Intervención nutricional en el paciente oncohematológico
Background: Oncohematological diseases such as
lymphoma or leukaemia affect an increasing number of
newly diagnosed patients in Spain and other countries.
Both disease and treatment may have a negatively impact
in the nutritional status of the patient. Malnutrition is not
uncommon among oncohematological patients. This
situation can compromised the course of the disease, the
clinical response of the treatment and the patient’s
quality of life.
Method: The implementation of a multidisciplinary
approach and a systematic and protocolled nutritional
assessment would be useful when dealing with haematological
malignancies.
Results: We present a proposal of protocol for nutritional
intervention in oncohematological patients. This
proposal is been developed from the analysis of the
published literature as well as clinical practice of a multidisciplinary
team specialized in the management of
patients with haematological malignanciesObjetivos: Las enfermedades oncohematológicas como
el Linfoma o la Leucemia afectan a un número importante
y creciente de personas en España. Tanto la enfermedad
como las distintas modalidades de tratamiento
que puede llegar a precisar el paciente a lo largo del curso
de la misma impactan negativamente en el estado nutricional
del paciente, no siendo infrecuente el desarrollo de
desnutrición, situación que compromete la evolución, la
respuesta al tratamiento y la calidad de vida del paciente.
Método: La implementación de una estrategia multidisciplinar,
sistematizada y protocolizada de valoración
nutricional puede resultar de utilidad a la hora de abordar
a los pacientes con enfermedades oncohematológicas.
Resultados: Se presenta una propuesta de protocolo de
evaluación y soporte nutricional en el paciente oncohematológico
elaborada a partir del análisis de la literatura
publicada al respecto, así como de la práctica clínica habitual
de un equipo sanitario multidisciplinar especialmente
implicado en el manejo de los pacientes con enfermedades
oncohematológica
Análisis de la relevancia y factibilidad de indicadores de calidad en las unidades de nutrición
Introducción: La evaluación de la calidad en las actividades
sanitarias exige la elección de unos indicadores
acordes con los resultados que queremos medir. De todos
los posibles, debemos priorizar aquellos que nos permitan
obtener la información más relevante sin sobrecargar el
trabajo habitual de nuestras Unidades.
Objetivo: Conocer la opinión de los socios de SENPE
respecto a la relevancia y la viabilidad del uso de una
selección de indicadores de calidad para su aplicación en
nutrición clínica.
Métodos: Encuesta remitida mediante correo electrónico
a los socios de SENPE solicitando a los mismos su
opinión sobre 12 indicadores de calidad, valorándose
cada uno en cuanto a su relevancia y factibilidad de la
aplicación en su medio.
Resultados: Contestaron 40 encuestados de 40 centros
diferentes de 12 comunidades autónomas. En general, los
indicadores fueron considerados más relevantes que factibles.
Los indicadores mejor puntuados fueron: “identificación
en las bolsas de nutrición artificial”, “posición
semi-incorporada del paciente con nutrición enteral por
sonda nasogástrica” y “protocolos clínicos básicos”. Considerando
los indicadores por grupos (de estructura, proceso
o resultado) los mejor valorados fueron: “identificación
del paciente en las bolsas de nutrición artificial”
(estructura), “posición semi-incorporada” y “protocolos
clínicos básicos” (proceso), y “cumplimiento del objetivo
calórico” (resultado).
Conclusión: Los resultados de la encuesta permiten
seleccionar indicadores prioritarios para su aplicación en
las Unidades de NutriciónIntroduction: The quality assessment in health activities
requires the choice of indicators in line with the
results we want to measure. Of all possible, we should prioritize
those that allow us to obtain the most relevant
information without overloading the regular work of our
units.
Objective: To determine the opinion of the members of
SENPE regarding the relevance and feasibility of using a
selection of quality indicators designed for use in clinical
nutrition.
Methods: E-mail survey sent to members of SENPE
asking them their views on 12 quality indicators, evaluating
each in terms of their relevance and feasibility of
implementation in their environment.
Results: 40 respondents answered from 40 centers in 12
different regions. In general, the indicators were considered
more relevant than feasible. The indicators best
rated were: “identification in artificial nutrition bags,
“semi-recumbent position in patient with nasogastric
tube feeding” and “basic clinical protocols”. Considering
the type of indicator: “patient identification in the bags of
artificial nutrition (structure),” a semi-incorporated
“and” basic clinical protocols (process), and “fulfillment
of the caloric goal” (result).
Conclusion: The results of the survey can make a selection
of indicators that could be considered for first-line
introduction in a Nutrition Uni
The prevalence and clinical characteristics of nonradiographic axial spondyloarthritis among patients with inflammatory back pain in rheumatology practices: a multinational, multicenter study
BACKGROUND: Patients with ankylosing spondylitis (AS), who by definition have radiographic sacroiliitis, typically experience symptoms for a decade or more before being diagnosed. Yet, even patients without radiographic sacroiliitis (i.e., nonradiographic axial spondyloarthritis [nr-axSpA]) report a significant disease burden. The primary objective of this study was to estimate the prevalence and clinical characteristics of nr-axSpA among patients with inflammatory back pain (IBP) in rheumatology clinics in a number of countries across the world. A secondary objective was to estimate the prevalence of IBP among patients with chronic low back pain (CLBP). METHODS: Data were collected from 51 rheumatology outpatient clinics in 19 countries in Latin America, Africa, Europe, and Asia. As consecutive patients with CLBP (N = 2517) were seen by physicians at the sites, their clinical histories were evaluated to determine whether they met the new Assessment of SpondyloArthritis international Society criteria for IBP. For those who did, their available clinical history (e.g., family history, C-reactive protein [CRP] levels) was documented in a case report form to establish whether they met criteria for nr-axSpA, AS, or other IBP. Patients diagnosed with nr-axSpA or AS completed patient-reported outcome measures to assess disease activity and functional limitations. RESULTS: A total of 2517 patients with CLBP were identified across all sites. Of these, 974 (38.70 %) fulfilled the criteria for IBP. Among IBP patients, 29.10 % met criteria for nr-axSpA, and 53.72 % met criteria for AS. The prevalence of nr-axSpA varied significantly by region (p < 0.05), with the highest prevalence reported in Asia (36.46 %) and the lowest reported in Africa (16.02 %). Patients with nr-axSpA reported mean ± SD Ankylosing Spondylitis Disease Activity Scores based on erythrocyte sedimentation rate and CRP of 2.62 ± 1.17 and 2.52 ± 1.21, respectively, indicating high levels of disease activity (patients with AS reported corresponding scores of 2.97 ± 1.13 and 2.93 ± 1.18). Similarly, the overall Bath Ankylosing Spondylitis Disease Activity Index score of 4.03 ± 2.23 for patients with nr-axSpA (4.56 ± 2.17 for patients with AS) suggested suboptimal disease control. CONCLUSIONS: These results suggest that, in the centers that participated in the study, 29 % of patients with IBP met the criteria for nr-axSpA and 39 % of patients with CLBP had IBP. The disease burden in nr-axSpA is substantial and similar to that of AS, with both groups of patients experiencing inadequate disease control. These findings suggest the need for early detection of nr-axSpA and initiation of available treatment options to slow disease progression and improve patient well-being
Nutrición enteral domiciliaria en España: registro Nadya del año 2011-2012
Objective: To describe the results of the home enteral
nutrition (HEN) registry of the NADYA-SENPE group in
2011 and 2012.
Material and methods: We retrieved the data of the patients
recorded from January 1st 2011 to December 31st 2012.
Results: There were 3021 patients in the registry
during the period from 29 hospitals, which gives 65.39 per
million inhabitants. 97.95% were adults, 51.4% male.
Mean age was 67.64 ± 19.1, median age was 72 years for
adults and 7 months for children. Median duration with
HEN was 351 days and for 97.5% was their first event
with HEN. Most patients had HEN because of neurological
disease (57.8%). Access route was nasogastric tube
for 43.5% and gastrostomy for 33.5%. Most patients had
limited activity level and, concerning autonomy, 54.8%
needed total help. Nutritional formula was supplied from
chemist’s office to 73.8% of patients and disposables,
when necessary, was supplied from hospitals to 53.8% of
patients. HEN was finished for 1,031 patients (34.1%)
during the period of study, 56.6% due to decease and
22.2% due to recovery of oral intake.
Conclusions: Data from NADYA-SENPE registry
must be explained cautiously because it is a non-compulsory
registry. In spite of the change in the methodology of
the registry in 2010, tendencies regarding HEN have been

de los años 2011 y 12.
Material y métodos: Se recopilaron los datos introducidos
en el registro desde el 1 de enero de 2011 al 31 de diciembre
de 2012.
Resultados: Hubo 3021 pacientes en el registro durante
el periodo, procedentes de 29 hospitales, lo que da una
prevalencia de 65,39 casos por millón de habitantes.
97.95% fueron adultos, 51,4% varones. La edad media
fue 67,64 ± 19,1 años y la mediana 72 años para los adultos
y 7 meses para los niños. La duración media de la NED
fue 351 días y para el 97,5% fue el primer episodio con
NED. La mayoría de pacientes tenían NED por una enfermedad
neurológica (57,8%). La vía de acceso fue sonda
nasogástrica para el 43,5% y gastrostomía para el 33,5%.
La mayoría de pacientes tuvieron un nivel de actividad física
limitado y, respecto a la autonomía, 54,8% necesitaba
ayuda total. La fórmula de nutrición se suministró desde
las oficinas de farmacia para el 73,8% y los fungibles,
cuando fueron necesarios, desde los hospitales para el
53,8%. La NED se suspendió en 1.031 pacientes (34,1%)
durante el periodo de estudio, 56,6% debido a fallecimiento
y 22,2% debido a recuperación de la vía oral.
Conclusiones: Los datos del registro NADYA-SENPE
deben ser interpretados con precaución ya que se trata de
un registro voluntario. A pesar del cambio de metodología
del registro en 2010, las tendencias en NED se han mantenido,
salvo la importancia cuantitativa de la vía ora
Nutrición parenteral domiciliaria en España 2018. Informe del Grupo de Nutrición Artificial Domiciliaria y Ambulatoria NADYA
Objetivo: comunicar los datos de nutrición parenteral domiciliaria (NPD) obtenidos del registro del grupo NADYA-SENPE (www.nadya-senpe.com) del año 2018.
Material y métodos: análisis descriptivo de los datos recogidos de pacientes adultos y pediátricos con NPD en el registro NADYA-SENPE del 1 de enero al 31 de diciembre de 2018.
Resultados: se registraron 278 pacientes (54, 7% mujeres), 23 niños y 255 adultos, procedentes de 45 hospitales españoles, lo que representa una tasa de prevalencia de 5, 95 pacientes/millón de habitantes/año 2018. El diagnóstico más frecuente en adultos fue “oncológico paliativo” (22, 0%), seguido de “otros”. En niños fue la enfermedad de Hirschsprung junto con la enterocolitis necrotizante, con cuatro casos (17, 4%). El primer motivo de indicación fue síndrome de intestino corto tanto en niños (60, 9%) como en adultos (35, 7%). El tipo de catéter más utilizado fue el tunelizado tanto en niños (81, 0%) como en adultos (41, 1%). Finalizaron 75 episodios, la causa más frecuente fue el fallecimiento (52, 0%) y el paso a vía oral (33, 3%).
Conclusiones: el número de centros y profesionales colaboradores en el registro de pacientes que reciben NPD se mantiene estable, así como las principales indicaciones y los motivos de finalización de la NPD.
Aim: To communicate home parenteral nutrition (HPN) data obtained from the HPN registry of the NADYA-SENPE group (www.nadya-senpe. com) for the year 2018.
Material and methods: Descriptive analysis of the data collected from adult and pediatric patients with HPN in the NADYA-SENPE group registry from January 1st, 2018 to December 31st, 2018.
Results: There were 278 patients from 45 Spanish hospitals (54.7% women), 23 children and 255 adults, which represent a prevalence rate of 5.95 patients/million inhabitants/year 2018. The most frequent diagnosis in adults was “palliative cancer” (22.0%), followed by “others”. In children it was Hirschsprung’s disease together with necrotizing enterocolitis, with four cases (17.4%). The first indication was short bowel syndrome in both children (60.9%) and adults (35.7%). The most frequently used type of catheter was tunneled in both children (81.0%) and adults (41.1%). Ending 75 episodes, the most frequent cause was death (52.0%) and change to oral feeding (33.3%).
Conclusions: The number of centers and collaborating professionals in the registry of patients receiving HPN remains stable, as well as the main indications and reasons for termination of HPN
