    Temporal and spatial variability of dissolved organic and inorganic phosphorus, and metrics of phosphorus bioavailability in an upwelling-dominated coastal system

The definitive version was published in Journal of Geophysical Research 110 (2005): C10S13, doi:10.1029/2004JC002837.High-frequency temporal and spatial shifts in the various dissolved P pools (total, inorganic, and organic) are linked to upwelling/relaxation events and to phytoplankton bloom dynamics in the upwelling-dominated Oregon coastal system. The presence and regulation of alkaline phosphatase activity (APA) is apparent in the bulk phytoplankton population and in studies of cell-specific APA using Enzyme Labeled Fluorescence (ELF®). Spatial and temporal variability are also evident in phytoplankton community composition and in APA. The spatial pattern of dissolved phosphorus and APA variability can be explained by bottom-controlled patterns of upwelling, and flushing times of different regions within the study area. The presence of APA in eukaryotic taxa indicates that dissolved organic phosphorus (DOP) may contribute to phytoplankton P nutrition in this system, highlighting the need for a more complete understanding of P cycling and bioavailability in the coastal ocean.KCR acknowledges WHOI for rapid-response funding that made possible participation on this first COAST cruise, and NSF-OCE grant 0119134 for support of subsequent work on these and other COAST samples

    Use of the internet as a source of health information by Spanish adolescents

    <p>Abstract</p> <p>Background</p> <p>The Internet is a fundamental part of the daily life of adolescents, they consider it as a safe and confidential source of information on health matters. The aims is to describe the experience of Spanish adolescents searching for health information on the Internet.</p> <p>Methods</p> <p>A cross-sectional study of 811 school-age adolescents in Granada was carried out. An adapted and piloted questionnaire was used which was controlled by trained personnel. Sociodemographic and health variables were included together with those concerning the conditions governing access to and use of information and communication technologies (ICT).</p> <p>Results</p> <p>811 adolescents were surveyed (99.38% response rate), mean age was 17 years old. Of these, 88% used the Internet; 57.5% used it on a daily or weekly basis and 38.7% used it occasionally. Nearly half the sample group (55.7%) stated that they used the Internet to search for health-related information. The main problems reported in the search for e-health were the ignorance of good web pages (54.8%) and the lack of confidence or search skills (23.2%).</p> <p>Conclusions</p> <p>In conclusion, it seems plausible to claim that websites designed and managed by health services should have a predominant position among interventions specifically addressed to young people.</p

    Preconditioning with Physiological Levels of Ethanol Protect Kidney against Ischemia/Reperfusion Injury by Modulating Oxidative Stress

    Oxidative stress due to excessive production of reactive oxygen species (ROS) and subsequent lipid peroxidation plays a critical role in renal ischemia/reperfusion (IR) injury. The purpose of current study is to demonstrate the effect of antecedent ethanol exposure on IR-induced renal injury by modulation of oxidative stress.Bilateral renal warm IR was induced in male C57BL/6 mice after ethanol or saline administration. Blood ethanol concentration, kidney function, histological damage, inflammatory infiltration, cytokine production, oxidative stress, antioxidant capacity and Aldehyde dehydrogenase (ALDH) enzymatic activity were assessed to evaluate the impact of antecedent ethanol exposure on IR-induced renal injury.After bilateral kidney ischemia, mice preconditioned with physiological levels of ethanol displayed significantly preserved renal function along with less histological tubular damage as manifested by the reduced inflammatory infiltration and cytokine production. Mechanistic studies revealed that precondition of mice with physiological levels of ethanol 3 h before IR induction enhanced antioxidant capacity characterized by significantly higher superoxidase dismutase (SOD) activities. Our studies further demonstrated that ethanol pretreatment specifically increased ALDH2 activity, which then suppressed lipid peroxidation by promoting the detoxification of Malondialdehyde (MDA) and 4-hydroxynonenal (HNE).Our results provide first line of evidence indicating that antecedent ethanol exposure can provide protection for kidneys against IR-induced injury by enhancing antioxidant capacity and preventing lipid peroxidation. Therefore, ethanol precondition and ectopic ALDH2 activation could be potential therapeutic approaches to prevent renal IR injury relevant to various clinical conditions

    Prioritization of non-recommended clinical activities in Primary Care

    Background: To prioritize non-recommended clinical activities in Primary Care (PC), from "Do not do" recommendations listed by the Sociedad Espanola de Medicina de Familia y Comunitaria (Semfyc), according to expert consensus (physicians, nurses and pharmacists).Methods. The consensus for the prioritization of non-recommended practices in PC was performed through an online procedure. We used as a base the list of "do not-do" recommendations of the SEMFYC. We asked the experts to prioritize practices that should be de-adopted in PC, based on four prioritization criteria: frequency of occurrence, cost of the activity, ease of disposal and damage caused, which were scored from one to five, according to their recommendation. Scores were summarized in median and quartile values. Two rounds were necessary to obtain a consensus. A modified e-Delphi technique was used.Results. 34 experts (62%) participated in the first consultation round and prioritized 19 recommendations with a score >= 3.5. These recommendations were again analyzed in a second round, in which 32 panelists agreed to prioritize 17 practices (13 related to prescription, three diagnostic tests, and one clinical analysis). The high priority list included seven practices with values >= 4.1) Prescription of a new drug in elderly patients without having reviewed the previous treatments; 2) Lipid-lowering drugs without calculating the overall cardiovascular risk; 3) Not systematically prescribing gastric protection with proton pump inhibitors to patients consuming Nonsteroidal anti-inflammatory drugs (NSAIDs); 4) Glucose self-analysis in non-insulinized type 2 diabetics; 5) Benzodiazepines in the long term; 6) Bisphosphonates in patients with low risk of fracture; and 7) Antibiotics in lower respiratory tract infections.Conclusion. This study provides information for the prioritization of 17 non-AP activities in which short-term de-adoption would significantly increase the efficiency of the public health system

    Inverse analysis of the planktonic food web dynamics related to phytoplankton bloom development on the continental shelf of the Bay of Biscay, French coast

    International audienceAlong the French coast of the Bay of Biscay, the seasonal progression of phytoplankton presents specific hydrographic conditions compared to other temperate areas due to the influence of two large river plumes (Loire and Gironde). However, in spite of the occurrence of a microphy-toplankton bloom during winter and phosphate limitation in spring, the dynamics of the planktonic food web on the continental shelf of the Bay seem to correspond with the characteristics typical of temperate oceans with the dominance of herbivory during spring and the development of the microbial food web under the stable post-bloom environment. Existing plankton data from recent scientific cruises were combined with inverse and network analyses to construct five model ecosystems between late winter and late spring and to investigate the structure and the function of the food web. The analysis of those models confirmed that the absolute amount of biogenic carbon export from the planktonic food web follows a continuum from high carbon export to depth when the microphytoplankton winter bloom is not consumed by grazers to carbon recycling within the microbial food web after the spring bloom when the water column is strongly stratified. Therefore, the winterespring period corresponds to a situation of high export. The particular hydrological conditions of the Bay of Biscay are not altering the capacity of the planktonic food web to export the greatest absolute amount of carbon during spring to plankton predators such as small pelagic fishes. However, this study showed that, in proportion to the primary production, the relative amount of biogenic carbon export to higher trophic levels does not vary considerably among the three types of planktonic food web function.     Non-steroidal anti-inflammatory drugs (NSAIDs) are among the most widely used drugs worldwide. This makes it necessary to carry out a comprehensive synthesis of the available evidence on the safe and adequate prescription of NSAIDs in patients with cardiovascular disease, chronic kidney disease, hypertension, heart failure or liver cirrhosis and in general population. For this, a review of systematic reviews was carried out. Data extraction and analysis were performed independently by two reviewers and a narrative synthesis of the results was carried out. The use of NSAIDs is associated with a significantly higher probability of hepatotoxicity and kidney damage, as well as increased risk of exacerbation of heart failure. Taking into account the increased cardiovascular, liver and kidney risk, the prescription of NSAIDs should be carried out with caution, considering the treatment duration and the patient's situation. For this reason, patients should be informed about their possible health consequences as well as ensuring adequate monitoring of them

    Lower trophic levels and detrital biomass control the Bay of Biscay continental shelf food web : Implications for ecosystem management

    The Bay of Biscay (North-East Atlantic) has long been subjected to intense direct and indirect human activities that lead to the excessive degradation and sometimes overexploitation of natural resources. Fisheries management is gradually moving away from single-species assessments to more holistic, multi-species approaches that better respond to the reality of ecosystem processes. Quantitative modelling methods such as Ecopath with Ecosim can be useful tools for planning, implementing and evaluating ecosystem-based fisheries management strategies. The aim of this study was therefore to model the energy fluxes within the food web of this highly pressured ecosystem and to extract practical information required in the diagnosis of ecosystem state/health. A well-described model comprising 30 living and two non-living compartments was successfully constructed with data of local origin, for the Bay of Biscay continental shelf. The same level of aggregation was applied to primary producers, mid-trophic-levels and top-predators boxes. The model was even more general as it encompassed the entire continuum of marine habitats, from benthic to pelagic domains. Output values for most ecosystem attributes indicated a relatively mature and stable ecosystem, with a large proportion of its energy flow originating from detritus. Ecological network analysis also provided evidence that bottom-up processes play a significant role in the population dynamics of upper-trophic-levels and in the global structuring of this marine ecosystem. Finally, a novel metric based on ecosystem production depicted an ecosystem not far from being overexploited. This finding being not entirely consistent over indicators, further analyses based on dynamic simulations are required

    Lymphocyte cell counts in middle age are positively associated with subsequent all-cause and cardiovascular mortality

    Background: There is an association between higher white blood cell counts and all-cause and cardiovascular disease (CVD) mortality. However, little is known about the prognostic significance of circulating lymphocyte and lymphocyte subset numbers. Aims: The present study examined the association between T-, CD4-, CD8- and B-cell numbers, and the CD4:CD8 ratio, and all-cause and CVD mortality. Methods: Lymphocyte and lymphocyte subset numbers were measured by flow cytometry in a cohort of 4256 male middle-aged Vietnam-era US veterans. Mortality was tracked for 15 years and cause of death was determined from death certificates. Results: In fully adjusted survival analyses, high circulating T-cells numbers were associated with increased risk of both all-cause [hazard ratio (HR)?=?1.75, 95% confidence interval (CI) 1.15–2.66] and cardiovascular (HR?=?3.57, 95% CI 1.53–8.33) mortality. The former association appeared to reflect an effect for high CD8-cells numbers, the latter an effect for high CD4-cell numbers. For all-cause mortality, a high CD4:CD8 ratio was protective (HR?=?0.58, 95% CI 0.41–0.81). Cardiovascular mortality was also predicted by high B-cells numbers (HR?=?1.87, 95% CI 1.10–3.17). Conclusion: Circulating lymphocyte and lymphocyte subset numbers may have substantial prognostic significance for both all-cause and CVD mortality. <br/
