    Atypical clinical presentation and long-term survival in a patient with optic nerve medulloepithelioma: a case report

    <p>Abstract</p> <p>Introduction</p> <p>Medulloepithelioma is a rare congenital tumor of the primitive medullary neuroepithelium. A significant proportion of patients with medulloepithelioma arising from the optic nerve die from intracranial spread or cerebral metastasis. Because it has no known distinct clinical features and because of its low frequency, this tumor presents within the first two to six years of life and is usually misdiagnosed clinically as a different type of optic nerve tumor. Here, we describe a new and atypical case of medulloepithelioma of the optic nerve in a 12-year-old boy. To the best of our knowledge, he is the oldest reported patient to present with this disease and, now as an adult, has the longest documented period of disease-free survival.</p> <p>Case presentation</p> <p>A 12-year-old Caucasian boy with headache and unilateral amaurosis was referred for a presumed optic nerve glioma to our hospital. A computed tomography scan showed optic nerve enlargement, and fundoscopy showed a whitish mass at the optic disc. Our patient had been followed at his local hospital for four years for an 'optic disc cyst' with no change or progression. He experienced mild progressive visual impairment during that period. He was admitted for resection, and a histopathological analysis revealed a medulloepithelioma of the optic nerve. Supplemental orbital radiotherapy was performed. He remained disease-free for 25 years.</p> <p>Conclusions</p> <p>Medulloepithelioma of the optic nerve can clinically mimic more common pediatric tumors, such as optic glioma, meningioma, or retinoblastoma. Thus, medulloepithelioma should be included in the differential diagnoses of pediatric optic nerve lesions. Fundoscopy in these patients may provide relevant information for diagnosis. Anterior optic nerve medulloepitheliomas may behave differently from and have a better prognosis than medulloepitheliomas that have a more posterior location. Our case report illustrates that long-term survival can be achieved in patients with this malignant tumor.</p

    Axicabtagene ciloleucel compared to tisagenlecleucel for the treatment of aggressive B-cell lymphoma

    Axicabtagene ciloleucel (axi-cel) and tisagenlecleucel (tisa-cel) are CD19-targeted chimeric antigen receptor (CAR) T cells approved for relapsed/refractory (R/R) large B-cell lymphoma (LBCL). We performed a retrospective study to evaluate safety and efficacy of axi-cel and tisa-cel outside the setting of a clinical trial. Data from consecutive patients with R/R LBCL who underwent apheresis for axi-cel or tisa-cel were retrospectively collected from 12 Spanish centers. A total of 307 patients underwent apheresis for axi-cel (n=152) and tisa-cel (n=155) from November 2018 to August 2021, of which 261 (85%) received a CAR T infusion (88% and 82%, respectively). Median time from apheresis to infusion was 41 days for axi-cel and 52 days for tisa-cel (P =0.006). None of the baseline characteristics were significantly different between both cohorts. Both cytokine release syndrome and neurologic events (NE) were more frequent in the axi-cel group (88% vs. 73%, P =0.003, and 42% vs. 16%, P <0.001, respectively). Infections in the first 6 months post-infusion were also more common in patients treated with axi-cel (38% vs. 25%, P =0.033). Non-relapse mortality was not significantly different between the axi-cel and tisa-cel groups (7% and 4%, respectively, P =0.298). With a median follow-up of 9.2 months, median PFS and OS were 5.9 and 3 months, and 13.9 and 11.2 months for axi-cel and tisa-cel, respectively. The 12-month PFS and OS for axi-cel and tisa-cel were 41% and 33% (P =0.195), 51% and 47% (P =0.191), respectively. Factors associated with lower OS in the multivariate analysis were increased lactate dehydrogenase, ECOG ≥2 and progressive disease before lympho-depletion. Safety and efficacy results in our real-world experience were comparable with those reported in the pivotal trials. Patients treated with axi-cel experienced more toxicity but similar non-relapse mortality compared with those receiving tisa-cel. Efficacy was not significantly different between both products

    El trabajo obtuvo un Premio Tomás García Verdejo a las buenas prácticas educativas en la Comunidad Autónoma de Extremadura para el curso académico 2015/2016. Modalidad ASe presenta un proyecto llevado a cabo entre varios centros educativos de la provincia de Cáceres que consistió en hacer actividades de fomento y animación a la lectura ligadas a los continentes del globo terrestre. Los objetivos del proyecto fueron: fomentar la relación y convivencia entre los centros; promover el trabajo colaborativo; propiciar ambientes que fomenten valores como la responsabilidad, el esfuerzo, la igualdad entre géneros y el respesto; conocer y disfrutar las diferentes formas literarias como relatos, poemas y obras dramáticas a través de la lectura, la escucha y la propia creación y recreación; comprender y respetar las diferentes culturas y las diferencias entre las personas; iniciarse en el uso de las nuevas tecnologías desarrollando un espíritu crítico ante los mensajes que reciben y elaboran y utilizar diferentes representaciones y expresiones artísticas e iniciarse en la construcción de propuestas visuales y audiovisualesExtremaduraES

    Los temas que en esta obra se tratan: Estado, territorio, espacios transfronterizos, procesos de integración regional, tradicionalmente se han abordado desde teorías de las Ciencias Sociales desarrolladas en Europa y en Estados Unidos. Esto podría representar una bofetada a la realidad, en este caso latinoamericana, en tanto que algunas de las experiencias de integración regional en América Latina, se dieron mucho antes de que Europa comenzara con su proceso integracionista. El usar marcos teóricos europeos y estadounidenses, por supuesto, no es ningún pecado, siempre y cuando se considere que tales paradigmas nacieron para explicar realidades de esos países y no para dar cuentas de los procesos socio-históricos de regiones como América Latina, África, Asia, Oceanía, ni del sur dentro del norte, es decir, las zonas periféricas en el interior de los países desarrollados. Este libro es un intento de romper con la pretensión de ciencia (social) única y absoluta con que se ha presentado (y nosotros, muchas veces, hemos aceptado) el pensamiento eurocéntrico. Pero, la misma receta debe aplicarse desde el sur: la humildad o la negación de la pretendida verdad absoluta. Es decir, lo decolonial pasa por el reconocimiento de que por más atrincherados de métodos cuantitativos y cualitativos que estemos, por más triangulación metodológica empleada, nuestros estudios serán una aproximación de algo, no un retrato y mucho menos, un video

    Exercise benefits in cardiovascular disease : beyond attenuation of traditional risk factors

    Despite strong scientific evidence supporting the benefits of regular exercise for the prevention and management of cardiovascular disease (CVD), physical inactivity is highly prevalent worldwide. In addition to merely changing well-known risk factors for systemic CVD, regular exercise can also improve cardiovascular health through non-traditional mechanisms. Understanding the pathways through which exercise influences different physiological systems is important and might yield new therapeutic strategies to target pathophysiological mechanisms in CVD. This Review includes a critical discussion of how regular exercise can have antiatherogenic effects in the vasculature, improve autonomic balance (thereby reducing the risk of malignant arrhythmias), and induce cardioprotection against ischaemia–reperfusion injury, independent of effects on traditional CVD risk factors. This Review also describes how exercise promotes a healthy anti-inflammatory milieu (largely through the release of muscle-derived myokines), stimulates myocardial regeneration, and ameliorates age-related loss of muscle mass and strength, a frequently overlooked non-traditional CVD risk factor. Finally, we discuss how the benefits of exercise might also occur via promotion of a healthy gut microbiota. We argue, therefore, that a holistic view of all body systems is necessary and useful when analysing the role of exercise in cardiovascular health
