    Coupling Mineralogical Analyses, Leaching Tests and Kinetic Modelling to Unravel Groundwater Flow-Paths in a Complex Landslide: An Attempt from the Vedriano Case Study (Northern Italian Apennines)

    As the rise in pore water pressure is one of the main factors in triggering landslides, the understanding of groundwater processes taking place at the hillslope scale is a crucial issue in slope stability analysis. However, identifying flow-paths travelled by water molecules from their infiltration is still a complex task. Hydrochemistry is recognized as a powerful tool that can help to gain useful hydrogeological information and has gradually become increasingly used in addition to conventional study methods. This manuscript presents a comprehensive geochemical investigation consisting of leaching tests and quantitative mineralogical analyses on soil samples, chemical analyses on groundwater samples and modelling. Our results highlighted the usefulness of coupling, even in hydrogeological studies focusing on landslides, geochemical surveys on both water and the soil matrix to constrain the interactions between host-rocks and groundwater. Moreover, it demonstrated that kinetic-based geochemical models, if properly calibrated on leaching tests, can provide valuable information on groundwater dynamics, allowing us to elucidate water-mixing processes beneath the soil surface

    Hydrogeological characterization of peculiar Apenninic springs

    Abstract. In the northern Apennines of Italy, springs are quite widespread over the slopes. Due to the outcropping of low-permeability geologic units, they are generally characterized by low-yield capacities and high discharge variability during the hydrologic year. In addition, low-flow periods (discharge lower than 1 Ls-1) reflect rainfall and snowmelt distribution and generally occur in summer seasons. These features strongly condition the management for water-supply purposes, making it particularly complex. The "Mulino delle Vene" springs (420 m a.s.l., Reggio Emilia Province, Italy) are one of the largest in the Apennines for mean annual discharge and dynamic storage and are considered as the main water resource in the area. They flow out from several joints and fractures at the bottom of an arenite rock mass outcrop in the vicinity of the Tresinaro River. To date, these springs have not yet been exploited, as the knowledge about the hydrogeological characteristics of the aquifer and their hydrological behaviour is not fully achieved. This study aims to describe the recharge processes and to define the hydrogeological boundaries of the aquifer. It is based on river and spring discharge monitoring and groundwater balance assessment carried out during the period 2012–2013. Results confirm the effectiveness of the approach, as it allowed the total aliquot of discharge of the springs to be assessed. Moreover, by comparing the observed discharge volume with the one calculated with the groundwater balance, the aquifer has been identified with the arenite slab (mean altitude of 580 m a.s.l.), extended about 5.5 km2 and located 1 km west of the monitored springs

    Duodenal metastasis from male breast cancer: a case report and review of the literature

    <p>Abstract</p> <p>Introduction</p> <p>Breast cancer is the most frequent type of tumor and the second leading cause of death in women. Metastases are present in nearly 60% of cases at the time of diagnosis with the lymph nodes, skeleton, lungs, brain and liver as the most frequent sites of metastases. Gastrointestinal involvement is rare, present in only 10% of all the cases. There is a very low risk of developing breast cancer in men.</p> <p>Case presentation</p> <p>A 68-year-old man, with a past history of ductal breast cancer, presented with duodenal obstruction. Medical treatment was attempted without success, so he underwent surgery with subtotal gastrectomy and resection of the first portion of the duodenum. Histological examination showed a duodenal metastasis originating from the previous carcinoma of the breast. Five months after surgery, the patient is alive and well.</p> <p>Conclusion</p> <p>Gastrointestinal metastases should be considered in patients with a past history of breast cancer. Surgical treatment should be performed in patients who are symptomatic and in good general condition. To our knowledge this is the only case of a gastrointestinal metastasis from breast carcinoma in a man.</p

    Estimating mass-wasting processes in active earth slides – earth flows with time-series of High-Resolution DEMs from photogrammetry and airborne LiDAR

    Abstract. This paper deals with the use of time-series of High-Resolution Digital Elevation Models (HR DEMs) obtained from photogrammetry and airborne LiDAR coupled with aerial photos, to analyse the magnitude of recently reactivated large scale earth slides – earth flows located in the northern Apennines of Italy. The landslides underwent complete reactivation between 2001 and 2006, causing civil protection emergencies. With the final aim to support hazard assessment and the planning of mitigation measures, high-resolution DEMs are used to identify, quantify and visualize depletion and accumulation in the slope resulting from the reactivation of the mass movements. This information allows to quantify mass wasting, i.e. the amount of landslide material that is wasted during reactivation events due to stream erosion along the slope and at its bottom, resulting in sediment discharge into the local fluvial system, and to assess the total volumetric magnitude of the events. By quantifying and visualising elevation changes at the slope scale, results are also a valuable support for the comprehension of geomorphological processes acting behind the evolution of the analysed landslides

    Estimating the Isotopic Altitude Gradient for Hydrogeological Studies in Mountainous Areas: Are the Low-Yield Springs Suitable? Insights from the Northern Apennines of Italy

    Several prior studies investigated the use of stable isotopes of water in hydrogeological applications, most on a local scale and often involving the isotopic gradient (evaluated by exploiting the so-called altitude effect), calculated on the basis of rainwater isotopes. A few times, this gradient has been obtained using the stable isotopic contents of low-yield springs in a limited time series. Despite the fact that this method has been recognized by the hydrogeological community, marked differences have been observed with respect to the mean stable isotopes content of groundwater and rainwater. The present investigation compares the stable isotopic signatures of 23 low-yield springs discharging along two transects from the Tyrrhenian sea to the Po Plain of Italy, evaluates the different isotopic gradients and assesses their distribution in relation to some climatic and topographic conditions. Stable isotopes of water show that groundwater in the study area is recharged by precipitation and that the precipitation regime in the eastern portion of the study area is strongly controlled by a shadow effect caused by the Alps chain on the air masses from central Europe. Stable isotopes (in particular the d18O and deuterium excess (d-excess) contents together with the obtained isotopic gradients) allow us to identify in the study area an opposite oriented orographic effect and a different provenance of the air masses. When the windward slope is located on the Tyrrhenian side, the precipitation shows a predominant oceanic origin; when the windward slope moves to the Adriatic side, the precipitation is characterized by a continental origin. The main results of this study confirm the usefulness of low-yield springs and the need for a highly detailed survey-scale hydrological investigation in the mountainous context

    Upper limb artery segmental occlusions due to chronic use of ergotamine combined with itraconazole, treated by thrombolysis

    <p>Abstract</p> <p>Background</p> <p>The ergotamine tartrate associated with certain categories of drugs can lead to critical ischemia of the extremities. Discontinuation of taking ergotamine is usually sufficient for the total regression of ischemia, but in some cases it could be necessary thrombolytic and anticoagulant therapy to avoid amputation.</p> <p>Case report</p> <p>A woman of 62 years presented with a severe pain left forearm appeared 10 days ago, with a worsening trend. The same symptoms appeared after 5 days also in the right forearm. Physical examination showed the right arm slightly hypothermic, with radial reduced pulse in presence of reduced sensitivity. The left arm was frankly hypothermic, pulse less on radial and with an ulnar humeral reduced pulse, associated to a decreased sensitivity and motility.</p> <p>Clinical history shows a chronic headache for which the patient took a daily basis for years Cafergot suppository (equivalent to 3.2 mg of ergotamine).</p> <p>From about ten days had begun therapy with itraconazole for vaginal candidiasis. The Color-Doppler ultrasound shown arterial thrombosis of the upper limbs (humeral and radial bilateral), with minimal residual flow to the right and no signal on the humeral and radial left artery.</p> <p>Results</p> <p>Angiography revealed progressive reduction in size of the axillary artery and right humeral artery stenosis with right segmental occlusions and multiple hypertrophic collateral circulations at the elbow joint. At the level of the right forearm was recognizable only the radial artery, decreased in size. Does not recognize the ulnar, interosseous artery was thin. To the left showed progressive reduction in size of the distal subclavian and humeral artery, determined by multiple segmental steno-occlusion with collateral vessels serving only a thin hypotrophic interosseous artery.</p> <p>Arteriographic findings were compatible with systemic drug-induced disease. The immediate implementation of thrombolysis, continued for 26 hours, with heparin in continuous intravenous infusion and subsequent anticoagulant therapy allowed the gradual disappearance of the symptoms with the reappearance of peripheral pulses.</p> <p>Conclusion</p> <p>Angiography showed regression of vasospasm and the resumption of flow in distal vessels. The patient had regained sensitivity and motility in the upper limbs and bilaterally radial and ulnar were present.</p

    Terrestrial Remote Sensing techniques to complement conventional geomechanical surveys for the assessment of landslide hazard: The San Leo case study (Italy)

    The San Leo village, located near to Rimini (northern Italy), was built in the medieval period on the top of a calcarenite and sandstone plateau, affected by lateral spreading associated with secondary rock falls and topples. In fact, a number of landslides endangered the historical town since centuries. In order to describe the structural features driving these slope instability phenomena, a complete Terrestrial Laser Scanner (TLS) survey all around the San Leo cliff was performed. Moreover, Close-Range Photogrammetric (CRP) surveys and conventional geomechanical surveys on scanlines have been carried out. The 3D geometry of the cliffs was extracted and critical areas have been investigated in detail using dense Digital Surface Models (DSMs) obtained from CRP or TLS. The results were used to define the structural features of the plateau, to recognize more fractured areas, and to perform kinematic analyses, in order to assess the joint sets predisposing to slope instability at the cliff scale. The creation of a 3D model was also fundamental for the implementation of the geological model to be used in numerical modelling for hydrogeological characterization and slope stability analyses

    Prospective randomized comparison of open versus laparoscopic management of splenic artery aneurysms: a 10-year study

    Abstract BACKGROUND: The literature does not support the choice between open and laparoscopic management of splenic artery aneurysms (SAA). METHODS: We designed a prospective, randomized comparison between open and laparoscopic surgery for SAA. Primary end points were types of surgical procedures performed and clinical outcomes. Analysis was developed on an intention-to-treat basis. RESULTS: Fourteen patients were allocated to laparotomy (group A) and 15 to laparoscopy (group B). Groups displayed similar patient- and aneurysm-related characteristics. The conversion rate to open surgery was 13.3 %. The type of surgical procedure performed on the splenic artery was similar in the two groups: aneurysmectomy with splenic artery ligature or direct anastomosis was performed in 51 % and 21 % of patients in group A and in 60 % and 20 % in group B, respectively. The splenectomy rate was similar (14 % vs. 20 %). Postoperative splenic infarction was observed in one case in each group. Laparoscopy was associated with shorter procedures (p = 0.0003) and lower morbidity (25 % vs. 64 %, p = 0.045). Major morbidity requiring interventional procedures and blood transfusion was observed only in group A. Laparoscopy was associated with quicker resumption of oral diet (p < 0.001), earlier drain removal (p = 0.046), and shorter hospital stay (p < 0.01). During a mean follow-up of 50 months, two patients in group A required hospital readmission. In group B, two patients developed a late thrombosis of arterial anastomoses. CONCLUSIONS: Our study demonstrates that laparoscopy permits multiple technical options, does not increase the splenectomy rate, and reduces postoperative complications. It confirms the supposed clinical benefits of laparoscopy when ablative procedures are required but laparoscopic anastomoses show poor long-term results
