    Los conocimientos tecnológicos, pedagógicos y de contenidos del profesorado universitario andaluz sobre las TIC. Análisis desde el modelo TPACK.

    The TPACK model is one of the most popular diagnose and teacher training models in recent times due to its interest in disciplinary, pedagogical and technological content. Scientific literature research that implements this model in teaching, not only helps to improve its quality but also allows us to determine what the teachers' allowance is and to design teacher training strategies. Based on these considerations, the objectives set out in the study were three: 1) to know if the reliability level of the instrument on the TPACK was similar to other investigations; 2) to analyze the amount of knowledge of the participants; 3) to know if there were significant differences between professors according to their University. For this reason, we have opted for a quantitative methodology with a descriptive and cross-sectional design. The sample is made up of 396 professors from four public Faculties of Education Sciences of western Andalusia. The results reflect high disciplinary knowledge in comparison with the low scores achieved in the domains with technological presence. It is also evident that Cronbach’s alpha is similar to other investigations and that there are no significant differences in teaching knowledge according to the University. As relevant conclusions, we highlight the homogeneity in the results on the knowledge of the participants and the usefulness of the model for teacher training.El modelo TPACK  es uno de los modelos de diagnóstico y formación del profesorado que mayor auge ha alcanzado en los últimos tiempos por su interés en los contenidos disciplinares, pedagógicos y tecnológicos. La literatura científica resalta que implementar este modelo en la enseñanza no solo ayuda a mejorar su calidad, sino que también permite determinar cuáles son los conocimientos del profesorado y diseñar estrategias de capacitación docente. Tomando como base estas consideraciones, los objetivos planteados en el estudio han sido tres: 1) conocer si el índice de fiabilidad del instrumento sobre el TPACK era similar al de otras investigaciones; 2) analizar los niveles de conocimientos de los participantes; 3) conocer si existían diferencias significativas entre los profesores según su Universidad. Por esta razón, hemos optado por una metodología cuantitativa con un diseño descriptivo y transversal. La muestra está formada por 396 profesores de cuatro Facultades públicas de Ciencias de la Educación de Andalucía occidental. Los resultados reflejan altos conocimientos disciplinares en comparación con las bajas puntuaciones alcanzadas en los dominios con presencia tecnológica. También se evidencia que el alfa de Cronbach es similar al de otras investigaciones y que no existen diferencias significativas en los conocimientos docentes según la Universidad. Como conclusiones relevantes, destacamos la homogeneidad en los resultados sobre los conocimientos de los participantes y la utilidad del modelo para la formación del profesorado

    Patrones de alimentación y trayectorias de crecimiento en niños con lactancia materna y con leche de fórmula durante la introducción de la alimentación complementaria

    Introduction: several investigations have identified breastfeeding as a protective factor for rapid infant weight gain and childhood obesity while other studies have found that this protective effect could be the result of confounding factors. Objectives: to assess the associations between lactation practices (breast-fed vs formula-fed infants) during the introduction of complementary food period, as well as the following: a) patterns of food intake; and b) trajectories of growth at six, nine and 12 months (z-score of weight, height and body mass index [BMI] and changes in these z-scores from six to 12 months). Methods: two hundred and three infants randomly selected from Spanish Primary Health Centres were measured. Parents recorded all infant's food consumption for three days (g/day). Linear regression models were applied. Results: breast-fed infants had a lower intake of cereals, fruit baby food, vegetables with meat/fish and a total intake of food compared to formula-fed infants at nine months of age. After adjusting for sex, parental education and total food intake, breastfed children continued to have lower intake of cereals (-5.82, 95% CI: -9.22, -2.43), and lower total food intake (-301.23, 95% CI: -348.50, -253.96). Breast-fed infants had a lower change in z-score of weight, height and BMI from six to 12 months of age and these differences remained when adjusting for all confounders. Conclusions: formula-fed infants during the complementary feeding period have a higher food intake and show higher rates of rapid infant weight gain compared to breast-fed infants. These differences in growth trajectories depending on breastfeeding maintenance and food intake during early life must be considered in adiposity risk evaluation.Introducción: varios estudios han identificado la lactancia materna como un factor protector frente a la ganancia rápida de peso y la obesidad infantil, mientras que otros estudios han encontrado que este efecto protector podría ser el resultado de la interferencia de factores de confusión. Objetivos: evaluar las asociaciones entre el tipo de lactancia (leche materna versus lactantes alimentados con fórmula) durante la introducción de la alimentación complementaria, así como: a) los patrones de ingesta de alimentos; y b) las trayectorias de crecimiento a los seis, nueve y 12 meses (z-score de peso, estatura e índice de masa corporal (IMC) y variaciones en estos z-score entre los seis y los 12 meses. Métodos: se midieron 203 neonatos seleccionados en centros de salud primaria españoles. Los padres registraron el consumo de alimentos de todos los bebés durante tres días (g/día). Se aplicaron modelos de regresión lineal. Resultados: los bebés alimentados con leche materna tuvieron una menor ingesta de cereales, alimentos para bebés a base de frutas, verduras con carne/pescado, así como una menor ingesta total de alimentos en comparación con los bebés alimentados con fórmula a los nueve meses de edad. Después de realizar el ajuste por sexo, educación de los padres y consumo total de alimentos, los niños alimentados con leche materna continuaron teniendo una menor ingesta de cereales (-5,82, IC 95%: -9,22, -2,43) y una ingesta total de alimentos más baja (-301,23, IC 95%: -348,50, -253,96). Los bebés alimentados al pecho tuvieron un menor incremento en la puntuación z-score de peso, talla e IMC entre los seis y los 12 meses de edad y estas diferencias se mantuvieron cuando se ajustaron para todos los factores de confusión. Conclusiones: los lactantes alimentados con fórmula durante el periodo de alimentación complementaria tienen una mayor ingesta de alimentos y muestran tasas más altas de ganancia de peso en comparación con los lactantes alimentados con leche materna. Estas diferencias en las trayectorias de crecimiento en función del mantenimiento de la lactancia materna y la ingesta de alimentos durante los primeros años de vida deben considerarse en la evaluación del riesgo de adiposidad

    Declared Reasons for Cessation of Breastfeeding during the First Year of Life: An Analysis Based on a Cohort Study in Northern Spain

    Background: Breastfeeding is the gold standard of infant feeding due to the many advantages it offers to both the child and the mother. Objective: To identity the main reasons for cessation of breastfeeding reported by mothers during the first year of life. Design: A prospective cohort study was conducted, recruiting 970 infants from a university hospital in Spain. The main maternal variables studied were maternal age, parity, educational level, work occupation, smoking habit, weeks of gestation at birth, birth weight, feeding type, and duration of breastfeeding. All participants were followed for one year to determinate the duration of breastfeeding and to gather reasons for abandoning breastfeeding. Results: At six months, the percentage of breastfeeding experienced a decline of 50%, and only 24.5% of these mothers maintained breastfeeding. Up to 15.8% of the mothers decided to give up exclusive breastfeeding by their own choice, whereas 15.4% did so because they suspected low milk production. Work-related causes represent the third reason of abandonment. Conclusions: Our results highlight the need to improve the health policies for the promotion, protection, and support for the initiation and maintenance of breastfeeding. In particular, our results highlight the importance of researching women's low milk production and work-related factors, with particular emphasis on improving conciliation measures.This research has been subsidized by the Valdecilla Health Research Institute (IDIVAL).     COVID-19 in a cohort of pregnant women and their descendants, the MOACC-19 study

    Purpose: The Mother and Child COVID-19 study is a cohort recruiting pregnant women and their children in Cantabria, North of Spain, during COVID-19 pandemic in order to ascertain consequences of SARS-CoV-2 infection on pregnant women and their descendants. This article reports the cohort profile and preliminary results as recruitment is still open. Participants: Three subcohorts can be identified at recruitment. Subcohort 1 includes women giving birth between 23 March and 25 May 2020; they have been retrospectively recruited and could have been exposed to COVID-19 only in their third trimester of pregnancy. Subcohort 2 includes women giving birth from 26 May 2020 on; they are being prospectively recruited and could have been exposed to COVID-19 in both their second and third trimesters of pregnancy. Subcohort 3 includes women in their 12 week of pregnancy prospectively recruited from 26 May 2020 on; they could have been exposed to COVID-19 anytime in their pregnancy. All women are being tested for SARS-CoV-2 infection using both RT-PCR for RNA detection and ELISA for anti-SARS-CoV-2 antibodies. All neonates are being tested for antibodies using immunochemoluminiscency tests; if the mother is tested positive for SARS-CoV-2 RNA, a nasopharyngeal swab is also obtained from the child for RT-PCR analysis. Findings to date: As of 22 October, 1167 women have been recruited (266, 354 and 547 for subcohorts 1, 2 and 3, respectively). Fourteen women tested positive to SARS-CoV-2 RNA by the day of delivery. All 14 children born from these women tested negative for SARS-CoV-2 RNA. Future plans: Children from women included in subcohort 3 are expected to be recruited by the end of 2020. Children will be followed-up for 1 year in order to ascertain the effect that COVID-19 on their development.This study is funding by a grant from Instituto de Salud Carlos III (ISCIII) (reference: COV20/00923).     Feeding in the first six months of life is associated with the probability of having bronchiolitis: a cohort study in Spain

    Background: Breastfeeding is associated with lower incidence and severity of lower respiratory tract disease. However, little is known about the relationship between feeding type and breastfeeding duration with bronchiolitis in a child's first year. Methods: A prospective cohort study of 969 newborn babies were followed-up for 12 months to determine breastfeeding duration, feeding type, feeding trajectory, and bronchiolitis episodes at Marqués de Valdecilla University Hospital, Spain in 2018. Type of feeding was recorded by interviewing mothers at the time of hospital discharge and at 2, 4, 6, 9 and 12 months of life, in three categories: breastfeeding, mixed feeding and infant formula. Type of feeding at hospital discharge refers to feeding from birth to discharge. In any other times studied, it refers to feeding in the last 24 h. The association between the feeding type and bronchiolitis was analysed using logistic regression. Poisson regression was used to evaluate the association between feeding type and the number of bronchiolitis episodes with Kaplan-Meier estimators presenting the cumulative probability of suffering bronchiolitis. The results were adjusted for mother and child characteristics. Results: Our data shows exclusive breastfeeding and mixed breastfeeding reduce the number of episodes of bronchiolitis. Regarding feeding at 4 months, exclusive breastfeeding reduced by 41% the number of episodes of bronchiolitis (adjusted incidence Ratio (aIR) 0.59, 95% CI 0.46, 0.76) and mixed feeding by 37% (aIR 0.63, 95% CI 0.47, 0.86). Moreover, changing from exclusive breastfeeding to mixed feeding increased the incidence of bronchiolitis compared with continuing exclusive breastfeeding. An early swap to mixed breastfeeding before months 2 or 4, was associated with a reduced the number of episodes of bronchiolitis, (aIR 0.53, 95% CI 0.39, 0.73 if introduction of mixed breastfeeding before month 2, and aIR 0.61, 95% CI 0.45, 0.83 if introduction of mixed breastfeeding before month 4), when compared with infant formula alone. Conclusions: Any breastfeeding was associated with lower incidence of bronchiolitis and number of episodes of bronchiolitis in the first year of life. Consequently, promoting programmes facilitating exclusive or mixed breastfeeding would be a relevant measure in the prevention of bronchiolitis

    Factors associated with duration of breastfeeding in Spain: a cohort study

    Background: Factors associated with duration of breastfeeding have been usually studied at specific times after birth. Little is known about how much time is added to breastfeeding by each associated factor. Methods: A cohort of 969 mother-child dyads was followed-up for twelve months at the Marqués de Valdecilla University Hospital, Spain, in 2018. Data on mother characteristics, pregnancy, delivery and children characteristics were obtained from medical records. Length of breastfeeding was reported by the mothers and recorded in paediatric medical record at hospital discharge and 2, 4, 6, 9 and 12 months of life. Factors associated with duration of breastfeeding were analysed via multivariate Weibull regression parameterized as accelerated time of failure. Results are presented as time ratios. Results: About four out of five children were breastfed at hospital discharge, although this proportion dropped to 65% in children born from smoker women, 70% in preterm children and 68% in neonates weighting less than 2500 g. Mother´s age was associated with longer breastfeeding, adding 2% more breastfeeding time per year (adjusted time ratio 1.02; 95% confidence interval 1.00, 1.04). Children born from mothers with university studies were breastfed 53% more time than those born from mothers with primary studies (adjusted time ratio 1.53; 95% confidence interval 1.21, 1.95); smoking in pregnancy decreased length of breastfeeding by 41% (adjusted time ratio 0.59; 95% confidence interval 0.46, 0.76). Other factors associated with longer breastfeeding were single pregnancy and newborn weight over 2500 g. Conclusions: Analysing factors associated with duration of breastfeeding as time parameters allows us to quantify the amount of time gained or lost by each factor, which could make it easier to evaluate the relevance of programmes directed to promote facilitating breastfeeding factors.This research has been subsidized by the Valdecilla Health Research Institute (IDIVAL).     Health Care Costs Associated to Type of Feeding in the First Year of Life

    Background: Breastfeeding is associated with lower risk of infectious diseases, leading to fewer hospital admissions and pediatrician consultations. It is cost saving for the health care system, however, it is not usually estimated from actual cohorts but via simulation studies. Methods: A cohort of 970 children was followed-up for twelve months. Data on mother characteristics, pregnancy, delivery and neonate characteristics were obtained from medical records. The type of neonate feeding at discharge, 2, 4, 6, 9 and 12 months of life was reported by the mothers. Infectious diseases diagnosed in the first year of life, hospital admissions, primary care and emergency room consultations and drug treatments were obtained from neonate medical records. Health care costs were attributed using public prices and All Patients Refined?Diagnosis Related Groups (APR?DRG) classification. Results: Health care costs in the first year of life were higher in children artificially fed than in those breastfed (1339.5€, 95% confidence interval (CI): 903.0?1775.0 for artificially fed vs. 443.5€, 95% CI: 193.7?694.0 for breastfed). The breakdown of costs also shows differences in primary care consultations (295.7€ for formula fed children vs. 197.9€ for breastfed children), emergency room consultations (260.1€ for artificially fed children vs. 196.2€ for breastfed children) and hospital admissions (791.6€ for artificially fed children vs. 86.9€ for breastfed children). Conclusions: Children artificially fed brought about more health care costs related to infectious diseases than those exclusively breastfed or mixed breastfed. Concretamente, tratamos de mostrar cómo el profesorado universitario puede evaluar competencias transversales y específicas mediante el uso de foros, blogs y wikis, cumpliéndose así con lo establecido en el Real Decreto 1393/2007.
