    A 10-Year Experience in Intraoperative Parathyroid Hormone Measurements for Primary Hyperparathyroidism: A Prospective Study of 91 Previous Unexplored Patients

    Introduction. Primary hyperparathyroidism (PHP) is characteristically determined by high levels of calcium and high or inappropriate levels of parathyroid hormone (PTH). Technological advances have dramatically changed the surgical technique over the years once intraoperative parathyroid hormone (IOPTH) assay had allowed for focused approaches. Objective. To evaluate our 10-year experience in employing a rapid intraoperative PTH assay for PHP. Methods. A prospective cohort of 91 PHP-operated patients in a tertiary institution in São Paulo, Brazil, from June 2000 to April 2011. Results. We had 85 (93.4%) successful parathyroidectomies, 6 (6.6%) failed parathyroidectomies in 91 previous unexplored patients, and 5 (100%) successful remedial surgeries. The IOPTH was true-positive in 88.5%, true-negative in 7.3%, false-positive in 2.1%, and false-negative in 2.1% of the procedures. IOPTH was able to obviate additional exploration or to ask for additional exploration in 92 (95.8%) procedures. Conclusion. The IOPTH revealed to be an important technological adjunct in the current parathyroid surgery for PHP

    Total Parathyroidectomy with Presternal Intramuscular Autotransplantation in Renal Patients: A Prospective Study of 66 Patients

    Surgical treatment of secondary (SHPT) and tertiary hyperparathyroidism (THPT) may involve various surgical approaches. The aim of this paper was to evaluate presternal intramuscular autotransplantation of parathyroid tissue as a surgical option in SHPT and THPT treatment. 66 patients with renal chronic disease underwent surgery from April 2000 to April 2005 at Universidade Federal São Paulo, Brazil. There were 38 SHPT patients (24 women/14 men), mean age of 39.yrs (range: 14–58), and 28 THPT patients (14 women/14 men), mean age of 43.4 yrs (range: 24–62). Postoperative average followup was 42.9 months (range: 12–96). Postoperative intact PTH increased throughout followup from 73.5 pg/mL to 133 pg/mL on average from 1st to the 5th year, respectively, in SHPT and from 54.9 pg/mL to 94.7 pg/mL on average from 1st to 5th year, respectively, in THPT group. Definitive hypoparathyroidism was observed in 4 (6.06%) patients and graft-dependent recurrence in 6 (9.09%). Presternal intramuscular autotransplantation of parathyroid tissue is a feasible and safe surgical option in SHPT and THPT treatment

    Variational state based on the Bethe ansatz solution and a correlated singlet liquid state in the one-dimensional t-J model

    The one-dimensional t-J model is investigated by the variational Monte Carlo method. A variational wave function based on the Bethe ansatz solution is newly proposed, where the spin-charge separation is realized, and a long-range correlation factor of Jastrow-type is included. In most regions of the phase diagram, this wave function provides an excellent description of the ground-state properties characterized as a Tomonaga-Luttinger liquid; Both of the amplitude and exponent of correlation functions are correctly reproduced. For the spin-gap phase, another trial state of correlated singlet pairs with a Jastrow factor is introduced. This wave function shows generalized Luther-Emery liquid behavior, exhibiting enhanced superconducting correlations and exponential decay of the spin correlation function. Using these two variational wave functions, the whole phase diagram is determined. In addition, relations between the correlation exponent and variational parameters in the trial functions are derived.Comment: REVTeX 3.0, 27 pages. 7 figures available upon request ([email protected]).     A new experimental technique for quantifying the galvanic coupling effects on stainless steel during tribocorrosion under equilibrium conditions

    Galvanic coupling during tribocorrosion of passive metals at open circuit potential (OCP) generates a wear-accelerated corrosion process within the depassivated area (worn surface) that is electrically in contact with the still passive one. The galvanic coupling effect at OCP was recently modeled using an electrochemical approach allowing for the theoretical quantification of the wear-accelerated corrosion under equilibrium conditions. Despite the usefulness of this model that mathematically determines the electrochemical conditions inside the wear track in terms of anode potential via the approximation of the net anodic current density, an experimental technique allowing for their experimental determination is essential in the effort to verify the galvanic coupling models and further understand the tribocorrosion mechanisms at OCP. In the present work, a new experimental technique based on galvanic current and potential measurements through a Zero-Resistance Ammeter (ZRA) for quantifying the electrode potential and anodic current inside the wear track during rubbing at OCP has been assessed. This experimental set-up has allowed for the first time to determine the prevailing electrochemical conditions (electrode potential and anodic current) inside the wear track by solely exposing the wear track to the electrolyte and physically separating the cathode from the anode (wear track). The effects of sliding wear at open circuit potential have been investigated for a super duplex stainless steel (UNS S32750) in 3.4 wt% NaCl. The new experimental set-up proposed in this work separates the cathode from the anode and exposes solely the wear track to the electrolyte. Using well-established electrochemical theories, the effect of the extent of the galvanic coupling on wear at the open circuit potential conditions has been quantified.The authors would like to thank the support from the European Union for funding C.     Perturbation theory of the dynamic inverse spin Hall effect with charge conservation

    We present gauge-invariant theory of the dynamic inverse spin Hall effect driven by the spin--orbit interaction in metallic systems. Charge conservation is imposed diagrammatically by including vertex corrections. We show the charge current is induced by an effective electric field that is proportional to the spin current pumped by the magnetization dynamics.     Methodological considerations in the analysis of fecal glucocorticoid metabolites in tufted capuchins (Cebus apella)

    Analysis of fecal glucocorticoid (GC) metabolites has recently become the standard method to monitor adrenocortical activity in primates noninvasively. However, given variation in the production, metabolism, and excretion of GCs across species and even between sexes, there are no standard methods that are universally applicable. In particular, it is important to validate assays intended to measure GC production, test extraction and storage procedures, and consider the time course of GC metabolite excretion relative to the production and circulation of the native hormones. This study examines these four methodological aspects of fecal GC metabolite analysis in tufted capuchins (Cebus apella). Specifically, we conducted an adrenocorticotrophic hormone (ACTH) challenge on one male and one female capuchin to test the validity of four GC enzyme immunoassays (EIAs) and document the time course characterizing GC me- tabolite excretion in this species. In addition, we compare a common field-friendly technique for extracting fecal GC metabolites to an established laboratory extraction methodology and test for effects of storing “field extracts” for up to 1 yr. Results suggest that a corticosterone EIA is most sensitive to changes in GC production, provides reliable measures when extracted according to the field method, and measures GC metabolites which remain highly stable after even 12 mo of storage. Further, the time course of GC metabolite excretion is shorter than that described yet for any primate taxa. These results provide guidelines for studies of GCs in tufted capuchins, and underscore the importance of validating methods for fecal hormone analysis for each species of interest

    Characteristics and outcomes of patients with advanced non-small-cell lung cancer who declined to participate in randomised clinical chemotherapy trials

    There are inadequate data on the outcomes of patients who declined to participate in randomised clinical trials as compared with those of participants. We retrospectively reviewed the patient characteristics and treatment outcomes of both participants and non-participants in the two randomised trials for chemotherapy-naive advanced non-small-cell lung cancer. Trial 1 compared four platinum-based combination regimens. Trial 2 compared two sequences of carboplatin plus paclitaxel and gefitinib therapies. Nineteen of 119 (16%) and 153 (37%) patients declined to participate in Trials 1 and 2, respectively. Among the background patient characteristics, the only variable associated with trial participation or declining was the patients' attending physicians (P<0.001). Important differences were not observed in the clinical outcomes between participants and non-participants, for whom the response rates were 30.6 vs 34.2% and the median survival times were 489 vs 461 days, respectively. The hazard ratio for overall survival, adjusted for other confounding variables, was 0.965 (95% confidence interval: 0.73–1.28). In conclusion, there was no evidence to suggest any difference in the characteristics and clinical outcomes between participants and non-participants. Trial designs and the doctor–patient relationship may have an impact on the patient accrual to randomised trials

    Fostering entrepreneurship and an entrepreneurial culture has become a key policy priority for governments. To encourage entrepreneurship and an entrepreneurial culture, however, there is a need to understand the factors that influence and shape individuals' intentions to start a business. This study extends models of entrepreneurial intentions by investigating the influence of various childhood-experience factors on the perceived feasibility and desirability of starting a business. A structured questionnaire was completed by over 1,000 university students and analysed using regression analysis. Results indicated that perceptions of entrepreneurship were influenced not only by parental ownership of a business, but also by a difficult childhood and frequent relocation

    Analysis of monitoring programmes and their suitability for ecotoxicological risk assessment in four Spanish basins

    Data from four Spanish basin management authorities were analysed. Chemical and biological data from four Spanish basin management authorities were analysed, focusing on three consecutive years. Aims were to i) determine the chemicals most likely responsible for the environmental toxicological risk in the four Spanish basins and ii) investigate the relationships between toxicological risk and biological status in these catchments. The toxicological risk of chemicals was evaluated using the toxic unit (TU) concept. With these data we considered if the potential risk properly reflects the risk to the community or, alternatively, if new criteria should be developed to improve risk assessment. Data study revealed inadequacies in processing and monitoring that should be improved (e.g., site coincidence for chemical and biological sampling). Analysis of the chemical data revealed high potential toxicological risk in the majority of sampling points, to which metals were the main contributors to this risk. However, clear relationships between biological quality and chemical risk were found only in one river. Further investigation of metal toxicity may be necessary, and future analyses are necessary to accurately estimate the risk to the environment.The present work was funded by the Spanish Ministry of Economics and Competitiveness through the Consolider-Ingenio 2010 Program (project Scarce CSD2009-00065).
