    Development of microelectrode arrays modified with inorganic–organic composite materials for dopamine electroanalysis

    New electrode materials based on composite inorganic redox material–organic conducting polymer for electroanalysis of dopamine were developed. The composite inorganic–organic coatings based on Prussian blue (PB) and poly(3,4-ethylenedioxythiophene) (PEDOT) have been prepared by electrochemical methods onto gold disk microelectrode arrays. Both insoluble and soluble forms of PB were prepared in situ inside the organic matrix and their electrochemical behaviour was investigated in electrolyte solution. The composite inorganic–organic coating containing the soluble PB form exhibited good electrocatalytic activity towards dopamine (DA) oxidation in aqueous buffered solutions. The DA concentration was determined by cyclic voltammetry in the presence of a high excess of ascorbic acid (AA). A linear response for DA concentrations ranging from 10 uM to 50 uM, in the presence of 1 mM AA, with a sensitivity of 460 uA/uM and a detection limit of 4.3 uM, have been obtained.Peer reviewe

    Rapid prototyping of electrochemical lateral flow devices: stencilled electrodes

    A straightforward and very cost effective method is proposed to prototype electrodes using pressure sensitive adhesives (PSA) and a simple cutting technique. Two cutting methods, namely blade cutting and CO2 laser ablation, are compared and their respective merits are discussed. The proposed method consists of turning the protective liner on the adhesive into a stencil to apply screen-printing pastes. After the electrodes have been printed, the liner is removed and the PSA can be used as a backing material for standard lateral flow membranes. We present the fabrication of band electrodes down to 250 μm wide, and their characterization using microscopy techniques and cyclic voltammetry. The prototyping approach presented here facilitates the development of new electrochemical devices even if very limited fabrication resources are available. Here we demonstrate the fabrication of a simple lateral-flow device capable of determining glucose in blood.     Integration of a zero dead-volume PDMS rotary switch valve in a miniaturised (bio)electroanalytical system

    This work features the design, fabrication and characterisation of a miniaturised electroanalytical lab on a chip that allows the performance of a complete bioassay, from the capture of magnetic particles through their functionalisation and sample incubation to the detection of electroactive reaction products. The system is built using mainly polymeric materials such as PMMA and PDMS and fast prototyping techniques such as milling and moulding. The system also includes a set of microelectrodes, photo-lithographed on a silicon chip. The novelty lies in the design of the rotary microvalve, which contains a microreactor so that various reaction and incubation steps can be carried out in isolation from the detection event with zero dead volume. This avoids contamination and fouling of the electrodes by proteins or other organic matter, and extends the useful lifetime of the detector. The system operation is demonstrated by a model example, consisting in the functionalisation of streptavidin-coated magnetic particles with biotinylated β-galactosidase over periods ranging from 5 to 15 minutes, at which point the particles saturate. Although the system is intended for the development of enzyme-based electrochemical bioassays, the concept of its rotary microreactor can be applied more broadly.Peer reviewe

    Microfluidic fuel cells on paper: Meeting the power needs of next generation lateral flow devices

    Lateral flow test strips have dominated the rapid diagnostics landscape for decades. Recently, the emergence of paper microfluidics has brought new functionalities to these porous materials, and the search for instrument-free point-of-care devices has driven the development of different types of energy sources to fulfill their power needs. This work presents the development of microfluidic fuel cells as paper-based power sources in a standard lateral flow test format. These fuel cells benefit from the laminar flow occurring in a porous material by capillarity to separately react with two parallel streams, anolyte and catholyte, without an ionic exchange membrane or external pumps. It has been shown that the devices are capable of delivering power densities in the range of 1-5 mW cm-2 using solutions of methanol and KOH. The incorporation of a conjugate pad to store the KOH electrolyte in a solid form and a methanol-rich agar gel on top of the reaction membrane allows the fuel cell to function soaking a single sample pad with just water. The presented microfluidic fuel cell approach would enable a more straightforward integration with typical lateral flow test strips and a cost-effective manufacturing. This work represents the starting point in the development of a power source for capillary-based autonomous sensing systems capable of harvesting the energy needed for the measurement from the biological sample to be analyzed.     The CARBA-MAP study: national mapping of carbapenemases in Spain (2014–2018)

    Introduction:Infections caused by carbapenem-resistant Enterobacterales (CRE) and carbapenem-resistant Pseudomonas aeruginosa, including isolates producing acquired carbapenemases, constitute a prevalent health problem worldwide. The primary objective of this study was to determine the distribution of the different carbapenemases among carbapenemase-producing Enterobacterales (CPE, specifically Escherichia coli, Klebsiella pneumoniae, Enterobacter cloacae complex, and Klebsiella aerogenes) and carbapenemase-producing P. aeruginosa (CPPA) in Spain from January 2014 to December 2018.Methods: A national, retrospective, cross-sectional multicenter study was performed. The study included the first isolate per patient and year obtained from clinical samples and obtained for diagnosis of infection in hospitalized patients. A structured questionnaire was completed by the participating centers using the REDCap platform, and results were analyzed using IBM SPSS Statistics 29.0.0.Results: A total of 2,704 carbapenemase-producing microorganisms were included, for which the type of carbapenemase was determined in 2692 cases: 2280 CPE (84.7%) and 412 CPPA (15.3%), most often using molecular methods and immunochromatographic assays. Globally, the most frequent types of carbapenemase in Enterobacterales and P. aeruginosa were OXA-48-like, alone or in combination with other enzymes (1,523 cases, 66.8%) and VIM (365 cases, 88.6%), respectively. Among Enterobacterales, carbapenemase-producing K. pneumoniae was reported in 1821 cases (79.9%), followed by E. cloacae complex in 334 cases (14.6%). In Enterobacterales, KPC is mainly present in the South and South-East regions of Spain and OXA-48-like in the rest of the country. Regarding P. aeruginosa, VIM is widely distributed all over the country. Globally, an increasing percentage of OXA-48-like enzymes was observed from 2014 to 2017. KPC enzymes were more frequent in 2017–2018 compared to 2014–2016.Discussion: Data from this study help to understand the situation and evolution of the main species of CPE and CPPA in Spain, with practical implications for control and optimal treatment of infections caused by these multi-drug resistant organisms

    IL-6 serum levels predict severity and response to tocilizumab in COVID-19: An observational study

    Background: Patients with coronavirus disaese 2019 (COVID-19) can develop a cytokine release syndrome that eventually leads to acute respiratory distress syndrome requiring invasive mechanical ventilation (IMV). Because IL-6 is a relevant cytokine in acute respiratory distress syndrome, the blockade of its receptor with tocilizumab (TCZ) could reduce mortality and/or morbidity in severe COVID-19. Objective: We sought to determine whether baseline IL-6 serum levels can predict the need for IMV and the response to TCZ. Methods: A retrospective observational study was performed in hospitalized patients diagnosed with COVID-19. Clinical information and laboratory findings, including IL-6 levels, were collected approximately 3 and 9 days after admission to be matched with preadministration and postadministration of TCZ. Multivariable logistic and linear regressions and survival analysis were performed depending on outcomes: need for IMV, evolution of arterial oxygen tension/fraction of inspired oxygen ratio, or mortality. Results: One hundred forty-six patients were studied, predominantly males (66%); median age was 63 years. Forty-four patients (30%) required IMV, and 58 patients (40%) received treatment with TCZ. IL-6 levels greater than 30 pg/mL was the best predictor for IMV (odds ratio, 7.1; P < .001). Early administration of TCZ was associated with improvement in oxygenation (arterial oxygen tension/fraction of inspired oxygen ratio) in patients with high IL-6 (P = .048). Patients with high IL-6 not treated with TCZ showed high mortality (hazard ratio, 4.6; P = .003), as well as those with low IL-6 treated with TCZ (hazard ratio, 3.6; P = .016). No relevant serious adverse events were observed in TCZ-treated patients.     Relevance of gastrointestinal manifestations in a large Spanish cohort of patients with systemic lupus erythematosus: what do we know?

    SLE can affect any part of the gastrointestinal (GI) tract. GI symptoms are reported to occur in >50% of SLE patients. To describe the GI manifestations of SLE in the RELESSER (Registry of SLE Patients of the Spanish Society of Rheumatology) cohort and to determine whether these are associated with a more severe disease, damage accrual and a worse prognosis. METHODS: We conducted a nationwide, retrospective, multicentre, cross-sectional cohort study of 3658 SLE patients who fulfil =4 ACR-97 criteria. Data on demographics, disease characteristics, activity (SLEDAI-2K or BILAG), damage (SLICC/ACR/DI) and therapies were collected. Demographic and clinical characteristics were compared between lupus patients with and without GI damage to establish whether GI damage is associated with a more severe disease. RESULTS: From 3654 lupus patients, 3.7% developed GI damage. Patients in this group (group 1) were older, they had longer disease duration, and were more likely to have vasculitis, renal disease and serositis than patients without GI damage (group 2). Hospitalizations and mortality were significantly higher in group 1. Patients in group 1 had higher modified SDI (SLICC Damage Index). The presence of oral ulcers reduced the risk of developing damage in 33% of patients. CONCLUSION: Having GI damage is associated with a worse prognosis. Patients on a high dose of glucocorticoids are at higher risk of developing GI damage which reinforces the strategy of minimizing glucocorticoids.
