Interactions of C+(2PJ) with rare gas atoms: incipient chemical interactions, potentials and transport coefficients
Accurate interatomic potentials were calculated for the interaction of a singly charged carbon cation, C+, with a single rare gas atom, RG (RG = Ne–Xe). The RCCSD(T) method and basis sets of quadruple-ζ and quintuple-ζ quality were employed; each interaction energy was counterpoise corrected and extrapolated to the basis set limit. The lowest C+(2P) electronic term of the carbon cation was considered, and the interatomic potentials calculated for the diatomic terms that arise from these: 2Π and 2Σ+. Additionally, the interatomic potentials for the respective spin-orbit levels were calculated, and the effect on the spectroscopic parameters was examined. In doing this, anomalously large spin-orbit splittings for RG = Ar–Xe were found, and this was investigated using multi-reference configuration interaction calculations. The latter indicated a small amount of RG → C+ electron transfer and this was used to rationalize the observations. This is taken as evidence of an incipient chemical interaction, which was also examined via contour plots, Birge–Sponer plots and various population analyses across the C+-RG series (RG = He–Xe), with the latter showing unexpected results. Trends in several spectroscopic parameters were examined as a function of the increasing atomic number of the RG atom. Finally, each set of RCCSD(T) potentials was employed, including spin-orbit coupling to calculate the transport coefficients for C+ in RG, and the results were compared with the limited available data
Probable detection of starlight reflected from the giant exoplanet orbiting tau Bootis
Giant planets orbiting stars other than the Sun are clearly detectable
through precise radial-velocity measurements of the orbital reflex motion of
the parent star. In the four years since the discovery of the companion to the
star 51 Peg, similar low-amplitude ``Doppler star wobbles'' have revealed the
presence of some 20 planets orbiting nearby solar-type stars. Several of these
newly-discovered planets are very close to their parent stars, in orbits with
periods of only a few days. Being an indirect technique, however, the
reflex-velocity method has little to say about the sizes or compositions of the
planets, and can only place lower limits on their masses. Here we report the
use of high-resolution optical spectroscopy to achieve a probable detection of
the Doppler-shifted signature of starlight reflected from one of these objects,
the giant exoplanet orbiting the star tau Bootis. Our data give the planet's
orbital inclination i=29 degrees, indicating that its mass is some 8 times that
of Jupiter, and suggest strongly that the planet has the size and reflectivity

Phenomenology and Cosmology of an Electroweak Pseudo-Dilaton and Electroweak Baryons
In many strongly-interacting models of electroweak symmetry breaking the
lowest-lying observable particle is a pseudo-Goldstone boson of approximate
scale symmetry, the pseudo-dilaton. Its interactions with Standard Model
particles can be described using a low-energy effective nonlinear chiral
Lagrangian supplemented by terms that restore approximate scale symmetry,
yielding couplings of the pseudo-dilaton that differ from those of a Standard
Model Higgs boson by fixed factors. We review the experimental constraints on
such a pseudo-dilaton in light of new data from the LHC and elsewhere. The
effective nonlinear chiral Lagrangian has Skyrmion solutions that may be
identified with the `electroweak baryons' of the underlying
strongly-interacting theory, whose nature may be revealed by the properties of
the Skyrmions. We discuss the finite-temperature electroweak phase transition
in the low-energy effective theory, finding that the possibility of a
first-order electroweak phase transition is resurrected. We discuss the
evolution of the Universe during this transition and derive an
order-of-magnitude lower limit on the abundance of electroweak baryons in the
absence of a cosmological asymmetry, which suggests that such an asymmetry
would be necessary if the electroweak baryons are to provide the cosmological
density of dark matter. We revisit estimates of the corresponding
spin-independent dark matter scattering cross section, with a view to direct

Increasing incidence of skin disorders in children? A comparison between 1987 and 2001
BACKGROUND: The increasing proportion of skin diseases encountered in general practice represents a substantial part of morbidity in children. Only limited information is available about the frequency of specific skin diseases. We aimed to compare incidence rates of skin diseases in children in general practice between 1987 and 2001. METHODS: We used data on all children aged 0–17 years derived from two consecutive surveys performed in Dutch general practice in 1987 and 2001. Both surveys concerned a longitudinal registration of GP consultations over 12 months. Each disease episode was coded according to the International Classification of Primary Care. Incidence rates of separate skin diseases were calculated by dividing all new episodes for each distinct ICPC code by the average study population at risk. Data were stratified for socio-demographic characteristics. RESULTS: The incidence rate of all skin diseases combined in general practice decreased between 1987 and 2001. Among infants the incidence rate increased. Girls presented more skin diseases to the GP. In the southern part of the Netherlands children consulted their GP more often for skin diseases compared to the northern part. Children of non-Western immigrants presented relatively more skin diseases to the GP. In general practice incidence rates of specific skin diseases such as impetigo, dermatophytosis and atopic dermatitis increased in 2001, whereas warts, contact dermatitis and skin injuries decreased. CONCLUSION: The overall incidence rate of all skin diseases combined in general practice decreased whereas the incidence rates of bacterial, mycotic and atopic skin diseases increased

Climate model forecast biases assessed with a perturbed physics ensemble
Perturbed physics ensembles have often been used to analyse long-timescale climate model behaviour, but have been used less often to study model processes on shorter timescales. We combine a transient perturbed physics ensemble with a set of initialised forecasts to deduce regional process errors present in the standard HadCM3 model, which cause the model to drift in the early stages of the forecast. First, it is shown that the transient drifts in the perturbed physics ensembles can be used to recover quantitatively the parameters that were perturbed. The parameters which exert most influence on the drifts vary regionally, but upper ocean mixing and atmospheric convective processes are particularly important on the 1-month timescale. Drifts in the initialised forecasts are then used to recover the ‘equivalent parameter perturbations’, which allow identification of the physical processes that may be at fault in the HadCM3 representation of the real world. Most parameters show positive and negative adjustments in different regions, indicating that standard HadCM3 values represent a global compromise. The method is verified by correcting an unusually widespread positive bias in the strength of wind-driven ocean mixing, with forecast drifts reduced in a large number of areas as a result. This method could therefore be used to improve the skill of initialised climate model forecasts by reducing model biases through regional adjustments to physical processes, either by tuning or targeted parametrisation refinement. Further, such regionally tuned models might also significantly outperform standard climate models, with global parameter configurations, in longer-term climate studies
American Society of Clinical Oncology/College ofAmerican Pathologists guideline recommendations forimmunohistochemical testing of estrogen andprogesterone receptors in breast cancer
Purpose: To develop a guideline to improve theaccuracy of immunohistochemical (IHC) estrogen receptor(ER) and progesterone receptor (PgR) testing in breastcancer and the utility of these receptors as predictivemarkers.Methods: The American Society of Clinical Oncologyand the College of American Pathologists convened aninternational Expert Panel that conducted a systematicreview and evaluation of the literature in partnership withCancer Care Ontario and developed recommendations foroptimal IHC ER/PgR testing performance.Results: Up to 20% of current IHC determinations ofER and PgR testing worldwide may be inaccurate (falsenegative or false positive). Most of the issues with testinghave occurred because of variation in preanalyticvariables, thresholds for positivity, and interpretationcriteria.Recommendations: The Panel recommends that ER andPgR status be determined on all invasive breast cancers andbreast cancer recurrences. A testing algorithm that relieson accurate, reproducible assay performance is proposed.Elements to reliably reduce assay variation are specified. It is recommended that ER and PgR assays be consideredpositive if there are at least 1% positive tumor nuclei in the sample on testing in the presence of expected reactivity of internal (normal epithelial elements) and external controls. The absence of benefit from endocrine therapy for women with ER-negative invasive breast cancers has been confirmed in large overviews of randomized clinical trials.(Arch Pathol Lab Med. 2010;134:907–922
Bioavailability of iodine in the UK-Peak District environment and its human bioaccessibility: an assessment of the causes of historical goitre in this area
Iodine is an essential micronutrient for human health. Its deficiency causes a number of functional and developmental abnormalities such as goitre. The limestone region of Derbyshire, UK was goitre-endemic until it declined from the 1930s and the reason for this has escaped a conclusive explanation. The present study investigates the cause(s) of goitre in the UK-Peak District area through an assessment of iodine in terms of its environmental mobility, bioavailability, uptake into the food chain and human bioaccessibility. The goitre-endemic limestone area is compared with the background millstone grit area of the UK-Peak District. The findings of this study show that ‘total’ environmental iodine is not linked to goitre in the limestone area, but the governing factors include iodine mobility, bioavailability and bioaccessibility. Compared with the millstone grit area, higher soil pH and calcium content of the limestone area restrict iodine mobility in this area, also soil organic carbon in the limestone area is influential in binding the iodine to the soil. Higher calcium content in the limestone area is an important factor in terms of strongly fixing the iodine to the soil. Higher iodine bioaccessibility in the millstone grit than the limestone area suggests that its oral bioaccessibility is restricted in the limestone area. Iodine taken up by plant roots is transported freely into the aerial plant parts in the millstone grit area unlike the limestone area, thus providing higher iodine into the human food chain in the millstone grit area through grazing animals unlike the goitre-prevalent limestone area
Sociodemographic variation in the use of chemotherapy and radiotherapy in patients with stage IV lung, oesophageal, stomach and pancreatic cancer: evidence from population-based data in England during 2013-2014.
BACKGROUND: Sociodemographic inequalities in cancer treatment have been generally described, but there is little evidence regarding patients with advanced cancer. Understanding variation in the management of these patients may provide insights into likely mechanisms leading to inequalities in survival. METHODS: We identified 50,232 patients with stage IV lung, oesophageal, pancreatic and stomach cancer from the English national cancer registry. A generalised linear model with a Poisson error structure was used to explore variation in radiotherapy and chemotherapy within 6 months from diagnosis by age, sex, deprivation, ethnicity, cancer site, comorbidity and, additionally, performance status. RESULTS: There was substantial variation by cancer site, large gradients by age, and non-trivial associations with comorbidity and deprivation. After full adjustment, more deprived patients were consistently least likely to be treated with chemotherapy alone or chemotherapy and radiotherapy combined compared with less deprived patients with equally advanced disease stage (treatment rate ratio: 0.82 95% CI (0.78, 0.87) for CT, 0.78 95% CI (0.71, 0.85) for CTRT p < 0.0001). CONCLUSIONS: There was marked variation in the management of patients with stage IV cancer. Routinely collected data could be used for surveillance across all cancers to help reduce treatment variation and optimise outcomes among patients with advanced cancer
Public health and economic impact of vaccination with 7-valent pneumococcal vaccine (PCV7) in the context of the annual influenza epidemic and a severe influenza pandemic
Background: Influenza pandemic outbreaks occurred in the US in 1918, 1957, and 1968. Historical evidence suggests that the majority of influenza-related deaths during the 1918 US pandemic were attributable to bacterial pneumococcal infections. The 2009 novel influenza A (H1N1) outbreak highlights the importance of interventions that may mitigate the impact of a pandemic. Methods: A decision-analytic model was constructed to evaluate the impact of 7-valent pneumococcal conjugate vaccine (PCV7) on pneumococcal disease incidence and mortality during a typical influenza season (13/100) and a severe influenza pandemic (30/100). Outcomes were compared for current PCV7 vaccination practices vs. no vaccination. The model was estimated using published sources and includes indirect (herd) protection of non-vaccinated persons. Results: The model predicts that PCV7 vaccination in the US is cost saving for a normal influenza season, reducing pneumococcal-related costs by 7.3 billion in costs and prevent 512,000 cases of IPD, 719,000 cases of pneumonia, 62,000 IPD deaths, and 47,000 pneumonia deaths; 84% of deaths are prevented due to indirect (herd) protection in the unvaccinated. Conclusions: PCV7 vaccination is highly effective and cost saving in both normal and severe pandemic influenza seasons. Current infant vaccination practices may prevent >1 million pneumococcal-related deaths in a severe influenza pandemic, primarily due to herd protection
