Expression of phosphorylated ribosomal protein S6 in mesothelioma patients - correlation with clinico-pathological characteristics and outcome: results from the European Thoracic Oncology Platform (ETOP) Mesoscape project
Pleural mesothelioma (PM) is an aggressive malignancy with poor prognosis. Although histology and pathologic stage are important prognostic factors, better prognostic biomarkers are needed. The ribosomal protein S6 is a downstream target of the phosphatidylinositol 3-kinase (PI3K) pathway involved in protein synthesis and cell proliferation. In previous studies, low phosphorylated S6 (pS6) immunoreactivity was significantly correlated with longer progression-free survival (PFS) and overall survival (OS) in PM patients. We aimed to correlate pS6 expression to clinical data in a large multi-centre PM cohort as part of the European Thoracic Oncology Platform (ETOP) Mesoscape project. Tissue Micro Arrays (TMAs) of PM were constructed and expression of pS6 was evaluated by a semiquantitatively aggregate H-score. Expression results were correlated to patient characteristics as well as OS/PFS. pS6 IHC results of 364 patients from 9 centres, diagnosed between 1999 and 2017 were available. The primary histology of included tumours was epithelioid (70.3%), followed by biphasic (24.2%) and sarcomatoid (5.5%). TMAs included both treatment-naive and tumour tissue taken after induction chemotherapy. High pS6 expression (181 patients with H-score>1.41) was significantly associated with less complete resection. In the overall cohort, OS/PFS were not significantly different between pS6-low and pS6-high patients. In a subgroup analysis nonepithelioid (biphasic and sarcomatoid) patients with high pS6 expression showed a significantly shorter OS (p< 0.001, 10.7 versus 16.9 months) and PFS (p < 0.001, 6.2 versus 10.8 months). In subgroup analysis, in non-epithelioid PM patients high pS6 expression was associated with significantly shorter OS and PFS. These exploratory findings suggest a clinically relevant PI3K pathway activation in non-epithelioid PM which might lay the foundation for future targeted treatment strategies
Serum and bronchoalveolar lavage levels of adiponectin in advanced non-small cell lung cancer: correlation with clinicopathological features
Aims: We aimed to explore the prognostic implications of adiponectin (APN) levels in the serum and bronchoalveolar lavage (BAL) of patients with advanced non-small cell lung cancer (NSCLC). Methods: Twenty-nine newly diagnosed patients with stage IV NSCLC were prospectively enrolled. Baseline serum and BAL levels of APN were assayed by enzyme-linked immunoassay (ELISA) and correlated with various clinicopathological parameters, including overall survival (OS). Results: No statistically significant correlations were observed between the serum or BAL levels of APN and the clinicopathological parameters evaluated. The only prognostic factor identified, both by univariate and multivariate survival analysis, was PS. Conclusions: The results of our cohort failed to reveal any prognostic significance of serum and BAL levels of APN in stage IV NSCLC.Η αδιπονεκτίνη είναι μια από τις κυριότερες αδιποκίνες που παράγονται από τον λιπώδη ιστό. Προηγούμενες μελέτες έδειξαν τη δυνητική χρησιμότητα της αδιπονεκτίνης ορού ως διαγνωστικού ή/και προγνωστικού βιοδείκτη σε διάφορους συμπαγείς όγκους, αλλά τα σχετικά δεδομένα για τον καρκίνο του πνεύμονα, ειδικότερα, είναι ελλιπή. Επιπρόσθετα, η διαγνωστική ή και προγνωστική αξία των επιπέδων αδιπονεκτίνης σε άλλα βιολογικά υγρά -εκτός του περιφερικού αίματος- ασθενών με καρκίνο πνεύμονα ή και με οποιαδήποτε άλλη μορφή καρκίνου, δεν έχει, απ’ όσο γνωρίζουμε, μελετηθεί. Ο πρωταρχικός σκοπός της παρούσας μελέτης ήταν η περαιτέρω διερεύνηση της δυνητικής προγνωστικής αξίας των προθεραπευτικών επιπέδων αδιπονεκτίνης στον ορό και σε δείγματα βρογχοκυψελιδικής έκπλυσης (bronchoalveolar lavage, BAL) ασθενών με μη μικροκυτταρικό καρκίνο πνεύμονα (ΜΜΚΠ) προχωρημένου σταδίου. Στον παρόν ερευνητικό έργο μελετήθηκαν προοπτικά 29 νεοδιαγνωσθέντες ασθενείς με ΜΜΚΠ σταδίου IV. Τα προθεραπευτικά επίπεδα αδιπονεκτίνης ορού και BAL μετρήθηκαν με τη χρήση ανοσοενζυμικής μεθόδου (ELISA), και, ακολούθως, συσχετίσθηκαν με δημογραφικές, κλινικές και παθολογοανατομικές παραμέτρους. Αναλυτικότερα, οι παράμετροι που αξιολογήθηκαν συμπεριελάμβαναν το φύλο, την ηλικία, το Performance Status (PS), το δείκτη μάζας σώματος (Body Mass Index, BMI), την απώλεια βάρους > 10%, καθώς και τον ιστολογικό τύπο, βαθμό διαφοροποίησης (grade) και μέγεθος του πρωτοπαθούς όγκου. Η συσχέτιση των επιπέδων αδιπονεκτίνης και των λοιπών μεταβλητών με τη συνολική επιβίωση των ασθενών αξιολογήθηκε με μονοπαραγοντική και πολυπαραγοντική ανάλυση παλινδρόμησης του Cox (univariate and multivariate Cox regression analysis).Σύμφωνα με τα αποτελέσματα της μελέτης μας, η μέση ηλικία των ασθενών ήταν 65.6 έτη (SD= 10.1 έτη), ενώ η πλειοψηφία ήταν άνδρες (24/29 περιπτώσεις, 82.8%). Ο κυρίαρχος ιστολογικός τύπος ήταν το αδενοκαρκίνωμα (18/29 περιπτώσεις, 62.1%). Η λειτουργική κατάσταση (performance status, PS) των ασθενών ήταν 0 και 1-2 σε 17/29 περιπτώσεις (58.6%) και 12/29 περιπτώσεις (41.4%), αντιστοίχως. Απώλεια βάρους μεγαλύτερη του 10% παρατηρήθηκε σε 10/29 ασθενείς (34.5%). Οι διάμεσες τιμές επιπέδων αδιπονεκτίνης σε ορό και BAL ήταν 17710 ng/ml και 911.5 ng/ml, αντιστοίχως. Δεν παρατηρήθηκαν στατιστικώς σημαντικές συσχετίσεις μεταξύ των επιπέδων αδιπονεκτίνης (τόσο στον ορό όσο και στο BAL) και των κλινικών και παθολογοανατομικών παραμέτρων που αξιολογήθηκαν. Η αδιπονεκτίνη είναι μια από τις κυριότερες αδιποκίνες που παράγονται από τον λιπώδη ιστό. Προηγούμενες μελέτες έδειξαν τη δυνητική χρησιμότητα της αδιπονεκτίνης ορού ως διαγνωστικού ή/και προγνωστικού βιοδείκτη σε διάφορους συμπαγείς όγκους, αλλά τα σχετικά δεδομένα για τον καρκίνο του πνεύμονα, ειδικότερα, είναι ελλιπή. The diverse roles of adiponectin in non-small-cell lung cancer: current data and future perspectives
In recent years, there is growing research interest for the biological
role of adipose tissue-derived bioactive factors, mainly including
adipokines, in various forms of cancer. Adiponectin (APN) is the most
abundant circulating adipokine, and a key mediator of several
cancer-related processes, such as cell proliferation, apoptosis,
regulation of tumor cell invasion and angiogenesis. In this review we
summarize and critically discuss the published literature on the diverse
roles of APN in non-small-cell lung cancer, including its implication in
lung cancer development, its use as a diagnostic and prognostic
biomarker, and its correlation with cancer-related cachexia. The main
challenges and future perspectives, mainly with regard to the potential
development of APN-targeted therapeutic agents in cancer therapeutics,
are also briefly presented and discussed
Bevacizumab-Induced Hypertension Pathogenesis and Management
Bevacizumab, a recombinant humanized monoclonal antibody targeting the
vascular endothelial growth factor (VEGF), has been approved in the US
as first- and second-line treatment of colorectal cancer and in the
first-line treatment of advanced non-small cell lung cancer. The US FDA
has also granted approval for the use of bevacizumab for the treatment
of patients with metastatic renal cell carcinoma and glioblastoma, and
in Europe, it is also approved in metastatic breast cancer in
combination with paclitaxel. Bevacizumab is under investigation in the
first-line and adjuvant setting of almost all types of solid tumors.
However, anti-VEGF therapy is associated with significant toxicity. The
incidence of grade 3-4 hypertension differs among the various
malignancies in which bevacizumab is administered, possibly because of
drug interactions with co-administered chemotherapy drugs. Hypertension
appears to be dose dependent, and it is under investigation as a
biomarker for VEGF inhibition efficacy. There are three main theories
concerning the underlying pathophysiology: (i) the nitric oxide theory;
(ii) the renal impairment theory; and (iii) the pre-eclampsia-like
theory. The correct evaluation of the levels of hypertension is of
critical importance and home blood pressure monitoring seems to be the
most effective technique. A baseline assessment and follow-up monitoring
of blood pressure is considered necessary for all patients receiving
bevacizumab. There are no evidence-based recommendations regarding which
antihypertensives are more appropriate for the management of
bevacizumab-related hypertension. It has been suggested that the
benefits from antihypertensive treatment are largely independent of the
drugs used, as long as they adequately lower blood pressure. Randomized
prospective studies are necessary to provide data that will be useful
for the development of specific guidelines for the management of
bevacizumab-related hypertension. In the meantime, treatment of
anti-VEGF-induced hypertension should follow current guidelines for
diagnosis and management of hypertension in general
Khorana Score: ew Predictor of Early Mortality in Patients With Lung Adenocarcinoma
Venous thromboembolism (VTE) is a typical complication in patients with
lung cancer. Khorana score is an established tool for thromboembolic
risk stratification of ambulatory patients with cancer undergoing
outpatient chemotherapy. The aim of this study was to evaluate the
predictive value of the Khorana score for VTE and death in patients with
lung adenocarcinoma during first-line or adjuvant chemotherapy. Medical
records of 130 patients with lung adenocarcinoma receiving first-line or
adjuvant chemotherapy were retrospectively studied during the time
period June 2013 to May 2015. Venous thromboembolism occurred in 13
(10.0%) patients. Thromboembolic events were significantly correlated
with reduced survival during treatment period (hazard ratio [HR]:
3.24; 95% confidence interval [CI]: 1.11-9.49; P = .032). The VTE
rates did not present statistically significant difference between
different Khorana score groups (P = .96). In univariate analysis, the
risk of death during treatment period (median: 16 weeks) was 3.75 times
higher in high-risk versus intermediate-risk patients (HR: 3.75, 95%
CI: 1.36-10.36; P = .001) and had 2.25 times higher per point increase
in the Khorana score (HR: 2.25, 95% CI: 1.36-3.73; P = .002); the above
results were also reproduced in multivariate analysis. Khorana score
represents a valuable tool for identifying patients with cancer in low
thromboembolic risk but does not preserve its predictive value for
higher risk individuals. Khorana score is an independent risk factor for
death in patients with lung adenocarcinoma receiving first-line or
adjuvant chemotherapy
Hypersensitivity Reactions to Oxaliplatin: A Retrospective Study and the Development of a Desensitization Protocol
Background: We recorded the epidemiologic and clinical features of
hypersensitivity reactions (HSRs) to oxaliplatin in colorectal cancer
(CRC) patients in order to provide information on the management of
these patients. We also developed a desensitization protocol and
evaluated its clinical application. For these reasons, we analyzed
retrospectively the records of 215 CRC patients treated with
oxaliplatin-containing regimens either as an adjuvant, first-line,
second-line, or more. Patients and Methods: Data from 52 patients with
HSRs were analyzed according to the Common Toxicity Criteria for Adverse
Events (CTCAE), v3.0. Three patients were chosen for the desensitization
protocol. After determining the starting point for desensitization, we
applied the standard protocol for parenteral desensitization to b-lactam
antibiotics. Oxaliplatin treatment was then carried out with serial
10-fold dilutions in sufficient volume to administer the total dose. A
total of 52 patients (24.2%) were recorded as having an allergic
reaction to oxaliplatin. Results: Hypersensitivity reactions were
recorded after a mean of 6.5 (SD +/- 4.5) cycles. Only 4 patients (4 of
215; 1.8%) developed CTCAE grade 3/4 HSRs. Hypersensitivity reactions
were seen more likely in patients receiving second-line or more of
chemotherapy compared with chemotherapy-naive patients. No other
correlations were seen (the presence of atopic disease, positive skin
prick test). The 3 patients, having completed the parenteral
desensitization protocol, completed their treatment uneventfully as
well. Conclusion: Hypersensitivity reactions from oxaliplatin respond
quickly to the discontinuation of the drug and appropriate support.
Premedication and an increased infusion time could allow for
readministration. The desensitization protocol we developed can provide
a reliable alternative to permanent discontinuation of oxaliplatin
