Faecal sludge management in Accra, Ghana: strengthening links in the chain
Achieving a fully operational Faecal Sludge Management (FSM) chain requires well managed and sustainable
services in all aspects of the collection (emptying), transport (haulage), disposal and treatment of faecal
sludge. A fully operational FSM chain offers one type of sustainable sanitation system, particularly for
urban populations in low and middle-income countries. Failure to ensure all links in the chain are strong and
working effectively results in untreated faecal sludge contaminating the environment, with serious implications
for human health and environmental degradation. Research in Accra, Ghana has identified important
constraints to achieving an efficient and fully functioning FSM chain, with consequences for both people
and the environment. Opportunities to improve the institutional and operating environment are identified,
particularly affecting engagement between the public and private sector and civil society. Improved stakeholder
engagement, together with better regulation, management and access to facilities, enhances services
in a way that bring wider benefits to all

fNIRS neuroimaging in olfactory research: A systematic literature review
There are a number of key features which make olfaction difficult to study; subjective processes of odor detection, discrimination and identification, and individualistic odor hedonic perception and associated odor memories. In this systematic review we explore the role functional near-infrared spectroscopy (fNIRS) has played in understanding olfactory perception in humans. fNIRS is an optical neuroimaging technique able to measure changes in brain hemodynamics and oxygenation related to neural electrical activity. Adhering to PRISMA guidelines, results of this search found that generally the majority of studies involving healthy adult subjects observed increased activity in response to odors. Other population types were also observed, such as infants, individuals with autism, attention deficit hyperactivity disorder (ADHD), post-traumatic stress disorder (PTSD), mild cognitive impairment (MCI) and dysosmia. fNIRS coverage heavily favored the prefrontal cortex, temporal and parietal regions. This review finds that odor induced cortical activation is dependent on multiple factors, such as odorant type, gender and population type. This review also finds that there is room for improvement in areas such as participant diversity, use of wearable fNIRS systems, physiological monitoring and multi-distance channels
A novel mutation in PTHLH in a family with a variable phenotype with brachydactyly, short stature, oligodontia and developmental delay
Mutations in PTHLH (PTH-like hormone), cause brachydactyly type E (BDE) characterized by shortening of metacarpals, metatarsals and/or phalanges with short stature. In this report we describe three siblings and their mother with a novel heterozygous mutation c.25 T > C, p.Trp9Arg in exon 2 of the PTHLH gene. Beside the known clinical features of PTHLH mutations all had a delay in speech and language development, unknown if this is related to the mutation. Patients with PTHLH mutation may have a variable phenotypic presentation.</p
Goals and outcomes of hospitalised older people:does the current hospital care match the needs of older people?
Background Due to the rising number of acutely hospitalised older people in the coming years, there is increased interest in tailoring care to the individual goals and preferences of patients in order to reach patient-centred care. Aims To investigate the goals of older hospitalised patients and the extent to which these goals were reached during hospitalisation. Methods A single-centre prospective cohort study was performed in The Netherlands between December 2017 and January 2018. Participants aged 70 years or older were included. In the first 3 days of hospitalisation, a semi-structured interview was conducted to assess the patient goals regarding the hospital admission. At 1-2 weeks after discharge, patients were asked to what extent the recent hospitalisation had contributed to reaching their goals. Results One hundred and four patients were included and follow up was completed for 86 patients. The main goals reported at hospital admission were 'remaining alive' (72.1%), 'feeling better' (71.2%) and 'improving condition' (65.4%). Hospitalisation seemed to have a positive contribution to reaching the goals 'remaining alive', 'knowing what is wrong', 'feeling better', 'reducing pain' and 'controlling disease'. Hospitalisation seemed to contribute little to reaching the goals in the categories 'enjoying life', 'independency and freedom', 'improving daily functioning', 'hobbies and work' and 'social functioning'. Conclusions It is important for healthcare professionals to know the goals of their patients. The majority of these goals were not achieved at hospital discharge. It is important to be aware of this, so sufficient aftercare can be arranged and patients can be prepared
Congruency between Word Position and Meaning is Caused by Task-Induced Spatial Attention
We report an experiment that compared two explanations for the effect of congruency between a word's on screen spatial position and its meaning. On one account, congruency is explained by the match between position and a mental simulation of meaning. Alternatively, congruency is explained by the polarity alignment principle. To distinguish between these accounts we presented the same object names (e.g., shark, helicopter) in a sky decision task or an ocean decision task, such that response polarity and typical location were disentangled. Sky decision responses were faster to words at the top of the screen compared to words at the bottom of the screen, but the reverse was found for ocean decision responses. These results are problematic for the polarity principle, and support the claim that spatial attention is directed by mental simulation of the task-relevant conceptual dimension

This policy letter adds to the current discussion on how to design a program of government assistance
for firms hurt by the Coronavirus crisis. While not pretending to provide a cure-all proposal, the
advocated scheme could help to bring funding to firms, even small firms, quickly, without increasing
their leverage and default risk. The plan combines outright cash transfers to firms with a temporary,
elevated corporate profit tax at the firm level as a form of conditional payback. The implied equity-like
payment structure has positive risk-sharing features for firms, without impinging on ownership
structures. The proposal has to be implemented at the pan-European level to strengthen Euro area
resilience
Superoxide dismutase downregulation in osteoarthritis progression and end-stage disease
Oxidative stress is proposed as an important factor in osteoarthritis (OA). To investigate the expression of the three superoxide dismutase (SOD) antioxidant enzymes in OA. SOD expression was determined by real-time PCR and immunohistochemistry using human femoral head cartilage. SOD2 expression in Dunkin–Hartley guinea pig knee articular cartilage was determined by immunohistochemistry. The DNA methylation status of the SOD2 promoter was determined using bisulphite sequencing. RNA interference was used to determine the consequence of SOD2 depletion on the levels of reactive oxygen species (ROS) using MitoSOX and collagenases, matrix metalloproteinase 1 (MMP-1) and MMP-13, gene expression. All three SOD were abundantly expressed in human cartilage but were markedly downregulated in end-stage OA cartilage, especially SOD2. In the Dunkin–Hartley guinea pig spontaneous OA model, SOD2 expression was decreased in the medial tibial condyle cartilage before, and after, the development of OA-like lesions. The SOD2 promoter had significant DNA methylation alterations in OA cartilage. Depletion of SOD2 in chondrocytes increased ROS but decreased collagenase expression. This is the first comprehensive expression profile of all SOD genes in cartilage and, importantly, using an animal model, it has been shown that a reduction in SOD2 is associated with the earliest stages of OA. A decrease in SOD2 was found to be associated with an increase in ROS but a reduction of collagenase gene expression, demonstrating the complexities of ROS function
Sublingual allergen immunotherapy with a liquid birch pollen product in patients with seasonal allergic rhinoconjunctivitis with or without asthma
Background: Sublingual allergen immunotherapy (SLIT) has been demonstrated to be both clinically efficacious and safe. However, in line with the current regulatory guidance from the European Medicines Agency, allergen immunotherapy (AIT) products must demonstrate their efficacy and safety in pivotal phase III trials for registration.
Objective: We sought to investigate the efficacy and safety of sublingual high-dose liquid birch pollen extract (40,000 allergy units native [AUN]/mL) in adults with birch pollen allergy.
Methods: A randomized, double-blind, placebo-controlled, parallel-group multicenter trial was conducted in 406 adult patients with moderate-to-severe birch pollen-induced allergic rhinoconjunctivitis with or without mild-to-moderate controlled asthma. Treatment was started 3 to 6 months before the birch pollen season and continued during the season in 40 clinical study centers in 5 European countries. For primary end point assessment, the recommended combined symptom and medication score of the European Academy of Allergy and Clinical Immunology was used. Secondary end points included quality-of-life assessments, immunologic parameters, and safety.
Results: Primary efficacy results demonstrated a significant (P < .0001) and clinically relevant (32%) reduction in the combined symptom and medication score compared with placebo after 3 to 6 months of SLIT. Significantly better rhinoconjunctivitis quality-of-life scores (P < .0001) and the patient's own overall assessment of his or her health status, including the visual analog scale score (Euro Quality of Life Visual Analogue Scale; P = .0025), were also demonstrated. In total, a good safety profile of SLIT was observed.
Conclusion: This study confirmed both the clinical efficacy and safety of a sublingual liquid birch pollen extract in adults with birch pollen allergy in a pivotal phase III trial (EudraCT: 2013-005550-30; ClinicalTrials. gov: NCT02231307)
