Conservative versus invasive strategy in elderly patients with non-ST-elevation myocardial infarction: insights from the international POPular age registry
This registry assessed the impact of conservative and invasive strategies on major adverse clinical events (MACE) in elderly patients with non-ST-elevation myocardial infarction (NSTEMI). Patients aged ≥75 years with NSTEMI were prospectively registered from European centers and followed up for one year. Outcomes were compared between conservative and invasive groups in the overall population and a propensity score-matched (PSM) cohort. MACE included cardiovascular death, acute coronary syndrome, and stroke. The study included 1190 patients (median age 80 years, 43% female). CAG was performed in 67% (N = 798), with two-thirds undergoing revascularization. Conservatively treated patients had higher baseline risk. After propensity score matching, 319 patient pairs were successfully matched. MACE occurred more frequently in the conservative group (total population 20% vs. 12%, adjHR 0.53, 95% CI 0.37–0.77, p = 0.001), remaining significant in the PSM cohort (18% vs. 12%, adjHR 0.50, 95% CI 0.31–0.81, p = 0.004). In conclusion, an early invasive strategy was associated with benefits over conservative management in elderly patients with NSTEMI. Risk factors associated with ischemia and bleeding should guide strategy selection rather than solely relying on age
Hamiltonian calculations on particle motion in linear electron accelerators
A Hamiltonian theory, in which electromagnetic space waves and longitudinal electric fields are incorporated by means of their vector potentials, is used to calculate particle motion in linear electron accelerators. In particular these calculations have been applied to the Eindhoven 10 MeV travelling-wave linac as well as to the Eindhoven racetrack microtron accelerating cavity. The calculations are in good agreement with simulations performed by particle-tracking codes

A Hamiltonian theory has been formulated, which is used to calculate accelerated particle motion in standing-wave RF structures. In particular, these calculations have been applied to the Eindhoven racetrack microtron accelerating cavity. Evolution of heterotopic bone in fibrodysplasia ossificans progressiva: An [F-18]NaF PET/CT study
Fibrodysplasia ossificans progressiva (FOP) is a rare, autosomal dominant disorder characterized by heterotopic ossification (HO) in muscles, ligaments and tendons. Flare-ups often precede the formation of HO, resulting in immobilization of joints. Due to progression of the disease without signs of a flare-up, co-existence of a chronic progression of HO has been postulated, but conclusive evidence is lacking. Recently, it has been shown that [18F]NaF PET/CT is able to identify early ossifying disease activity during flare-ups. Therefore, the purpose of the present study was to assess whether [18F]NaF PET/CT might also be able to identify the possible presence of chronic progressive HO in FOP. A total of thirteen [18F]NaF PET/CT scans from five FOP patients were analysed. Scans were acquired over a period of 0.5 to 2 years. Volumes of HO and standardized uptake values (SUV) were obtained based on manual segmentation of CT images. SUVpeak values, defined as the average SUV value of a 1 mL sphere containing the hottest voxel pixels, were obtained. Two out of five patients experienced ≥1 active clinical flare-ups at the time of the [18F]NaF PET/CT scan. In addition, in four out of five patients, serial scans showed radiological progression of HO (3 to 8 cm3), as assessed by CT volume, in the absence of a clinical flare-up. This volumetric increase was present in 6/47 (12.8%) of identified HO structures and, in all cases, was accompanied by increased [18F]NaF uptake, with SUVpeak ranging from 8.4 to 17.9. In conclusion, HO may progress without signs of a flare-up. [18F]NaF PET/CT is able to identify these asymptomatic, but progressive HO lesions, thereby demonstrating the presence of chronic activity in FOP. Consequently, future drugs should not only target new HO formation, but also this chronic HO progression
Nanoscale electrical contacts grown by focused ion beam (FIB)-induced deposition
A detailed description of the use of the focused ion beam (FIB) to grow electrical contacts is presented. The combination of FIB with precursor compounds, which are delivered on the area of interest by means of gas-injection systems, allows the growth of electrical contacts with nanometric resolution on targeted places. The technique has been coined focused ion beam-induced deposition (FIBID). Pros and cons with respect to other existing techniques for contacting are discussed. The FIBID contacts reported in this chapter are based on the use of Pt and W precursors, which result in the growth of deposits with resistivities down to 100 μΩ cm without any post-treatment. A comparison of FIBID with focused electron beam-induced deposition, the sister technique that uses focused electrons instead of ions, is also presented. The steps to follow in order to be successful in the contacting process by means of FIBID are described. Examples of the contacting to individual nanowires and nanoparticles carried out in our laboratory are shown, together with the corresponding four-probe transport measurements. Below 5 K, W deposits are superconducting and can be therefore used for zero-resistance lead contacts, superconductor-based nanocontacts and probing of proximity effects, opening new perspectives as described here.Financial support by the Spanish Ministry of Economy (through project MAT2011-27553-C02, including FEDER funding) and the Aragón Regional Government is acknowledged.Peer reviewe
Saccular Abdominal Aortic Aneurysms Patient Characteristics, Clinical Presentation, Treatment, and Outcomes in the Netherlands
Objective: The aim of this was to analyze differences between saccularshaped abdominal aortic aneurysms (SaAAAs) and fusiform abdominal aortic aneurysms (FuAAAs) regarding patient characteristics, treatment, and outcome, to advise a threshold for intervention for SaAAAs.Background: Based on the assumption that SaAAAs are more prone to rupture, guidelines suggest early elective treatment. However, little is known about the natural history of SaAAAs and the threshold for intervention is not substantiated.Methods: Observational study including primary repairs of degenerative AAAs in the Netherlands between 2016 and 2018 in which the shape was registered, registered in the Dutch Surgical Aneurysm Audit (DSAA). Patients were stratified by urgency of surgery; elective versus acute (symptomatic/ruptured). Patient characteristics, treatment, and outcome were compared between SaAAAs and FuAAAs.Results: A total of 7659 primary AAA-patients were included, 6.1% (n = 471) SaAAAs and 93.9% (n = 7188) FuAAAs. There were 5945 elective patients (6.5% SaAAA) and 1714 acute (4.8% SaAAA). Acute SaAAApatients were more often female (28.9% vs 17.2%, P = 0.007) compared with acute FuAAA-patients. SaAAAs had smaller diameters than FuAAAs, in elective (53.0mm vs 61 mm, P = 0.000) and acute (68mm vs 75 mm, P = 0.002) patients, even after adjusting for sex. In addition, 25.2% of acute SaAAA-patients presented with diameters <55mm and 8.4% <45 mm, versus 8.1% and 0.6% of acute FuAAA-patients (P = 0.000). Postoperative outcomes did not significantly differ between shapes in both elective and acute patients.Conclusions: SaAAAs become acute at smaller diameters than FuAAAs in DSAA patients. This study therefore supports the current idea that SaAAAs should be electively treated at smaller diameters than FuAAAs. The exact diameter threshold for elective treatment of SaAAAs is difficult to determine, but a diameter of 45mm seems to be an acceptable threshold.Vascular Surger
