    INTERACTIVE POLYPHENOLS-BASED BIOPACKAGING FOR FOOD PRESERVATION: AN IN VITRO STUDY

    Producing green, sustainable and renewable materials is one of the major challenge nowadays in the food-packaging sector. In this context, the aim of our study is to develop an active cellulose-based packaging, cellulose in fact is one of the most plentiful polymer on the earth. Tetrahydrocurcumin (THC), a derivative of curcuma, chosen as antioxidant and antimicrobial natural substance, was added into a cellulose matrix (1.5% w/w) and the resulting material was then studied. In addition to this active compound, the paper also contained chitosan and carboxymethylcellulose, to improve the retention of THC and the mechanical properties. Mechanical, chemical and microbiological analyses were done to completely characterize the active papers. Grammage, dry and wet strength were determined. The polyphenol content was determined by the Folin-Ciolcalteu method. Antimicrobial activity of THC, in solution and after its incorporation into the papers, was tested against Gram negative bacteria (Escherichia coli and Pseudomonas putida), Gram positive bacteria (Staphylococcus aureus and Listeria innocua) and moulds (Penicillium chrysogenum and Aspergillus niger). Antioxidant capacity was also tested through the DPPH method. THC solution presented a good antioxidant capacity: it showed an EC50 equal to 4,49 ppm ( EC50 of Trolox was 2,86 ppm). Its minimal inhibitory concentration is 0,4 g/L for Gram positive bacteria, and higher than 0,6 g/L for Gram negative. THC solution was found effective in delaying the development of moulds. THC polyphenols were quantified as 0,8% on paper weight for the paper containing THC and these quantities together with chitosan resulted able to slow down the growth of microorganism. Pseudomonas is inhibited by the presence of chitosan and THC is able to amplify the antimicrobial activity of chitosan, by inhibiting also the growth of Staphylococcus and E. Coli. The presence of THC does neither affect the mechanical properties of papers, nor the color and the odor. Based on these results, it is possible to conclude that THC exhibits good antioxidant and moderate antimicrobial properties. Paper sheets didn’t lose their mechanical properties. These data will pave the way to the use of THC for the production of an active paper-based packaging to improve the shelf life of food items

    Study of cellulose-lysozyme interactions aimed to a controlled release system for bioactives

    The potential use of cellulose based materials for active food packaging applications was explored. For this purpose, the sorption of lysozyme in a cellulose based paper pulp was optimized using the experimental design methodology. A Face-Centered Composite design was applied to investigate the effect of short/long fibers ratio and carboxymethyl cellulose concentration on linking lysozyme to the fibers surface and to optimize the cellulosic matrix composition. The related polynomial model showed good fitting ability (R2[0.9) as well as good prediction ability (Q2[0.8). The best combination of factors was 50 % short fibers, 50 % long fibers and at least 4 % carboxymethyl cellulose. Afterwards the lysozyme release was investigated using a mathematical model based on Fick\u2019s second law. It was able to describe the release kinetics of lysozyme from the paper sheets made with the optimized cellulosic matrix into water at different temperatures (between 4 and 23 C), pH (from 4 to 7) and NaCl concentrations (from 0.2 to 2 M). The apparent diffusion coefficients for the lysozyme were in the range of 3.1 X 10-16 6.6x10-14 and the Ea value was 22.1 kJ/mol at pH 5 and NaCl concentration of 0.5 M

    Antimicrobial activity of lysozyme and lactoferrin incorporated in cellulose-based food packaging

    The antimicrobial proteins lysozyme and lactoferrin were incorporated into paper containing carboxymethyl cellulose, that allowed non-covalent binding of the positively charged proteins to the paper matrix. More than 60 percent of the proteins added alone or in combination during the papermaking process were released in buffered saline. The released proteins retained their structural and functional features, indicating that the papermaking process did not affect their structure. The antimicrobial activity on common food contaminants was also retained in the released protein, and a synergism between the two proteins was evident in tests carried out with paper containing both proteins against Listeria. Tests on thin meat slices laid on paper sheets containing either or both antimicrobial proteins indicated that lysozyme was most effective in preventing growth of this particular microbiota

    Epidemiology and complications of late-onset sepsis: An Italian area-based study

    Background Most studies regarding late-onset sepsis (LOS) address selected populations (i.e., neonates with low birth weight or extremely preterm neonates). Studying all age groups is more suitable to assess the burden of single pathogens and their clinical relevance. Methods This is a retrospective regional study involving paediatric departments and NICUs in Emilia-Romagna (Italy). Regional laboratory databases were searched from 2009 to 2012. Records of infants (aged 4 to 90 days) with a positive blood or cerebrospinal fluid (CSF) culture were retrospectively reviewed and analysed according to acquisition mode (whether hospital- or community-acquired). Results During the study period, there were 146,682 live births (LBs), with 296 patients experiencing 331 episodes of LOS (incidence rate: 2.3/1000 LBs). Brain lesions upon discharge from the hospital were found in 12.3% (40/296) of cases, with death occurring in 7.1% (23/296; 0.14/ 1000 LBs). With respect to full-term neonates, extremely preterm or extremely low birth weight neonates had very high risk of LOS and related mortality (> 100- and > 800-fold higher respectively). Hospital-acquired LOS (n = 209) was significantly associated with very low birth weight, extremely preterm birth, pneumonia, mechanical ventilation, and death (p< 0.01). At multivariate logistic regression analysis, catecholamine support (OR = 3.2), central venous line before LOS (OR = 14.9), and meningitis (OR = 44.7) were associated with brain lesions or death in hospital-acquired LOS (area under the ROC curve 0.81, H-L p = 0.41). Commonly identified pathogens included coagulase-negative staphylococci (CoNS n = 71, 21.4%), Escherichia coli (n = 50, 15.1%), Staphylococcus aureus (n = 41, 12.4%) and Enterobacteriaceae (n = 41, 12.4%). Group B streptococcus was the predominant cause of meningitis (16 of 38 cases, 42%). Most pathogens were sensitive to first line antibiotics. Conclusions This study provides the first Italian data regarding late-onset sepsis (LOS) in all gestational age groups. Compared to full-term neonates, very high rates of LOS and mortality occurred in neonates with a lower birth weight and gestational age. Group B streptococcus was the leading cause of meningitis. Excluding CoNS, the predominant pathogens were Escherichia coli and Staphylococcus aureus. Neonates with hospital-acquired LOS had a worse outcome.     Escherichia coli Is Overtaking Group B Streptococcus in Early-Onset Neonatal Sepsis

    The widespread use of intrapartum antibiotic prophylaxis (IAP) to prevent group B streptococcus (GBS) early-onset sepsis (EOS) is changing the epidemiology of EOS. Italian prospective area-based surveillance data (from 1 January 2016 to 31 December 2020) were used, from which we identified 64 cases of culture-proven EOS (E. coli, n = 39; GBS, n = 25) among 159,898 live births (annual incidence rates of 0.24 and 0.16 per 1000, respectively). Approximately 10% of E. coli isolates were resistant to both gentamicin and ampicillin. Five neonates died; among them, four were born very pre-term (E. coli, n = 3; GBS, n = 1) and one was born full-term (E. coli, n = 1). After adjustment for gestational age, IAP-exposed neonates had ≥95% lower risk of death, as compared to IAP-unexposed neonates, both in the whole cohort (OR 0.04, 95% CI 0.00–0.70; p = 0.03) and in the E. coli EOS cohort (OR 0.05, 95% CI 0.00–0.88; p = 0.04). In multi-variable logistic regression analysis, IAP was inversely associated with severe disease (OR = 0.12, 95% CI 0.02–0.76; p = 0.03). E. coli is now the leading pathogen in neonatal EOS, and its incidence is close to that of GBS in full-term neonates. IAP reduces the risk of severe disease and death. Importantly, approximately 10% of E. coli isolates causing EOS were found to be resistant to typical first-line antibiotics

    Body composition parameters, immunonutritional indexes, and surgical outcome of pancreatic cancer patients resected after neoadjuvant therapy: A retrospective, multicenter analysis

    Background and aims: Body composition parameters and immunonutritional indexes provide useful information on the nutritional and inflammatory status of patients. We sought to investigate whether they predict the postoperative outcome in patients with pancreatic cancer (PC) who received neoadjuvant therapy (NAT) and then pancreaticoduodenectomy. Methods: Data from locally advanced PC patients who underwent NAT followed by pancreaticoduodenectomy between January 2012 and December 2019 in four high-volume institutions were collected retrospectively. Only patients with two available CT scans (before and after NAT) and immunonutritional indexes (before surgery) available were included. Body composition was assessed and immunonutritional indexes collected were: VAT, SAT, SMI, SMA, PLR, NLR, LMR, and PNI. The postoperative outcomes evaluated were overall morbidity (any complication occurring), major complications (Clavien-Dindo ≥ 3), and length of stay. Results: One hundred twenty-one patients met the inclusion criteria and constituted the study population. The median age at the diagnosis was 64 years (IQR16), and the median BMI was 24 kg/m2 (IQR 4.1). The median time between the two CT-scan examined was 188 days (IQR 48). Skeletal muscle index (SMI) decreased after NAT, with a median delta of −7.8 cm2/m2 (p < 0.05). Major complications occurred more frequently in patients with a lower pre-NAT SMI (p = 0.035) and in those who gained in subcutaneous adipose tissue (SAT) compartment during NAT (p = 0.043). Patients with a gain in SMI experienced fewer major postoperative complications (p = 0.002). The presence of Low muscle mass after NAT was associated with a longer hospital stay [Beta 5.1, 95%CI (1.5, 8.7), p = 0.006]. An increase in SMI from 35 to 40 cm2/m2 was a protective factor with respect to overall postoperative complications [OR 0.43, 95% (CI 0.21, 0.86), p < 0.001]. None of the immunonutritional indexes investigated predicted the postoperative outcome. Conclusion: Body composition changes during NAT are associated with surgical outcome in PC patients who receive pancreaticoduodenectomy after NAT. An increase in SMI during NAT should be favored to ameliorate the postoperative outcome. Immunonutritional indexes did not show to be capable of predicting the surgical outcome
