    Manifest Dream/Association Comparison: A Criterion to Monitor the Psychotherapeutic Field (2nd part) Field Transformations: A Clinical Case

    Summary The present work focuses on the transformations of the psychotherapeutic field through the relationship dynamics that occur within it. The first part of this article starts with a brief outline of the Gestalt psychological understanding of the field concept, also in its application to the psychotherapeutic situation, followed by a brief review of the introduction of the field concept into the psychoanalytic theory formation. After this, the first author first presents the theoretical concept underlying a new approach he has developed for observing the relationship dynamics in psychotherapy. Mirroring a formation both psychoanalytic and gestaltic of the main author, this new approach is based on the combination of psychoanalytic and Gestalt psychological concepts. According to the clinical experience and insights of the author, the phenomenological and relational approach of Gestalt theory fits well with the psychoanalytic approach; on this basis, a criterion for recording the progress of therapy can be developed. This criterion is the phenomenology of the development of the qualities of the relationships of the client, as they become visible in his dream narrations and the subsequent associations in the analysis room, and continue to develop during the session and the further course of therapy. The relationship dynamics in the dream narration is thus compared with those that develop in the course of the subsequent associations. This is demonstrated and further elaborated in the second part of this paper on the basis of a clinical case. The clinical example shows how the relationship dynamics develop in this sense in the individual therapy sessions and over a longer course of therapy. The associated transformations of the therapeutic field give a good indication of the progress of therapy. The main author gained such insights into the transformations of the therapeutic field and the progression of therapy, which are visible in the course of therapy, from the careful application of the criterion "manifest dream/associations comparison of relational dynamics". In the specific case, there was also a high degree of correspondence between the results of the application of this phenomenological criterion and the empirical evidence of the symptom questionnaire, a self-report measure requested by the patient himself during the course of the therapy

    Integration of Context Information through Probabilistic Ontological Knowledge into Image Classification

    The use of ontological knowledge to improve classification results is a promising line of research. The availability of a probabilistic ontology raises the possibility of combining the probabilities coming from the ontology with the ones produced by a multi-class classifier that detects particular objects in an image. This combination not only provides the relations existing between the different segments, but can also improve the classification accuracy. In fact, it is known that the contextual information can often give information that suggests the correct class. This paper proposes a possible model that implements this integration, and the experimental assessment shows the effectiveness of the integration, especially when the classifier’s accuracy is relatively low. Such phenomenon is often supported by captivating marketing strategies and social media advertising providing unscientifically founded claims, thus raising safety concerns. The aim of our study is to provide a comprehensive analysis of the online market, patterns of use, perceived risks and other safety issues on supplement use as reported in online fitness communities. Methods: A mixed method approach was employed. An automatized web-based monitoring tool (Brand24®) was used to track the most popular supplements and related discussions according to the number of interactions between users and shares; the number and category of websites; the social media reach; and the most popular hashtags. Results were assessed through a netnographic qualitative analysis of online fitness fora, to identify motivations of intake, self-reported side effects andthe overall safety perception reliability of supplements information online. Results: A social media reach of over four million individuals, inclusive of 18595 posts, emerged from our search. The most cited supplements were “Whey Protein”, “Branched Chain Amino-Acid”, “Creatine”, “Multivitamin supplements” and “Nitric Oxide boosters”. Supplements were mainly taken for muscle gain (23%), increase energy (17%), and weight loss (8%). Although the web narrative on supplementation was overall positive, a wide range of side effects were reported by 19% of fitness fora users. These included acne (9%), water retention (9%), stomach pain (9%), rashes (7%), erectile dysfunctions (7%) and weight gain (5%). Concerns about contamination (47%), counterfeit content (17%) and the presence of hidden ingredients (11%) were also recorded. Conclusions: In a poorly regulated context, where unsolicited social media posts have replaced the typical advice provided by professionals, efforts should be made to ensure the reliability of the provided information to avoid the insurgence of unwanted adverse effects and safeguard public health.Peer reviewe

    Web application testing: Using tree kernels to detect near-duplicate states in automated model inference

    Background: In the context of End-to-End testing of web applications , automated exploration techniques (a.k.a. crawling) are widely used to infer state-based models of the site under test. These models, in which states represent features of the web application and transitions represent reachability relationships, can be used for several model-based testing tasks, such as test case generation. However, current exploration techniques often lead to models containing many near-duplicate states, i.e., states representing slightly different pages that are in fact instances of the same feature. This has a negative impact on the subsequent model-based testing tasks, adversely affecting, for example, size, running time, and achieved coverage of generated test suites. Aims: As a web page can be naturally represented by its tree-structured DOM representation, we propose a novel near-duplicate detection technique to improve the model inference of web applications, based on Tree Kernel (TK) functions. TKs are a class of functions that compute similarity between tree-structured objects, largely investigated and successfully applied in the Natural Language Processing domain. Method: To evaluate the capability of the proposed approach in detecting near-duplicate web pages, we conducted preliminary classification experiments on a freely-available massive dataset of about 100k manually annotated web page pairs. We compared the classification performance of the proposed approach with other state-of-the-art near-duplicate detection techniques. Results: Preliminary results show that our approach performs better than state-of-the-art techniques in the near-duplicate detection classification task. Conclusions: These promising results show that TKs can be applied to near-duplicate detection in the context of web application model inference, and motivate further research in this direction to assess the impact of the technique on the quality of the inferred models and on the subsequent application of model-based testing techniques

    Seizures Do Not Affect Disability and Mortality Outcomes of Stroke: A Population-Based Study

    Although seizures are frequently seen after cerebrovascular accidents, their effects on long-term outcome in stroke patients are still unknown. Therefore, the aim of this study was to investigate the relationship between post-stroke seizures and the risk of long-term disability and mortality in stroke patients. This study is part of a larger population-based study. All patients were prospectively followed up by a face-to-face interview or a structured telephone interview. We enrolled 635 patients with first-ever stroke and without a history of seizures. Prevalence of ischemic stroke (IS) was 85.2%, while the remaining 14.8% of patients were affected by intracerebral hemorrhage (ICH). During the study period, 51 subjects (8%) developed post-stroke seizures. Patients with post-stroke seizures were younger, had a higher prevalence of ICH, had a more severe stroke at admission, were more likely to have an IS involving the total anterior circulation, and were more likely to have a lobar ICH than patients without seizures. Moreover, subjects with seizures had more frequently hemorrhagic transformation after IS and cortical strokes. At 24 months, the risk of disability in patients with seizures was almost twice than in those without seizures. However, the negative effect of seizures disappeared in multivariate analysis. Kaplan-Meier survival curves at 12 years were not significantly different between patients with and without post-stroke seizures. Using the Cox multivariate analysis, age, NIHSS at admission, and pre-stroke mRS were independently associated with all-cause long-term mortality. In our sample, seizures did not impair long-term outcome in patients affected by cerebrovascular accidents. The not significant, slight difference in favor of a better survival for patients with seizures may be attributed to the slight age difference between the two groups

    Computational design of cyclic peptides for the customized oriented immobilization of globular proteins

    The oriented immobilization of proteins, key for the development of novel responsive biomaterials, relies on the availability of effective probes. These are generally provided by standard approaches based on in vivo maturation and in vitro selection of antibodies and/or aptamers. These techniques can suffer technical problems when a non-immunogenic epitope needs to be targeted. Here we propose a strategy to circumvent this issue by in silico design. In our method molecular binders, in the form of cyclic peptides, are computationally evolved by stochastically exploring their sequence and structure space to identify high-affinity peptides for a chosen epitope of a target globular protein: here a solvent-exposed site of β2-microglobulin (β2m). Designed sequences were screened by explicit solvent molecular dynamics simulations (MD) followed by experimental validation. Five candidates gave dose-response surface plasmon resonance signals with dissociation constants in the micromolar range. One of them was further analyzed by means of isothermal titration calorimetry, nuclear magnetic resonance, and 250 ns of MD. Atomic-force microscopy imaging showed that this peptide is able to immobilize β2m on a gold surface. In short, we have shown by a variety of experimental techniques that it is possible to capture a protein through an epitope of choice by computational design

    Gambling at the time of COVID-19: results from interviews in an Italian sample of gamblers

To explore the variation of symptoms related to gambling, 46 outpatients of the mental health services in the Trento Province were invited to take part in a phone interview at the start of the national lockdown. Although only 2.17% increased gambling activity during this period, half of the sample (50.00%) experienced irritability, mood fluctuation (43.48%) and anxiety (39.13%). Follow-up studies should assess modifications in their behaviors that occurred after the reopening of gambling venues.Peer reviewedFinal Published versio

    On the cardinal system in Italian Sign Language (LIS)

    This paper offers a comprehensive discussion of the cardinal system of Italian Sign Language (LIS). At the lexical level, we present the different formational strategies used to generate cardinal numerals and we provide evidence that in the younger generations of signers, the sign ONE has lost the function of indefinite determiner and is now used as cardinal only. At the syntactic level, we show that the attested variation in order between the cardinal and the noun is in part due to definiteness and contrastive focus. We account for this variation within the cartographic approach to syntax. Finally, we offer a principled explanation for the reason why cardinals inside Measure Phrases are not subject to word order variation, but always precede the measure noun
