    The Impact of Chemotherapy on Hepatitis B Antibody Titer in Patients with Hematological Malignancies

    Objective: To investigate the influence of chemotherapy (CT) on HBsAb titer in patients receiving CT due to hematological malignancy. Materials and Methods: The data of 75 patients who received CT with the diagnosis of various hematological malignancies and who had serum HBsAb levels measured prior to and after the cessation of CT were evaluated retrospectively. Results: The median age of the patients was 52 years (range: 16-78) with 49 (65\%) males and 26 (35\%) females. Median HBsAb titer decreased significantly after CT compared to the pre-CT median HBsAb titer {[}68 (range: 0-1000) vs. 100 (range: 6.2-1000)] (p=0.001). In subgroup analysis, median HBsAb titer decreased significantly after CT in acute leukemia patients {[}110 (range: 6.2-1000) vs. 67.8 (range: 0-1000)] (p=0.003) and in patients receiving intensive CT {[}97.2 (range: 6.2-1000) vs. 71 (range: 0-1000)] (p=0.036). The decrease in median HBsAb titer was significant in male patients (p<0.001). HBsAb became negative after CT in 9 patients who were HBcAb-negative and had lower pre-CT HBsAb levels. Conclusion: HBsAb decreased after CT, especially in acute leukemia and male patients, and in patients receiving intensive CT

    Prognostic role of pre-transplantation serum C-reactive protein levels in patients with acute leukemia undergoing myeloablative allogeneic stem cell transplantation

    The aim of this study was to identify indicators of outcome prior to transplantation in allogeneic hematopoietic stem cell transplantation (HCT). Clinical data of 106 patients with acute leukemia were retrospectively analyzed. We examined the role of pre-conditioning serum C-reactive protein (CRP) and ferritin levels, HCT-CI and European Group for Blood and Marrow Transplantation (EBMT) scores on transplant toxicities, transplant-related mortality (TRM), progression-free survival (PFS), and overall survival (OS). High pre-conditioning serum CRP levels showed a positive correlation with higher EBMT scores (p < 0.001), HCT-CI (p = 0.004), and ferritin levels (p < 0.001). In univariate Cox regression analysis, serum CRP =10 mg/L, serum ferritin =1500 ng/mL, and HCT-CI =3 had a significant adverse effect on OS. Serum CRP =10 mg/L and HCT-CI =3 predicted increased risk of TRM in univariate analysis. Multivariate Cox regression analysis showed that HCT-CI score =3 independently predicted increased risk of TRM and CRP =10 mg/L predicted increased risk of disease progression. Although CRP lost its significance on TRM in multivariate analysis, as an inexpensive and readily available serum biomarker of inflammation, the prognostic role of pre-transplant CRP levels should be analyzed in selected diseases and increased number of patient groups

    Impacts of post-transplantation cyclophosphamide treatment after allogeneic hematopoietic stem cell transplantation in acute myeloid leukemia

    Abstract Post-transplant cyclophosphamide has become a promising medical option after allogeneic HSCT. In this study we aimed to evaluate the efficacy of cyclophosphamide and cyclosporine combination in acute and chronic graft-versus-host disease (GvHD) prophylaxis in acute myeloid leukemia (AML) cases scheduled for allogeneic hematopoietic stem cell transplantation (allo-HSCT). Retrospective analysis of data from 40 cases who underwent allogeneic HSCT under GvHD prophylaxis with cyclophosphamide and cyclosporine combination between April 2016 and August 2017 was made. Cyclophosphamide was given at daily doses of 50 mg/kg on post-transplant 3rd and 4th days, and cyclosporine was applied at daily doses of 3 mg/kg/day starting from the 5th post-transplant day. Cyclosporine dose was tapered beginning from the 45th postoperative day and completely discontinued on the 90th post-transplant day. Mean age was 38.25 ± 15.25 years. Posttransplant median follow-up was six months (6–17 months). Post-transplant, the number of deaths and mortality rates related and unrelated to transplantation were 5 (12.5%), and 2 (5%), respectively. Acute GvHD was diagnosed in 7 cases (17.5%), and relapse was noted in 9 cases (22.5%). Myeloablative or reduced intensity conditioning was performed in 22 (55%) and 18 (45%) patients, respectively. The distribution of the donors was as follows: match-related (n = 26; 65%), match-unrelated (n = 9, 22.5%) and haploidentical donors (n = 5; 12.5%). There was no statistically significant correlation between the transplant-related and unrelated mortality and parameters under investigation.Cyclophosphamide use appears to be a highly effective and promising strategy for acute GvHD prophylaxis in non-haploidentical allogeneic HSCT cases. Identification of the impact of cyclophosphamide use on the development of chronic GvHD needs further investigation

    High-dose thiotepa, etoposide and carboplatin as conditioning regimen for autologous stem cell transplantation in patients with relapsed or refractory germ cell tumors

    Purpose: As well as standard chemotherapy, autologous stem cell transplantation (ASCT) is also seen as a good therapeutic alternative in the relapsed/refractory germ cell tumors (GCT). The combination of thiotepa, carboplatin and etoposide (TECA) is also one of the high-dose chemotherapy options that can be used before ASCT. Except a phase-II study there are no large studies conducted with the TECA regimen in GCT. In this study, we aimed to evaluate the efficacy and toxicity of the TECA regimen in patients who underwent ASCT. Methods: Patients who underwent ASCT with TECA for relapsed/refractory GCT in our center between 2013-2020 were included in the study. Results: The median age of 15 patients included in the study was 31 years (19-46). The majority of patients (n=12; 80.0%) had a diagnosis of non-seminoma GCT. All of the patients had previously received bleomycin, etoposide, cisplatin (BEP) combination chemotherapy. They were relapsed/refractory to platinums and had at least one distant metastasis. ASCT was administered as a second-line therapy in 12 (80.0%) patients. In all patients etoposide, thiotepa and carboplatin were administered before ASCT as myeloablative therapy. Complete response was obtained in 6 (40.0%) patients and partial response in 5 (33.3%). The objective response rate was 73.3%. Three-year progression-free survival (PFS) was 43.1% and the estimated median PFS was 12.6 months (2.7- 41.7). The estimated median overall survival (OS) was 37.3 months and 3-year OS was 54.5%. None of the patients had ASCT-related death. Conclusions: High-dose TECA is an effective and safe myeloablative regimen for ASCT in relapsed/refractory GCT

    Brentuximab vedotin and bendamustine: an effective salvage therapy for relapsed or refractory Hodgkin lymphoma patients

    The prognosis is poor for relapsed or refractory (R/R) classical Hodgkin Lymphoma (cHL) patients. The brentuximab vedotin (Bv) and bendamustine (B) combination has been used as a preferable salvage regimen in R/R cHL patient trials. We retrospectively evaluated response rates, toxicities, and the survival in R/R cHL patients treated with the BvB combination. In a multi-centre real-life study, 61 R/R HL patients received intravenous doses of 1.8 mg/kg Bv on the first day plus 90 mg/m(2) B on the first and second days of a 21-day cycle as a second-line or beyond-salvage regimen. Patients' median age at BvB initiation was 33 (range: 18-76 years). BvB was given as median third-line treatment for a median of four cycles (range: 2-11). The overall and complete response rates were 82% and 68.9%, respectively. After BvB initiation, the median follow-up was 14 months, and one- and two-year overall survival rates were 85% and 72%, respectively. Grade 3/4 toxicities included neutropenia (24.6%), lymphopenia (40%), thrombocytopenia (13%), anaemia (13%), infusion reactions (8.2%), neuropathy (6.5%), and others. The BvB combination could be given as salvage regimen aiming a bridge to autologous stem cell transplant (ASCT), in patients relapse after ASCT or to transplant-ineligible patients with manageable toxicity profiles
