    The Experiences of Patients in the Synchrony Group Music Therapy Trial for Long-term Depression

    Music therapy may be particularly well placed to address features of depression. The Synchrony study used group songwriting in a feasibility trial of group music therapy for longterm depression. The current study explored the lived experiences of the participants in these groups. Ten individuals participated in semi-structured interviews. These were analysed by a music therapist, research psychologist and lived experience researcher, using interpretative phenomenological analysis (IPA). Three superordinate themes were identified: „the group as a happy and safe place‟, „music stimulates new feelings and songwriting aids expression into words‟ and „uncertainty, unmet needs and the ending were challenging‟. Findings underscore the importance of early group cohesion and the role of music and song-writing in promoting enjoyment, exploration and a sense of achievement. Group setup requires careful consideration of expectation setting, levels of musicianship and readiness for songwriting and preparation for ending, so as not to reinforce hopelessness, disappointment and loss

    Expert nurses' perceptions of the relevance of Carper's patterns of knowing to junior nurses.

    This study explored with expert nurses in the UK how nursing wisdom can be developed in new and junior nurses. Carper's patterns of knowing and Benner's novice-to-expert continuum formed the theoretical framework. Employing a constructionist research methodology with participant engagement in co-construction of findings, data were collected via two separate cycles comprising four consecutive sessions followed by a nationally-advertised mini-conference. Empirical, ethical, personal, and aesthetic knowing were considered evident in junior nurses. Junior nurses in the UK seem to lack a previously-unrecognized domain of organizational knowing without which they cannot overcome hegemonic barriers to the successful development of nursing praxis

    IoT and Its Implications for Informed Consent

    This report is based on a three-hour long workshop between representatives of the PETRAS IoT Hub, Pinsent Masons, and the HMG Department for Transport. The workshop is part of an ongoing investigation that explores the connections between some of the different dimensions likely to shape conceptions and applications of consent in the emerging Internet of Things (IoT). The impetus for the workshop was the recognition that two significant developments will challenge conventional approaches to online consent. From a technical perspective, the IoT will significantly increase personal data collection, use and re-use. From a regulatory perspective, the General Data Protection Regulation (GDPR) which comes into force in May 2018, will make much higher demands on practices of giving and obtaining consent. Combined, these two factors suggest that consent will be a major issue for all actors in the next five years and it requires some careful analysis now in order to adequately prepare for these developments

    New standards for permanent tooth emergence in Australian children

The aim of this study was to present new data on the timing and sequence of permanent tooth emergence in a large sample of Australian children and to compare the findings with those of earlier studies. Methods: Records of a randomly selected sample of 8676 children, aged between 4 to 16 years, who had attended the clinics of the South Australian Dental Service were scanned optically and coded to provide the data for this study. A logistic regression analysis enabled median times of emergence, together with percentile ranges, to be calculated for each tooth, except third molars, in boys and girls. Comparisons were made between the sexes and with previously published values. The frequencies of occurrence of emergence polymorphisms for different tooth pairs were also computed. Results: The earliest teeth to emerge in both boys and girls were the central incisors and first molars, whereas the second premolars and molars and maxillary canines tended to be last to emerge. The mandibular teeth tended to precede the corresponding maxillary teeth in emergence in both sexes. Tooth emergence was advanced in girls compared with boys, averaging 4.5 months in the maxilla and 5.3 months in the mandible. In general, emergence times of children in the present sample were later than those reported previously for Australian children. The most common emergence polymorphism in the maxilla involved the canine and second premolar, whereas common polymorphisms in the mandible were noted for the central incisor and first molar, canine and first premolar, and the second premolar and second molar. Conclusions: Given that they have been collected relatively recently from a large sample of children, the new data reported in this paper can now be used as standards when assessing permanent tooth emergence of Australian children.J Diamanti, GC Townsen

    The impact of pre-restoration land-use and disturbance on sediment structure, hydrology and the sediment geochemical environment in restored saltmarshes

    Saltmarshes are being lost or degraded as a result of human activity resulting in loss of critical ecosystem services including the provision of wild species diversity, water quality regulation and flood regulation. To compensate, saltmarshes are being restored or re-created, usually driven by legislative requirements for increased habitat diversity, flood regulation and sustainable coastal defense. Yet, there is increasing evidence that restoration may not deliver anticipated ecosystem services; this is frequently attributed to poor drainage and sediment anoxia. However, physical sediment characteristics, hydrology and the sediment geochemical environment are rarely examined in restoration schemes, despite such factors being critical for plant succession. This study presents the novel integration of 3D-computed X-ray microtomography to quantify sediment structure and porosity, with water level and geochemical data to understand the impact of pre-restoration land use and disturbance on the structure and functioning of restored saltmarshes. The study combines a broad-scale investigation of physical sediment characteristics in nine de-embanked saltmarshes across SE England, with an intensive study at one site examining water levels, sediment structure and the sediment geochemical environment. De-embankment does not restore the hydrological regime, or the physical/chemical framework in the saltmarshes and evidence of disturbance includes a reduction in microporosity, pore connectivity and water storage capacity, a lack of connectivity between the sub-surface environment and overlying floodwaters, and impeded sub-surface water flow and drainage. This has significant consequences for the sediment geochemical environment. This disturbance is evident for at least two decades following restoration and is likely to be irreversible. It has important implications for plant establishment in particular, ecosystem services including flood regulation, nutrient cycling and wild species diversity and for future restoration design

    Homozygosity for a missense mutation in the 67 kDa isoform of glutamate decarboxylase in a family with autosomal recessive spastic cerebral palsy: parallels with Stiff-Person Syndrome and other movement disorders

    Background Cerebral palsy (CP) is an heterogeneous group of neurological disorders of movement and/or posture, with an estimated incidence of 1 in 1000 live births. Non-progressive forms of symmetrical, spastic CP have been identified, which show a Mendelian autosomal recessive pattern of inheritance. We recently described the mapping of a recessive spastic CP locus to a 5 cM chromosomal region located at 2q24-31.1, in rare consanguineous families. Methods Here we present data that refine this locus to a 0.5 cM region, flanked by the microsatellite markers D2S2345 and D2S326. The minimal region contains the candidate gene GAD1, which encodes a glutamate decarboxylase isoform (GAD67), involved in conversion of the amino acid and excitatory neurotransmitter glutamate to the inhibitory neurotransmitter Îł-aminobutyric acid (GABA). Results A novel amino acid mis-sense mutation in GAD67 was detected, which segregated with CP in affected individuals. Conclusions This result is interesting because auto-antibodies to GAD67 and the more widely studied GAD65 homologue encoded by the GAD2 gene, are described in patients with Stiff-Person Syndrome (SPS), epilepsy, cerebellar ataxia and Batten disease. Further investigation seems merited of the possibility that variation in the GAD1 sequence, potentially affecting glutamate/GABA ratios, may underlie this form of spastic CP, given the presence of anti-GAD antibodies in SPS and the recognised excitotoxicity of glutamate in various contexts
