    Whole-exome sequencing in osteosarcoma reveals important heterogeneity of genetic alterations

    BACKGROUND: Whole-genome sequencing studies have recently shown that osteosarcomas (OSs) display high rates of structural variation, i.e. they contain many somatic mutations and copy number alterations. TP53 and RB1 show recurrent somatic alterations in concordant studies, suggesting that they could be key players in bone oncogenesis. PATIENTS AND METHODS: we carried out whole-genome sequencing of DNA from seven high-grade OS samples matched with normal tissue from the same patients. RESULTS: We confirmed the presence of genetic alterations of the TP53 (including novel unreported mutations) and RB1 genes. Most interestingly, we identified a total of 84 point mutations and 4 deletions related to 82 different genes in OS samples, of which only 15 have been previously reported. Interestingly, the number of mutated genes (ranging from 4 to 8) was lower in TP53mut cases compared with TP53wt cases (ranging from 14 to 45). This was also true for the mutated RB1 case. We also observed that a dedifferentiated OS harboring MDM2 amplification did not carry any other mutations. CONCLUSION: This study suggests that bone oncogenesis driven by TP53 or RB1 mutations occurs on a background of relative genetic stability and that the dedifferentiated OS subtype represents a clinico-pathological entity with distinct oncogenic mechanisms and thus requires different therapeutic managemen

    Ce volume bilingue, comprenant un ensemble de 28 contributions disponibles en français et en anglais (dans leur version longue ou abrĂ©gĂ©e), propose d’établir un Ă©tat des lieux des rĂ©flexions, recherches et Ă©tudes conduites sur le fait funĂ©raire Ă  l’époque tardo-antique au sein des provinces de l’Empire romain et sur leurs rĂ©gions limitrophes, afin d’ouvrir de nouvelles perspectives sur ses Ă©volutions possibles. Au cours des trois derniĂšres dĂ©cennies, les transformations considĂ©rables des mĂ©thodologies dĂ©ployĂ©es sur le terrain et en laboratoire ont permis un renouveau des questionnements sur les populations et les pratiques funĂ©raires de l’AntiquitĂ© tardive, pĂ©riode marquĂ©e par de multiples changements politiques, sociaux, dĂ©mographiques et culturels. L’apparition de ce qui a Ă©tĂ© initialement dĂ©signĂ© comme une « Anthropologie de terrain », qui fut le dĂ©but de la dĂ©marche archĂ©othanatologique, puis le rĂ©cent dĂ©veloppement d’approches collaboratives entre des domaines scientifiques divers (archĂ©othanatologie, biochimie et gĂ©ochimie, gĂ©nĂ©tique, histoire, Ă©pigraphie par exemple) ont Ă©tĂ© dĂ©cisives pour le renouvellement des problĂ©matiques d’étude : rĂ©vision d’anciens concepts comme apparition d’axes d’analyse inĂ©dits. Les recherches rassemblĂ©es dans cet ouvrage sont articulĂ©es autour de quatre grands thĂšmes : l’évolution des pratiques funĂ©raires dans le temps, l’identitĂ© sociale dans la mort, les ensembles funĂ©raires en transformation (organisation et topographie) et les territoires de l’empire (du cƓur aux marges). Ces Ă©tudes proposent un rĂ©examen et une rĂ©vision des donnĂ©es, tant anthropologiques qu’archĂ©ologiques ou historiques sur l’AntiquitĂ© tardive, et rĂ©vĂšlent, Ă  cet Ă©gard, une mosaĂŻque de paysages politiques, sociaux et culturels singuliĂšrement riches et complexes. Elles accroissent nos connaissances sur le traitement des dĂ©funts, l’emplacement des aires funĂ©raires ou encore la structure des sĂ©pultures, en rĂ©vĂ©lant une diversitĂ© de pratiques, et permettent au final de relancer la rĂ©flexion sur la maniĂšre dont les sociĂ©tĂ©s tardo-antiques envisagent la mort et sur les Ă©lĂ©ments permettant d’identifier et de dĂ©finir la diversitĂ© des groupes qui les composent. Elles dĂ©montrent ce faisant que nous pouvons vĂ©ritablement apprĂ©hender les structures culturelles et sociales des communautĂ©s anciennes et leurs potentielles transformations, Ă  partir de l’étude des pratiques funĂ©raires.This bilingual volume proposes to draw up an assessment of the recent research conducted on funerary behavior during Late Antiquity in the provinces of the Roman Empire and on their borders, in order to open new perspectives on its possible developments. The considerable transformations of the methodologies have raised the need for a renewal of the questions on the funerary practices during Late Antiquity, a period marked by multiple political, social, demographic and cultural changes. The emergence field anthropology, which was the beginning of archaeothanatology, and then the recent development of collaborative approaches between various scientific fields (archaeothanatology, biochemistry and geochemistry, genetics, history, epigraphy, for example), have been decisive. The research collected in this book is structured around four main themes: Evolution of funerary practices over time; Social identity through death; Changing burial grounds (organisation and topography); Territories of the Empire (from the heart to the margins). These studies propose a review and a revision of the data, both anthropological and archaeological or historical on Late Antiquity, and reveal a mosaic of political, social, and cultural landscapes singularly rich and complex. In doing so, they demonstrate that we can truly understand the cultural and social structures of ancient communities and their potential transformations, based on the study of funerary practices

    Tumeurs d'Ewing extra Osseuses, quels traitements ?

    Les tumeurs d'Ewing Extra Osseuses (EEO) appartiennent à la fois aux Tumeurs Mésenchymateuses Malignes (TMM), et aux tumeurs d'Ewing Osseuses (EO). Ainsi, la question se pose de les traiter selon un protocole de TMM (MMT89) ou de tumeurs d'EO (Ew93). Parmi 63 cas consécutifs d'EEO traités entre 1989 et 1999 en France, nous avons comparé ceux traités selon le MMT89 (n=32) et ceux traités selon l'Ew93 (n=31). Les caractéristiques des deux groupes sont similaires. La survie globale (SG) et la survie sans événement (SSE) sont de 70 et 58% pour le groupe Ew93, ce qui est identique à celles des 215 enfants traités pour un EO pendant cette période. Inversement, les taux de SG et de SSE sont significativement inférieurs pour le groupe MMT89 (59 et 44%) comparés à ceux des 503 enfants présentant une TMM (71 et 57%), avec un taux élevé de récidives métastatiques. En analyse multivariée, seul le protocole reste un facteur pronostique.     Sex-Cord Stromal Tumors in Children and Teenagers: Results of the TGM-95 Study

    International audienceBACKGROUND:We present the results of the TGM-95 study for gonadal sex-cord stromal tumors (SCT).METHODS:Between 1995 and 2005, children (<18 years) with gonadal SCT were prospectively registered. Primary gonadal resection was recommended whenever feasible. Patients with disseminated disease or an incomplete resection received neoadjuvant or adjuvant VIP chemotherapy (etoposide, ifosfamide, cisplatinum).RESULTS:Thirty-eight children with ovarian SCT were registered. Median age was 10.7y. Endocrine symptoms were present in 21 cases. The histological diagnoses were as follows: juvenile (23) and adult (3) granulosa cell tumors, Sertoli-Leydig cell tumors (11), and mixed germ cell SCT (1). An initial oophorectomy ± salpingectomy led to complete resection in 23 patients who did not receive adjuvant treatment; two of them relapsed: one achieved second complete remission whereas the other one died of disease. Fifteen patients had tumor rupture and/or malignant ascites: 11 received chemotherapy and did not relapse, four did not receive chemotherapy and relapsed with a fatal outcome in two cases. With a median follow-up of 5.9y, the 5-y EFS and OS rates were respectively 85% and 94%. Eleven patients had localized testicular tumors (median age 0.83y): juvenile granulosa cell tumors (4), Sertoli or Leydig cell tumors (5) and not otherwise specified SCT (2). Treatment was surgery alone with an inguinal orchiectomy. None have relapsed (median follow-up: 5.4y).CONCLUSIONS:Childhood SCT carry favorable prognosis. In ovarian SCT, surgery should be complete and non-mutilating. Adjuvant chemotherapy efficiently prevents recurrences in cases of tumor rupture. In childhood testicular SCT, the prognosis is excellent with an inguinal orchiectomy, prompting the debate on testis-sparing surgery

    SRF-FOXO1 and SRF-NCOA1 Fusion Genes Delineate a Distinctive Subset of Well-differentiated Rhabdomyosarcoma

    International audienceRhabdomyosarcoma (RMS) encompasses a heterogenous collection of tumors in which new groups have recently been identified that improved the World Health Organization (WHO) classification. While performing RNA-sequencing in our routine practice, we identified 3 cases of well-differentiated RMS harboring new fusion genes. We also analyzed these tumors through array-comparative genomic hybridization. Clinically, these tumors were deep paraspinal tumors, occurring in neo-nat and young children. The patients underwent resection and adjuvant therapy. At the time of last follow-up (ranging from 12 to 108 mo), they were alive without disease. Histologically, these tumors consisted of well-differentiated rhabdomyoblastic proliferations with nuclear atypia, infiltrative borders, and a specific growth pattern. These tumors harbored new fusion genes involving SRF and either FOXO1 or NCOA1. We compared the expression profiles of these 3 tumors to the expression data of a series of 33 skeletal muscle tumors including embryonal RMSs, alveolar rhandomyosarcomas, RMSs with VGLL2 fusions, RMSs with the myoD1 mutation, EWSR1/FUS-TFCP2 epithelioid and spindle cell RMSs of the bone, and rhabdomyomas with PTCH1 loss. According to clustering analyses, the 3 SRF-fused tumors formed a distinct group with a specific expression profile different from that of the other types of skeletal muscle tumors. Array-comparative genomic hybridization showed a recurrent gain of chromosome 11. These 3 tumors define a new group of RMS associated with a fusion of the SRF gene. FOXO1 rearrangements, usually used to confirm the diagnosis of alveolar RMS and identify poor-outcome RMSs, were identified in a nonalveolar RMS for the first time

    The French FRACTURE database: A way to improve knowledge on management of children with very rare tumors

    International audienceIntroduction: Very rare pediatric tumors (VRTs), defined by an annual incidence ≀2 per million inhabitants, represent a heterogeneous group of cancers. Due to their extremely low incidence, knowledge on these tumors is scant. Since 2012, the French Very Rare Tumors Committee (FRACTURE) database has recorded clinical data about VRTs in France. This study aims: (a) to describe the tumors registered in the FRACTURE database; and (b) to compare these data with those registered in the French National Registry of Childhood Cancer (RNCE).Methods: Data recorded in the FRACTURE database between January 1, 2012 and December 31, 2018 were analyzed. In addition, these data were compared with those of the RNCE database between 2012 and 2015 to evaluate the completeness of the documentation and understand any discrepancies.Results: A total of 477 patients with VRTs were registered in the FRACTURE database, representing 97 histological types. Of the 14 most common tumors registered in the RNCE (772 patients), only 19% were also registered in the FRACTURE database. Total 39% of children and adolescent VRTs registered in the RNCE and/or FRACTURE database (323 of a total of 828 patients) were not treated in or linked to a specialized pediatric oncology unit.Conclusion: VRTs represent many different heterogenous entities, which nevertheless account for 10% of all pediatric cancers diagnosed each year. Sustainability in the collection of these rare tumor cases is therefore important, and a regular systematic collaboration between the FRACTURE database and the RNCE register helps to provide a more exhaustive picture of these VRTs and allow research completeness for some peculiar groups of patients
