    Solitary Peutz-Jeghers Polyp in a Paediatric Patient

    Hamartomatous polyps of Peutz-Jeghers are mostly found in patients affected by Peutz-Jeghers syndrome (PJS), but they can be rarely encountered in the general population. It is unclear whether a solitary Peutz-Jeghers polyp (PJP) is an incomplete form of PJS or a separate entity. We report a case of solitary PJP in a paediatric patient in whom the other features of PJS were absent. The patient underwent laparotomy due to small bowel intussusception secondary to an ileac polyp. Histological examination showed the characteristic features of PJP, but the patient did not fulfill the WHO criteria for PJS diagnosis (negative family history for PJS and absence of mucocutaneous pigmentation); moreover analysis of the STK11/LKB1 gene did not reveal any genomic abnormality. The clinical and investigative findings in our case suggest that the solitary PJP can be considered a different clinical entity from PJS

    Analysis of Systemic Inflammatory Factors and Survival Outcomes in Endometrial Cancer Patients Staged I-III FIGO and treated with Postoperative External Radiotherapy

    Background: The causal link between elevated systemic inflammation biomarkers and poor survival has been demonstrated in cancer patients. However, the evidence for this correlation in endometrial cancer (EC) is too weak to influence current criteria of risk assessment. Here, we examined the role of inflammatory indicators as a tool to identify EC patients at higher risk of death in a retrospective observational study. Methods: A total of 155 patients surgically diagnosed with EC stage I-III FIGO 2009 and treated with postoperative External Beam Radiotherapy (EBRT) brachytherapy and chemotherapy according to ESMO-ESTRO-ESGO recommendation for patients at high risk of recurrence at the Gustave Roussy Institut, France, and Hospital Clínic, Spain, between 2008 and 2017 were evaluated. The impact of pre-treatment Neutrophil-to-Lymphocyte Ratio (NLR 2.2), Monocyte-to-Lymphocyte Ratio (MLR 0.18), Systemic Immune-Inflammatory Index (SII 1100) and lymphopenia (<1.0 109/L) on overall survival (OS), cancer-specific survival and progression-free survival was evaluated. Subsequently, a cohort of 142 patients within high-advanced risk groups according to ESMO-ESGO-ESTRO classification was evaluated. Results: On univariate analysis, NLR (HR = 2.2, IC 95% 1.1-4.7), SII (HR = 2.2, IC 95% 1.1-4.6), MLR (HR = 5.0, IC 95% 1.1-20.8) and lymphopenia (HR = 3.8, IC 95% 1.6-9.0) were associated with decreased OS. On multivariate analysis, NLR, MLR, SII and lymphopenia proved to be independent unfavorable prognostic factors. Conclusions: lymphopenia and lymphocytes-related ratio are associated with poorer outcome in surgically staged I-III FIGO EC patients classified as high risk and treated with adjuvant EBRT and could be considered at cancer diagnosis. External validation in an independent cohort is required before implementation for patients' stratification

    Assembly-Induced Bright-Light Emission from Solution-Processed Platinum(II) Inorganic Polymers

    Synthesis, processing, and characterization are reported for a series of tetracyanoplatinate Magnus' salt (TCN-MS) derivatives-soluble derivatives of the generally intractable Magnus' green salt-that feature the general structure [Pt(NH2R)(4)] [Pt(CN)(4)] where R is a branched alkyl group or a w-phenylalkyl group. In solutions, these coordination compounds generally dissolve on the level of individual ion pairs as shown by X-ray diffraction analysis. To enable the formation of quasi-one-dimensional linear stacks of Pt(II) atoms in thin films, the matrix-assisted assembly is employed, whereby the compounds are codissolved with poly(ethylene oxide) (PEO), followed by film casting, thermally activated assembly, and eventual removal of PEO. Remarkably, assembled TCN-MS inorganic polymers exhibit bright blue-green photoluminescence. A detailed investigation of the assembly process and simultaneously modified solid-state optical properties is performed using a range of microscopy, optical and vibrational spectroscopy, and thermal analysis techniques. Given their unusual combination of optical properties, namely, transparency in the visible region, high photoluminescence quantum efficiencies (up to 13% in first-demonstration samples), and large Stokes shifts (up to 1 eV), TCN-MS derivatives are proposed as a promising class of light-emitting materials for emerging applications in molecular optoelectronics, the potential and challenges of which are discussed.The work in Barcelona was financially supported by the Ministerio de Economía y Competitividad of Spain through the Severo Ochoa Programme for Centres of Excellence in R&D (SEV-2015-0496) and project MAT2015-70850-P and the European Research Council (ERC) under grant agreement no. 648901.     Characteristics and patterns of care of endometrial cancer before and during COVID-19 pandemic

    Objective: Coronavirus disease 2019 (COVID-19) outbreak has correlated with the disruption of screening activities and diagnostic assessments. Endometrial cancer (EC) is one of the most common gynecological malignancies and it is often detected at an early stage, because it frequently produces symptoms. Here, we aim to investigate the impact of COVID-19 outbreak on patterns of presentation and treatment of EC patients. Methods: This is a retrospective study involving 54 centers in Italy. We evaluated patterns of presentation and treatment of EC patients before (period 1: March 1, 2019 to February 29, 2020) and during (period 2: April 1, 2020 to March 31, 2021) the COVID-19 outbreak. Results: Medical records of 5,164 EC patients have been retrieved: 2,718 and 2,446 women treated in period 1 and period 2, respectively. Surgery was the mainstay of treatment in both periods (p=0.356). Nodal assessment was omitted in 689 (27.3%) and 484 (21.2%) patients treated in period 1 and 2, respectively (p&lt;0.001). While, the prevalence of patients undergoing sentinel node mapping (with or without backup lymphadenectomy) has increased during the COVID-19 pandemic (46.7% in period 1 vs. 52.8% in period 2; p&lt;0.001). Overall, 1,280 (50.4%) and 1,021 (44.7%) patients had no adjuvant therapy in period 1 and 2, respectively (p&lt;0.001). Adjuvant therapy use has increased during COVID-19 pandemic (p&lt;0.001). Conclusion: Our data suggest that the COVID-19 pandemic had a significant impact on the characteristics and patterns of care of EC patients. These findings highlight the need to implement healthcare services during the pandemic
