    Use of Prescription Medication by Individuals Who Died by Suicide in Northern Ireland

    OBJECTIVES/: To understand medication use prior to suicide in relation to patterns, polypharmacy and adherence. METHODS: 1,371 suicide cases were coded and latent class analysis used to identify combinations of medications prescribed prior to death. RESULTS: Two thirds had been prescribed medication with 30.7% prescribed three or more. Latent class analysis revealed three classes: Mixed medication use, primarily mental medication use, and baseline/low medication use. There are potentially high rates of medication non-adherence. CONCLUSIONS: Both medication use but also non-adherence rates were high in this sample of individuals who died by suicide. Potential implications and areas for future research are discussed

    Patterns of Suicidal Ideation and Behavior in Northern Ireland and Associations with Conflict Related Trauma

    In this study, data from the World Mental Health Survey's Northern Ireland (NI) Study of Health and Stress (NISHS) was used to assess the associations between conflict- and non-conflict-related traumatic events and suicidal behaviour, controlling for age and gender and the effects of mental disorders in NI. DSM mental disorders and suicidal ideation, plans and attempts were assessed using the Composite International Diagnostic Interview (CIDI) in a multi-stage, clustered area probability household sample (N = 4,340, response rate 68.4%). The traumatic event categories were based on event types listed in the PTSD section of the CIDI. Suicidal ideation and attempts were more common in women than men, however, rates of suicide plans were similar for both genders. People with mood, anxiety and substance disorders were significantly more likely than those without to endorse suicidal ideation, plan or attempt. The highest odds ratios for all suicidal behaviors were for people with any mental disorder. However, the odds of seriously considering suicide were significantly higher for people with conflict and non-conflict-related traumatic events compared with people who had not experienced a traumatic event. The odds of having a suicide plan remain significantly higher for people with conflict-related traumatic events compared to those with only non-conflict-related events and no traumatic events. Finally, the odds of suicide attempt were significantly higher for people who have only non-conflict-related traumatic events compared with the other two categories. The results suggest that traumatic events associated with the NI conflict may be associated with suicidal ideation and plans, and this effect appears to be in addition to that explained by the presence of mental disorders. The reduced rates of suicide attempts among people who have had a conflict-related traumatic event may reflect a higher rate of single, fatal suicide attempts in this population

    A Python-Based Open Source System for Geographic Object-Based Image Analysis (GEOBIA) Utilizing Raster Attribute Tables

    A modular system for performing Geographic Object-Based Image Analysis (GEOBIA), using entirely open source (General Public License compatible) software, is presented based around representing objects as raster clumps and storing attributes as a raster attribute table (RAT). The system utilizes a number of libraries, developed by the authors: The Remote Sensing and GIS Library (RSGISLib), the Raster I/O Simplification (RIOS) Python Library, the KEA image format and TuiView image viewer. All libraries are accessed through Python, providing a common interface on which to build processing chains. Three examples are presented, to demonstrate the capabilities of the system: (1) classification of mangrove extent and change in French Guiana; (2) a generic scheme for the classification of the UN-FAO land cover classification system (LCCS) and their subsequent translation to habitat categories; and (3) a national-scale segmentation for Australia. The system presented provides similar functionality to existing GEOBIA packages, but is more flexible, due to its modular environment, capable of handling complex classification processes and applying them to larger datasets

    Chimeric IgH-TCR/ translocations in T lymphocytes mediated by RAG

    Translocations involving the T cell receptor alpha/delta (TCRα/δ) chain locus, which bring oncogenes in the proximity of the TCRα enhancer, are one of the hallmark features of human T cell malignancies from ataxia telangiectasia (AT) and non-AT patients. These lesions are frequently generated by the fusion of DNA breaks at the TCRα/δ locus to a disperse region centromeric of the immunoglobulin heavy chain (IgH) locus. Aberrant VDJ joining accounts for TCRα/δ associated DNA cleavage, but the molecular mechanism that leads to generation of the "oncogene partner" DNA break is unclear. Here we show that in ATM deficient primary mouse T cells, IgH/TCRα/δ fusions arise at a remarkably similar frequency as in human AT lymphocytes. Recombinase-activating gene (RAG) is responsible for both TCRα/δ as well as IgH associated breaks on chromosome 12 (Chr12), which are subject to varying degrees of chromosomal degradation. We suggest a new model for how oncogenic translocations can arise from two non-concerted physiological DSBs
