    HP-LT metamorphism in Elba Island: Implications for the geodynamic evolution of the inner Northern Apennines (Italy)

    The inner Northern Apennines belt (i.e., northern Tyrrhenian Sea and Tuscany) is an Alpine chain affectedby high-P metamorphic conditions during its evolution. Although Elba Island is structurally located closeto the Adria-Europe suture zone, for several authors it represents a sector of the orogen affected bylow-P metamorphism. The involvement of Elba Island tectonic units in high-P metamorphism was onlysuspected for the sparse presence of phengitic white mica in the metasedimentary rocks. This paperpresents the first clear evidence of high-P and low-T metamorphism found in metabasite rocks embed-ded in the Cretaceous calcschist of eastern Elba Island. Mineral composition of metabasite includesGln + Cpx + Ep + Ab + Act + Qtz + Ilm ± Ti-oxide ± Spn and is indicative of a former equilibration in the epi-dote blueschist subfacies and subsequent retrogression in the greenschist facies. Recorded metamorphicconditions are P = 0.9–1.0 GPa and T = 330–350◦C. Tectonic discrimination using immobile elements inthe metabasite does not point to an oceanic setting. As a consequence, the metasedimentary succes-sion containing metabasite is explained as belonging to the Tuscan continental domain and not to theLigurian-Piedmont Ocean, as previously interpreted. Our results have two significant implications: (i)it is confirmed and strengthened that the tectonic stacking of the Elba Island units did not occur in alow-pressure context; (ii) Elba Island is now completely reconciled in the tectonic and metamorphicevolution of the inner Northern Apennines

    A meta-analysis of the validity of FFQ targeted to adolescents

    Objective: The present work is aimed at meta-analysing validity studies of FFQ for adolescents, to investigate their overall accuracy and variables that can affect it negatively. Design: A meta-analysis of sixteen original articles was performed within the ASSO Project (Adolescents and Surveillance System in the Obesity prevention). Setting: The articles assessed the validity of FFQ for adolescents, compared with food records or 24 h recalls, with regard to energy and nutrient intakes. Subjects: Pearson’s or Spearman’s correlation coefficients, means/standard deviations, kappa agreement, percentiles and mean differences/limits of agreement (Bland–Altman method) were extracted. Pooled estimates were calculated and heterogeneity tested for correlation coefficients and means/standard deviations. A subgroup analysis assessed variables influencing FFQ accuracy. Results: An overall fair/high correlation between FFQ and reference method was found; a good agreement, measured through the intake mean comparison for all nutrients except sugar, carotene and K, was observed. Kappa values showed fair/moderate agreement; an overall good ability to rank adolescents according to energy and nutrient intakes was evidenced by data of percentiles; absolute validity was not confirmed by mean differences/limits of agreement. Interviewer administration mode, consumption interval of the previous year/6 months and high number of food items are major contributors to heterogeneity and thus can reduce FFQ accuracy. Conclusions: The meta-analysis shows that FFQ are accurate tools for collecting data and could be used for ranking adolescents in terms of energy and nutrient intakes. It suggests how the design and the validation of a new FFQ should be addressed

    Protein supplementation and dietary behaviours of resistance trained men and women attending commercial gyms: a comparative study between the city centre and the suburbs of Palermo, Italy

    Background: It is anecdotally recognized that commercial gym users assume supplements in order to improve performance or health. However, dietary behaviours of people and athletes attending commercial gyms have been poorly studied. The exact amount and frequency of dietary supplements consumption are still needed to be investigated. The main purpose of this study is to understand the quantity and quality of food intake, as well as dietary supplementation in people attending commercial gyms. Secondly to compare the city centre and the suburbs of Palermo, Italy. Methods: A face-to-face questionnaire was administered to 561 subjects, 207 from the city centre (CC) and 354 from the suburbs (SB) of Palermo, Italy. Frequency of protein supplements use and association with dietary behaviours were investigated. Subsequently, the frequency distribution was used for demographic assessment. Results: Frequency of protein consumption was similar in both groups (30% for CC and 28.8% for SB). Males show greater consumption percentages than females (30.5% in males and 6.9% in females). Milk and chicken are the most frequently consumed foods. Data show that non-supplement users (NSU) consume significantly more snacks and bakery products than supplement users (SU) (P < 0.001). While, SU consume significantly higher quantities of vegetables, nuts, fresh fish, eggs and canned tuna (P < 0.001). SU consume less low protein food and higher protein foods than NSU. No differences were found between CC and SB. Conclusions: Protein consumption among commercial gym users is 30% for the CC and 28.8% for the SB. Significant differences were found between CC and SB females, underlining an interesting discrepancy, indicating to dietary supplement industries regarding regional implications.     Hydroxytyrosol, an olive oil phenolic compound, inhibits cyclooxygenase-2 transcription in phorbol--ester-treated human mocytes

    Introduction: Inflammation plays a critical role in atherosclerotic plaque development and instability. Cyclooxygenase(COX)-2 is a key mediator of inflammation. It is up-regulated in activated monocytes and macrophages of human atherosclerotic lesions. The extra-virgin olive oil is a source of bioactive compounds which can be responsible of Mediterranean diet athero-protective effect. Aim of the study was to examine the effects of hydroxytyrosol (HT), a phenolic compound from olives and extra-virgin olive oil, on COX-2 expression in phorbol-ester stimulated monocytic-macrophagic cells and to explore the mechanisms involved. Methods: U937 monocytoid cells were pre-treated with HT (0-10 micromol/L) for 60 min before stimulation with 20 nmol/L phorbol myristate acetate (PMA) in RPMI 1640 medium with 5%SFB for 20 h. Cell supernatants were then tested for the release of PGE2 by EIA kit and cell extracts were analysed for COX-2 expression by Western blot analysis. COX-2 mRNA was investigated by semi-quantitative RT-PCR and COX-2 promoter activity was assessed by transient transfection of full length and partially deleted or mutagenized COX-2 promoter constructs. Results: Stimulation of U937 cells with 20 nmol/L PMA for 20 h caused a marked increase in the production of prostaglandin(PG) E2. This effect was inhibited by 1 micromol/L HT both in PMA-treated U937 (by about 25%) and even more in U937 derived-macrophages (> 40%). Pre-treatment of U937 cells with 0.1-10 micromol/L HT reduced PMA-induced COX-2 protein and mRNA in a concentration dependent manner with a 30% reduction already at 1 mmol/L (p<0.01). HT inhibitory effect was isoform specific given that the constitutive COX-1 expression was not affected. In transient transfection assay, HT reduced PMA-induced COX-2 promoter activity of full length construct. Transient transfections utilizing COX-2 promoter deletion constructs and COX-2 promoter constructs, in which specific enhancer elements were mutagenized, indicated that the effects of HT were, at least in part, mediated via NF-kB elements. Conclusions: HT inhibited PGE2 release and COX-2 expression in phorbol-ester-stimulated monocytes and U937-derived macrophages. These data provide new insight into the anti-inflammatory properties of HT and may contribute to explain the cardiovascular protection by specific components of Mediterranean diets

    The predictive role of intolerance of uncertainty and trait of worry in breast cancer patients: A prospective, observational, single-center clinical study

    BackgroundBreast cancer diagnosis and treatment compromise well-being in a pervasive way, and negative consequences may remain after recovery. The psychological side of breast cancer has been extensively investigated; however, the role of intrusive thoughts and intolerance of uncertainty have been studied less systematically.ObjectivesThe present study aimed to prospectively evaluate worry content, depression, anxiety, and post-traumatic stress symptoms and to define the role of the trait of worry and intolerance of uncertainty (IU) related to breast cancer.MethodsPatients with their first breast cancer diagnosis were enrolled in a single-center, prospective observational trial. The trait of worry and IU were assessed using the Penn State Worry Questionnaire (PSWQ) and the Intolerance of Uncertainty Scale-Revised (IUS-R). The psychological aspects were evaluated using the Worry Domains Questionnaire (WDQ), the Beck Anxiety (BAI), Beck Depression Inventory-II (BDI-II), and the Impact of Event Scale-Revised (IES-R). Questionnaires were administered in a randomized sequence at diagnosis (T0), 3 months post-diagnosis (T1), and 12 months post-diagnosis (T2).ResultsOne hundred and fifty eligible patients were enrolled in the study and provided the T0 assessment. Further compliance rates were 57% at T1 and 64% at T2. All patients showed a significant and continuous increase in the IES-R scale (p &lt; 0.0001) from diagnosis to the end of the study, while no significant changes were observed for the WDQ, BAI, and BDI-II scales. The clinical PSWQ levels and/or high levels of the IUS-R score were the only variables that aided the distinction between patients who maintain high levels of depression, anxiety, and post-traumatic disorders and those who did not.ConclusionAn early assessment of the components of the trait of worry and intolerance of uncertainty could be critical in identifying patients with a higher psychopathological risk. Furthermore, if future studies confirm the present findings, support and monitoring throughout the prognosis may present crucial benefits, and possibly affect the course of treatment

    Intraventricular flow patterns during right ventricular apical pacing

    Objectives To assess differences in blood flow momentum (BFM) and kinetic energy (KE) dissipation in a model of cardiac dyssynchrony induced by electrical right ventricular apical (RVA) stimulation compared with spontaneous sinus rhythm. Methods We cross-sectionally enrolled 12 consecutive patients (mean age 74\ub18 years, 60% male, mean left ventricular ejection fraction 58%\ub16 %), within 48 hours from pacemaker (PMK) implantation. Inclusion criteria were: age>18 years, no PMK-dependency, sinus rhythm with a spontaneous narrow QRS at the ECG, preserved ejection fraction (>50%) and a low percentage of PMKstimulation (<20%). All the participants underwent a complete echocardiographic evaluation, including left ventricular strain analysis and particle image velocimetry. Results Compared with sinus rhythm, BFM shifted from 27\ub13.3 to 34\ub17.6\ub0 (p=0.016), while RVA-pacing was characterised by a 35% of increment in KE dissipation, during diastole (p=0.043) and 32% during systole (p=0.016). In the same conditions, left ventricle global longitudinal strain (LV GLS) significantly decreased from 17\ub13.3 to 11%\ub12.8% (p=0.004) during RVA-stimulation. At the multivariable analysis, BFM and diastolic KE dissipation were significantly associated with LV GLS deterioration (Beta Coeff.=0.54, 95% CI 0.07 to 1.00, p=0.034 and Beta Coeff.=0.29, 95% CI 0.02 to 0.57, p=0.049, respectively). Conclusions In RVA-stimulation, BFM impairment and KE dissipation were found to be significantly associated with LV GLS deterioration, when controlling for potential confounders. Such changes may favour the onset of cardiac remodelling and sustain heart failure

    Design, Implementation, and Evaluation of the Adolescents and Surveillance System for the Obesity Prevention Project

    The Adolescents Surveillance System for Obesity prevention (ASSO) Project aimed at developing standardized and web-based tools for collecting data on adolescents’ obesity and its potential determinants. This has been implemented and piloted in the local area of Palermo city, Italy. The aim of the present study is to provide an overview of the Project’s design, implementation, and evaluation, highlighting all the aspects for a potential scale-up of the surveillance system on the whole national territory and abroad, as a sustainable and effective source of data. The overall structure and management, the ASSO-toolkit, the ASSO-NutFit software, and all developed and used procedures for recruiting, training, and data collecting/analyzing are addressed. An interim evaluation has been performed through a feasibility study; a final Project evaluation has been performed reporting the Strengths, Weaknesses, Opportunities, and Threats (SWOT) and the attributes that a surveillance system should have. This article provides a detailed overview of the Project and highlights that ASSO can be considered a valid, logical, coherent, efficient, and sustainable surveillance system that is consistent with countries’ needs and priorities. The system developed by the ASSO Project provides high-quality data and complies with several characteristics typical of a suitable surveillance system. It has a potential of being adopted within the National Health Service and other countries’ Health Services for monitoring adolescents’ obesity and its determinants, such as food intakes, behaviors, physical activity, and fitness profiles

    Immune activation, immune senescence and levels of Epstein Barr Virus in kidney transplant patients: Impact of mTOR inhibitors

    Abstract Post-transplant lymphoproliferative disorders (PTLD) represent a severe complication in transplanted patients and Epstein-Barr Virus (EBV) is the main driver. Besides immunodepression, immune activation/chronic inflammation play an important role in both virus reactivation and expansion of EBV-positive B cells. The aim of this study was to assess the impact of immunosuppressive strategies on factors involved in the PTLD's pathogenesis. 124 kidney transplanted patients were enrolled in this study: 71 were treated with mycophenolic acid (MPA) and 53 treated with mTOR inhibitor (mTORi), both in combination with different doses of calcineurin inhibitor. At the time of the transplant (T0), profile of inflammation/immune activation and immune senescence didn't differ between the two groups, but after one year of treatment (T1) markers were significantly higher in MPA-treated patients; their immunosenescence process was supported by the greater erosion of telomeres despite their younger age. Percentages of activated B cells and levels of EBV-DNA significantly increased in MPA-treated patients, and at T1 were significantly higher in MPA- than in mTORi-treated patients. Overall, these findings indicate that mTOR inhibitors constrain the inflammation/immune activation and senescence status, thus reducing the expansion of EBV-infected B cells and the risk of virus-associated PTLD in kidney transplant recipients
