Modeling Antibiotic Resistance when Adding a New Antibiotic to a Hospital Setting.
As of now, not many pharmaceutical companies are producing new categories of antibiotics to fight bacterial infections. Therefore, bacteria are building up a resistance to the medications commonly used. Often, antibiotic resistance begins within a hospital. To combat resistance, researchers completed several studies using cycling of the medications that are already in place, but they found either no improvement or the resistance increased with this type of setting. In addition, although preventative infection control measures have been shown to decrease antibiotic resistance for some antibiotics, the level of antibiotic resistance found in hospitals is still extremely high. This motivates the main goal of this thesis: to quantify how much the overall resistance can be lowered by simply adding one new drug to the regimen.
The process of adding a new antibiotic can be quantified using mathematical models that show the flow of patients colonized with various types of bacteria into, out of, and within the hospital. Deterministic models can be used to model the spread of resistant bacteria in hospitals with a relatively large number of beds. However, not all hospitals are large enough to accurately determine the effects using a deterministic model; thus, we must use stochastic models, where mathematical formulations include probability in ways that describe intrinsic random fluctuations, typical of infection processes at smaller scales.
In examining the addition of a new antibiotic within a hospital, we consider different administration protocols, either assuming that physicians are equally likely to prescribe the new antibiotic as they are to prescribe existing antibiotics or that physicians prescribe the new antibiotic to only a targeted population of patients. We will examine the variation in the expected level of overall resistance in a hospital depending on the administration procedure as well as the whether the hospital is large (deterministic model) or small (stochastic model). We will conclude with initial results for fitting these models to simulated data using common inverse problem methodology
Fragmentation of Positronium (Ps) in collision with Li ion
Fragmentation of ground state ortho Positronium (Ps) in collision with Li ion
(Li+) is studied in the framework of post collisional Coulomb distorted eikonal
approximation (CDEA) for the target elastic case . The present model takes
account of the two center effect on the ejected e which is crucial for a proper
description of the projectile ionization involving an ionic target. Both the
fully differential (TDCS) and the doubly differential (DDCS) cross sections
(energy spectra) are investigated at intermediate and high incident energies. A
broad distinct Electron loss peak (ELP) centered around v_e ~ v_p is noted in
the e energy spectrum in contrast to the sharp ELP for a heavy projectile. Two
salient features are noted in the present study: i) the shift of the e DDCS
peak (summed over e+ angles) towards higher ejection energy with respect to
half the residual energy of the system, ii) comparison of the e& e+ energy
spectra reflect a strong e - e+ asymmetry with respect to the ratio v_e/v_p =1
>. Both these features could be attributed to the post collisional two center
effect on the e due to its parent nucleus (e+) and the screened target ion .
Two different wave functions of the Li ion are chosen in order to test the
sensitivity of the present results with respect to the choice of the wave

Offenders' Crime Narratives across Different Types of Crimes
The current study explores the roles offenders see themselves playing during an offence and their relationship to different crime types. One hundred and twenty incarcerated offenders indicated the narrative roles they acted out whilst committing a specific crime they remembered well. The data were subjected to Smallest Space Analysis (SSA) and four
themes were identified: Hero, Professional, Revenger and Victim in line with the recent theoretical framework posited for Narrative Offence Roles (Youngs & Canter, 2012). Further analysis showed that different subsets of crimes were more like to be associated with different narrative offence roles. Hero and Professional were found to be associated with property offences (theft, burglary and shoplifting), drug offences and robbery and Revenger
and Victim were found to be associated with violence, sexual offences and murder. The theoretical implications for understanding crime on the basis of offenders' narrative roles as well as practical implications are discussed
Semantic categories underlying the meaning of ‘place’
This paper analyses the semantics of natural language expressions that are associated with the intuitive notion of ‘place’. We note that the nature of such terms is highly contested, and suggest that this arises from two main considerations: 1) there are a number of logically
distinct categories of place expression, which are not always clearly distinguished in discourse about ‘place’; 2) the many non-substantive place count nouns (such as ‘place’, ‘region’, ‘area’, etc.) employed in natural
language are highly ambiguous. With respect to consideration 1), we propose that place-related expressions
should be classified into the following distinct logical types: a) ‘place-like’ count nouns (further subdivided into abstract, spatial and substantive varieties), b) proper names of ‘place-like’ objects, c) locative property phrases, and d) definite descriptions of ‘place-like’ objects. We outline possible formal representations for each of these. To address consideration 2), we examine meanings, connotations and ambiguities of the English vocabulary of abstract and generic place count nouns, and identify underlying elements of meaning, which explain both
similarities and differences in the sense and usage of the various terms
Expressive and Instrumental Offending: Reconciling the Paradox of Specialisation and Versatility
Although previous research into specialisation has been dominated by the debate over the existence of specialisation versus versatility, it is suggested that research needs to move beyond the restrictions of this dispute. The current study explores the criminal careers of 200 offenders based on their criminal records, obtained from a police database in the North West of England, aiming to understand the patterns and nature of specialisation by determining the presence of differentiation within their general offending behaviours and examining whether the framework of Expressive and Instrumental offending styles can account for any specialised tendencies that emerge. Fifty-eight offences were subjected to Smallest Space Analysis. Results revealed that a model of criminal differentiation could be identified and that any specialisation is represented in terms of Expressive and Instrumental offending styles
FIRST-line support for Assistance in Breathing in Children (FIRST-ABC): protocol for a multicentre randomised feasibility trial of non-invasive respiratory support in critically ill children.
INTRODUCTION: Over 18 000 children are admitted annually to UK paediatric intensive care units (PICUs), of whom nearly 75% receive respiratory support (invasive and/or non-invasive). Continuous positive airway pressure (CPAP) has traditionally been used to provide first-line non-invasive respiratory support (NRS) in PICUs; however, high-flow nasal cannula therapy (HFNC), a novel mode of NRS, has recently gained popularity despite the lack of high-quality trial evidence to support its effectiveness. This feasibility study aims to inform the design and conduct of a future definitive randomised clinical trial (RCT) comparing the two modes of respiratory support. METHODS AND ANALYSIS: We will conduct a three-centre randomised feasibility study over 12 months. Patients admitted to participating PICUs who satisfy eligibility criteria will be recruited to either group A (primary respiratory failure) or group B (postextubation). Consent will be obtained from parents/guardians prior to randomisation in 'planned' group B, and deferred in emergency situations (group A and 'rescue' group B). Participants will be randomised (1:1) to either CPAP or HFNC using sealed, opaque envelopes, from a computer-generated randomisation sequence with variable block sizes. The study protocol specifies algorithms for the initiation, maintenance and weaning of HFNC and CPAP. The primary outcomes are related to feasibility, including the number of eligible patients in each group, feasibility of randomising >50% of eligible patients and measures of adherence to the treatment protocols. Data will also be collected on patient outcomes (eg, mortality and length of PICU stay) to inform the selection of an appropriate outcome measure in a future RCT. We aim to recruit 120 patients to the study. ETHICS AND DISSEMINATION: Ethical approval was granted by the National Research Ethics Service Committee North East-Tyne&Wear South (15/NE/0296). Low-energy quenching of positronium by helium
Very low-energy scattering of orthopositronium by helium has been
investigated for simultaneous study of elastic cross section and pick-off
quenching rate using a model exchange potential. The present calculational
scheme, while agrees with the measured cross section of Skalsey et al,
reproduces successfully the parameter ^ 1Z_{\makebox{eff}}, the effective
number of electrons per atom in a singlet state relative to the positron.
Together with the fact that this model potential also leads to an agreement
with measured medium energy cross sections of this system, this study seems to
resolve the long-standing discrepancy at low energies among different
theoretical calculations and experimental measurements.Comment: 4 latex pages, 3 postscript figure
Intravesical rAd-IFNα/Syn3 for Patients With High-Grade, Bacillus Calmette-Guerin-Refractory or Relapsed Non-Muscle-Invasive Bladder Cancer: A Phase II Randomized Study.
Purpose Many patients with high-risk non-muscle-invasive bladder cancer (NMIBC) are either refractory to bacillus Calmette-Guerin (BCG) treatment or may experience disease relapse. We assessed the efficacy and safety of recombinant adenovirus interferon alfa with Syn3 (rAd-IFNα/Syn3), a replication-deficient recombinant adenovirus gene transfer vector, for patients with high-grade (HG) BCG-refractory or relapsed NMIBC. Methods In this open-label, multicenter (n = 13), parallel-arm, phase II study ( ClinicalTrials.gov identifier: NCT01687244), 43 patients with HG BCG-refractory or relapsed NMIBC received intravesical rAd-IFNα/Syn3 (randomly assigned 1:1 to 1 × 10(11) viral particles (vp)/mL or 3 × 10(11) vp/mL). Patients who responded at months 3, 6, and 9 were retreated at months 4, 7, and 10. The primary end point was 12-month HG recurrence-free survival (RFS). All patients who received at least one dose were included in efficacy and safety analyses. Results Forty patients received rAd-IFNα/Syn3 (1 × 10(11) vp/mL, n = 21; 3 × 10(11) vp/mL, n = 19) between November 5, 2012, and April 8, 2015. Fourteen patients (35.0%; 90% CI, 22.6% to 49.2%) remained free of HG recurrence 12 months after initial treatment. Comparable 12-month HG RFS was noted for both doses. Of these 14 patients, two experienced recurrence at 21 and 28 months, respectively, after treatment initiation, and one died as a result of an upper tract tumor at 17 months without a recurrence. rAd-IFNα/Syn3 was well tolerated; no grade four or five adverse events (AEs) occurred, and no patient discontinued treatment because of an adverse event. The most frequently reported drug-related AEs were micturition urgency (n = 16; 40%), dysuria (n = 16; 40%), fatigue (n = 13; 32.5%), pollakiuria (n = 11; 28%), and hematuria and nocturia (n = 10 each; 25%). Conclusion rAd-IFNα/Syn3 was well tolerated. It demonstrated promising efficacy for patients with HG NMIBC after BCG therapy who were unable or unwilling to undergo radical cystectomy
Resonance Production
Recent results on rho(770)^0, K(892)^*0, f_0(980), phi(1020), Delta(1232)^++,
and Lambda(1520) production in A+A and p+p collisions at SPS and RHIC energies
are presented. These resonances are measured via their hadronic decay channels
and used as a sensitive tool to examine the collision dynamics in the hadronic
medium through their decay and regeneration. The modification of resonance

Forgiveness-Reconciliation and Communication-Conflict-Resolution Interventions Versus Retested Controls in Early Married Couples
The first 6 months of marriage are optimal for marriage enrichment interventions. The Hope-Focused Approach to couple enrichment was presented as two 9-hr interventions--(a) Handling Our Problems Effectively (HOPE), which emphasized communication and conflict resolution, and (b) Forgiveness and Reconciliation through Experiencing Empathy (FREE). HOPE and FREE were compared with repeated assessment controls. Couples were randomly assigned and were assessed at pretreatment (t1); 1 month posttreatment (t2) and at 3- (t3), 6- (t4), and 12-month (t5) follow-ups using self-reports. In addition to self-report measures, couples were assessed at t1, t2, and t5 using salivary cortisol, and behavioral coding of decision making. Of 179 couples who began the study, 145 cases were analyzed. Both FREE and HOPE produced lasting positive changes on self-reports. For cortisol reactivity, HOPE and FREE reduced reactivity at t2, but only HOPE at t5. For coded behaviors, control couples deteriorated; FREE and HOPE did not change.
