs and relevant EU and national level policies.Les systèmes alimentaires doivent être réorientés pour mieux prendre en compte les interactions avec l’environnement, l’économie, la santé et la société. Les transitions vers des pratiques et des systèmes agricoles agroécologiques peuvent relier les domaines de l’action publique et permettre d'atteindre des objectifs environnementaux, économiques et sociaux. Ces transitions fournissent une perspective pour examiner les politiques, les pratiques et les comportements des acteurs des systèmes agricoles et des chaînes de valeur, l’identifi cation des obstacles à l'adoption et les domaines d’action publique auxquels ils contribuent ou qui présentent des lacunes. Le développement du capital social et humain et de structures de gouvernance qui permettent les transitions vers l’agroécologie est la clé de leurs chances de succès. L’éducation et l’apprentissage tout au long de la carrière sont essentiels pour développer les connaissances et les compétences des jeunes générations de gestionnaires fonciers et d’autres acteurs des chaînes de valeur, tout au long de leur carrière. Le résultat recherché est une compréhension partagée des avantages des pratiques et des systèmes agroécologiques, créant des opportunités pour rééquilibrer les politiques vers la neutralité climatique, inverser la perte de biodiversité et renforcer les droits des citoyens. Pour concrétiser ces opportunités, les politiques et stratégies doivent être cohérentes à tous les niveaux territoriaux et systémiques, et adaptées au lieu, au système, aux personnes et aux étapes des transitions. Dans l’ensemble, elles devraient être conçus pour garantir que tous les citoyens bénéfi cient des transitions vers des systèmes agricoles agroécologiques, sur le long terme, et que personne ni aucun lieu ne soit désavantagé par les processus de changement ou les résultats escomptés.Die Lebensmittelsysteme müssen neu ausgerichtet werden, um den Wechselwirkungen zwischen der Umwelt, der Wirtschaft, der Gesundheit und der Gesellschaft stärker Rechnung zu tragen. Die Umstellung auf agrarökologische Anbaumethoden und - systeme kann die Politikbereiche miteinander verbinden und zur Erreichung ökologischer, wirtschaftlicher und sozialer Ziele beitragen. Sie bieten einen Ansatzpunkt für die Überprüfung der Politik, der Praxis und der Verhaltensweisen der Akteure in landwirtschaftlichen Systemen und Wertschöpfungsketten. Des Weiteren helfen sie bei der Ermittlung von Umsetzungsbarrieren und von Politikbereichen, zu denen sie beitragen oder in denen es Lücken gibt. Die Entwicklung von Sozial- und Humankapital sowie von Governance- Strukturen – die den Übergang zur Agrarökologie ermöglichen – sind ein wesentlicher Erfolgsfaktor. Bildung und lebenslanges Lernen sind ein zentraler Baustein, um die Kenntnisse und Fähigkeiten der jüngeren Generationen von Landwirten und Landwirtinnen und anderen Akteuren in den Wertschöpfungsketten zu entwickeln. Das angestrebte Ergebnis ist ein gemeinsames Verständnis über die Vorteile agrarökologischer Praktiken und Systeme. Dieses schafft Möglichkeiten für eine Neuausrichtung der Politik im Hinblick auf Klimaneutralität, Umkehrung des Verlusts der biologischen Vielfalt und Stärkung der Rechte der Bürger. Zur Verwirklichung der Möglichkeiten, sollten Politiken und Strategien über territoriale und Systemebenen hinweg kohärent und auf Ort, System, Menschen und Phasen des Übergangs zugeschnitten sein. Insgesamt sollten sie so konzipiert sein, dass alle Bürgerinnen und Bürger langfristig von der Umstellung auf agrarökologische Bewirtschaftungssysteme profi tieren und dass niemand oder kein Ort durch den Veränderungsprozess oder das angestrebte Ergebnis benachteiligt wird

    Numerical and experimental analysis of wrinkling during the cup drawing of an AA5042 aluminium alloy

    The recent trend to reduce the thickness of metallic sheets used in forming processes strongly increases the likelihood of the occurrence of wrinkling. Thus, in order to obtain defect-free components, the prediction of this kind of defect becomes extremely important in the tool design and selection of process parameters. In this study, the sheet metal forming process proposed as a benchmark in the Numisheet 2014 conference is selected to analyse the influence of the tool geometry on wrinkling behaviour, as well as the reliability of the developed numerical model. The side-wall wrinkling during the deep drawing process of a cylindrical cup in AA5042 aluminium alloy is investigated through finite element simulation and experimental measurements. The material plastic anisotropy is modelled with an advanced yield criterion beyond the isotropic (von Mises) material behaviour. The results show that the shape of the wrinkles predicted by the numerical model is strongly affected by the finite element mesh used in the blank discretization. The accurate modelling of the plastic anisotropy of the aluminium alloy yields numerical results that are in good agreement with the experiments, particularly the shape and location of the wrinkles. The predicted punch force evolution is strongly influenced by the friction coefficient used in the model.     The epigenetic evolution of glioma is determined by the IDH1 mutation status and treatment regimen

    Tumor adaptation or selection is thought to underlie therapy resistance in glioma. To investigate longitudinal epigenetic evolution of gliomas in response to therapeutic pressure, we performed an epigenomic analysis of 132 matched initial and recurrent tumors from patients with IDH-wildtype (IDHwt) and IDH-mutant (IDHmut) glioma. IDHwt gliomas showed a stable epigenome over time with relatively low levels of global methylation. The epigenome of IDHmut gliomas showed initial high levels of genome-wide DNA methylation that was progressively reduced to levels similar to those of IDHwt tumors. Integration of epigenomics, gene expression, and functional genomics identified HOXD13 as a master regulator of IDHmut astrocytoma evolution. Furthermore, relapse of IDHmut tumors was accompanied by histological progression that was associated with survival, as validated in an independent cohort. Finally, the initial cell composition of the tumor microenvironment varied between IDHwt and IDHmut tumors and changed differentially following treatment, suggesting increased neo-angiogenesis and T-cell infiltration upon treatment of IDHmut gliomas. This study provides one of the largest cohorts of paired longitudinal glioma samples with epigenomic, transcriptomic, and genomic profiling and suggests that treatment of IDHmut glioma is associated with epigenomic evolution towards an IDHwt-like phenotype
