Identifying Cardiac Amyloid in Aortic Stenosis: ECV Quantification by CT in TAVR Patients
OBJECTIVES: To validate computed tomography measured ECV (ECVCT) as part of routine evaluation for the detection of cardiac amyloid in patients with aortic stenosis (AS)-amyloid. BACKGROUND: AS-amyloid affects 1 in 7 elderly patients referred for transcatheter aortic valve replacement (TAVR). Bone scintigraphy with exclusion of a plasma cell dyscrasia can diagnose transthyretin-related cardiac amyloid noninvasively, for which novel treatments are emerging. Amyloid interstitial expansion increases the myocardial extracellular volume (ECV). METHODS: Patients with severe AS underwent bone scintigraphy (Perugini grade 0, negative; Perugini grades 1 to 3, increasingly positive) and routine TAVR evaluation CT imaging with ECVCT using 3- and 5-min post-contrast acquisitions. Twenty non-AS control patients also had ECVCT performed using the 5-min post-contrast acquisition. RESULTS: A total of 109 patients (43% male; mean age 86 ¹ 5 years) with severe AS and 20 control subjects were recruited. Sixteen (15%) had AS-amyloid on bone scintigraphy (grade 1, n = 5; grade 2, n = 11). ECVCT was 32 ¹ 3%, 34 ¹ 4%, and 43 ¹ 6% in Perugini grades 0, 1, and 2, respectively (p < 0.001 for trend) with control subjects lower than lone AS (28 ¹ 2%; p < 0.001). ECVCT accuracy for AS-amyloid detection versus lone AS was 0.87 (0.95 for 99mTc-3,3-diphosphono-1,2-propanodicarboxylic acid Perugini grade 2 only), outperforming conventional electrocardiogram and echocardiography parameters. One composite parameter, the voltage/mass ratio, had utility (similar AUC of 0.87 for any cardiac amyloid detection), although in one-third of patients, this could not be calculated due to bundle branch block or ventricular paced rhythm. CONCLUSIONS: ECVCT during routine CT TAVR evaluation can reliably detect AS-amyloid, and the measured ECVCT tracks the degree of infiltration. A comunicação da suspensão de cirurgias pediåtricas: sentimentos dos familiares envolvidos no processo
A hospitalização ĂŠ uma experiĂŞncia complexa, sendo agravada no caso de necessidade de intervenção cirĂşrgica, principalmente quando o paciente ĂŠ criança. Quando a cirurgia ĂŠ suspensa, paciente e seus familiares podem apresentar sentimentos de insegurança, angĂşstia e ansiedade. Este estudo teve como objetivo identificar e descrever a percepção de quinze mĂŁes e/ou responsĂĄveis por crianças de 0 a 18 anos, internadas em um hospital, apĂłs receberem a notĂcia de que a cirurgia de seu filho foi suspensa. Ă um estudo descritivo, qualitativo, que utilizou o referencial teĂłrico da Comunicação Interpessoal e o metodolĂłgico da AnĂĄlise de ConteĂşdo. Os resultados evidenciaram que as suspensĂľes de cirurgias pediĂĄtricas sĂŁo fatos presentes na instituição, que trazem repercussĂľes ao paciente e familiares, e Ă organização institucional; que a comunicação entre profissionais da saĂşde, pacientes e familiares ĂŠ inadequada; que a atuação do enfermeiro na notĂcia da suspensĂŁo da cirurgia precisa ser efetiva.La hospitalizaciĂłn es una experiencia compleja, agravada en el caso de ser necesaria una intervenciĂłn quirĂşrgica, en particular cuando el enfermo es un niĂąo. Este estudio tuve como objetivo identificar e describir la percepciĂłn de quince madres y/o responsables por niĂąos con edades entre 0 y 18 aĂąos internados en un hospital despuĂŠs de recibir la noticia de que la cirugĂa de su hijo fue suspendida. Se trata de un estudio descriptivo cualitativo que utilizĂł el referencial teĂłrico de la ComunicaciĂłn Interpersonal y la metodologĂa del AnĂĄlisis de Contenido. Los resultados evidenciaron que las suspensiones de cirugĂas pediĂĄtricas son hechos presentes en la instituciĂłn, que traen repercusiones al enfermo y familiares y en la propia organizaciĂłn institucional, que la comunicaciĂłn entre profesionales de la salud, enfermos y familiares es inadecuada y que la actuaciĂłn del enfermero en la noticia de la suspensiĂłn de la cirugĂa debe ser necesariamente efectiva.Hospitalization is a complex experience; this can be made worse if surgical intervention is required, especially when the patient is a child. When surgery is suspended, patient and family can experience feelings of insecurity, distress, and anxiety. The objective of this study was to identify and describe the perceptions of fifteen mothers or guardians of children between 0 and 18 years old admitted in a teaching hospital, after receiving news that surgery for their child was suspended. This was a descriptive qualitative study which used Interpersonal Communication as theoretical reference and Analysis of Content as the methodology. Results showed that suspension of pediatric surgery in our institution causes repercussions to patients, their families, and institution organisation; that communication between health team professionals, patients and families is inadequate; and that the nurses' participation in informing surgery suspension must be effective
A Draft of the Human Septin Interactome
Background: Septins belong to the GTPase superclass of proteins and have been functionally implicated in cytokinesis and the maintenance of cellular morphology. They are found in all eukaryotes, except in plants. In mammals, 14 septins have been described that can be divided into four groups. It has been shown that mammalian septins can engage in homo- and heterooligomeric assemblies, in the form of filaments, which have as a basic unit a hetero-trimeric core. In addition, it has been speculated that the septin filaments may serve as scaffolds for the recruitment of additional proteins. Methodology/Principal Findings: Here, we performed yeast two-hybrid screens with human septins 1-10, which include representatives of all four septin groups. Among the interactors detected, we found predominantly other septins, confirming the tendency of septins to engage in the formation of homo- and heteropolymeric filaments. Conclusions/Significance: If we take as reference the reported arrangement of the septins 2, 6 and 7 within the heterofilament, (7-6-2-2-6-7), we note that the majority of the observed interactions respect the ""group rule"", i.e. members of the same group (e. g. 6, 8, 10 and 11) can replace each other in the specific position along the heterofilament. Septins of the SEPT6 group preferentially interacted with septins of the SEPT2 group (p<0.001), SEPT3 group (p<0.001) and SEPT7 group (p<0.001). SEPT2 type septins preferentially interacted with septins of the SEPT6 group (p<0.001) aside from being the only septin group which interacted with members of its own group. Finally, septins of the SEPT3 group interacted preferentially with septins of the SEPT7 group (p<0.001). Institutional shelter to protect adolescent victims of domestic violence: theory or practice?
OBJECTIVE: To understand and analyze, from the perspective of adolescent victims of domestic violence who were cared for in an institution in Campinas-SP, the protective factors to which they are submitted and / or have access. METHOD: This was qualitative research, with data collection occurring through focus groups with 17 adolescents, and semistructured interviews with seven of them; the data analysis was based on content analysis, using a thematic modality. RESULTS: Two themes emerged, entitled Four Walls and Trust. We discuss the context of institutional care, that despite the efforts made contemporaneously, still maintains an authoritarian environment; the importance of the bond and trust established with some employees, acting as protective factors for the adolescents and the protective aspect of religiosity. CONCLUSIONS: It is understood that these considerations should be valued and reinforced through the healthcare services provided to children and adolescents, as they contribute to the promotion of the physical and mental health of this population
