    Report 1 PHIN-CARE-JRA2-WP3 Second Task: Pulse Shaping

    This report presents the activity developed on laser pulse shaping argument in years 2004-2005 by Milano-INFN within the framework of CARE /JRA2 \Charge production with Photoinjectors" second task \Pulse Shaping". A dedicated laser system with the relative diagnostic tools have been developed. A liquid crystal programmable spatial light modulator(LCP-SLM) shaper have been studied and set for the generation of di®erent waveforms. The shaper is integrated in the laser system for an automatic generation of the target waveforms via the insetion of a computer which drives the system through the developed software. The system can be programmed to generate any target waveform compatible with the spectral bandwidth of the laser system and some exemples are presented. The following issues are treated: (i) the operation stability as function of perturbations of the set-up parameters, (ii) the design of the shaper for the SPARC project, (iii) a new shaper concept for the generation of long target waveforms and (iiii) the rectangular pulse generation at the second harmonic

    Stretching stenoses of the external auditory canal: a report of four cases and brief review of the literature

    Acquired stenosis of the external auditory canal may be caused by a variety of insults, all sharing a common pathogenesis, namely a cascade of inflammatory changes leading to medial canal fibrosis. Previous surgery (canaloplasty or meatoplasty) and radiotherapy, especially if associated with a history of parotid surgery extended to the external auditory canal, have been implicated as possible causes. The literature offers advice on the management of stenosis consequent to otosurgery for congenital and acquired defects, but nothing on forms secondary to radiotherapy to the head and neck region. The proposed solutions are often cumbersome and difficult to fabricate, and therefore expensive. The aim of this paper, in which the cases of four patients are reported, is to present a new technique initially used for the most severe form - i.e. external auditory canal stenosis after surgery and radiotherapy - and then extended to forms due to different causes. This new technique involves the use of a series of surgical steel tubes of increasing dimension commonly used for tissue expansion in a body piercing practice called stretching and known as ear stretching tunnels or ear stretchers. This innovative approach proved effective in solving external auditory canal stenosis in our patients, with the least discomfort for the patient and the lowest cost. We consider this new solution to be feasible and practical and are convinced that it provides a new approach to an old problem. Further studies are needed to increase the number of clinical cases to verify how long the ear stretcher should be kept in place for the stenosis to stabilise, and to establish whether surgery is always necessary after ear stretcher application and, if so, the best timing for surgery

    Time domain add-drop multiplexing scheme enhanced using a saw-tooth pulse shaper

    We experimentally demonstrate the use of saw-tooth optical pulses, which are shaped using a fiber Bragg grating, to achieve robust and high performance time-domain add-drop multiplexing in a scheme based on cross-phase (XPM) modulation in an optical fiber, with subsequent offset filtering. As compared to the use of more conventional pulse shapes, such as Gaussian pulses of a similar pulse width, the purpose-shaped saw-tooth pulses allow higher extinction ratios for the add and drop windows and significant improvements in the receiver sensitivity for the dropped and added channels

    In this article we present a novel approach for the characterization of cold appliances and in particular of refrigerators based on the standard vapour compression cycle with a reciprocating on/off compressor. The test procedure is based on a virtual instrument that perform both the stimulus and the data acquisition on the device under test. Acquired data is elaborated to fit a semi-empirical model based on the energetic balances between thermal and electrical sub systems and the heat exchanged with the environment. This approach results in a simple method to calculate useful parameters of the refrigerator, such as energetic performance, cooling effect and limit values of thermal loads. The test procedure requires only a few temperatures and the electric power consumption to be monitored, resulting in a low impact on the refrigerator. Preliminary tests showed a good estimation of parameters and prediction of energy consumption and heat extraction capacity of the refrigerator under test

    Tocilizumab Effects on Coagulation Factor XIII in Patients with Rheumatoid Arthritis

    Introduction: Rheumatoid arthritis (RA) is a chronic systemic auto-immune disease associated with a prothrombotic state. Tocilizumab, an interleukin-6 receptor inhibitor, is highly effective in controlling disease activity and thrombotic risk. Factor XIII (FXIII), involved in thrombotic complications, has been reported to be reduced in RA patients during maintenance treatment with tocilizumab, but no data are available before and after the drug administration. Thus, we investigated the effects of tocilizumab on FXIII, thrombin generation and inflammation in patients with RA na\uefve for the drug. Methods: We studied 15 consecutive adult patients with RA at baseline and 4 weeks after the onset of parenteral administration of tocilizumab, measuring disease activity and plasma levels of C-reactive protein (CRP), FXIII, and prothrombin fragments F1+2 by immunoenzymatic methods. Fifteen healthy subjects, sex-and age-matched with patients, served as normal controls for laboratory measurements. Results: At baseline, patients with established RA had a median DAS28 of 4.8 (3.2\u20138.3) and, compared to healthy controls, had higher plasma levels of CRP (p < 0.0001), FXIII (p = 0.017) and F1+2 (p < 0.0001). Four weeks after starting treatment with tocilizumab, based on the EULAR response criteria, eight patients were classifiable as responders and seven as non-responders. In responders, we observed a statistically significant reduction not only of the values of DAS28 and CRP (p = 0.012 for both), ut also of plasma levels of FXIII (p = 0.05) and F1+2 (p = 0.025). In non-responders, all the studied parameters were unchanged. Conclusion: The decrease of FXIII and F1+2 levels after tocilizumab treatment observed only in those patients who responded to the drug indicates that the effect of tocilizumab on the prothrombotic state is linked to the control of inflammation and disease activity and not to a direct effect of the drug, thus contributing to the reduction of the cardiovascular risk
