    Mutant pattern of p53 predicts local recurrence and poor survival rate in gastric cancer

    Background. TP53 mutation is a poor prognostic factor for various organ malignancies such as colorectal cancer, breast cancer, ovarian cancer, hepatocellular carcinoma, lung adenocarcinoma and clinical pathologists previously evaluated it using immunohistochemistry for p53. The clinicopathologic significance of p53 expression in gastric cancer remains unclear due to inconsistent classification methods. Methods. Immunohistochemistry for p53 protein was performed using tissue microarray blocks generated from 725 cases of gastric cancer, and p53 expression was divided into three staining patterns using a semiquantitative ternary classifier: heterogeneous (wild type), overexpression, and absence (mutant pattern). Results. Mutant pattern of p53 expression had a male predominance, greater frequency in cardia/fundus, higher pT stage, frequent lymph node metastasis, local recurrence clinically, and more differentiated histology microscopically compared with wild type. In survival analysis, p53 mutant pattern was associated with worse recurrent-free survival and overall survival rates, and significance was maintained in subgroup analysis of early versus advanced gastric cancers. In Cox regression analysis, p53 mutant pattern was a significant predicting factor for local recurrence (relative risk (RR=4.882, p<0.001)) and overall survival (RR=2.040, p=0.007). The p53 mutant pattern remained significant for local recurrence (RR=2.934, p=0.018) in multivariate analyses. Conclusions. Mutant p53 pattern on immunohistochemistry was a significant prognostic factor for local recurrence and poor overall survival in gastric cancer

    Clinical application of functional near-infrared spectroscopy for burn assessment

    Significance: Early assessment of local tissue oxygen saturation is essential for clinicians to determine the burn wound severity.Background: We assessed the burn extent and depth in the skin of the extremities using a custom-built 36-channel functional near-infrared spectroscopy system in patients with burns.Methods: A total of nine patients with burns were analyzed in this study. All second-degree burns were categorized as superficial, intermediate, and deep burns; non-burned skin on the burned side; and healthy skin on the contralateral non-burned side. Hemodynamic tissue signals from functional near-infrared spectroscopy attached to the burn site were measured during fNIRS using a blood pressure cuff. A nerve conduction study was conducted to check for nerve damage.Results: All second-degree burns were categorized into superficial, intermediate, and deep burns; non-burned skin on the burned side and healthy skin on the contralateral non-burned side showed a significant difference distinguishable using functional near-infrared spectroscopy. Hemodynamic measurements using functional near-infrared spectroscopy were more consistent with the diagnosis of burns 1 week later than that of the degree of burns diagnosed visually at the time of admission.Conclusion: Functional near-infrared spectroscopy may help with the early judgment of burn extent and depth by reflecting differences in the oxygen saturation levels in the skin

    Generalized Pustular Psoriasis and Hepatic Dysfunction Associated with Oral Terbinafine Therapy

    We report a case of 61-yr-old man with stable psoriasis who progressively developed generalized pustular eruption, erythroderma, fever, and hepatic dysfunction following oral terbinafine. Skin biopsy was compatible with pustular psoriasis. After discontinuation of terbinafine and initiating topical corticosteroid and calcipotriol combination with narrow band ultraviolet B therapy, patient'S condition slowly improved until complete remission was reached 2 weeks later. The diagnosis of generalized pustular psoriasis (GPP) induced by oral terbinafine was made. To our knowledge, this is the first report of GPP accompanied by hepatic dysfunction associated with oral terbinafine therapy

    Serial Measurement of WT1 Expression and Decrement Ratio Until Hematopoietic Cell Transplantation as a Marker of Residual Disease in Patients with Cytogenetically Normal Acute Myelogenous Leukemia

    AbstractUsing real-time quantitative PCR, we monitored Wilms tumor gene 1 (WT1) expression from diagnosis to hematopoietic stem cell transplantation (HSCT) in adult patients with cytogenetically normal acute myelogenous leukemia (CN-AML) and FLT3-ITD and NPM1 mutations. The values at diagnosis were evaluated in 104 patients. Data collected after induction chemotherapy were available for all patients, but only 68 patients were treated with HSCT. Significant WT1 expression cut-offs were determined by receiver operation characteristic curve analysis, and rates of overall survival (OS) and disease-free survival (DFS) were estimated. WT1 decrement ratios (DR) at postinduction chemotherapy and at pre- and post-HSCT compared with the diagnostic level were calculated. Higher WT1 expression at diagnosis, postinduction chemotherapy, and pre-HSCT showed inferior OS (P = .015, <.001, and .002) and DFS (P = .006, <.001, and .003). The cut-offs were determined at the median for diagnostic WT1 expression and at the 25% level from the top for other time points excluding post-HSCT. The WT1 DR ≥ 1-log after induction chemotherapy showed superior OS and DFS (P = .009 and .002) and WT1 DR ≥ 1-log preceding HSCT also showed superior OS and DFS (P = .009 and .003). Results of WT1 DR were consistently applicable in each subgroup with higher (≥1.0) and lower (<1.0) WT1 expression at diagnosis and also in NPM1-wild-type/FLT3-ITD–negative CN-AML. The WT1 DR therefore predicted survival outcomes after HSCT more accurately than did the diagnostic WT1 expression. WT1 expression may serve as a reliable marker for residual disease and WT1 DR as a prognostic indicator, particularly in NPM1-wild-type/FLT3-ITD–negative CN-AML. These measures may be applied throughout the course of treatment and even after HSCT

    Development of a 2-Channel Embedded Infrared Fiber-Optic Temperature Sensor Using Silver Halide Optical Fibers

    A 2-channel embedded infrared fiber-optic temperature sensor was fabricated using two identical silver halide optical fibers for accurate thermometry without complicated calibration processes. In this study, we measured the output voltages of signal and reference probes according to temperature variation over a temperature range from 25 to 225 °C. To decide the temperature of the water, the difference between the amounts of infrared radiation emitted from the two temperature sensing probes was measured. The response time and the reproducibility of the fiber-optic temperature sensor were also obtained. Thermometry with the proposed sensor is immune to changes if parameters such as offset voltage, ambient temperature, and emissivity of any warm object. In particular, the temperature sensing probe with silver halide optical fibers can withstand a high temperature/pressure and water-chemistry environment. It is expected that the proposed sensor can be further developed to accurately monitor temperature in harsh environments

    Role of age and sex in determining antibiotic resistance in febrile urinary tract infections

    SummaryObjectivesTo identify the age- and sex-specific antimicrobial susceptibility patterns of Gram-negative bacteria (GNB) in outpatient febrile urinary tract infections (UTIs) in Korea.MethodsA total 2262 consecutive samples collected from patients aged 1–101 years with febrile UTIs, during the period January 2012 to December 2014, were analyzed in this multicentre, retrospective cohort study.ResultsThe sensitivities to cefotaxime and cefoxitin were over 85% for females but under 75% for males. Sex played an important role in the susceptibility of GNB to cefotaxime (p<0.001) and cefoxitin (p<0.001). The sensitivity to ciprofloxacin (age >20 years) was under 75% in both sexes, and was not influenced by sex (p=0.204). Age distributions of the incidences of resistance to cefotaxime, cefoxitin, and ciprofloxacin (age >20 years) were similar to the age distribution of the incidence of GNB, which indicates that the resistance patterns to these drugs were not affected by age (Kolmogorov–Smirnov test, female/male: p=0.927/p=0.509, p=0.193/p=0.911, and p=0.077/p=0.999, respectively).ConclusionsAge is not a considerable factor in determining the antibiotic resistance in febrile UTIs. Ciprofloxacin should be withheld from both sexes until culture results indicate its use. Second- or third-generation cephalosporins such as cefoxitin and cefotaxime can be used empirically only in females

    Recent studies suggest that liver cirrhosis is reversible after administering oral nucleos(t)ide analogue therapy to patients with hepatitis B virus (HBV) infection. However, few studies have addressed whether esophageal varices can regress after such therapy. We report a case of complete regression of esophageal varices during entecavir therapy in patients with HBV-related liver cirrhosis, suggesting that complications of liver cirrhosis such as esophageal varices can regress after the long-term suppression of HBV replication

    Bladder Recovery by Stem Cell Based Cell Therapy in the Bladder Dysfunction Induced by Spinal Cord Injury: Systematic Review and Meta-Analysis

    Background Bladder dysfunction induced by spinal cord injury (SCI) can become problematic and severely impair the quality of life. Preclinical studies of spinal cord injury have largely focused on the recovery of limb function while neglecting to investigate bladder recovery. Objective The present study was performed to investigate and review the effect of stem cell-based cell therapy on bladder recovery in SCI. Methods We conducted a meta-analysis of urodynamic findings of experimental trials that included studies of stem cell-based cell therapy in SCI. Relevant studies were searched using MEDLINE, EMBASE and Cochrane Library (January 1990 -December 2012). Final inclusion was determined by a urodynamic study involving detailed numerical values. Urodynamic parameters for analysis included voiding pressure, residual urine, bladder capacity and non-voiding contraction (NVC). Meta-analysis of the data, including findings from urodynamic studies, was performed using the Mantel-Haenszel method. Results A total of eight studies were included with a sample size of 224 subjects. The studies were divided into different subgroups by different models of SCI. After a stem cell-based cell therapy, voiding pressure (-6.35, p < 0.00001, I-2 = 77%), NVC (-3.58, p < 0.00001, I-2 = 82%), residual urine (-024, p = 0.004, I-2 = 95%) showed overall significant improvement. Bladder capacity showed improvement after treatment only in the transection type (-0.23, p = 0.0002, I-2 = 0%). Conclusion After stem cell-based cell therapy in SCI, partial bladder recovery including improvement of voiding pressure, NVC, and residual urine was demonstrated. Additional studies are needed to confirm the detailed mechanism and to obtain an ideal treatment strategy for bladder recovery.open1156sciescopu

    Import 26/02/2015Tato diplomová práce je zaměřena na vyprojektování multifunkčního domu, ve kterém bude zajištěno strojní větrání a zajištěno vytápění pomocí alternativního zdroje tepla. Multifunkční dům podsklepený a má čtyři nadzemní podlaží s pochozí terasou. Objekt je navržen ze sortimentu firmy Porotherm. Funkčně je navržen tak, že v prvním podzemním podlaží se nachází technické zázemí a kóje bytů, první nadzemní podlaží tvoří dvě nezávislé prodejní plochy. Ve druhém patře jsou umístěny kancelářské prostory. Druhou horní polovinu objektu , třetí a čtvrté nadzemní podlaží, tvoří bytové jednotky. Základním kamenem pro nutnou výměnu vzduchu v objektu jsou tři vzduchotechnické systémy, které se starají o dodávku a výměnu vzduchu ve třech celcích. První celkem je jednotka pro prodejní plochy v prvním nadzemním podlaží a kancelářské prostory ve druhém nadzemním podlaží. Druhá jednotka má na starosti byty ve třetím a čtvrtém podlaží a poslední, třetí, jednotka zajišťuje přívod čerstvého vzduchu do společných prostor a odvod odpadního vzduchu do exteriéru. Všechny jednotky jsou vybaveny rekuperátorem. V objektu je také řešeno vytápění pomocí alternativních zdrojů. Jako alternativní zdroje energie jsou použity tepelné čerpadlo vzduch – voda a solární kolektory. Tepelné čerpadlo se stará o pokrytí tepelných ztrát prostupem, větráním a přípravou teplé vody. Solární kolektory umístěné na terase pomáhají s ohřevem teplé vody v objektu.This Master thesis is focused on the projection Multifunctional building, which will ensure mechanical ventilation and heating ensured by using alternative heat sources. Multifunctional building basement and has four floors with walkable terrace. The building is designed from company Porotherm. Functionally, it is designed so that on the first floor there is a technical background and cubicle apartments, first floor consists of two independent sales area. On the second floor there are office spaces. The second half of the upper house, the third and fourth floors, consists of residential units. The cornerstone for the necessary air exchange in the building are three air handling systems that are responsible for the delivery and exchange of air in three units. The first is the total unit sales area on the first floor and office space on the second floor. The second unit is in charge of flats in the third and fourth floors and the last third unit provides fresh air to the common area and the exhaust to the exterior. All units are equipped with a recuperator. There is also heating solutions using alternative sources. As alternative energy sources are used heat pump air - water and solar collectors. The heat pump takes care of the cover heat losses through transmission, ventilation and hot water.
