    AMPHIBIAN DISTRIBUTION IN THE GEORGIA SEA ISLANDS: IMPLICATIONS FROM THE PAST AND FOR THE FUTURE

    We summarized amphibian distributions for 12 coastal islands in Georgia, USA. Occurrence among islands was correlated with life history traits, habitats, island size, distance to other islands, and island geological age. Species’ distributions were determined from published literature. Island sizes and vegetation types were derived from 2011 Georgia Department of Natural Resources habitat maps, which included both federal and state vegetation classification systems. Species occurring on more islands tended to have greater total reproductive output (i.e., life span >4 years, and annual egg production >1,000 eggs) and adults had tolerance of brackish environs. Larger islands had great­er area of freshwater wetlands, predominantly short hydroperiod (<6 months). Species tied to long hydroperiod wetlands (>6 months) were more restricted in their distribution across islands. Overall, larger islands supported more species, but the correlation was weaker for geologically younger Ho­locene islands (age <11,000 years). While Euclidean distance between islands does not necessarily preclude inter-island dispersal, inhospitable habitat for amphibians (brackish tidal marshes and creeks interspersed with wide rivers) suggests that inter-island dispersal is very limited. The paucity of recent occurrence data for amphibians in this dynamic coastal region, let alone standardized annual moni­toring data, hinders efforts to model species’ vulnerability in a region susceptible to sea level rise and development pressure. The most common survey method, standardized amphibian vocal surveys, will detect Anuran reproductive efforts, but is unlikely to ascertain if breeding was successful or to detect salamanders. While it will not replace actual population data, consideration of critical life-history traits and breeding habitat availability can be used to direct management to support long-term species per­sistence in changing environs.     Cardiorespiratory requirements of the 6-min walk test in patients with left ventricular systolic disfunction and no major structural heart disease

    The six-minute walk test (6-MWT) is widely used to assess functional status in patients with chronic heart failure (CHF). The aims of the present study were: (1) to compare metabolic gas exchange during the 6-MWT in older patients with left ventricular systolic dysfunction (LVSD) and in breathless patients with no major structural heart disease (MSHD); (2) to determine the exercise intensity of the 6-MWT relative to peak oxygen uptake; (3) to establish the accuracy and reproducibility of the Metamax 3B ergospirometer during an incremental workload. Twenty four older patients with LVSD (19 male; age 76 ± 5 years; BMI 27 ± 4), and 18 patients with no MSHD (12 male; age 75 ± 8 years; BMI 27 ± 4) attended on consecutive days at the same time. Patients completed a 6-MWT with metabolic gas exchange measurements using the Metamax 3B portable ergospirometer, and an incremental cycle ergometry test using both the Metamax 3B and Oxycon Pro metabolic cart. Patients returned and performed a second 6-MWT and an incremental treadmill test, metabolic gas exchange was measured with the Metamax 3B. In patients with LVSD, the 6-MWT was performed at a higher fraction of maximal exercise capacity (p = 0.02). The 6-MWT was performed below the anaerobic threshold in patients with LVSD (83 %) and in patients with no MSHD (61 %). The Metamax 3B showed satisfactory to high accuracy at 10 W and 20 W in patients with LVSD (r = 0.77 - 0.97, p < 0.05), and no MSHD (r = 0.76 - 0.94, p < 0.05). Metabolic gas exchange variables measured during the 6-MWT showed satisfactory to high day-to-day reproducibility in patients with LVSD (ICC = 0.75 - 0.98), but a higher variability was evident in participants with no MSHD (ICC = 0.62 - 0.97). The Metamax 3B portable ergospirometer is an accurate and reproducible device during submaximal, fixed rate exercise in older patients with LVSD and no MSHD. In elderly patients with LVSD and no MSHD, the 6-MWT should not be considered a maximal test of exercise capacity but rather a test of submaximal exercise performance. Our study demonstrates that the 6-MWT takes place at a higher proportion of peak oxygen uptake in patients with LVSD compared to those with no MSHD, and may be one reason why fatigue is a more prominent symptom in these patients

    Validating a virtual human and automated feedback system for training doctor-patient communication skills

    Effective communication between a clinician and their patient is critical for delivering healthcare maximizing outcomes. Unfortunately, traditional communication training approaches that use human standardized patients and expert coaches are difficult to scale. Here, we present the development and validation of a scalable, easily accessible, digital tool known as the Standardized Online Patient for Health Interaction Education (SOPHIE) for practicing and receiving feedback on doctor-patient communication skills. SOPHIE was validated by conducting an experiment with 30 participants. We found that participants who underwent SOPHIE performed significantly better than the control in overall communication, aggregate scores, empowering the patient, and showing empathy (p<0.05p < 0.05 in all cases). One day, we hope that SOPHIE will help make communication training resources more accessible by providing a scalable option to supplement existing resources.Comment: 10 pages, 5 figures, 2 table

    Identification of C-C Chemokine Receptor 1 (CCR1) as the Monocyte Hemofiltrate C-C Chemokine (HCC)-1 Receptor

    Hemofiltrate C-C chemokine (HCC)-1 is a recently cloned C-C chemokine that is structurally similar to macrophage inflammatory protein (MIP)-1α. Unlike most chemokines, it is constitutively secreted by tissues and is present at high concentrations in normal human plasma. Also atypical for chemokines, HCC-1 is reported not to be chemotactic for leukocytes. In this paper, we have investigated the chemokine receptor usage and downstream signaling pathways of HCC-1. Cross-desensitization experiments using THP-1 cells suggested that HCC-1 and MIP-1α activated the same receptor. Experiments using a panel of cloned chemokine receptors revealed that HCC-1 specifically activated C-C chemokine receptor (CCR)1, but not closely related receptors, including CCR5. HCC-1 competed with MIP-1α for binding to CCR1-transfected cells, but with a markedly reduced affinity (IC50 = 93 nM versus 1.3 nM for MIP-1α). Similarly, HCC-1 was less potent than MIP-1α in inducing inhibition of adenylyl cyclase in CCR1-transfected cells. HCC-1 induced chemotaxis of freshly isolated human monocytes, THP-1 cells, and CCR1-transfected cells, and the optimal concentration for cell migration (100 nM) was ∼100-fold lower than that of MIP-1α (1 nM). These data demonstrate that HCC-1 is a chemoattractant and identify CCR1 as a functional HCC-1 receptor on human monocytes

    In mammalian cells, the primary function of the endoplasmic reticulum (ER) is to synthesize and assemble membrane and secreted proteins. As the main site of protein folding and posttranslational modification in the cell, the ER operates a highly conserved quality control system to ensure only correctly assembled proteins exit the ER and misfolded and unfolded proteins are retained for disposal. Any disruption in the equilibrium of the ER engages a multifaceted intracellular signaling pathway termed the unfolded protein response (UPR) to restore normal conditions in the cell. A variety of pathological conditions can induce activation of the UPR, including neurodegenerative disorders such as Parkinson\u27s disease, metabolic disorders such as atherosclerosis, and conformational disorders such as cystic fibrosis. Conformational disorders are characterized by mutations that modify the final structure of a protein and any cells that express abnormal protein risk functional impairment. The monocyte is an important and long-lived immune cell and acts as a key immunological orchestrator, dictating the intensity and duration of the host immune response. Monocytes expressing misfolded or unfolded protein may exhibit UPR activation and this can compromise the host immune system. Here, we describe in detail methods and protocols for the examination of UPR activation in peripheral blood monocytes. This guide should provide new investigators to the field with a broad understanding of the tools required to investigate the UPR in the monocyte

    The incidence of malaria in travellers to South-East Asia: is local malaria transmission a useful risk indicator?

    BACKGROUND: The presence of ongoing local malaria transmission, identified though local surveillance and reported to regional WHO offices, by S-E Asian countries, forms the basis of national and international chemoprophylaxis recommendations in western countries. The study was designed to examine whether the strategy of using malaria transmission in a local population was an accurate estimate of the malaria threat faced by travellers and a correlate of malaria in returning travellers. METHODS: Malaria endemicity was described from distribution and intensity in the local populations of ten S-E Asian destination countries over the period 2003-2008 from regionally reported cases to WHO offices. Travel acquired malaria was collated from malaria surveillance reports from the USA and 12 European countries over the same period. The numbers of travellers visiting the destination countries was based on immigration and tourism statistics collected on entry of tourists to the destination countries. RESULTS: In the destination countries, mean malaria rates in endemic countries ranged between 0.01 in Korea to 4:1000 population per year in Lao PDR, with higher regional rates in a number of countries. Malaria cases imported into the 13 countries declined by 47% from 140 cases in 2003 to 66 in 2008. A total of 608 cases (27.3% Plasmodium falciparum (Pf)) were reported over the six years, the largest number acquired in Indonesia, Thailand and Korea. Four countries had an incidence > 1 case per 100,000 traveller visits; Burma (Myanmar), Indonesia, Cambodia and Laos (range 1 to 11.8-case per 100,000 visits). The remaining six countries rates were 1 case per 100,000 visits to consider targeted malaria prophylaxis recommendations to minimize use of chemoprophylaxis for low risk exposure during visits to S-E Asia.     Supernova Limits on the Cosmic Equation of State

    We use Type Ia supernovae studied by the High-Z Supernova Search Team to constrain the properties of an energy component which may have contributed to accelerating the cosmic expansion. We find that for a flat geometry the equation of state parameter for the unknown component, alpha_x=P_x/rho_x, must be less than -0.55 (95% confidence) for any value of Omega_m and is further limited to alpha_x<-0.60 (95%) if Omega_m is assumed to be greater than 0.1 . These values are inconsistent with the unknown component being topological defects such as domain walls, strings, or textures. The supernova data are consistent with a cosmological constant (alpha_x=-1) or a scalar field which has had, on average, an equation of state parameter similar to the cosmological constant value of -1 over the redshift range of z=1 to the present. Supernova and cosmic microwave background observations give complementary constraints on the densities of matter and the unknown component. If only matter and vacuum energy are considered, then the current combined data sets provide direct evidence for a spatially flat Universe with Omega_tot=Omega_m+Omega_Lambda = 0.94 +/- 0.26 (1-sigma).Comment: Accepted for publication in ApJ, 3 figure
