    The Effect of a Community-Based, Primary Health Care Exercise Program on Inflammatory Biomarkers and Hormone Levels

    The aim of this study was to analyze the impact of a community-based exercise program in primary care on inflammatory biomarkers and hormone levels. The 1-year quasiexperimental study involved 13 women (mean age = 56.8 ± 11.4 years) and it was developed in two basic health care units in Rio Claro City, Brazil. The physical exercise intervention was comprised of two, 60-minute sessions/week. The inflammatory biomarkers were measured at baseline, 6 months, and 1 year. Repeated measures ANOVA analyses indicated that the intervention was effective in reducing CRP and TNFα after 1 year compared to baseline and 6 months . There were no changes in IL10, IL6, and insulin after 1 year. However, leptin significantly increased at 1 year . The major finding of this study is that a community-based exercise program can result in a decrease or maintenance of inflammatory biomarkers after 1 year, and thus has the potential to be a viable public health approach for chronic disease prevention

    O departamento de Educação Física da Universidade Estadual Paulista (UNESP)- Campus Rio Claro desenvolve desde 1999 o Programa de Ginástica Laboral (GL). O programa tem como objetivos prevenir os Distúrbios Osteomusculares Relacionados ao Trabalho, melhorar a qualidade de vida, reduzir a inatividade física e promover um estilo de vida saudável. Atualmente, a equipe é formada por três professores, um pósgraduando e oito estagiários do curso de educação física, além da participação de 40 funcionários da própria universidade. As aulas são realizadas duas vezes na semana e são constituídas de exercícios de flexibilidade, coordenação, força, atividades lúdicas e massagem. No decorrer do ano, acontecem avaliações da capacidade funcional, nível de atividade física, qualidade de vida, estágio de mudança de comportamento para atividade física e estado de ânimo de todos os praticantes da GL. Os resultados indicaram que o programa de GL é capaz de melhorar a qualidade de vida, estado de ânimo e manter as capacidades funcionais dos seus praticantes.The Physical Activity department of São Paulo State University (SPSU) developed since 1999 the Active Breaks Program.     Health related quality of life is differently associated with leisure-time physical activity intensities according to gender: a cross-sectional approach

    Background: Several studies have demonstrated a positive association between physical activity (PA) and health-related quality of life (HRQL). However, studies have suggested that this association depends both on the PA intensity and the domain of HRQL evaluated. This study aimed to explore the association between physical, mental and overall HRQL with recommended levels of PA. PA levels were divided into moderate and vigorous intensity leisure-time PA and total leisure-time PA.Methods: The study included 1001 adults, 582 women (46 +/- 17 years) and 419 men (43 +/- 16 years), residents in Rio Claro-SP, Brazil. All participants completed the SF-36 questionnaire to assess HRQL and the long version of the International Physical Activity Questionnaire (IPAQ) to assess level and intensities of leisure-time PA. Total leisure-time PA at moderate intensity was classified as: less than 9 min/week, 10-149 min/week, 150-299 min/week and 300 min/ week or more. Total leisure-time PA at vigorous intensity was classified as: less than 9 min/week, 10 to 74.9 min/week, 75-149 min/week and 150 min/week or more. Multiple linear regression was performed in STATA version 12.0.Results: Among women, moderate intensity and total leisure-time PA were associated with physical health. Among men, moderate and vigorous intensity and total leisure-time PA were associated with physical health and overall HRQL. Furthermore, moderate intensity and total leisure-time PA were associated with mental health in men. However, vigorous intensity PA was not associated with mental health for this group.Conclusion: The different domains of HRQL were associated with different levels and intensities of PA in leisure-time according to gender of adults.     The study aims to verify the effect of community-based physical exercise interventions carried out in primary health care on plasma inflammatory biomarkers and cardiorespiratory fitness. It was performed a 1year quasi-experimental study with female participants of Physical Exercise Interventions carried out in Health Units in Rio Claro City, Sao Paulo State. Two different protocols were implemented. The Intervention 1 (two, 60 minute sessions/week) was based in the Saude Ativa Rio Claro Program (existing since 2001). The intervention 2 (three, 90 minute sessions/week) was based in the physical activity recommendation. In both interventions, aerobic, resistance muscle and stretching exercises were performed. The variables evaluated were: a) Anthropometric variables (body mass, stature, body mass index, waist and hip circumferences); b) Physical Activity level through pedometer; c) Cardiorespiratory fitness test (incremental ergoespirometric test); d) Inflammatory biomarkers (blood plasmatic dosage). The results are present in 3 articles. In the chapter 1, the results showed that the Intervention 1 was effective in reducing CRP and TNFa; and in maintaining the IL10, IL6 and insulin after 1 year of intervention. In the chapter 4, it was demonstrated that Intervention 2 decreased the LDL and glucose levels and improved the resistance insulin of participants. In chapter 5, the results indicated that participants of Intervention 2 developed a greater exercise tolerance and improved the energy cost during walking. These data suggest that cardiorespiratory fitness was enhanced. Some additional analyses indicated that participants of intervention 1 developed a greater exercise tolerance but they did not improve the energy cost during walking. The development of ...O presente projeto tem como objetivo verificar o efeito de programas de exercício físico sobre biomarcadores de inflamação e a capacidade cardiorrespiratória em participantes de programas de exercícios físicos oferecidos em Unidades de Saúde do município de Rio Claro-SP. Para atingir os objetivos propostos foi realizado um estudo quase-experimental com duração de 12 meses, onde a amostra foi composta por mulheres participantes de programas de Intervenção com Exercício Físico em Unidades de Saúde. Foram implementados 2 Programas distintos. A Intervenção 1 foi baseada no Programa Saúde Ativa Rio Claro (já existente no Município desde 2001), composta de 2 sessões semanais de exercício físico com duração de 60 minutos. A intervenção 2 foi fundamentada nas recomendações preconizadas para a prática de atividade física, sendo oferecida 3 sessões semanais com duração de 90 minutos. Em ambas intervenções foram desenvolvidas atividades aeróbias, exercícios de força e flexibilidade. As variáveis avaliadas foram: a) Antropometria (Massa Corporal, Estatura, Cálculo do Índice de Massa Corporal, Circunferência da Cintura e Quadril); b) Pedometria; c) Aptidão Cardiorrespiratória (Teste Ergoespirométrico Incremental na esteira); d) Biomarcadores de Inflamação (Dosagem plasmática do sangue). Os resultados estão apresentados em forma de artigo. No capítulo 3, os resultados demonstram que a Intervenção 1 foi efetiva em reduzir concentrações de PCR e TNFα e manter concentrações de IL10, IL6 e insulina após 12 meses de intervenção. No capítulo 4, foi demonstrado que a Intervenção 2 reduziu concentrações de LDL e glicose e ainda melhorou o quadro de resistência à insulina das participantes. No capítulo 5 os dados indicaram que as participantes desenvolveram uma maior tolerância ao exercício e melhoraram o custo energético dos estágios do teste.     Associações entre nível, oferta de atividade física no trabalho e atividade física de lazer

    O objetivo desse estudo foi investigar associações entre nível, oferta de Atividade Física (AF) no trabalho e AF de lazer. Foi realizado um estudo transversal de base populacional. Participaram do estudo 864 trabalhadores com idade média e desviopadrão de 41 (± 13,4) anos, sendo 451 homens e 413 mulheres e eles responderam o Questionário Internacional de AF (versão longa– classificados em ativos e sedentários de acordo com o nível de AF de lazer), o Questionário baseado no modelo Transteorético (Estágios de Prontidão – classificados em fase de adoção e fase antecedentes), o Questionário sobre convênios e instalações para a prática de AF pela empresa e o Questionário sócio-demográfico. Para verificar as associações foi realizada a Regressão de Poisson no programa STATASE.10, obtendo a Razão de Prevalência (RP) com Intervalo de Confiança (IC - 95%). Como variáveis dependentes utilizamos o sedentarismo (nível de AF de lazer) e fase antecedentes (participação em AF de lazer). Para as variáveis independentes utilizamos: sexo, idade, índice de massa corporal (IMC), escolaridade, nível sócio-econômico, nível de AF no trabalho, oferta de ginástica laboral, participação na ginástica laboral e oferta de programas de condicionamento físico (PCF) através de convênios e instalações no local de trabalho. Os resultados indicam que os trabalhadores do sexo masculino são menos sedentários quando comparados com as trabalhadoras do sexo feminino. Para o sexo masculino, menor nível sócio-econômico está associado ao sedentarismo e a oferta de PCF oferecidos através de convênios está associada a níveis mais elevadosl de AF de lazer. Para o sexo feminino, maior IMC e menor nível sócio-econômico foram associados ao sedentarismo. Com relação a participação em AF de lazer o sexo feminino tem menos chances de estarem na fase de adoção...The aim of this study was to investigate the associations between Physical Activity’s (PA) level and PA’s offer at worksite and Leisure time PA. The Population-based cross-sectional study was realized with 864 workers with average age and standard deviation 41 (± 13,4) years old, being 451 men and 413 women. They answered the International PA Questionnaire (long version – classified in activity and sedentary according to leisure time PA’s level), the Questionnaire based on Transteorethic Model (Stages of change – classified in adoption and antecedents phases), the Questionnaire about facilities and installations PA’s practice in the worksite and the demographic-status Questionnaire. The Poisson Regression in the STATASE.10 program, getting the Prevalence Ratio (PR) with Confident Interval (CI - 95%) was conducted to verify the associations between the variables. The dependents variables were the sedentarism (leisure time PA’s level) and antecedents phase (leisure time PA’s participation). For the independents variables were used: sex, age, body mass index (BMI), schooling level, social status, PA’s level in work, stretchbreak’s offer, stretch-break’s participation and fitness program’s (FP) through facilities and installations PA’s practice in the worksite. The results indicated that the male workers are less sedentary than female. For men the lower social status was associated with sedentarism and the FP facilities was associated with bigger leisure time PA’s level. For female, bigger IMC and lower social status were associates with sedentarism. About leisure time PA’s participation, the female has less chance to be in the adoption phase than male.     Severidades Ocupacionais associadas à inatividade física no lazer em trabalhadores

    The aim of this study was to verify associations between occupational severities and leisure time physical inactivity level in workers. Two-hundred workers of Rio Claro County participated in this study. The leisure time physical activity (active or inactive) and some occupational severities were evaluated. The statistic analyses showed that working day, overtime work and occupational intensity had not been associated with leisure time physical inactivity in workers. Only the willingness at the end of the working day had been associated to the leisure time physical inactivity; showing that workers who leave their work with some willingness have more possibilities to be active in leisure time.O objetivo do estudo foi verificar associações entre severidades ocupacionais e nível de atividade física no lazer em trabalhadores. Para isso participaram do estudo 200 trabalhadores (71 homens e 129 mulheres) do Município de Rio Claro e região. Foram avaliados o nível de atividade física no lazer (ativo ou inativo) e algumas severidades ocupacionais. A análise estatística revelou que o tempo da jornada diária de trabalho, realização de horas extra e intensidade do trabalho não foram associados à inatividade física no lazer em trabalhadores. Somente a disposição no fim da jornada de trabalho foi associada à inatividade física no lazer, mostrando que trabalhadores que tem alguma disposição depois do trabalho são mais ativos no tempo de lazer
