Le tri médical en radiologie, une réalité quotidienne. En sommes-nous conscients ?

International audienceIntroduction: Medical triage, the result of resource rationing and technical progress, is seen as scandalous, because every individual is supposed to benefit from the maximum that science can provide. However, any situation where there is a tension between medical needs and available human and non-human resources gives rise to procedures of triage, whether these are conscious or not. We therefore felt it necessary to “zoom in” on these day-to-day practices of triage in radiology. Main messages: We describe processes of patients triage in radiology, distinguishing between urgent and non-urgent situations, where the notion of time is by nature different. In emergencies, medical criteria predominate, but ethical issues remain when several emergencies occur simultaneously in a radiology department. In non-emergency contexts, the possible delay between the request and the performance of the examination encourages requests for cut-files from the various stakeholders (patients, doctors, friends and family), and prioritization criteria are often subjective and unequal. Awareness of triage strategies in the nooks and crannies of the radiology care pathway makes it possible to discuss, modify or justify these practices by making them transparent. Resource constraints resulting from health policy choices have an impact not only on patients, but also on radiologists, who experience a permanent conflict of values and loyalties. Conclusion: As in other disciplines, triage practices in radiology mobilize the principles of equality, justice, utilitarianism and responsibility. Collective, multidisciplinary, and societal reflection are essential for them, and radiology can be a powerful driving force
