    Physician Time Management

    Introduction: Time management is an essential skill set for physicians. The importance of time management is not routinely emphasized in undergraduate or graduate medical education curricula, often resulting in the development of poor time-management practices early in training. Improving time-management practices may lead to decreased stress, increased productivity, and improved well-being for physicians. Methods: This interactive workshop targeted trainees and junior faculty. It aimed to highlight common physician knowledge gaps with respect to cognitive limitations and to teach effective time-management strategies. It also aimed to educate learners about how time management may increase physician career satisfaction. The workshop included a detailed presentation with structured resources to reinforce skill development. Results: This workshop was given four times to 54 residents in two different training paradigms. Evaluations were based on a 4-point Likert scale (1 = Strongly Disagree, 4 = Strongly Agree). Overall, participants indicated that the workshop addressed an educational need (M = 3.72) and would recommend this workshop to a colleague (M = 3.83). Follow-up survey results at 4 months indicated that most workshop participants had noticed some degree of improved productivity and well-being, that only a small minority had not incorporated new elements of time management into routine practices. Discussion: This workshop offers an effective way to teach time-management strategies to physicians. Our results imply that this workshop meets an early career physician need by addressing a necessary skill set. Effective time-management skills may promote physician career sustainability

    Floodplains by Design: Advancing a new generation of holistic floodplain practices and projects

    In 2011, the Puget Sound Partnership created a floodplain recovery goal and embraced an integrated, multi-benefit approach to addressing one of the most critical elements of Salish Sea recovery – mainstem river and estuary habitat restoration. The Floodplains by Design partnership was launched in 2012 to build the know-how, support and incentives to accelerate the adoption of integrated floodplain management approaches to advance floodplain recovery. This presentation will focus on the results of an intensive review and re-visioning process aimed at taking stock of what’s worked over the last 5 years and developing a roadmap for the next 5. Input has been garnered through interviews, online surveys and a large workshop with stakeholders, partners and leaders from across the region and representing a broad diversity of interests. The effort will culminate in a 5-year Plan to define what is to be accomplished over the next few years, and address how the partnership should be organized and funded. The goal is to build on the foundation that’s been laid and significantly increase our collective abilities to make floodplain communities and ecosystems more resilient. The presentation will include a mix of quantitative and qualitative results from the stakeholder/partner input process, a snapshot of key lessons learned relevant to local and regional practitioners, and an overview of the path forward as the region moves toward more holistic, collaborative, and resilient floodplain management

    Obesity‐Related Hormones in Low‐Income Preschool‐Age Children: Implications for School Readiness

    Mechanisms underlying socioeconomic disparities in school readiness and health outcomes, particularly obesity, among preschool‐aged children are complex and poorly understood. Obesity can induce changes in proteins in the circulation that contribute to the negative impact of obesity on health; such changes may relate to cognitive and emotion regulation skills important for school readiness. We investigated obesity‐related hormones, body mass index ( BMI ), and school readiness in a pilot study of low‐income preschoolers attending Head Start (participating in a larger parent study). We found that the adipokine leptin was related to preschoolers' BMI z ‐score, the appetite‐regulating hormones ghrelin and glucagon‐like peptide 1 ( GLP ‐1), and pro‐inflammatory cytokines typically associated with early life stress; and that some of these obesity‐related biomarkers were in turn related to emotion regulation. Future work should evaluate how obesity may affect multiple domains of development, and consider modeling common physiological pathways related to stress, health, and school readiness.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/101799/1/mbe12034.pd

    Patterns of genomic loss of heterozygosity predict homologous recombination repair defects in epithelial ovarian cancer

    Background: Defects in BRCA1, BRCA2, and other members of the homologous recombination pathway have potential therapeutic relevance when used to support agents that introduce or exploit double-stranded DNA breaks. This study examines the association between homologous recombination defects and genomic patterns of loss of heterozygosity (LOH). Methods: Ovarian tumours from two independent data sets were characterised for defects in BRCA1,BRCA2, and RAD51C, and LOH profiles were generated. Publically available data were downloaded for a third independent data set. The same analyses were performed on 57 cancer cell lines. Results: Loss of heterozygosity regions of intermediate size were observed more frequently in tumours with defective BRCA1 or BRCA2 (P=10−11). The homologous recombination deficiency (HRD) score was defined as the number of these regions observed in a tumour sample. The association between HRD score and BRCA deficiency was validated in two independent ovarian cancer data sets (P=10−5 and 10−29), and identified breast and pancreatic cell lines with BRCA defects. Conclusion: The HRD score appears capable of detecting homologous recombination defects regardless of aetiology or mechanism. This score could facilitate the use of PARP inhibitors and platinum in breast, ovarian, and other cancers

    A modeling tool to evaluate regional coral reef responses to changes in climate and ocean chemistry

    This is the published version.We developed a spreadsheet-based model for the use of managers, conservationists, and biologists for projecting the effects of climate change on coral reefs at local-to-regional scales. The COMBO (Coral Mortality and Bleaching Output) model calculates the impacts to coral reefs from changes in average SST and CO2 concentrations, and from high temperature mortality (bleaching) events. The model uses a probabilistic assessment of the frequency of high temperature events under a future climate to address scientific uncertainties about potential adverse effects. COMBO offers data libraries and default factors for three selected regions (Hawai’i, Great Barrier Reef, and Caribbean), but it is structured with user-selectable parameter values and data input options, making possible modifications to reflect local conditions or to incorporate local expertise. Preliminary results from sensitivity analyses and simulation examples for Hawai’i demonstrate the relative importance of high temperature events, increased average temperature, and increased CO2 concentration on the future status of coral reefs; illustrate significant interactions among variables; and allow comparisons of past environmental history with future predictions
