Es descriu la infraestructura de recursos d’assistència sanitària a les Illes Balears i se n’examina l’evolució al llarg del període 1999-2009. Les Illes es configuren com una regió singular pel fet que disposen d’una de les més àmplies xarxes hospitalàries d’aguts de l’Estat, tant pública com privada, amb un nombrós grup de professionals, i altament dotada tecnològicament, que contrasta amb el pes relatiu menor que presenta l’atenció primària i els hospitals de mitjana i llarga estada. L’evolució dels recursos durant el període 1999-2008 s’ha caracteritzat pel notable creixement del sector públic a l’atenció hospitalària d’aguts, amb l’obertura de quatre hospitals de nova planta i amb la multiplicació per dos del nombre de professionals.The infrastructure of resources in health assistance of the Balearic Islands is described and the evolution of it through the period 1999-2009 examined. The Balearic Islands are configured as an outstanding area because they boast one of the most complete nets of acute hospitalisation in the whole State in terms of public and private sector. They have a large group of professionals and it is technologically highly equipped, which contrasts with the low relative weight of primary care as well as medium and long-stay hospitals. The evolution of resources during the period of 1999-2008 is characterised by the significant growth of the public sector in acute care hospitalisation, with the opening of four new hospitals and with the doubling in the number of professionals

Es descriuen els aspectes determinants de demanda d’assistència sanitària i d’assegurament privat a les Illes Balears, així com s’examina la seva evolució al llarg del període 1999-2009. D’acord amb l’anàlisi d’aquests determinants, les Illes es configuren com una regió singular ja que presenten característiques diferents respecte a la resta de comunitats autònomes en relació a variables com el major nombre de població estacional i estrangera, la menor proporció de persones majors, el baix nivell d’estudis o l’elevat augment demogràfic i escàs creixement econòmic. Els notables canvis experimentats en els determinants durant el període considerat han modificat el tipus de demanda d’assistència sanitària cap a una major presència del sector públic en relació al sector privat i una disminució de l’assegurament privat.We describe the key aspects of the demand for health services and of the private health insurance in the Balearic Islands. We also examine the changes in these aspects throughout the period 1999-2009. The analysis of these aspects suggests that the Islands are a singular region, with different characteristics compared to the other Spanish regions. These differences are related to variables such as the seasonal and foreign population, the proportion of older people, the low levels of education and the demographic and economic growth. The important changes in the aspects during the analyzed period have modified the type of demand for health services, leading to a greater presence of the public sector than the private sector and a decrease of private health insurance.Se describen los aspectos determinantes de demanda de asistencia sanitaria y de la compra de seguro privado en las Illes Balears, así como se examina su evolución a lo largo del periodo 1999-2009. De acuerdo con el análisis de estos determinantes, las Illes se configuran como una región singular dado que presentan características diferentes respecto al resto de comunidades autónomas en relación a variables como el mayor número de población estacional y extranjera, la menor proporción de personas mayores, el bajo nivel de estudios o el elevado aumento demográfico y escaso crecimiento económico. Los notables cambios experimentados en los determinantes durante el periodo considerado han modificado el tipo de demanda de asistencia sanitaria hacia una mayor presencia del sector público en relación al sector privado y una disminución de aseguramiento privado

Palliative care program of the Balearic Islands

Los cuidados paliativos (CP) mejoran la calidad de vida de los pacientes en la última etapa de la enfermedad terminal y disponemos de evidencia de su eficiencia al dar respuesta a las necesidades integrales de atención de las personas -pacientes y familiares- en el periodo de final de vida. En las Islas hay experiencia consolidada de equipos expertos, aunque con una insuficiente cobertura de atención. Ante esta situación, la Consejería de Salud, a través de la Estrategia de Cuidados Paliativos de las Islas Baleares, ha diseñado este programa cuyo objetivo principal es proporcionar a los pacientes con enfermedad avanzada/terminal y a sus familiares, una valoración y atención integrales, adaptadas a cada situación y en cualquiera de los niveles asistenciales. Partiendo del supuesto de que, si las personas con necesidades de atención paliativa y sus familias recibiesen atención desde el modelo de los cuidados paliativos, obtendrían una mayor calidad de vida, mayor satisfacción y una reducción de carga en los cuidadores y menores costes en el sistema, se ha elaborado el programa con una metodología participativa, basándose en la opinión de más de 100 profesionales, gestores, representantes de la Administración y usuarios así como en las referencias, y estándares nacionales y europeos. El programa se fundamenta en acercar los cuidados al entorno familiar y social del paciente; para ello el equipo de atención primaria y el Equipo de soporte de atención domiciliaria (ESAD) son el eje de atención. Según la complejidad del proceso se distinguen dos niveles de atención: básico y especializado. Partiendo de los recursos existentes y las necesidades sanitarias se planifican una serie de actuaciones para cada uno de los sectores de salud.Palliative care (PC) improves patients’ quality of life in the last stage of the terminal disease and we have evidence of its efficiency in responding to comprehensive care needs of people –patients and families– at the end-of-life period. There is a consolidated experience of expert teams in the Balearic Islands, although with inadequate care coverage. In view of this situation, the Balearic Department of Health, through the Palliative Care Strategy of the Balearic Islands, has designed this program, whose main purpose is to provide patients with advanced/terminal disease and their families, a whole assessment and care, adapted to each situation and at any level of care. Based on the assumption that, if the people with palliative care needs and their families received attention from the palliative care model, they would obtain a greater quality of life and satisfaction, a reduced burden in caregivers, and lower costs in the system. The program has been developed with a participative methodology, based on the views of more than 100 professionals, managers, Administration representatives, and users, as well as on national and European references and standards. This program is based on approaching care to the familial and social environment of the patient, for which the primary care team and the Homecare support team (ESAD) are the focus of attention. Depending on the complexity of the process, there are two levels of care: basic and specialized. Building on the existing resources and health needs, a series of actions for each health sector are planned

Activation of PKA via asymmetric allosteric coupling of structurally conserved cyclic nucleotide binding domains

Cyclic nucleotide-binding (CNB) domains allosterically regulate the activity of proteins with diverse functions, but the mechanisms that enable the cyclic nucleotide-binding signal to regulate distant domains are not well understood. Here we use optical tweezers and molecular dynamics to dissect changes in folding energy landscape associated with cAMP-binding signals transduced between the two CNB domains of protein kinase A (PKA). We find that the response of the energy landscape upon cAMP binding is domain specific, resulting in unique but mutually coordinated tasks: one CNB domain initiates cAMP binding and cooperativity, whereas the other triggers inter-domain interactions that promote the active conformation. Inter-domain interactions occur in a stepwise manner, beginning in intermediate-liganded states between apo and cAMP-bound domains. Moreover, we identify a cAMP-responsive switch, the N3A motif, whose conformation and stability depend on cAMP occupancy. This switch serves as a signaling hub, amplifying cAMP-binding signals during PKA activation

[spa] El contenido del presente trabajo, tratará de la elaboración de un Plan de Negocio de una bodega. 7103 Petit Celler, va ha ser el objeto de estudio de dicho plan, una bodega de reciente apertura, situada en Santa Maria, en la isla de Mallorca. En el trabajo se va a tratar de hacer un análisis exhaustivo de la bodega. A través de la descripción del negocio y su entorno, se va a determinar cuales van a ser sus estrategias a seguir para la consecución de las diferentes variedades de vino. Posteriormente se hará un plan de marketing, y un plan operativo para terminar con un análisis financiero. Finalmente se propondrán unas conclusiones y consideraciones finales.[eng] The content of the present work would be the development of a Business Plan for a wine cellar. 7103 Petit Cellar is going to be the object of study of the plan, a wine cellar of recent opening, located in Santa Maria, on the island of Mallorca. In the work, I am going to try to make a comprehensive analysis of the winery. Through the description of the business and its environment, I am going to determine which strategies will be the ones to follow for the achievement of the different varieties of wine. Later there will be a marketing plan, and an operational plan finishing with a financial analysis. Finally conclusions and considerations will be proposed

Consideracions historiogràfiques sobre la plasmació de la «puella pilosa» en l’edat moderna i la seva repercussió en l’actualitat. Antonietta González, Brígida del Río i Magdalena Ventura

[cat] El present estudi aborda les diferents connotacions iconològiques de les dones barbades existents dins la historiografia hispànica recent, la qual tracta aquestes representacions en la forquilla cronològica de l’època moderna. Tenint en compte els precedents literaris medievals i renaixentistes, en el text es desenvoluparan les nocions que tenia la societat moderna sobre les dones barbades a partir dels estudis crítics sobre la pintura i literatura dels segles XVI i XVII. Prenent com a referència el retrat d’Antonietta González de Lavinia Fontana datat c. 1595, el retrat de Brígida del Río de Juan Sánchez Cotán pintat en el 1590 i el retrat de Magdalena Ventura efectuat per José de Ribera l’any 1631, es desenvoluparà la rellevància del pensament modern sobre aquestes dones, s’explicarà la seva incidència en la consciència social i els canvis de significat en la seva representació en l’art fins a l’actualitat.[eng] The present study broaches the different iconological connotations of bearded women existing in the recent Hispanic historiograpy which adresses these representations in the modern period. Considering medieval and Renaissance literary precedents, it is developed troughout this text the notions that modern society had about bearded women, based on studies about painting and literature from the 16th and 17th centuries. Taking the portrait of Antonietta González painted by Lavinia Fontana c. 1595, the portrait of Brígida del Río by Juan Sánchez Cotán dated in 1590 and the portrait of Magdalena Ventura executed by José de Ribera in 1631 as references, it is developed the relevance of modern thinking about these women and it is explained their impact on social consciousness and the changes in meanings in their representations in Art until nowadays
